    Psychological and Clinical Factors Mediate Post-COVID-19 Irritable Bowel Syndrome

    Background: Exposure to COVID-19 has been shown previously to be associated with a higher risk for irritable bowel syndrome (IBS). This study aimed to better explain this relationship using mediation analysis. Methods: This post hoc analysis of a multicenter cohort study includes 623 patients with and without COVID-19 infection. All participants completed the ROME IV criteria, gastrointestinal symptom rating scale (GSRS), and hospital anxiety and depression scale (HADS) over 1 year. Mediation analysis utilized the PROCESS macro and Baron and Kenny's method for parametric and nonparametric mediating variables, respectively. Key Results: The impact of COVID-19 on the development of post-COVID-19 IBS is completely mediated by dyspnea at baseline (adjusted OR = 3.561, p = 0.012), severity of acid regurgitation at 1 month [indirect effect, log-odds metric = 0.090, 95% CI (0.006–0.180)], hunger pains at 1 [indirect effect, log-odds metric = 0.094, 95% CI (0.024–0.178)], and 6 months [indirect effect, log-odds metric = 0.074, 95% CI (0.003–0.150)], depression at 6 [indirect effect, log-odds metric = 0.106, 95% CI (0.009–0.225)] and 12 months [indirect effect, log-odds metric = 0.146, 95% CI (0.016–0.311)] as well as borborygmus [indirect effect, log-odds metric = 0.095, 95% CI (0.009–0.203)], abdominal distention [indirect effect, log-odds metric = 0.162, 95% CI (0.047–0.303)], and increased flatus [indirect effect, log-odds metric = 0.110, 95% CI (0.005–0.234)] at 12 months. Conclusions and Inferences: Our findings provide evidence for psychological and clinical mediators between COVID-19 and post-COVID-19 IBS, which may be promising targets for interventions tailored for treating or preventing depression. The presence of specific GI symptoms at COVID-19 onset and their persistence should increase awareness of a potential new onset of IBS diagnosis

Google Trends verileri,turizm talebinin tahmini, pazarlama stratejilerinin planlanması ve bölgeselyatırım kararlarının verilmesinde kullanılabilecek nitelikli bir açık verikaynağı olarak ele alınmıştır. Google Trends, dijital pazarlama stratejileriiçin oldukça değerli bir araçtır çünkü gerçek zamanlı arama verilerine dayalıiçgörüler sunar. Dijital pazarlamada hedef kitleye doğru zamanda, doğruiçerikle ulaşmak kritik önemdedir[1,2,3]. Literatürde açık verinin önemini veiş zekası uygulamalarının değerini ortaya koyan pek çok çalışmagerçekleştirilmiştir [4,5,6,7,8,9,10]. Çalışmada anahtar kelimeler üzerindenyapılan arama eğilimleri incelenmiş; veriler analiz edilerek zamansal grafiklerile desteklenmiştir. Elde edilen veriler Microsoft Power BI gibi iş zekasıuygulamaları ile görselleştirilmiş ve politika yapıcılar için anlaşılır birformatta raporlanmıştır. Bu yöntem, turizm verilerinin yönetilmesinde açık verive iş zekâsı bileşenlerinin birlikte kullanılmasının önemine dikkatçekmektedir. Sonuçlar, özellikle yaz aylarında yoğunlaşan arama hacimlerinin,bölgesel tanıtım kampanyalarının zamanlaması ve içerik planlaması açısındanönemli birer gösterge niteliği taşıdığını ortaya koymuştur. Ayrıca, aramatrendlerindeki ani değişimler, kriz dönemlerinde turizm algısındaki dönüşümlerierken evrede tespit etmek açısından stratejik önem taşımaktadır. Bu bağlamda,Türkiye'deki turizm politika yapıcılarına, Google Trends gibi ücretsiz veerişilebilir veri kaynaklarını düzenli olarak kullanmaları; bu verileri yerelyönetimler ve turizm işletmeleri ile entegre biçimde değerlendirmeleriönerilmektedir. Veri temelli karar alma süreçlerinin benimsenmesi, sadecetanıtım faaliyetlerinin etkinliğini değil, aynı zamanda sürdürülebilir ve hedefodaklı turizm politikalarının oluşturulmasını da mümkün kılacaktır.     Doxorubicin (DOX), a commonly used anti-neoplastic agent, has been associated with significant cardiotoxic effects, which limit its clinical utility. Recent studies suggest that mesenchymal stem cells (MSCs) may offer therapeutic potential in mitigating DOX-induced cardiotoxicity through their regenerative properties. This study aimed to evaluate the cardioprotective effects of fetal kidney-derived mesenchymal stem cells (FKD-MSCs) in a DOX-induced cardiotoxicity rat model. Thirty male Sprague-Dawley rats were divided into three groups: control, sham, and treatment. DOX (10 mg/kg) was administered to the sham and treatment groups to induce cardiotoxicity. The treatment group received intraperitoneal FKD-MSCs (2 × 106) three times at weekly intervals post-DOX administration. Immunohistochemical analyses were conducted to assess cardiac recovery. The 5-bromo-2-deoxyuridine (BrdU) labeling technique was used to track FKD-MSC localization in the cardiac tissue. The immunohistochemical findings demonstrated a significant improvement in the treatment group compared to the sham group. The BrdU-labeled FKD-MSCs were predominantly localized in cardiac muscle tissues, indicating their successful homing and integration into damaged cardiac regions. The results of the study indicate that FKD-MSCs significantly attenuated DOX-induced cardiotoxicity in rats, suggesting their potential as a novel therapeutic approach for cardioprotection. Further studies are warranted to investigate their clinical applications in managing chemotherapy-induced cardiotoxicity

