Clinical Outcomes Following the Use of Archived Proviral HIV-1 DNA Genotype to Guide Antiretroviral Therapy Adjustment.
Background. Evidence regarding the safety of using proviral HIV-1 DNA genotype (DNA GT) to guide antiretroviral therapy (ART) is limited. We hypothesized that HIV RNA would not increase following ART adjustment guided by DNA GT in a university HIV clinic. Methods. Data were obtained from electronic medical records of adult persons living with HIV-1 (PWH) who underwent DNA GT testing and changed ART between October 2014 and November 2017. Logistic regression was used to evaluate the effect of ART switch on HIV RNA over time. Results. Eighty-three PWH had DNA GT performed, 66 (80%) switched ART, and 59 had postswitch follow-up. Data were analyzed pre-/postswitch for these 59 PWH (median age, 54 years; 71% LWH ≥10 years; 46% ≥2 previous regimens; 36% recent low-level viremia; 34% unknown medication history). On DNA GT, 58% had ≥1-class ART resistance, 34% ≥2-class, and 10% 3-class. Median follow-up (range) was 337 (34-647) days. There was no change in probability of HIV RNA ≥50 copies/mL over time (P \u3e .05). At baseline, 76% had HIV RNA \u3c50 vs 88% at last postswitch follow-up (P = .092). Protease inhibitor use decreased from 58% to 24% (P \u3c .001). Average daily pills and dosing frequency decreased from 3.48 to 2.05 (P \u3c .001) and 1.39 to 1.09 (P \u3c .001), respectively; ART cost did not change. Conclusions. DNA GT facilitated changes in ART in a treatment-experienced population without increases in HIV RNA. Decreased pill burden occurred without increased ART cost. A Causal Comparative Analysis of the Academic Self-Efficacy of Black Male High School Students taught by a Black or White Male Teacher.
The airm of this study was to determine if there was a difference in the academic-self efficacy among Black male students taught by a Black Male teacher compared to the academic self-efficacy of Black male students that are taught by a Whilte male teacher. Academically, Black male students lag behind their peers in academic achievement indicators such as grade point average, standardized test scores, and high school graduation rates (Schottt Report, 2015). Existing literature regarding Black male academic achievement focuses on exploring the academic achievement gap that exists, but little to no research investigates how to close that gap. Using Albert Bandura\u27s (1977) academic self-efficacy theory as a theoretical framework, this study investigated whether or not Black male teachers have an impact on the academic self-efficacy of Black male students by comparing the academic self-efficacy of Black male students taught by a Black male teacher compared to the academic self-efficacy of Black male students taught by a White male teacher.
The study was guided by the following research question:
Is there a difference in the academic self-efficacy of Black male students taught by a Black male teacher compared to the academic self-efficacy of Black male students taught by a White male teacher.
To answer the question, a quantitative causal comparative research study was employed using an adapted Academic Self-Efficacy Scale (Gafor & Ashraf, 2006). Findings resulting from this study is significant, as it aims to serve as a platform for future research on methods to close the gap in academic achievement for Black male students
Colorblind Racism and Identity Amongst College Students
The purpose of this to study is to observe the changes in Black Identity and Colorblind Racism amongst black students following the murders of George Floyd, Breonna Taylor and Ahmaud Arbery. In 2019 I conducted an educational study that examined the relationship between Black Identity and Racial Colorblindness amongst college students within the psychology department. Although the purpose of that study was to determine a correlation between Black Identity and Racial Colorblindness, the data indicated that a substantial number of students scored high in Colorblind Racism and low on Black Identity. Best Practices: The Relationship Between School Counselors\u27 Self-Efficacy And Conducting Suicide Risk Assessments
The purpose of this quantitative non-experimental cross-sectional study is to investigate the self-efficacy of middle and high school counselors in the state of Louisiana as it relates to their ability to conduct youth suicide risk assessments. Broadly, the study compares the Council for Accreditation of Counseling & Related Educational Programs (CACREP). CACREP accredited and non-accredited school counselors to determine if CACREP accreditation equips school counselors with higher self-efficacy. To this end, the study employs the Counselor Suicide Assessment Efficacy Survey (CSAES) scale to analyze degrees of self-efficacy in conjunction with a demographic survey of sample characteristics associated with educational preparation, training, and professional experiences of school counselors. This study will examine school counselor’s self-efficacy in order to provide data for practical and evidenced-based training in suicide intervention through existing CACREP accredited and non-CACREP accredited counseling programs. Results from the study may be used to improve the content of vi counselor education programs and better prepare school counselors with identifying suicide ideation, performing risk assessments, and overall, preventing suicidal acts by youth

Cardiac Collapse Secondary to Phenytoin Toxicity in a Neonate Treated with Extracorporeal Membrane Oxygenation Support (ECMO)
Introduction: Although medication toxicity is uncommon in neonates, there are several medications used in this population that pose a risk. Phenytoin has an increased risk of toxicity given its narrow therapeutic window and variations in drug elimination. Case Report: We describe the case of a 3-day-old male infant who developed cardiovascular collapse secondary to severe phenytoin toxicity (max phenytoin level 86 μg/mL) and was placed on extracorporeal membrane oxygenation support (ECMO). Several ancillary treatments were utilized in an attempt to decrease serum phenytoin concentrations and limit toxicity including albumin boluses, phenobarbital administration, intravenous lipid infusion, and folic acid supplementation. Discussion: Although uncommon, drug toxicity should be considered in patients with acute changes who are exposed to medications with potential toxicity. With elevated levels of phenytoin, the half-life can be prolonged resulting in longer exposure to elevated levels of the drug as seen in our patient. This case report highlights the importance of ECMO utilization for cardiac support in neonates with medication toxicity and other potential ancillary treatments to decrease serum phenytoin concentrations
