    Clinical and Functional Outcomes of Patients Undertaking Posterior Cruciate Ligament (PCL) Reconstruction Using Standard and Posteromedial Portal Fixation

    Background: Currently handling patients with posterior cruciate ligament (PCL) injury is still controversial. Arthroscopic techniques can reduce surgical morbidity and allow for an earlier and aggressive rehabilitation program. This study aimed to evaluate the clinical and functional outcomes of patients with isolated PCL ruptures undergoing PCL reconstruction with PCL residual, tendon hamstring autopsies, standard portable interference fixation, and posteromedial.Subjects and Method: This was a retrospective cohort study conducted at Dr. Soeradji Tirtonegoro Hospital, Klaten, Central Java. A total sample of 6 patients with isolated PCL rupture was selected for this study. The independent variables were PCL reconstruction with PCL residual, autologous tendon hamstring, standard portal interference fixation, and posteromedial for 9 months (range 5-17 months). The dependent variable was clinical and functional outcomes, which were measured by the International Knee Documentation Committee (IKDC), Knee Society Score, Tigner Lysholm Knee rating scale, and a modified Cincinnati Assessment System Questionnaire. Scores of clinical and functional outcomes before and after surgery were tested by t-test.Results: The Tigner score after surgery (mean = 94.50; SD = 4.18) was higher than before surgery (mean = 63.17; SD = 12.48) with p = 0.001. The IKDC score after surgery (mean = 81.48; SD = 10.12) was higher than before surgery (mean = 55.73; SD = 18.72) with p = 0.014. Cincinnati modification score after operation (mean = 92.83; SD = 8.11) higher than before surgery (mean = 66.17; SD = 13.23) with p = 0.002. The Knee Society Score after surgery (mean= 82.16; SD= 4.91) was higher than before surgery (mean= 60.83; SD= 10.51) with p= 0.001. The percentage of patients with knee stability according to Lysholm score after surgery (6 patients, 100%) was higher than before surgery (3 patients, 50%).Conclusion: PCL reconstruction with PCL residual, autologous tendon hamstring, standard portal interference fixation, and posteromedial for 9 months can improve clinical and functional outcomes in patients with isolated PCL ruptureKeywords: posterior cruciate ligament, portal posteromedial, reconstructionCorrespondence: Paramita Dyah Lasmana.     The Effectiveness of Adding Probiotic to Antimicrobial Agents for the Treatment of Bacterial Vaginosis: A Systematic Review

    Background: Common antimicrobial regimens for treating bacterial vaginosis often cause recurrence and bacterial resistance. Previous studies have reported that a combination of antimicrobial and probiotic consisting of Lactobacillus spp. Is an effective treatment for recurrent bacterial vaginosis. Lactobacillus plays an important role in vaginal health by replacing the pathogenic colonies in vagina. This study aimed to determine the effectiveness of adding probiotic to antimicrobial therapeutic for the prevention of  bacterial vaginosis(BV) recurrence.Subjects and Method: A systematic review was conducted by searching the following databases: PubMed, Science Direct, Web of Science, Springer Link and the Cochrane. The review included randomized controlled trials (RCTs) conducted in primary hospitals and private clinics. The Amsel criteria and Nugent score were used for diagnosis appraisal of bacterial vaginosis. were included in this systematic review consisting of the two types of bacterial vaginosis treatment.  All pooled data analyses were based on random-effects models and intention to treat (ITT). Data were analyzed using Rev Man 5 software.Results: The review included 5 RCTs involving 692 women on reproductive age underwent treatment of BV for 5 days or more.The studies showed that combination of probiotic and antimicrobial treatment reduced the risk of bacterial vaginosis recurrence a half time compared to antimicrobial treatment alone(RR= 0.49; 95% CI= 0.17 to 1.44).Conclusion: Adding probiotic to antimicrobial regimens is more effective than antimicrobial regimens alone for treating bacterial vaginosis recurrence.Keywords: probiotic, antimicrobial, bacterial vaginosis recurrence, randomized controlled trialCorrespondence: Aquartuti Tri Darmayanti.     Correlation between Hemoglobin Levels and Muscular Endurance in Soccer Athletes

