The effectiveness of postoperative delirium prevention, diagnosis, and intervention protocol in patients monitored in the intensive care unit after cardiac surgery: a quasi-experimental study
Background: The incidence of delirium is high in the intensive care unit (ICU) after cardiac surgery. The development of evidence-based care protocols for delirium management and training of nurses in this regard can ensure effective management of delirium. This quasi-experimental study aimed to assess the effectiveness of a postoperative delirium prevention, diagnosis, and intervention protocol in patients undergoing monitoring in the ICU after cardiac surgery. Methods: This study included 64 patients who underwent cardiac surgery and met the inclusion criteria, along with 14 nurses working in the ICU. Patients were divided into control (n = 32) and intervention (n = 32) groups. The study comprised three phases: determining the incidence of delirium in the control group and the delirium diagnosis status of the nurses; providing training to nurses on postoperative delirium prevention, diagnosis and intervention protocol; implementing a preliminary study of the protocol; and finally, implementing the protocol in the intervention group. Statistical significance was set at p 0.05), significant agreement was observed in the intervention group (kappa: 1.00) (p < 0.001). The delirium diagnosis rate of the nurses was 14.3% in the control group and 100% in the intervention group, which was a significant difference. The incidence of delirium was 21.9% in the control group and 9.4% in the intervention group, although the difference was not significant. Conclusion: Postoperative delirium prevention, diagnosis, and intervention protocol effectively enhance delirium diagnosis compliance among researchers and nurses and improve the accuracy of delirium diagnosis among postcardiac surgery ICU patients. The implementation of this protocol is recommended for delirium management in such patients. Cell proliferation and apoptosis in canine oral papillomatosis
This study was aimed at the evaluation of cell proliferation, p53 level and apoptotic index by immunohistochemical methods in canine oral papillomatosis. The study material comprised of tumor tissue samples taken from six dogs being admitted to the Pathology Department of Faculty of Veterinary Medicine, Kafkas University, Kars, Türkiye. Choice of immunohistochemical staining was avidin-biotin peroxidase method. Cases of canine oral papillomatosis, determined to have been caused by canine papillomavirus-1, were found to have a rather high cell proliferation index. Furthermore, all cases were immunohisto-chemically demonstrated to carry a mutant p53 gene. Despite the mutation of p53 gene, the shift in the Bax/Bcl-2 ratio of dogs diagnosed with tumor was in favor of the pro-apoptotic Bax gene. The apoptotic mechanism was determined to occur through both the caspase-dependent and caspase-independent pathways. While the lesions occupied the entire oral cavity in some cases, histopathologically, malignant transformation was not detected in any of the six cases

COMPARISON OF EARLY OUTCOMES BETWEEN LICHTENSTEIN AND LAPAROSCOPIC REPAIR IN INGUINAL HERNIA SURGERY
Introduction and Aim: Inguinal hernia repair is one of the most commonly performed surgeries. Alongside many open repair techniques, laparoscopic repair methods have gained prominence in recent decades. This study was designed to investigate the early postoperative outcomes and complications of open and laparoscopic hernia repair methods. Patient files of those who underwent Lichtenstein and transabdominal pre-peritoneal repair (TAPP) between January 2021 and January 2024 were retrospectively reviewed. The patients' age, gender, type of anesthesia, length of hospital stay, pain VAS score, type of hernia, operation duration, complications, and use of drains were recorded and compared. A total of 163 patients were included in the study. Of these, 57 patients (35.0%) underwent TAPP, while 106 patients (65.0%) underwent Lichtenstein repair. The average age of patients in the TAPP group was lower compared to those in the Lichtenstein group (p<0.001). Postoperative pain levels, operation duration, and length of hospital stay were higher in the TAPP group (p<0.001, p<0.001, p<0.001). There were no significant differences between the groups in terms of complications such as bleeding, seroma, or atelectasis. In the early postoperative period, there was no significant difference in the development of bleeding, seroma, or atelectasis between TAPP and Lichtenstein repairs for inguinal hernia. However, within the first 24 hours postoperatively, patients in the TAPP group experienced higher levels of pain and required longer hospital stays. We assess that the extended hospitalization observed in the TAPP group is more likely attributable to side effects of general anesthesia— such as nausea, vomiting, and malaise—rather than the surgical technique itself.</p
Investigation of sensory and motor functions of wrist in patients with rheumatoid arthritis
Background: Rheumatoid arthritis (RA) is a systemic and autoimmune disease that affects many tissues and organs in the body, especially the synovial joint and periarticular structures. One of the most affected joints is the hand joints, and there is also involvement in the wrist joint. Objectives: The purpose of this study was to assess the sensory and motor functions of the wrist with a gold standard method and to determine the relationship between the motor and sensory functions of the wrist and wrist-related pain/disability level and hand grip strength in patients with RA. Method: Flexor and extensor muscle strength and joint position sense of the wrist were evaluated using isokinetic dynamometer, hand grip strength was measured using dynamometer, and wrist related pain and disability was assessed by Patient Rated Wrist Evaluation (PRWE) questionnaire. Results: Thirty-one patients with RA and 31 healthy controls were included in the study. Control group had better results regarding wrist flexor and extensor muscle strength and wrist joint position sense than RA patients for both hands/wrists. A positive and strong correlation and a negative and weak correlation were found between muscle strength and joint position sense of the wrist and hand grip strength, respectively. Conclusions: Compared to healthy subjects, RA patients showed wrist-related sensorimotor deficits. Therefore, evaluation of the wrist should not be ignored in the examination of patients with RA. In addition, when planning the treatment program of the patients, not only hand exercises but also exercises involving the wrist should be taken into consideration