    Fresh and cryopreserved stem cell transplantation in myeloma patients: Does it make a difference on transplant outcomes?

    Objective: The standard approach for multiple myeloma patients eligible for transplantation includes 4–6 cycles of induction therapy, followed by autologous stem cell transplantation (aHSCT). The aHSCT process starts with stem cell mobilization and collection, followed by high-dose chemotherapy and reinfusion of the harvested stem cells. These cells can be infused fresh within 24–48 h post-collection or cryopreserved for future use. Herein we analyzed the outcomes of aHSCT patients receiving infusions of fresh versus cryopreserved hematopoietic stem cells. Materials and methods: This multicenter retrospective study analyzed 88 adult patients diagnosed with multiple myeloma who underwent aHSCT (n = 43 for cryopreserved; n = 45 for fresh infused group). Results: A total of 88 patients were included in the study. 39.7 % of patients were female and 60.3 % of patients were male. No correlation was observed between pre-mobilization disease status, mobilization regimen, and disease risk status as defined by the R-ISS system (p = 0.1, p = 0.8). The median neutrophil engraftment time was 10 days in the fresh group and 12 days in the cryopreserved group (p < 0.01). In contrast, the median platelet engraftment time was 12 days in the fresh group and 11 days in the cryopreserved group (p < 0.01). Engraftment was achieved in all patients included in the study. Conclusion: The shorter neutrophil engraftment time in the fresh group and the shorter platelet engraftment time in the cryopreserved group, along with successful engraftment in all patients, suggest that both options are reasonable within the MM aHSCT protocol

    Retrospective Evaluation of Clinical, Demographic, and Radiological Data of Orbital Lymphoma Patients: A Single Tertiary Center Experience

    Objectives: The objective of the study was to present different clinical presentations, clinical and histopathological features, and treatment outcomes of intraorbital lymphoma. Methods: Medical records of 18 eyes of 17 patients with histopathologically proven diagnoses of intraorbital lymphoma at Dokuz Eylül University Ophthalmology Department, between 2007 and 2022, were reviewed retrospectively. The age, gender, location, laterality, stage, type of involvement, histopathological features, systemic involvement, recurrence, time from initial symptoms to diagnosis, follow-up time, and survival times were recorded. Visual acuities at the time of diagnosis and following treatment were evaluated. Results: The mean age of the patients was 63±19.11 (range: 15-79, median age 65). The most common presenting findings were periorbital swelling, conjunctival mass, and ptosis. Periorbital region involvement was present in 10 of 17 cases (58.8%) and conjunctival involvement was present in 7 cases (41.2%). Biopsy samples were taken from all cases. Non-Hodgkin lymphoma was the most common type of lymphoma, whereas the most common histopathological diagnoses were marginal zone lymphoma and diffuse large B-cell lymphoma. One case was diagnosed with mature T-cell lymphoma. Nine cases had bone marrow involvement. While two cases refused treatment, two cases were not treated due to age and comorbid diseases. Nine cases received chemotherapy, two cases received radiotherapy (RT), 1 case received chemotherapy and RT, and one case received antibiotherapy treatment. In one case with proptosis and exposure keratitis, lateral canthatomy, temporary tarsorrhaphy, and aggressive topical treatment resulted in complete epithelialization of the cornea. Conclusion: Orbital lymphoma is a painless and slowly progressive clinical picture that requires high clinical suspicion because it is a common tumor of the orbit. Although clinical symptoms and findings vary according to the intraorbital location of lymphoma, patients should be examined for systemic involvement and followed up in a multidisciplinary manner