    Background: Muscular endurance is one of the important components of physical fitness in the sport of soccer player. Soccer athletes are required to contracting the muscle explosively as maximum as possible and must have high aerobic capability. This study aimed to determine the correlation between hemoglobin levels with muscular endurance in soccer athletes.Subjects and Method: This was an analytic observational study with a cross-sectional design. The population was soccer player athlete who was incorporated in soccer clubs and soccer schools at Medan, North Sumatera, Indonesia. A sample of 96 soccer athletes was selected for this study. The dependent variable was muscular endurance. The independent variables were hemoglobin levels. Hemoglobin level was measured by the cyanmethemoglobin method, and muscular endurance was measured by the push-up test. The data was analyzed using a Spearman test.Results: Hemoglobin levels were positively correlated with muscular endurance levels and it was statistically significant (r = 0.226; p<0.05).Conclusion: This study showed hemoglobin levels correlates with muscular endurance.Keywords: hemoglobin, muscular endurance, soccer  athletesCorrespondence: Nur Afni Heryanti Octavia.     Survival Predictors of People Living with HIV/ AIDS in Wamena, Papua

    Background: Quality of life (QoL) is an important component in the evaluation of the well-being of people living with HIV and AIDS (PLWHA), especially with the appreciable rise in the longevity of PLWHA. The analysis of the quality of life is essential for the healthcare policies and services directed toward PLWHA, since this indicator values the perception of people about their own life and health. However, little is known about the determinant of QoL in PLWHA in Papua. The aim of this study was to analyze survival predictors of people living with HIV/ AIDS in Wamena, Papua, Indonesia.Subjects and Method: This was a retrospective cohort study conducted in Wamena Hospital, Papua, from December 2017 to February 2018. A cohort of 304 PLWHA was selected for this study from the medical record at Wamena Hospital. The dependent variable was survival. The independent variables were adherence to ART, nutritional status, age, and coinfection. The data were collected from medical record and questionnaire. The data were analyzed by Cox regression model.Results: The risk of dying of PLWHA if adherent to take the ART was lower than not adherent to take the ART (HR=0.45; 95% CI= 0.06 to 0.33; p= 0.002).The risk of dying of PLWHA with poor nutritional status was higher than good nutritional status (HR= 12.78; 95% CI= 6.81 to 23.98; p< 0.001). The risk of dying of PLWHA aged ?35 years was slightly higher than <35 years (HR= 1.47; 95% CI= 1.00 to 2.17; p= 0.050). The risk of dying of PLWHA with coinfected was higher than without coinfected (HR= 1.59; 95% CI= 1.15 to 2.23; p= 0.006).Conclusion: The risk of dying of PLWHA decreases if adherent to take the ART, but increases with poor nutritional status, age ?35 years, and coinfection.Keywords: survival, adherence to treatment, people living with HIV/ AIDS.Correspondence

    Intraventricular Neurocysticercosis: A Case Report

    Background: Neurocysticercosis is the most common parasitic disease of nervous system in humans and the single most common cause of acquired epileptic seizures in the developing country. But in Indonesia, it was rarely reported. It has wide variety of clinical manifestation due to its pleomorphic nature.Subjects and Method: We reported a case of a 12 years old male with seizure and hydrocephalus due to intraventricular neurocysticercosis. A ventriculoperitoneal shunt (V-P shunt) performed to reduce the intracranial pressure due to hydrocephalus and followed by endoscopic surgery to taking sample of the neurocysticercosis and reducing the mass.Results: Pathological anatomy examination of excised mass found worm eggs. The patient received medications of albendazole and metil prednisolon.Conclusion: Intraventricular neurocysticercosis causing hydrocephalus is the commonest surgical indication, with V-P shunt placement followed by endoscopic surgery as the procedure of choice as it minimally invasive.Keyword: Intraventricular neurocysticercosis, hydrocephalus, headache, V-P shunt, endoscopic surgery.Correspondence

    Effect of Gestational Biological, Social, Economic Factors on Undernutrition in Infants 6-12 Months in Cilacap

