Analysis of Risk Factors of Sarcopenia in the Elderly and Nursing Measures of Traditional Chinese Medicine
Sarcopenia is also known as skeletal muscular dystrophy, which has a great relationship with aging. The core symptoms are further reduction of skeletal muscle mass and significant decline of strength. As the name implies, sarcopenia mainly occurs in the elderly, which often has a huge negative impact on the health of older people. The Role of Key Early Warning Indicators in Crisis Management: Analytical Review
This study came to identify the role of applying key early indicators in managing crisis through answering the following main question: What is the role of the key early indicators in crisis management? Methodology: The study used an analytical descriptive approach based on induction, analysis, and comparison of the literature on early warning indicators, crisis management, its divisions and how to be built. Subsequently, the study achieves answers for the study questions by analyzing and inferring the reviewed related literature. Results: Key early warning indicators for crisis management play a major role in the notification and information about the imminent occurrence of risk that cause the spread of crisis. This helps to move forward in a proactive manner towards the crisis before it occurs. The successful role of key early warning indicators in crisis management requires certain characteristics. It should be a real indication of the imminent crisis and valid. Conclusion: The study concludes that there is need for key early warning indicators for their effective role in assisting senior leaders in taking proper decisions. The higher management of the organization should select the appropriate and qualified planning team and develop their skills
Research Progress on Intestinal Barrier Dysfunction and Treatment of Severe Acute Pancreatitis
Severe acute pancreatitis (SAP) often leads to intestinal epithelial cell damage, disrupts intestinal epithelial cell structure, increases intestinal mucosal permeability, and leads to intestinal barrier dysfunction (IBD). IBD is prone to cause intestinal toxin and bacterial translocation, further accelerating the course of severe pancreatitis, leading to systemic inflammatory response or multiple organ failure, and even death. It can be seen that IBD plays a key role in the development of pancreatic tissue necrosis infection and severe pancreatitis. Intestinal barriers, ie, chemical, physical, immune, and biological barriers, can interact and promote positive feedback. Factors such as intestinal ischemia, inflammatory factors, intestinal flora, immune damage, intestinal nutrition and motility lead to IBD disease. Therefore, the assessment and protection of intestinal barrier function is essential for the treatment of severe pancreatitis. This article reviews the pathogenesis and treatment of IBD disease
Application of Strengthening Detail Management in Intensive-Care Unit Nursing Management
As the core work of the hospital, the intensive care unit (ICU) has gathered many of the most cutting-edge facilities and related medical staff. Study on the Risk of Metabolic Syndrome Based on UA/Cr Analysis
To explore the epidemiological trend of the relationship between serum uric acid/creatinine ratio(UA/Cr) and metabolic syndrome (MS) in middle-aged and old people in Wuhan, Hubei Province. The study used the method of cluster sampling, In 2016, 6570 residents over 45 years old in Wuhan City, Hubei Province were investigated with standardized questionnaire, physical examination and biochemical examination. MS is defined according to the standard of the International Diabetes Union. According to the quartile grouping of UA/Cr, the regression analysis of risk metabolism indexes between UA / Cr and MS was carried out. Analyze the correlation between UA / Cr and MS to determine whether UA / Cr can be an independent risk factor of MS.6414 subjects were included in this study including 3313 MS patients (52.7%). Blood CR level is 77(70~87) mmol/L,Blood UA level is 314(267~372) mmol/L,UA/Cr level is 4.01(3.46~4.65). The study population was divided into four groups according to the UA / Cr quartile.Group 1 UA/Cr <1.0, Group 2 1.0≤UA/Cr<1.5,Group 3 1.5≤UA/Cr<2.0,Group 4 UA/Cr≥2.0. From the first group to the fourth group, with the increase of UA / Cr level, the body mass index (BMI), waist circumference, blood pressure, triacylglycerol, low-density lipoprotein cholesterol, total cholesterol, C-reactive protein and insulin resistance increased gradually (P < 0.01). The prevalence of MS and its components also increased significantly (P < 0.01).Conclusion:There was an independent positive correlation between UA / Cr and MS in the middle-aged and old people in the community, UA / Cr can be used as early biomarker of MS. It provides a new scientific basis for the early detection of MS, the intervention of high-risk population and the reversal of cardiovascular disease
Splenic Artery Aneurysms
Splenic artery aneurysms (SAAs) are a rare arterial disease (1) and defined as a diameter of more than 50% of expected (0.9 - 1 cm) splenic artery diameter. Â They account for 60-70% of visceral artery aneurysms making them the most common of this group (4). Â They are often difficult to diagnose due to their vague or non-existing symptoms but present a high risk of rupture in certain patient groups making this a clinically important differential diagnosis. Â
