    Differences in Post-Operative Complaints between Patients with and without Anatomical Variations of Ostiomeatal Complex Based on CT Scan of Chronic Rhinosinusitis at Dr. Moewardi Hospital

    Background: The ostiomeatal complex has various anomalous variants that can vary in type and number for each person. These ano­malies can contribute to the development of chronic rhinosinusitis and the accompanying symptoms. This study aims to determine the differences in post-treatment complaints between patients with and without anatomical variations of the ostiomeatal complex in chro­nic rhinosinusitis at RSUD Dr. Moewardi.Subjects and Method: A cross-sectional study was conducted in Dr. Moewardi hospital, Surakarta. A sample of 30 of chronic rhino­sinusitis patients who were subjected to a CT scan in the radiology department and surgery by an ENT-KL specialist at Dr. Moewardi, from January 2019 to June 2020, was selected for this study. The study variables were anatomical variations of ostimeatal complex and post-operative complaints. Difference of post-opera­tive complaints between chronic rhino­sinusitis patients with and without anatomical variations of ostiomeatal complex were described in frequency (n) and percent (%).Results: This study indicated that there is no difference in post-operative complaints between patients with and without anatomical variations of the ostiomeatal complex based on CT Scan of chronic rhinosinusitis (p= 0.856).Conclusion: There is no difference in post-operative complaints between patients with and without anatomical variations of the ostio­meatal complex based on CT Scans of chronic rhinosinusitis.Keywords: ostiomeatal complex, CT scan, post-operative complaintsCorrespondence: Yudo Duswanto.     Determinants of Stunting in Children Aged 12 to 60 Months in Kota Waingapu District, East Nusa Tenggara, Indonesia

    Background: Stunting is a problem that is often not recognized in the community because short stature is so common and often considered normal. In Indonesia, the incidence of stunting under five is a major nutritional problem, with an average prevalence from 2015-2017 of around 36.4%. This study aimed to investigate the determinants of stunting in children aged 12 months - 60 months in the Waingapu District.Subjects and Method: This was a cross-sectional study conducted at Posyandu, Waing­apu District, in December 2020. A total of 105 samples were selected based on inclusion and exclusion criteria using non-probability conse­cutive sampling. The dependent variable was stunting. The independent variables were exclusive breastfeeding, birth weight, house­hold income, maternal education, mater­nal nutritional status, and maternal age. The data were collected by questionnaire and analyzed by Chi-square.Results: The prevalence of stunting in children aged 12 to 60 months in the Waingapu District was 24.8%. The bivariate analysis elucidated that the incidence of stunting was significantly increased by poor maternal education (OR= 3.40; 95% CI= 1.29 to 8.97; p= 0.011), poor maternal nutritional status (OR= 3.06; 95% CI= 1.05 to 8.89; p= 0.034), and low household income (p = 0.002).Conclusion: Poor maternal education, poor maternal nutritional status, and low household income significantly increase the incidence of stunting.Keywords: stunting, determinant, nutritional status, childrenCorrespondence: Samuel Bungaran Partahi Saud Manalu.     Functional Outcome Evaluation of Grade III Open Tibial Fracture Treated by External Fixation as Definitive Treatment

    Background: Tibial shaft fractures including open tibial fractures grade III are one of the most common fractures of long bones. There are many methods of conservative and operative treatment, one of them is external fixation. External fixation is more common used temporary in polytraumatized patients with tibial shaft fractures. The study was undertaken to see if the patient can be treated with external fixation as the definitive treatment and evaluate the functional outcome after the treatment.Subjects and Method: A retrospective review of a prospectively-collected database was performed. Data was taken from the Orthopaedic Department of RSUD Dr. Moewardi Hospital patients’ database. The study included all patients who underwent grade III open tibial fracture treatment from May 2018 to May 2019. A total of 8 patients who were included in our study were a patient with open tibial fractures grade III planned for external fixaton as definitive treatment. They were evaluated radiographically and clinically to determine the union rate. The data were reported descriptively.Results: External fixator time ranged in this study around 240 days. In this study, there were a few patients whose progress were remain unknown due to loss of contact. Fractures studied 5 out of 8, no patient were union after 8 months of external fixator used. 4 out of 8 patients were reported non-union after 8 months based on their radiological examination (50%) and 1 out of 8 patients were reported has been performed Removal of External Fixation (ROEF) after 5 months of treatment. The rest of the 3 patients’ results were remain unknown.Conclusion: Open tibial fractures grade III  of the leg can be managed with use of external fixator as a definitive treatment.     The Effectiveness of Acupuncture in Insomnia Patients: Meta Analysis

