    Obesity and Diabetes Mellitus as Predictors of Mortality in Patients with Stroke: A Meta Analysis

    Background: Stroke is the leading cause of death and disability throughout the world. Based on studies in the United States (US), a person experiences a stroke every 40 seconds and every 4 minutes someone dies. The problem of comorbidity or the presence of more than one disease is the risk of stroke. This causes a person with comorbidities to have a higher risk of mortality than stroke patients without comorbidities. This study aims to examine the effect of obesity and diabetes mellitus as predictors of mortality in stroke patients.Subjects and Method: This was a meta-analysis and systematic review. The articles were obtained from Google Scholar, PubMed, Springer Link, Clinical key, and ProQuest databases. The articles have been published from 2010-2019. Keywords to search for articles are as follows: "comorbidity and stroke", "comorbidity and mortality and stroke and cross sectional study", "comorbidity and mortality and stroke and adjusted odd ratio", "comorbidity or stroke", "comorbi­dity or mortality or stroke or cross sectional study ", "comorbidity or mortality or stroke or adjusted odd ratio". The articles studied are full text articles with observational study design. Articles are collected using PRISMA diagram, and analyzed using the Review Manager application 5.3.Results: Obesity (aOR = 1.51; 95% CI = 1.03 to 2.21; p = 0.030) and diabetes mellitus (aOR = 1.75; 95% CI = 1.33 to 2.29; p <0.001) are predictors of mortality in stroke patients.Conclusion: Obesity and diabetes mellitus are predictors of mortality in stroke patients.Keywords: obesity, diabetes mellitus, strokeCorrespondence: Karlinda.     Determinants of the Quality of Life for Hemodialysis Patients

    Background: Chronic kidney disease (CKD) is a major cause of changes in activity, psychology and life. Analysis of the use of hemodialysis on quality of life was carried out as a treatment eva­lu­a­tion to maintain the survival of CKD patients. This study aimed to determine the determinants qu­a­­lity of life among he­modialysis patients.Subjects and Method: This was a cross-sectional study conducted at the Hospital Hemodialysis Unit Dr. Hardjono, Ponorogo, East Java, in April 2019. A total sample of 200 hemodialysis patients we­­re selected using simple random sampling. The dependent variable was the quality of life of he­mo­­dialysis patients. The independent variables were age, gender, education, type of financing, fa­mi­ly­ income, stress, frequency of hemodialysis, level of physical dependence, comorbidity, and so­cial group. The data of life quality were collected using the Kidney Disease Quality of Life (KDQoL) SF-36 questionnaire and analyzed using multiple logistic regressions.Results: The quality of life of hemodialysis patients was affected by age ≥ 50 years (b = -1.40; CI 95% = -2.42 to -0.36; p = 0.008), severe stress (b = -1.73; CI 95% = -2.91 to -0.54; p = 0.004), had co­­­morbidity (b = -1.25; CI 95% = -2.29 to -0.23; p = 0.017), male sex (b = 1.48; CI 95% = 0.41 to 2.54; p = 0.007), education high (b = 1.46; 95% CI = 0.43 to 2.50; p = 0.006), family income ≥ mi­ni­­mum wage (b = 1.98; 95% CI = 0.75 to 3.20; p = 0.002), PBI financing type (b = 1.69; 95% CI = 0.40 to 2.98; p = 0.010), frequency of hemodialysis often (b = 1.21; 95% CI = 0.20 to 2.22; p = 0.019), independent physical dependence level (b = 1.41; 95% CI = 0.33 to 2.49 ; p = 0.011), and has a social group (b = 3.28; 95% CI = 1.12 to 9.65; p = 0.031).Conclusions: Age, gender, education, type of financing, family income, stress, frequency of he­mo­di­a­lysis, level of physical dependence, comorbidity, and social group affect the quality of life of he­mo­­dialysis patients.Keywords: Chronic kidney disease, quality of life, hemodialysis, social group.Correspondence: Danies Tunjung Pratiwi.     Bilateral Simultaneous Rupture of the Quadriceps Tendon in a Healthy Elderly Woman: A Case Report

