Association between Hemodialysis Adequacy, Family Support, and Quality of Life in Chronic Renal Failure Patients
Background: Hemodialysis is one of renal replacement therapy for patients with chronic renal failure (CRF). The quality of hemodialysis is largely determined by the recommended dose of hemodialysis. Biological and psychological changes are often faced by patients undergoing hemodialysis, which can affect their quality of life. Family support is an important factor that serves as a support system for the patients to face the health problems. This study aimed to determine the association between hemodialysis adequacy, family support, and quality of life in chronic renal failure patients.Subjects and Method: This was an analitic observational study with cross sectional design. It was conducted at Kasih Ibu Hospital, Surakarta, Central Java. A total sample of 102 patients with chronic renal failure who underwent hemodialysis were selected for this study using random sampling. The dependent variable was quality of life. The independent variables were hemodialysis adequacy and family support. The quality of life was assessed using the WHOQoL questionnaire. The hemodialysis adequacy was measured by Ureum Reduction Rate (URR) formula. Family support was measured by family support questionnaire. The data were analyzed using Chi Square test, Mann Whitney test, and logistic regressionResults: Patients with chronic renal failure had better quality of life if they underwent adequate hemodialysis (OR= 5.34 95% CI= 2.20 to 12.98 p= 0.001) and received strong family support (OR= 7.74; 95% CI= 3.13 to 19.13 p= 0.001).Conclusion: Quality of life of the patients with chronic renal failure is determined by hemodialysis adequacy and family support.Keywords: chronic renal failure, hemodialysis, adequacy, family support, quality of lifeCorespondence: Leo Chandra Wisnu Pandu Winata. The Roles of Medical Doctor and Family on Patient Health Behavior in Controlling HbA1C Level Among Patients with Type 2 Diabetes Mellitus at Dr. Moewardi Hospital
Background: Diabetes Mellitus is one of non-communicable diseases with high prevalence of complication and mortality at productive age. Patients with type 2 DM need to control their blood glucose level to attain optimal blood glucose. Thereby, it is expected to prevent or minimize the incidence of complication. This study aimed to examine the roles of medical doctor and family on patient health behavior in controlling HbA1C level among patients with type 2 diabetes mellitus at Dr. Moewardi Hospital, Surakarta.Subjects and Methods: This was an analytic observational study with case control design. This study was conducted at Dr. Moewardi Hospital, Surakarta, from April to May, 2017. A sample of 50 patients with type 2 Diabetes Mellitus and 100 control group were selected by fixed disease sampling. The dependent variable was HbA1C blood level. The independent variables were roles of medical doctor, roles of family, treatment compliance, physical activity, diet, and duration of illness. HbA1C blood level was measured by clinical laboratory test. The independent variable was measured by a set of questionnaire. The data were analyzed by path analysis. Results: HbA1C blood level was affected by duration of illness (b= 0.04; SE= 0.01; p<0.001), diet (b=0.18; SE=0.09; p=0.044), and physical activity (b=0.16; SE=0.07; p=0.024). Diet was affected by the role of medical doctor (b= 0.02; SE= 0.07; p= 0.766), the role of family (b= 0.13; SE=0.07; p=0.082), duration of illness (b=0.01; SE=0.01; p=0.063), and treatment compliance (b=0.32; SE=0.11; p=0.002). Physical activity was affected by the role of medical doctor (b=0.17; SE=0.08; p=0.025), the role of family (b=0.21; SE=0.09; p=0.017), diet (b=0.27; SE=0.10; p=0.005) and duration of illness (b= 0.03; SE= 0.01; p< 0.001). Treatment compliance was affected by the role of medical doctor (b= 0.18; SE= 0.05; p<0.001) and the role of family (b=0.24; SE= 0.05; p< 0.001).Conclusion: HbA1C blood level was directly affected by duration of illness, diet, and physical activity. The Effectiveness of Chronic Disease Management Program in Blood Pressure Control among Hypertensive Patients