The relationship of the position of mandibular third molar impaction with the development of dental and periodontal lesions in adjacent second molars
Objective: The aim of this study was to evaluate the effect of different positions of Mandibular third molar impaction teeth on dental and periodontal lesions of mandibular second molars and correlate the lesion severity with patient age.Materials and methods: This retrospective study assessed 212 CBCT images of patients who visited the department of Oral and Maxillofacial radiology of XX university between 2017 and 2021. Periodontal and dental lesion effects of impacted third molars on mandibular second molars were evaluated. While dental lesions were classified as mild, moderate, and severe, periodontal lesions were classified as mild in cases not exceeding 2/3 of the root, and severe in cases exceeding 2/3 of the root. Pearson's chi-square tests, Mann-Whitney U or Kruskal-Wallis test were used for data analsis. A multivariate logistic regression model was constructed for further analysis.Results: Periodontal lesions were frequently observed in the second molar adjacent to the third molar (157 teeth, 74.1%), dental lesions were observed in 10%. Among the third molar positions, the mesioangular position was associated with a high (88%) prevalence of periodontal lesions (p<0.001), followed by the horizontal position (71%). The periodontal lesion was more severe in cases 30-39 years old and over 40 years old than in cases under 20 years old and 21-24 years old. Distoangular, horizontal and vertical/inverse positions are less likely to increase the severity of the periodontal lesion than the mesioangular position. (OR = 0.02; p <0.001, OR = 0.28; p =0.008, OR = 0.14; p <0.001,respectively).Conclusions: The third molars are often impacted in the mesioangular position, and this position most increases the risk of periodontal infection in the second molars. Accurate diagnosis and evaluation can minimize complications, and CBCT is a reliable imaging technique in which the position of the third molars and their relationship with the 2nd molars can be evaluated
Analyzing the EKC hypothesis for the top 10 energy-importing countries: a perspective for the COP27 targets
The environmental Kuznets curve (EKC) has been analyzed in many studies, but none of them has focused on countries with high energy imports. COP27 reiterated that fossil fuels are the main cause of carbon dioxide (CO2) emissions and highlighted the importance of net-zero targets for CO2 reduction by 2050. Analyzing the determinants of CO2 emissions in countries that import a lot of fossil energy is an important issue for achieving the net-zero targets within the scope of COP27. The study therefore empirically analyzes the effects of resource rents, energy prices, urbanization, and income on CO2 emissions. To this end, the study uses second-generation panel data analyses for the top 10 energy-importing countries for the period 1990–2020. The results suggest that (i) natural resources rent and urbanization increase CO2 emissions; (ii) an increase in energy prices helps to achieve carbon neutrality goals; (iii) GDP has a U-shaped link with CO2 emissions; and (iv) the EKC hypothesis is not valid for energy import-dependent countries. These findings suggest that energy import-dependent countries should strive for COP27 goals by adopting green urbanization policies, resource rent regulation laws, and energy price adjustments in domestic markets that accompany the transition to clean energy
The osteoinductive potential of different root-filling materials in a rat femur model
In pediatric dentistry, the enduring success of root-end filling materials employed for the establishment of apical plugs in immature teeth undergoing endodontic intervention is contingent upon their possessing a robust osteoinductive capacity. Hence, the primary objective of this study was to histologically assess the osteoinductive potential of four distinct dental materials, specifically recommended for retrograde filling applications, utilizing an animal hard tissue model. Within the designed experimental model, two bone defects measuring 2 mm in diameter and 2 mm in depth were created in both femurs of a cohort comprising 21 male Wistar albino rats. The first defect in the right femur was left blank as the control group, and Neo MTA Plus was placed in the second defect. The EndoSequence BCRRM Fast Set Putty was placed in the first of the defects on the left femur, and Endo Repair was placed in the second defect. Subjects were sacrificed after 7, 14, and 28 days of follow-up, and sections were examined to assess the degree of inflammation, connective tissue formation, and new bone formation. The data were statistically evaluated with Kruskal‒Wallis and post hoc Dunn's tests using SPSS 12 software. The bone healing levels of the Neo MTA Plus group were significantly higher than those of the other groups in all periods (p < 0.05). Bone organization in all groups decreased over time, and fibrous tissue was enriched. The results of this study demonstrate that NeoMta Plus has superior osteoinductive properties compared to other materials but that EndoSequence and Endo Repair have the potential to be developed