    Background: Quadriceps tendon ruptures are uncommon injury that affects middle-aged women, predominantly it affects middle aged men. However, the bilateral quadriceps tendon rupture is extremely rare in the absence of systemic disease.Subjects and Method: We reported a case of bilateral quadriceps tendon rupture in a healthy 65 year old female. She felt pain in both knees since 4 months before entering the hospital. Local physical examination on both knees reveal the loss of normal contour of rectus femoris, tenderness at superior pole patella the ruptur site, and palpable defect within 2 cm of superior pole patella. Patient unable to extend knee against resistance, and unable to perform straight leg raise. The patient was carried out a surgical repair with consent.Results: After the surgery both knees were immobilized with knee extension splint for 6 weeks. Patient evaluation revealed no pain and progressive recovery of muscular strength after four months and could be able to walk and returned to normal activity.Conclusion: Surgery is needed to repair quadriceps rupture.Keywords: Quadriceps tendon, quadriceps rupture, repair tendon.Correspondence: Pamudji Utomo.     Metformin/Glimepiride and Metformin/Glibenclamide, Which is Better?: A Systematic Review and Meta-Analysis

    Background: Type 2 diabetes mellitus (DM) is associated with the microvascular and macrovascular complication. Metformin and sulphonylurea (glimepiride and glibenclamide) combination is widely used for the treatment of type 2 DM. This study aimed to examine the difference of Metformin/Glimepiride and Metformin/Glibenclamide administrations in reducing HbA1C among type 2 DM patients.Subjects and Method: This was a systematic review and meta-analysis according to PRISMA guideline with PICO construction using MeSH and text-word. 214 article were identified from PubMed, Cochrane, other source databases. Two articles with 183 type 2 DM  patients were selected for this study.Results: No significant difference on HbA1C level, fasting plasma glucose, and hypoglycemia adverse events between glimepiride/metformin and glibenclamide/metformin combinations. However, glimepiride/metformin combination demonstrated lower HbA1C (-0.11; 95% CI= -0.41 to 0.18; p= 0.450) and lower hypoglycemia adverse events (OR= 0.52; 95% CI= -1.02 to 3.05; p= 0.450), while glibenclamide/metformin combination demonstrated lower fasting plasma glucose concentration (1.01; 95% CI= -1.02 to 3.05; p= 0.450).Conclusion: Glimepiride/metformin combination is preferable in HbA1C lowering and hypoglycemia risk than glibenclamide/metformin combination.Keywords: Glimepiride-metformin, Glibenclamide-metformin, type 2 diabetes mellitusCorrespondence: Yedy Purwandi Sukmawan.     The Effectiveness of Goat Milk to Increase the Volume of Breast Milk and Protein Content among Lactating Women: Randomized Controlled Trial Evidence

    Background: Breast milk is a natural food to fulfill the nutritional needs of infants during the first 6 months of life. Nutrition is one of the factors that influence milk production. Protein is one of the most important nutrient factors needed by nursing mothers to increase milk production. This study aimed to analyze the effectiveness of giving goat milk to increase breast milk production.Subject and Method: A randomized controlled trial (RCT) was conducted in Madiun, East Java, in October 2019. A sample of 100 lactating mothers who had infants aged 1-3 months was selected by purposive sampling and divided into 2 groups. The dependent variables were an increase in milk production and protein intake. The independent variable was the effectiveness of giving goat milk. The data were collected by observation and 24-hour food recall sheets. The data were analyzed by t test run on Stata 13 program.Results: After having goat milk, the average of breast milk production in the intervention group (Mean= 121.5; SD= 12.81) was higher than in the control group (Mean= 97.06; SD= 13.59), (ES= 0.81; p<0.001). Protein intake after having goat milk was higher in the intervention group (Mean= 92.60; SD= 4.54) than control group (Mean 83.36; SD= 4.26), (ES= 2.10; p < 0.001).Conclusion: Goat milk is effective for increasing milk production and protein intake in lactating mothers.Keywords: goat milk, breast milk, proteinCorrespondence: Prita Alvina Reviana, Masters Program in Public Health, Universitas Sebelas Maret, Jl.     The Effect of Holistic Programs on Cortisol Serum Levels in Polycistic Ovarium Syndrome