Background: The prevention and management of hypertension are major public health challenges for Indonesia and the rest of the world. If the rise in blood pressure with age could be prevented or diminished, much of hypertension, cardiovascular and renal disease, and stroke might be prevented. In Indonesia, the Social Security Administration Agency (BPJS) has administered a government program to control chronic diseases, including hypertension, namely Prolanis (chronic disease management program). This study aimed to determine the effectiveness of chronic disease management program in controlling systolic blood pressure in hypertensive patients.Subjects and Method: This was an analytic observational study with case control design. The study was conducted at Pratama Griya Husada Clinic 2 Tasikmadu, Karanganyar, Central Java, in April 2017. A sample of 120 hypertensive patients, consisting of 60 controlled hypertensive patients and 60 uncontrolled hypertension patients, were selected for this study using fixed disease sampling. The dependent variable was systolic blood pressure. The independent variable was chronic disease management program for systolic blood pressure control, consisting of health status monitoring, reminder, club activity, home visit, dietary education, and physical activity education. Data on diet were collected by Food Frequency Questionnaire (FFQ) based on the DASH eating plan. Physical activity data were collected by short-form International Physical Activity Questionnaire (IPAQ). The other data were collected using questionnaires. Multiple linear regression was used to analyze the data.Results: Blood pressure of hypertensive patients was reduced by chronic disease management program, including monitoring health status (ƅ= -6.34; 95% CI= -12.42 to -0.26; p= 0.041), reminder (b= -6.22; 95% CI = -13.30 to -0.87; p= 0.085), club activity (b= -3.46; 95% CI= -6.40 to -0.53; p= 0.021), home visit (b= -22.01; 95% CI= -41.85 to -2.17; p= 0.030), dietary education (b= -3.61; 95% CI= -6.66 to -0.57; p = 0.020), and physical activity education (b= -0.40; 95% CI= -0.79 to -0.01; p= 0.001).Conclusion: Chronic disease management program is effective to control blood pressure of hypertensive patients. The program’s components including monitoring health status, reminder, club activity, home visit, dietary education, and physical activity education, are effective in reducing blood pressure in hypertensive patients.Keywords: hypertension, chronic disease management programCorrespondence: Frieska Dyanneza. Antimicrobial Activity of Cladosporiumoxysporum Endophytic Fungus Extract Isolated From Aglaiaodorata Lour
Background: Endophytic fungi have economic potential as enzymes, medicines and biological control agents. Cladosporiumoxysporum endophytic fungi can be isolated from a plant named Aglaiaodorata Lour (Indonesian: Paca rCina). This plant can be found inPurwodadi Botanical Garden, Pasuruan, East Java. This study aimed to assess the antimicrobial activity from the ethyl acetate extract of the Cladosporiumoxysporum endophytic fungi against Escherichia coli, Staphylococcusaureus, and Candida albicans.Subjects and Method: This was a descriptive study. C. oxysporum was cultivated, extracted, and fractionated. The fractions then were applied to antimicrobial activity assay. Disc diffusion method was used in this project with 2 mg/disc concentration extracts.Results: Some fractions of ethyl acetate extracts C. oxysporum showed antimicrobial activity against all microbials tested. 6 of 13 fractions exhibited inhibition zone against S. aureusATCC 6538, E. coli ATCC 8739, and C. albicansATCC 10231. Anthrax Seroprevalence in Central Java, Indonesia
Background: Anthrax is a zoonotic disease that is caused by Bacillus Anthracis is transmitted to humans through infected animal. The transmission to humans occurs when there is a contact to animals or animal products contracting anthrax. Clinical skin manifestations and anthrax serum Ig G antibody can be used to diagnose infected anthrax animals. This study aimed to determine the prevalence of anthrax based on ELISA serum Ig G antibody and clinical skin manifestations occurring in patients with anthrax.Subjects and Method: This was a descriptive study with cross sectional design conducted in Sragen district, Central, Indonesia, in 2015. A sample of 101 patients infected with anthrax was examined based on clinical skin manifestations and anthrax serum Ig G antibody.Results: 39.6% of the sample was 21 to 40 years of age. 57.4% of the sample was female. 