    Background: Insomnia is a disease that has a negative impact on the health and well-being of the sufferer. Insomnia can occur in anyone, both men and women with various ages of vulnerability and in individuals with various socioeconomic backgrounds. This study aims to examine the effectiveness of acupuncture in insomnia patients.Subjects and Method: Meta-analysis was performed with PICO which is presented as follows: the population in this study was insomnia patients aged 16-85. The intervention is in the form of acupuncture therapy. The com­parison used was a sham/ placebo acupuncture. The outcome was insomnia. The meta-analysis study was applied to this study with electronic data sources: Clinical Key, Google Scholar, MEDLINE/ PubMed, Science Direct, Scopus. The article used is a full-text article with a randomized control trial (RCT) study design. There were 9 articles used in this study with a sample size of 649 people who were divided into two groups (372 people in the acupuncture group and 277 people in the sham / placebo acupuncture group). Articles were analyzed using the Review Manager 5.4 application. The results of this study aim to determine the Standardized Mean difference (SMD) and hete­rogeneity of the study sample.Results: There is high heterogeneity between one experiment and another (I2= 94%; p <0.001) so that what is used is the Random Effect Model (REM). The provision of acu­puncture therapy was able to reduce the degree of insomnia with the Standardized Mean Diffe­rent (SMD) of 0.98 compared to sham acu­puncture (SMD= -0.98; 95% CI= -1.71 to -0.25; p = 0.008).Conclusion: The provision of acupuncture therapy was able to reduce the degree of insomnia with the Standardized Mean Different (SMD) of 0.98 compared to sham acupuncture.Keywords: Acupuncture, InsomniaCorrespondence: Kurnia Eka Putri.     Meta-Analysis: The Effect of Diabetes Mellitus Comorbidity on the Risk of Death in Covid-19 Patients

    Background: Corona Virus Deases 2019 (COVID-19) is an infectious disease caused by the corona virus. COVID-19 is currently a challenge for diabetes patients. Diabetes mellit­us predisposes to a very severe disease course and increases the risk of death from COVID-19. This study aims to estimate the average mag­ni­tude of the effect of diabetes mellitus comor­bi­dity on the risk of death of COVID-19 patients.Subjects and Method: This was a systematic review and meta-analysis. This research was conducted using secondary data that will be conducted by searching and selecting data from the results of clinical trials conducted around the world. Search for articles through syste­matic and comprehensive databases from Pub­med, Google Scholar, Springer Link. The ana­lysis in this study was carried out using the Review Manager 5.3 software. The results of the analysis are the effect size, heterogeneity and then the study model. Variations between studies are used to determine the analysis model which is divided into two models, namely the fixed effect model (FEM) and the random effect model (REM).Results: Based on the results of the analysis, there was high heterogeneity between experi­ments (I2= 85%; p= 0.04), so that the Random Effects Model (REM) was used. COVID-19 patients with comorbid diabetes mellitus were 2.17 times more likely to die than patients without comorbid diabetes mellitus (aOR 2.17; 95% CI 1.04 to 4.54).Conclusion: Diabetes mellitus co-morbidity increases 2.17 times the risk of death in COVID-19 patients compared to patients without dia­betes mellitus comorbidity.Keywords: COVID-19, Comorbidity, Diabetes Mellitus, DeathCorrespondence: Utin Ilma Agni Kun'ain.     Hypnosis and Dyspareunia: A Meta Analysis