    Background: The increasing prevalence of polycystic ovary syndrome (PCOS) in Indonesia reaches 8-10%. Patients with polycystic ovary syndrome are at risk of experiencing complex psychological disorders and decreased well-being. Psychological disorders in polycystic ovary syndrome are at risk of developing physical symptoms and increasing insulin resistance. The Holistic Program consists of a combination of diet yoga and remembrance therapy as an integrated holistic therapy. There is no non-pharmacological therapy that is holistically integrated to reduce serum cortisol levels in polycystic ovary syndrome. This study aimed to examine the effect of holistic programs on cortisol serum levels in polycistic ovarium syndrome.Subjects and Method: This was a randomized controlled trials conducted in Yogyakarta. A sample of 30 women patients with PCOS aged 19-40 years old was selected for this study. The sample was divided into 2 groups. The intervention group received holistic program for 14 days. The dependent variable was cortisol serum. The independent variable was holistic program. Cortisol serum and insulin levels were measure by ELISA method at the Integrated Research and Testing Laboratory, Universitas Gadjah Mada, Yogyakarta. Insulin resistance was calculated using the HOMA IR method. The data were analyzed by independent t-test and different test Paired t-testResults: Serum cortisol levels in the intervention group were lower than the control group.     The Effect of Lactapuncture Massage on Breast Milk Production through Prolactin Hormone Levels Changes in Dr. M. Ashari Hospital, Pemalang, Central Java

    Background: Problems that inhibit breastfeeding in the first week include less breast milk production. Increasing breast milk production can be done by stimulating or massage the breast. Breast massage can be done by lactapuncture massage. This study aimed to examine the effect of lactapuncture massage on prolactin hormone levels and breast milk production among primiparous mothers.Subjects and Method: This was a quasy experiment study conducted at Dr. M. Ashari hospital, Pemalang, Central Java, Indonesia. A sample of 32 primiparous postpartum mothers was selected by simple random sampling. The dependent variable was breast milk production. The independent variables were lactapuncture massage for 7 consecutive days. The data were analyzed by t-test.Results: Prolactin hormone level in intervention group (mean= 313.80; SD= 45.54) was higher than in control group (mean= 292.09; SD= 97.87) but it was statistically non-significant (p= 0.428). Breast milk production in intervention group was higher (mean= 9.36; SD= 0.71) than in control group (mean= 7.39; SD= 0.23) and it was statistically significant (p<0.001).Conclusion: The lactapuncture massage on 2-3 times clockwise at each point for 7 consecutive days increasing prolactin hormone levels and on breast milk production.Keywords: lactapuncture massage, prolactin hormone, breast milk productionCorrespondence: Siti Patimah.     Correlation between Schizas Score, Degree of Disability, and Neurogenic Claudication in Lumbar Spinal Stenosis

    Background: Magnetic Ressonance Imaging (MRI) is one of modality to diagnosed lumbar spinal stenosis (LSS) however there were some mismatch between the degree of canal stenosis to functional score of LSS. There are some methods to measure the degree of canal stenosis, on of them is Schizas Score. To asses functional score of patients with LSS, there are some tools can be used. Oswesstry Disability Index (ODI) and Neurogenic Claudication Outcome Score (NCOS) are questionnaire commonly used in LSS. This study aimed to analyze the correlation between the degree of stenosis grading using Schizas Score to the degree of disability using ODI score and Neurogenic Claudication using NCOS.Subjects and Method: This was an analytic observational study conducted at Prof. Dr. R. Soeharso Orthopaedic Hospital, Surakarta, Central Java. A sample of 35 patients with LSS in were assessed by filling ODI and NCOS questionnaire, then evaluate the lumbar axial MRI to identify the Schizas Score. The data were analyzed by Spearman test.Results: There was a significant correlation between Schizas Score to ODI score and NCOS. Schizas score increased ODI score (r= 0.91; p<0.001).     Relationship between Body Mass Index, Age, and Muscular Endurance among Soccer Players in Medan, North Sumatra