The Looming Success in Cancer Vaccination
Cancer vaccination projects are on trial worldwide and the results are far-off being a remarkable success. Albeit, thousands of clinical trials are taking place, only a several of those are producing a significant result to increase the survival rate of the patients. Four vaccines (Human papillomavirus - HPV vaccines, Hepatitis B virus - HBV vaccines, Sipuleucel-T and Oncophage) are approved for market in the United States and Russia so far. Most of the prototype vaccines are yielding at phase III clinical trials after being successful at phase I and II. Apparently, new visions and approaches are required to guide these projects to harvest better results
A Rare Case Od Meckel’s Diverticulum Perforation Caused By a Toothpick Mimicking Acute Appendicitis
Meckel’s diverticulum (MD) is the most common congenital abnormality of the gastrointestinal tract and is caused by the failure of omphalomesenteric duct to obliterate during embryonal maturation. Most patients are asymptomatic with only 4-16% presenting complications which include hemorrhage, intestinal obstruction, inflamation and occasionally perforation. A preoperative diagnosis of a complicated Meckel’s diverticulum may be challenging because of the overlapping clinical and imaging features of other acute surgical and inflammatory conditions of the abdomen. A 32-year old male was admitted to the emergency department with a 48-hour history of colicky like pain in the lower part of the abdomen, which was gradually increased and was localised in the lower right quadrant, with no history of fever, nausea or vommiting, no urinary symptoms nor altered bowel habits. Laboratory revealed leukocytosis and elevated CRP, while on physical examination guarding and rebound tenderness were found in the right lower quadrant. Abdominal ultrasound  established a distended small bowel with very slow peristalsis in ileocecal region. Complete workup raised suspicion for an acute appendicitis, therefore the patient was operated. Intraoperatively, the perforated Meckel’s diveriticulum was found, caused by a toothpick. Diverticulectomy and appendectomy were done. Postoperative course was uneventful. Regardless of the fact that a variety of diagnostic procedures are at the disposal of surgeons, we feel that the diagnose of a complicated MD remains a challenging task, largely in thanks to the fact that it mimics some other acute abdominal pathology and, often, is misdiagnosed
Frequency of urinary incontinence with pelvic organ prolapse and associated factors.
 Introduction : To determine the frequency of common types of urinary incontinence (UI) in women with pelvic organ prolapse (POP) attending Uro-gynaecology clinics at Aga Khan Hospital Karachi. METHODS: A total of 85 women attending the Uro-gynaecology clinics with symptoms of pelvic organ prolapse were included in a cross sectional survey  RESULTS: Out of 85 women presented with pelvic organ prolapse, 61(71.7%) were aged above 50 years and 24(28.4%)  below 50 year of age . The frequency of SUI was reported by 46(54.1%) women, 16(18.8%) women presented with UUI and 23(27.1%) with MUI. Large majority of women were postmenopausal(69%) ,  multiparous(71%), , and obese with a BMI >25 (69.4%). Thirty nine (46%) women had III grade of POP and 46( 54%) had SUI. Grade of POP was significantly associated with UI (SUI). Grade II POP was associated statistically significant  with SUI [Adjusted Odd Ratio (AOR) 0.04; 95% CI: 0.004, 0.418; P value: 0.007].The other factors like age ,parity and BMI were not found to be statistically significant in association with UI.CONCLUSION: Pelvic organ prolapse and urinary and incontinence are significant problems in developing countries .Both of these conditions badly impact quality of life of women .In our study only SUI was found to be significantly associated with increasing grades of POP
Clinical analysis of chemotherapy-related anemia in non-small cell lung cancer patients
The objective is to analyze the clinical features and factors of non-small cell lung cancer patients with chemotherapy-related anemia to prevent and treat anemia. 78 cases of hospitalized patients with advanced non-small cell lung cancer treated with chemotherapy from January 2008 to October 2014 were recruited as subjects. The anemia information after chemotherapy was analyzed to understand the clinical features and the factors of anemia after chemotherapy. The incidence of anemia after chemotherapy was 82.05%, while the rate moderate and severe anemia was 34.61%. No significant difference was observed from the incidence of anemia after chemotherapy in patients in terms of gender. Among the stage IV patients, patients with bone metastasis has higher incidence of anemia after chemotherapy when compared with patients without bone metastasis (P < 0.05). Non-small cell lung cancer has a higher incidence of anemia after chemotherapy, patients should be given attention on the anemia therapy in addition to other clinical treatment. Patients with bone metastases are more prone to have anemia after chemotherapy thus, early intervention should be taken to reduce the incidence of anemia