    Background: Sexual satisfaction refers to a person's pleasant feelings from the type of sexual relationship that forms an important part of one partner's impression of another. 75% of women experience sexual problems. Symptoms of sexual dysfunction occur in nearly half of the marriage period and 75% of couples need therapy. This study aims to analyze the effect of hypnosis on pain during sexual activity and the intensity of pain.Subjects and Method: This study was a meta-analysis and a systematic study. The arti­cles used were obtained from several elec­tronic databases including Pubmed, Science Direct, Springer Link, and Google Scholar. Articles were collected for 4 weeks. The keywords to search for articles were as follows: "non-medi­cal therapy", "non-medical care", "randomized control trial", "sexual disturbance", "sexual function", "sexual satisfaction". The articles studied were full text articles with an obser­vational research design. Articles were collected using PRISMA diagrams, and analyzed using the Review Manager 5.3 application.Results: The effects of hypnosis on pain during sexual activity (aOR= 3.55; 95% CI= 2.63 to 4.79; p = 0.001) and intensity of pain (aOR=  2.39; 95% CI= 1.96 to 2.82; p= 0.006).Conclusion: Hypnosis can reduce pain during sexual activity and the intensity of pain.Keywords: Hypnosis, sexual dysfunctionCorrespondence: Kumalasari.     Meta-Analysis: Effects of Obesity and Family History of Diabetes Mellitus on the Risk of Gestational Diabetes Mellitus

    Background: Gestational diabetes mellitus is a serious complication of pregnancy, this is due to impaired carbohydrate tolerance which results in increased blood sugar levels and was first noticed during pregnancy. Gestational dia­betes mellitus is a global problem in terms of its incidence and impact. Studies suggest that obe­sity and a family history of diabetes mellitus are important risk factors for gestational diabetes mellitus. This study aimed to analyze the influ­ence of obesity and family history of diabetes mellitus on the incidence of gestational dia­betes mellitus by meta-analysis.Subjects and Method: This was a systematic review and meta-analysis carried out by follow­ing the PRISMA flow diagram. The electronic database for article searches is as follows: Pub­Med, Science Direct, Springer Link, Scopus, and Google Scholar. The search for articles was carried out on articles published from 2010 to 2020. The keywords used were "gestational dia­betes mellitus AND obesity", "gestational dia­betes mellitus AND family history. The inclu­sion criteria were full text articles, used Eng­lish, cross-sectional study design, and results reported in adjusted odds ratio (aOR). P= Preg­nant mother. I= Obesity and family history of diabetes mellitus. C= No obesity and no family history of diabetes mellitus. O= gestational dia­betes mellitus. Articles that meet the require­ments are analyzed using the Revman 5.3.Results: Nine articles were reviewed in this study with a total of 11,657 subjects. The results of the meta-analysis showed that obesity incre­ased the risk of gestational diabetes mellitus 1.81 times compared with non-obese (aOR= 1.81; 95% CI= 1.53 to 2.15; p <0.001). Having a family history of diabetes mellitus increased the risk of gestational diabetes mellitus 2.08 times compared with those without a family history of diabetes mellitus (aOR= 2.08; 95% CI= 1.34 to 3.22; p <0.001).Conclusion: Obesity and a family history of diabetes mellitus increase the risk of gesta­tional diabetes mellitus.Keywords: obesity, family history of diabetes mellitus, gestational diabetes mellitusCorrespondence: Galuh Sitorukmi.     The Effect of Kinesio Taping on the Reduction of Pain in Patients with Carpal Tunnel Syndrome: Meta-Analysis

    Background: Carpal tunnel syndrome (CTS) is the most common occurrence of nerve com­pression. Some cases of CTS have chronic sym­p­toms. The most common complaint is "Noc­turnal Acroparesthesia", which is tingling pain at night. Physiotherapy interventions can be do­ne to relieve complaints of CTS one of which is the use of Kinesio Taping. This study aimed to analyze the effect of Kinesio taping on the re­d­uction of pain in patients with carpal tunnel syn­drome from some previous studies.Subjects and Method: This was a systematic and meta-analysis study. The study used the PICO model as follows Population=Patients with Carpal Tunnel Syndrome, Intervention= Gi­­ven Kinesio Taping, Comparison= Not Given Kinesio Taping, and Outcome=Reductionof Pa­in. The articles used in this study were obtained from several databases including PubMed, Sci­enceDirect, and Google Scholar. The articles we­re collected in a month. The keywords for sear­ching the articles were as follows: "kinesio taping" OR "kinesio tape" OR "pain" OR "carpal tunnel syndrome" AND "Randomized Control­led Trial". The articles included in this study we­re full-text articles with a randomized con­trol­led trial study design. The articles were collected using PRISMA flow diagrams and ana­lyzed using the Review Manager 5.3 ap­p­l­i­cation.Results: The total of seven articles reviewed in this meta-analysis study was from Turkey, Po­land, the Republic of Korea, Egypt, and the USA.     The Effect of Ear Acupuncture in Reducing Body Weight in Obesity Patients: A Meta-Analysis

