    Regulation in health and social care: Improving the quality of care?

    When people access health and social care services, they do so in the expectation that it will improve the quality of their lives. Where quality of care is affected, this can be a matter of life and death. However, a specific issue is how to ensure that the large volume of health and social care activities is of good quality, especially in the context of financial pressures, staffing pressure and workload pressures. This chapter analyses the functions and purposes of regulation to ensure good quality care. It examines the processes of regulation for health and social care to provide an understanding of how it works. This includes key bodies involved in the process of regulation. The chapter provides a critical analysis of the process of regulation to consider whether regulation can improve the quality of care

    Examining peer mentoring with women who experience multiple and complex disadvantage: a feminist ethical model of practice

    Despite the recognized strengths and benefits of peer mentoring as a process, this practice remains an underexplored area for women in communities who experience multiple and complex disadvantage linked to trauma. This article explores the perspectives of those who deliver and receive peer mentoring, with the aim of building a more comprehensive understanding of such practice in developing a model of peer mentoring based on feminist ethics of care. Three women’s organizations located in England participated in this study and in-depth, semi-structured interviews were undertaken with decision makers (n = 5), peer mentors (n = 11) and mentees (n = 8). This research is framed within feminist qualitative research where reflexive thematic analysis considered the findings from the data collected from 24 interviews (Braun & Clarke, 2022). The findings from this study identify that peer mentoring is a complex form of community level support that provides a welcome approach from the more formal, statutory, community provision many women with multiple and complex disadvantage experience. A feminist ethical model of delivery is devised within this article to be considered within women’s sector community practice. The elements of the model being: community strength, meeting expressed need and having clear feminist ethical foundations of trust and care

    Background and Objectives: As the life expectancy of the multiple sclerosis (MS) community increases, new innovations and understandings of what it is to age ‘well’ are needed. Building on a line of work exploring the meaning and experiences of aging with a disabling condition ,and showing how and why people aging with MS experience this phenomenon differently, this paper progresses aging and disability literature by (re)conceptualizing what ‘wellbeing’ means to people aging with MS, and how wellbeing may be enhanced or compromised. Research Design and Methods: Working with 40 persons with MS over the age of 60, we used a Heideggerian phenomenological framework to co-construct what wellbeing meant among persons aging with MS. Results: Emphasizing the importance of the ‘everdayness’ of wellbeing experiences, persons aging with MS discussed how wellbeing was related to ‘Doing, Being, and Becoming; the ability to do the things they wanted to do, be the person they wanted to be, and the autonomy, opportunity, and ability to do something or become someone different’. The ability to be, do or become was, however dependent on, ‘the power of people’, ‘sociocultural privilege’ and ‘writing one’s own health narrative’. Implications: These findings, that are contextualized within the socio-cultural boundaries of participants’ situations, can help support persons with MS, families and friends, caregivers, health care professionals and interventionists that are working towards enhancing the quality of life among persons aging with MS

    Anthocyanin-rich blackcurrant supplementation as a nutraceutical ergogenic aid for exercise performance and recovery: A narrative review

    Athletes and physically active individuals consume sports nutrition supplements to enhance competitive sport performance and exercise recovery. Polyphenols have emerged as a promising area of research with application for sport and exercise nutrition due to affecting physiological mechanisms for exercise performance and recovery. The anthocyanin is a polyphenol that can be abundantly present in dark-colored fruits, berries and vegetables. Anthocyanins and anthocyanin-induced metabolites will provide antioxidant and anti-inflammatory effects. The focus in this narrative review is on the observations with intake of anthocyanin-rich blackcurrant supplements on whole-body exercise performance and exercise recovery. This review included a total number of 17 studies with a randomized placebo-controlled cross-over design (10 studies on performance and 8 on recovery effects) and 1 with a randomized placebo-controlled parallel group design (recovery effects). Among the performance studies, 6 studies (60%) reported positive effects, 3 studies (30%) reported no significant effects and 1 study (10%) reported a mixed outcome. Among the recovery studies, 7 studies (78%) reported positive effects, 1 study (11%) reported no significant effects and 1 study (11%) reported a negative effect. Studies with intake of supplements made from New Zealand blackcurrants (dose: 1.8 to 3.2 mg·kg-1 and 105 to 315 mg of anthocyanins, acute to 7-day intake) have provided meaningful (but not always consistent) effects on continuous and intermittent exercise performance tasks (i.e. rowing, cycling and running) and markers for exercise recovery. A mechanistic understanding for the beneficial exercise effects of anthocyanins for athletes and physically active individuals is still limited. Future work requires a better understanding of the specific types of anthocyanins and anthocyanin-induced metabolites and their effects on altering cell function that can enhance exercise performance and recovery

    A head, heart and hands approach to collaborative mentoring

    Drawing on the ideas of Pestalozzi (1746–1827), a Swiss educator and social reformer, this chapter considers collaborative mentoring in settings, exploring three key areas woven together through a Head, Heart and Hands approach. We demonstrate how when engaging in the process of collaborative mentoring, which is underpinned by theoretical knowledge, mentors (experienced practitioners) and mentees (trainee practitioners) are not just using academic literature to make informed choices but also engaging in action research and contributing to communities of enquiry that have the potential to empower individuals and transform practice. At the heart of collaborative mentoring is the holistic relationship between the mentors and mentees. We consider the importance of nurturing trusting non-hierarchical and empathetic relationships that are also productive and developmental. For the ‘hands' aspect of the approach, we explore practical strategies that are essential for embedding new skills. This holistic approach to mentoring, rooted in pedagogical love, empowers and motivates both the mentor and mentee by embracing their full humanity and fostering lifelong learning
