Comprehensive analysis of the role of deep inspiration breath-hold in right-sided breast cancer radiotherapy: A focus on cardiac substructures and right coronary artery
Purpose: The deep inspiration breath-hold (DIBH) technique, a method used to minimize radiation exposure to normal tissues in breast cancer patients, has primarily been applied to patients with left breast cancer. However, this study, with its unique focus on right breast cancer, aims to evaluate the potential benefits of DIBH on the right coronary artery (RCA), cardiac substructures, and liver dose in right breast cancer radiotherapy. Materials and Methods: Forty right-sided breast cancer patients were included in this study. Radiotherapy planning using 3D, IMRT, and VMAT was performed using both free-breathing (FB) and DIBH techniques. Two treatment plans were generated per patient, and organ-at-risk (OAR) doses were compared using the Wilcoxon signed-rank test. Patients were analyzed based on regional nodal irradiation (RNI) status. Results: The mean radiation doses (Dmean) to the liver, ipsilateral lung, heart, and RCA region were 5.4, 15.9, 2.5, and 6 Gy with FB, decreasing to 4.6, 12.6, 1.6, and 3.6 Gy with DIBH. For the left ventricle, right ventricle, left atrium, and right atrium, the Dmean values were 1.2, 2.2, 2, and 4.6 Gy with FB, versus 0.76, 1.8 Gy, 1.2, and 3.2 Gy with DIBH. DIBH significantly reduced liver and lung doses in all patients, while heart and RCA doses reductions were observed only in those receiving RNI. Conclusion: The use of the DIBH technique can significantly reduce radiation exposure to OARs in right-sided breast cancer radiotherapy. Despite a slight increase in treatment duration, DIBH should be considered for right breast cancer radiotherapy to minimize radiation-related toxicity
Genetic heterogeneity in childhood leukemia/lymphoma: a Turkish cohort with strong predisposition
Background: Leukemia is the most common cancer in children, and 10%–15% of patients with leukemia/lymphoma carry pathogenic germline cancer-predisposing variants. Identifying these variants is critical for understanding the genetic predisposition and optimizing clinical management. Methods: We performed germline short-read sequencing in 36 individuals from 20 families with suspected leukemia/lymphoma predisposition, including 20 index cases, 9 affected relatives, and 7 unaffected members. Results: We identified 13 clinically relevant germline variants in known cancer predisposition genes including TP53, ETV6, MSH6, MLH1, and BRCA1. Notably, we uncovered novel candidate variants in ATR, TNFRSF9, ETAA1, and KSR1, which was supported by segregation analysis, consanguinity patterns, and secondary malignancy phenotypes. Several index cases exhibited striking familial cancer syndromes involving both hematologic and solid tumors, with progression from ALL to AML or glioma. Deep clinical–genomic correlation enabled reclassification of variants and refined diagnostic and therapeutic decision-making in multiple cases. The patients were referred to genetic counseling for surveillance of carriers and risk assessment for various family members. Conclusion: These findings emphasize the clinical utility of germline testing in pediatric hematologic cancers by providing novel insights into the predisposition to leukemia/lymphoma and contributing to treatment regimens, donor selection, and diagnostic refinement, particularly in populations with high consanguinity
RESVERATROL STABILIZES CYTOSOLIC ß- CATENIN THROUGH THE ACTIVATION OF THE THERMOGENIC CASCADE
Purpose: Resveratrol is a polyphenolic compound that has been shown to stimulate fatty acid oxidationand increase the expression of genes associated with thermogenic processes. Therefore, it is importantto investigate the effects of resveratrol beyond intracellular thermogenic pathways to determine sideeffects at the cellular level. The aim of this study was to evaluate the effect of resveratrol on intracellularß-catenin, in addition to its thermogenic effect.Materials and Methods: The effect of resveratrol on thermogenic genes such as SIRT1 gene, UCP1,PGC1α, PRDM16 and ß-catenin gene expression levels was analyzed by RT-PCR. The cytosolic ßcateninlevel analyzed by western-blot.Results: The effect of resveratrol on thermogenic genes such as SIRT1 gene, UCP1, PGC1α, PRDM16and ß-catenin gene expression levels was analyzed by RT-PCR. The cytosolic ß-catenin level analyzedby western-blot. We found that the expression levels of UCP1, PPARγ, PRDM16 were increased by SIRT1protein activated by resveratrol, but it had no effect on ß-catenin mRNA expression. While ß-catenin didnot change at the mRNA level, we found that ß-catenin increased at the protein level in the cellindependently of the canonical Wnt pathway.Conclusion: Our results suggest that resveratrol, which is widely used as a supplement, may also act ondifferent signaling pathways in the cell.Keywords: Resveratrol, obesity, thermogenesis, β-Catenin</p
Evaluation of Adolescents with Anxiety Disorders in the Context of Cognitive Distortion
Background: This study aimed to investigate the differences in cognitive distortions between patients with anxiety disorder (AD) and healthy controls, investigate the relationship between anxiety levels and cognitive distortions in AD, and investigate whether accompanying symptoms in AD had an impact on cognitive distortions Methods: Eighty-nine adolescents diagnosed with AD and 94 healthy adolescents were assessed using The Kiddie Schedule for Affective Disorders and Schizophrenia, Screen for Child Anxiety Related Emotional Disorders (SCARED), Beck Depression Inventory (BDI), Cognitive Distortions Scale (CDS), and the effects of major depressive disorder (MDD) were evaluated. Results: All cognitive distortions except catastrophizing were more problematic in AD. It also assessed the effects of comorbid MDD with AD on cognitive distortions. Another finding that was obtained in this study was that thought characteristics such as mindreading, catastrophizing, all-or-nothing thinking, emotional reasoning, personalization, minimizing the positive, and overgeneralization, CDS total scores were statistically significantly higher in the AD group with comorbid MDD. Conclusion: The study indicates that some cognitive distortions may be prevalent in AD or AD with comorbid depression. The importance of this study is the probability of laying the groundwork for future research in adolescents, the development of cognitive anxiety models, and guiding treatment practices
