    Comparison of Postpartum Sexual Function in Patients who had Undergone Episiotomy and had Perineal Tears

    BACKGROUND/AIMS: To make a comparison regarding postpartum sexual function according to the presence of episiotomy and the degree of perineal tears. MATERIALS AND METHODS: Two hundred ninety-four primiparous patients in total between the ages of 18 and 45 who arrived at the obstetrics outpatient clinic between 2017 and 2024, underwent pregnancy follow-up and delivery, and fulfilled the inclusion criteria were analyzed in our research. The study included 131 women with perineal tears and 163 women who underwent episiotomy. Retrospectively, at 6 months postpartum, the Female Sexual Function Index (FSFI), Quality of Sexual Experience Scale (QSES), and visual analog scale (VAS) for dyspareunia values of all individuals were reviewed from participant files. RESULTS: The VAS, QSES, and FSFI values were significantly greater in the episiotomy (+) group than the episiotomy (-) group (p<0.01). The VAS value was significantly greater in the episiotomy (+) group in comparison with the other groups (p<0.01). The QSES and FSFI values were significantly smaller in the tear degree 4 group than those in the other groups (p<0.01). The values of arousal and lubrication were significantly smaller in the tear degree 4 group than those of the other groups (p<0.01). The orgasm and satisfaction values were significantly smaller in the tear degree 4 group compared with the other groups (p<0.01). CONCLUSION: Sexual dysfunction in the postpartum period can be affected by social, physical, psychogenic, and demographic factors. Our study showed that preventing advanced-stage tears through episiotomy might contribute positively to sexual function aspects. Prospective studies with larger samples are also needed to evaluate the effects of other maternal, demographic, and cultural factors that might influence females' sexual functions in the postpartum period

    Transforming Urban Planning with LLMs: A GPT Model for Plan Notes in Turkiye

    In the urban planning process in Turkey, plans are integrated with plan notes and reports, and the plan notes produced during the plan-making process undertake a critical function in shaping the urban forms in terms of ensuring the applicability of plan decisions and their compliance with spatial features. Plan notes provide professionals with either a restrictive or detailed framework in the design process of urban forms. However, the writing of these notes is a time-consuming process that requires intensive specialized knowledge and is open to interpretation. Differences in the content and style of plan notes in different cities also make standardization difficult. This study aims to develop a GPT-based language model prototype that can support planners' plan note production processes by associating plan notes from various cities in Turkey with urban morphology indicators and fine-tuning them on OpenAI GPT models. First, plan notes were collected from different cities and these notes were matched with the urban form features (morphological indicators such as urban density, number of floors, building block typology) of the generated plan. The resulting prototype generates draft plan notes that can be used in plan-making processes based on spatial inputs received from the user (low density residential area, maximum 3 floors). Preliminary results show that the GPT model can provide planners with fast and consistent recommendations for producing plan notes supported by urban form data. In particular, providing morphological factors such as land use characteristics, city block typology and density as input data to the model has enabled the model to produce content that is more compatible with local planning decisions. This study provides an important application example in the field of urban planning and design on the use of large language models in specialized fields such as urban planning by fine-tuning them with local data.</p

    The effect of eHealth-based guided self help interventions for binge eating disorder : a meta-analysis of randomized controlled trials

    With rapid technological advancements, eHealth-based guided self-help interventions have become accessible, flexible, cost-effective, and stigma-reducing treatment options for binge eating disorder (BED). This meta-analysis evaluated the effectiveness of these interventions in individuals diagnosed with BED or showing BED symptoms, based on eight randomized controlled trials with 1,575 participants. Intervention length varied between a single session to four months. Six studies focused solely on web-based guided self-help interventions, one study implemented a hybrid approach combining face-to-face and online components, and another study employed two distinct online guided self-help methods. The interventions included psychoeducational modules, therapist feedback, online behavior monitoring, and self-assessments. eHealth-based guided self-help significantly reduced binge eating psychopathology (SMD: 0.53; 95% CI: 0.20–0.86) and objective binge eating (OBE) days (SMD: 0.49; 95% CI: 0.12–0.85) compared to controls.     YEŞİL SENTEZ YÖNTEMİ İLE ACHILLEA MILLEFOLIUM VE ECHINACEA PURPUREA ÖZÜTLERİ KULLANILARAK PLATİN NANOPARTİKÜLLERİNİN SENTEZİ VE KARAKTERİZASYONU

    Bu çalışmada, yeşil sentez yöntemi kullanılarak Achillea millefolium (civan perçemi) ve Echinacea purpurea (ekinezya) bitki özütleri aracılığıyla platin nanopartikülleri (PtNP) sentezlenmiştir. Bitki özütlerinde bulunan fenolik bileşikler, flavonoidler ve protein benzeri biyomoleküllerin Pt⁴⁺ iyonlarının indirgenmesinde ve oluşan nanopartiküllerin stabilize edilmesindeki etkisi FTIR ve UV-Vis analizleri ile incelenmiştir. X-ışını kırınımı (XRD) analizleri her iki bitki özütü kullanılarak sentezlenen PtNP’lerin kristal yapıda olduğunu göstermiştir. Elde edilen sonuçlar, bitki türüne bağlı olarak Pt nanopartiküllerinin yapısal ve fonksiyonel özelliklerinin değiştiğini ortaya koymuştur. A. millefolium özütü ile sentezlenen PtNP’lerin, E. purpurea özütü ile elde edilen PtNP’lere kıyasla daha yüksek katalitik aktivite sergilediği belirlenmiştir. Bu durum, A. millefolium özütünün içeriğindeki biyolojik aktif bileşenlerin nanopartikül yüzeyinde daha etkin bir fonksiyonelleşme sağlaması ve katalitik açıdan daha elverişli aktif yüzeyler oluşturması ile ilişkilendirilmiştir. Ayrıca, A. millefolium özütü kullanılarak sentezlenen PtNP’lerin katalitik performansı, etanolün indirgeyici ajan olarak kullanıldığı konvansiyonel yöntemle sentezlenen PtNP’ler ile karşılaştırılmış ve yeşil sentez yöntemiyle elde edilen PtNP’lerin daha yüksek katalitik etkinlik gösterdiği tespit edilmiştir. Bunun yanı sıra, sentez sürecinde stabilize edici olarak kullanılan polivinil pirolidon (PVP) miktarının PtNP’lerin katalitik performansı üzerinde belirleyici bir etkisi olduğu gözlenmiştir.&nbsp;&nbsp;</p