    Background: Obesity is now no longer consi­dered a health problem in developed countries, but has also been faced by developing countries such as Indonesia. According to WHO, obesity worldwide has increased by more than two times from 1980. In 2016, there were 1.9 billion adults who were overweight and more than 600 million of them were obese or overweight. This study aims to analyze the effect of ear acu­puncture therapy on weight loss.Subjects and Method: This study was a systematic review and meta-analysis using the Randomized Controlled Trial design. The arti­cles used in this study were obtained from seve­ral databases including PubMed, Google Scholar, Springerlink, Hindawi, and Science­direct. The articles used in this study were those published from 2008-2020. The article search was carried out by considering the eli­gibility criteria defined using the PICO model. P: obese patients (BMI ≥25), I: ear acu­puncture therapy, C: no ear acupuncture thera­py O: weight loss. The keywords for searching articles are as follows: "auricular acupuncture", AND "obesity", "auricular acupuncture for obe­sity" and "acupuncture obesity randomized con­trolled trial". The articles included in this study are full text articles with a Randomized Con­trolled Trial. Articles were collected using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 7 articles were reviewed in this study. The meta-analysis showed the results that ear acupuncture therapy was effec­tive for weight loss in obese people (SMD= -0.74; 95% CI= -1.31 to-0.16;  p<0.001).     The Agreement of Immunoglobulin Gamma Release Assay (IGRA)/ T-SPOT Tuberculosis and Tuberculin Skin Test to Detect Latent TB Infection in Diabetes Mellitus Patients

    Background: WHO identifies diabetes mellitus (DM) as a neglected risk factor for tuberculosis (TB). Currently, there is no gold standard test for latent TB infection (LTBI). Centers for Disease Control and Prevention (CDC) recommends using Tuberculin Skin Test (TST) and Immunoglobulin Gamma Release Assay (IGRA) to diagnose LTBI. TST is an LTBI classic diagnostic tool that has low sensitivity and specificity. But it is still preferred in diagnosing LTBI due to its lower price, and more health facilities can perform the examination compared to IGRA (T-SPOT.TB). The study aimed to measure the agreement of TST and T-SPOT.TB testing in detecting LBTI in DM and the correlation of HbA1c with TST and T-SPOT.TB.Subjects and Method: Subjects were DM patients who underwent TST and T-SPOT.TB testing. If the results of TST and T-SPOT.TB was positive, the test would be continued with Xpert MTB/RIF microbiological testing. TST used PPD RT23 2TU. T-SPOT.TB was performed toward peripheral blood mononuclear cells. The degree of agreement between TST and T-SPOT.TB testing was calculated using the Test of Agreement (Kappa Cohen). The degree of correlation between the two variables was calculated by using Pearson correlation.Results: The selected 30 study subjects with DM undergoing antidiabetic therapy showed 6 (20%) detected LTBI and 24 (80%) without LTBI using TST and T-SPOT.TB test. There was a substantial agreement level between TST and T-SPOT.TB testing in detecting LTBI among diabetes mellitus patients undergoing anti-diabetic therapy with kappa value= 0.62 (p<0.001). HbA1c increased T-SPOT (r= 0.07; p= 0.716) and TST (r= 0.11; p= 0.956).Conclusion: TST testing may substitute T-SPOT.TB to detect LTBI among diabetes patients undergoing antidiabetic therapy.Keywords: latent tuberculosis infection, diabetes mellitus, TST, Immunoglobulin Gamma Release Assay (IGRA), T-SPOT.TBCorrespondence: Reviono.