    Background: Undernutrition in children under five can cause disorder in the physical growth, mental development, low intellegence, or even death. This study aimed to test the hypothesis that under nutrition in infants aged 6-12 months is associated with gestational biological, social, and economic factors.Subjects and Method: This was an analytic and observational study with case control design. This study was conducted in Cilacap, Central Java. A total of 144 infants aged 6-12 months were selected by fixed disease sampling, consisting of 72 undernutrition cases and 72 control. The dependent variable was undernutrition. The independent variables were birthweight, breastfeeding complementary food intake, exclusive breastfeeding, maternal education, paternal education, family income, MUAC at pregnancy, intra-household food allocation, and rural-urban residence. The data were collected by questionnaire and anthropometry. The data were analyzed by multiple logistic regression.Results: Birthweight <2500 gram (OR=8.12; 95% CI=0.92 to 71.64; p=0.059), inadequate intake of breastfeeding complementary food (OR=3.93; 95% CI=1.59 to 9.75; p=0.003), and non-exclusive breastfeeding (OR=2.46; 95% CI=1.13 to 5.36;  p=0.023) increased the risk of undernutrition in infants aged 6-12 months. Paternal education (OR=1.50; 95% CI=0.61 to 3.71; p=0.377), family income (OR=2.54; 95% CI= 1.03 to 6.28; p=0.43), MUAC (OR=1.32; 95% CI=0.512 to 3.38; p=0.562), intra-household food allocation (OR=1.84; 95% CI=0.85 to 4.02; p=0.123), maternal education (OR=0.78; 95% CI=0.31 to 1.97; p=0.599) and rural-urban residence (OR=0.82; 95% CI= 0.36 to 1.85; p=0.630) had non-significant effect on the risk of undernutrition in infants aged 6-12 months.Conclusion: Birthweight <2500 gram, inadequate intake of breastfeeding complementary food, and non-exclusive breastfeeding increase the risk of undernutrition in infants aged 6-12 months.     The Proportion of Human Immunodeficiency Virus Patients in Circumcised and Uncircumcised Men with Unsafe Sexual Practices in Special Care Center in General Hospital Center Haji Adam Malik Hospital Medan

    Background: The incidence of HIV was found to be higher in uncircumcised men compared to circumcised men. Circumcision was known to protect and fight against the acquired HIV infection. HIV infection was classified into sexual transmitted infections since it is mostly transmitted sexually (95%). The risk was higher in unsafe sexual practices, such as having multiple sexual partners and not using protections like condoms.Subjects and methods: This was a descriptive study with cross-sectional design study. There were 340 patients who came to the special care center in General Hospital Center Haji Adam Malik Hospital and participated in this study. Data such as history taking and clinical examination were then collected.Result: The proportion of HIV patients with unsafe sexual practices was lower in circumcised men group, which was 141 patients (41.5%), compared to uncircumcised men group which was 199 patients (58.5%).Conclusion: The proportion of HIV infection in uncircumcised men with unsafe sexual practices was higher than in circumcised men.Keywords: HIV, Circumcision, Sexual practiceCorespondence: Juliyanti.     The Influencial Factors on Counselling Effectiveness Using Decision-Making Tools in Selecting Long-Term Contraceptive Methods in Kendal District

    Background: One of the efforts to increase the usage of Long-term Contraceptive Methods is an effective counselling using decision-making tools. This counselling is very important to assist acceptors in making decisions and providing convenience to problem solving, behaviour changes or attitudes towards contraception. This study aimed to analyse the counselling effectiveness using decision-making tools in selecting methods of long-active reversible contraception.Subjects and Method: This was an analytic observational study with case-control design. This study was conducted at District of Kendal, Central Java, from May 18 to August 18, 2016. Population of this study was the entire contraceptive acceptors who were at district of Kendal. The samples were as much as 100 people, 35 contraceptive acceptors in case group and 65 in control group using quota sampling technique. The exogenous variables were counselling on Decision-Making Tools, self-efficacy, subjective norms, behaviour of women of reproductive age, ages of women of reproductive age, parity of women of reproductive age, levels of education of women of reproductive age. The endogenous variables were the usage of long-term contra­ceptive methods. The data were collected by a set of questionnaires and analyzed by path analysis.Results: Age≥35 years old (b=-2.19; CI 95%=-3.60 to -0.77; p=0.002), multiparity (b=-2.04; CI 95%=-3.99 to -0.09; p=0.040). Primary education level(b=-0.55; CI 95%=0.12 to 1.85; p= 0.359). There was a positive correlation between counselling and behaviour (b=0.98; CI 95%=0.12 to 1.85; p=0.025). There was a correlation between counselling and efficacy (b=1.56; CI 95%=0.62 to 2.50; p= 0.001). There was a correlation between behaviour and participation in the long-term contraceptive methods (b=4.02; CI 95%=1.71 to 6.34;p=0.001). There was a correlation between self-efficacy and participation in the long-term contraceptive methods (b=3.23; CI 95%=0.71 to 5.75; p=0.012). There was a correlation between subjective norms and participation in the long-term contraceptive methods (b=3.25; CI 95%=0.92 to 5.59; p=0.006).Conclusion: Counselling using decision-making tools influences behaviour and self-efficacy in using the long-term contraceptive methods indirectly.