    Multilingual TESOL: De-constructing and Re-constructing Power 'in', 'of' and 'for' Language Classrooms

    &nbsp;This chapter explores the power dynamics that are inherent in the way teachers and learners construct their identities in multilingual TESOL classrooms. Utilizing imagined communities (Anderson, 1991) as our conceptual framework and a sociocultural perspective on identity as dynamic, reimagined, and redefined through imagined communities, we trace the identity navigations of (n:22) pre-service teachers in an English Language Teaching program in Türkiye through their language autobiographies (LAs) and written reflection (WRs) as identity texts (Anderson, 1991; Akkerman &amp; Meijer, 2011; Beauchamp &amp; Thomas, 2009; Cummins, 2006; Varghese, et al., 2005). Data were analyzed in an abductive manner recursively by two pairs of authors and discussed in online meetings to maintain consensus. Findings overall indicate a shift from monolingual to multi/plurilingual identities for participants that we map on a journey of becoming. With the intention of staying true to the narratives of the participants, we organize the main theme into four sub-themes to illustrate and discuss the findings: discover, 2 explore, embrace, and enact. The study contributes an international perspective to the field on critical and identity oriented TESOL teacher preparation.&nbsp;</p

    Bu çalışma, 11 Eylül 2001 terör saldırılarının ardından Amerikan toplumunda yaşanan ideolojik ve kültürel dönüşümleri, bu değişimlerin yetişkinlik algısı üzerindeki etkilerini incelemektedir. Örneklem olarak Aqua Teen Hunger Force adlı animasyon dizisini ele alan makale, dizinin 11 Eylül sonrası Amerikan toplumundaki yetişkinlik kavramının yeniden tanımlanmasına nasıl katkıda bulunduğunu araştırmaktadır. Adult Swim platformunun bir parçası olan bu yapım, absürt mizahı ve geleneksel toplumsal normlara meydan okuyan anlatımıyla, dönemin bireyci ve sorgulayıcı gençlik kültürünü yansıtmaktadır. Makale, Aqua Teen Hunger Force’un yalnızca bir eğlence aracı olmadığını, aynı zamanda Amerikan toplumundaki geniş çaplı ideolojik ve kültürel değişimlerin bir yansıması olduğunu savunmaktadır. Bu bağlamda, dizi hem izleyicilere politik ve toplumsal baskılardan uzaklaşma imkânı sunmakta hem de geleneksel yetişkinlik ve kimlik anlayışını sorgulamaya teşvik etmektedir. Çalışma, medya ürünlerinin ideolojik etkilerine ışık tutarak, 11 Eylül sonrası Amerikan toplumu ve kültürünün anlaşılmasına katkıda bulunmayı amaçlamaktadır.This study explores the ideological and cultural transformations in American society following the terrorist attacks of September 11, 2001, and examines the impact of these changes on the perception of adulthood. Using the animated series Aqua Teen Hunger Force as a case study, the article investigates how the series contributed to redefining the concept of adulthood in post-9/11 American society. This production, part of the Adult Swim platform, uses absurd humor and a narrative style that challenges traditional social norms, reflecting the individualistic and questioning youth culture of the time. The article argues that Aqua Teen Hunger Force is not merely a form of entertainment but also a reflection of broader ideo-logical and cultural shifts in American society. In this context, the series provides viewers with both a reprieve from political and social pressures and an opportunity to question traditional concepts of adulthood and identity. By highlighting the ideological implications of media products, this study seeks to enhance understanding of post-9/11 American society and culture.</p