74% of the sample completed primary school. 21% worked as farmers. 30.5% of the sample who cooked and consumed meat showed positive Ig G. Test results showed serum Ig G antibody negative 50%, 15.8% and 33.7% borderline positive. Clinical manifestations in the skin as much as 11.9%, which is the eschar on all respondents and 92.8% showed positive Ig G. While 88.1% did not show any clinical signs of anthrax.Conclusion: The increase in serum antibody titer Ig G anthrax is not all respondents were exposed, in an area that otherwise outbreak of anthrax, which is only a third of all respondents, and when it comes up eschar will be followed by an increase in Ig G antibody titer.Keywords: cutaneous anthrax, Ig G antibody ELISA, escharCorrespondence: Dhani Redhon. Comparison of β-Hydroxylase Enzyme 11 Serum in Obese, Overweight, and Normoweight Young Men
Background: Previous studies showed that cardiovascular risk factor was increased in obese and overweight subjects. Obesity and cardiovascular risk factor are associated with hypothalamic-pituitary-adrenal (HPA) axis hyperactivity that causes hypercortisolism, cortisol level is associated with cardiovascular risk factor on obesity. 11 β-hydroxylase is an enzyme that involved in cortisol synthesis. The aim of this study was to investigate 11 β-Hydroxylase concentration in obesity, overweight, and normal weight young men.Subjects and Method: This was analytic-observational study using cross-sectional design. The study was conducted at HKBP Nommensen University, Medan. The study subjects included by 76 young men aged 18-28 years old, consisting of 25 obese subjects, 25 overweight, and 25 normoweight. The concentration of 11 β-Hydroxylase was evaluated in blood sample after 10 hours fasting. The data was analyzed bivariately.Results: Mean of 11 β-Hydroxylase concentration was 52.76 ± 44.27 in obese subjects, 70.16 ± 46.83 in overweight subjects, and 43.42 ± 27.75 in normoweight subjects. The 11 β-Hydroxylase concentration in overweight subjects was statistically higher than normoweight subjects (p = 0.007), but the 11 β-Hydroxylase concentration on obese subjects statistically was not different from normoweight subjects (p = 0.362).Conclusion: The 11 β-Hydroxylase concentration on overweight subject is higher than normoweight subject. There is no difference of 11 β-Hydroxylase concentration on obese and normoweight subject. The Effect of Depression on the Quality 0f Life of Patient with Cervical Cancer at Dr. Moewardi Hospital in Surakarta
Background: The share of cervical cancer of all types of cancer was 37.65% (9.113 cervical cases of the total 24,204 cancer cases) in Central Java in 2009. Cancer deteriorates physical, psychological, social, and spiritual states of the persons affected, especially in patients of terminal stage. Depression may interfere the quality of life of the affected. This study aimed to determine the effect depression on the quality life of patients with cervical cancer.Subjects and Method: This was an analytic observational study with cross sectional design. This study was conducted at Dr. Moewardi Hospital, Surakarta, from May to August 2014. A total of 36 patients with cervical cancer at Dr. Moewardi Hospital were selected for this study. The independent variables were age, education, employment status, duration of illness, and depression. The dependent variable was quality of life. The depression was measured by Beck Depression Inventory (BDI). The quality of life was measured by European Organization for Research and Treatment of Cancer (EORTC) QOQ-C30 and EORTC QOQ-CX24. The data were analyzed by logistic regression analysis.Results: After controlling for age, education, employment status, and duration of illness, depression showed strong effect (OR= 14.83; p= 0.005) on the quality of life of patients with cervical cancer. Patients with severe depression were 14.83 times more likely to have poor quality of life than those with moderate or mild depression.Conclusion: Depression affects the quality of life in patients with cervical cancer.