    Background: Maintaining body mass index is one of the social problems, especially for young athletes. This study aimed to determine the relationship of body mass index (BMI) and age to muscular endurance in soccer athletes in Medan.Subjects and Method: This was a cross-sectional study soccer clubs in Medan, North Sumatera, Indonesia, in April 2018. A sample of 96 soccer players was selected for this study. The dependent variable was muscular endurance. The independent variables were BMI and age. Body weight was measured by weight scale. Body height was measured by microtoise. The other data were collected by questionnaire. Muscle endurance was assessed by 1 minute push up test. The data were analyzed by Person product moment.Results: BMI was positively correlated with muscle endurance but it was not statistically significant (r = 0.09; p = 0.390). Age was positively correlated with muscle endurance and it was statistically significant (r= 0.62; p= 0.001).Conclusion: BMI and age are positively correlated with muscle endurance among soccer players.Keywords: BMI, muscle endurance, soccer athleteCorrespondence: Dewi Maya Sarah.     Factors Associated with Diabetic Peripheral Neuropathy among Patients with Type 2 Diabetes Mellitus in Surakarta, Central Java

    Background: Peripheral neuropathy is the most common chronic complication and often occurs in diabetes mellitus. Peripheral neuropathy can cause morbidity, decreased quality of life,and mortality. This study aimed to analyze the determinants of peripheral neuropathy in patients with type 2 diabetes mellitus in Surakarta, Central Java.Subjects and Method: This study was an observational analytic with cross sectional design. This study was conducted at Surakarta Regional Hospital, Central Java on July 2019. A total of 200 type 2 diabetes mellitus outpatients was selected by simple random sampling. The dependent variable was peripheral neuropathy. The independent variables were age, education level, income, self-efficacy, diet, medication adherence, physical activity, health literacy, duration of illness, and fasting blood sugar. Peripheral neuropathy was measured using the Michigan Neu-black Screening Instrument (MNSI). Other variable data were obtained using questionnaires. Analysis of data used multiple logistic regression.Results: The risk of peripheral neuropathy increased with age of  ≥55 years (b = 3.61; 95% CI = 1.51 to 5.70; p= 0.001), duration of illness ≥ 5 years (b = 2.78; 95% CI = 0.51 to 5.04; p = 0.016 ), and fasting sugar ≥110 mg / dL (b= 1.64; 95% CI= -0.01 to 3.03; p= 0.052). The risk of peripheral neuropathy decreased with education level ≥senior high school (b= -2.41; 95% CI = -4.31 to -0.51; p= 0.013), income ≥ Rp 1,800,000 (b= -1.58; 95% CI= -3.12 to -0.04; p = 0.044), high self-efficacy (b= -1.67; 95% CI= -3.26 to -0.07; p= 0.040), controlled diet (b= -1.89; 95% CI= -3.64 to -0.14; p= 0.034), high medication adherence (b= -2.34; 95% CI= -4.13 to -0.56; p= 0.010), high physical activity (b= -2.33; 95% CI= -4.17 to -0.49; p = 0.013), and high health literacy (b= -1.52; 95% CI = -3.18 to 0.14; p = 0.073).Conclusion: The risk of peripheral neuropathy increases with age of ≥55 years, duration of illness ≥ 5 years, and fasting blood sugar ≥ 110 mg/dL. The risk of peripheral neuropathy decreases with a high education level, income of ≥1,800,000, high self-efficacy, controlled diet, high medication adherence, high physical activity, and high health literacy.Keywords: peripheral neuropathy, diabetesCorrespondence: Akhmad Azmiardi.
