Fracture healing is a complex process that partially over-
laps successive stages, from inflammation stages to bone re-
modeling stage. Among other risk factors, the patient’s nu-
tritional status posses a major role in the fracture healing
process. (Meesters et al. , 2018) A few studies have de-
scribed the effect of malnutrition on fracture healing. Protein
malnutrition has been reported to endanger on quality of
fracture callus. Hughes et al. proposed a significant effect on
the protein intake of fracture healing in rats. Compared with
the low protein intake group, the high protein intake group
had increased muscle mass and bone mineral density in the
fracture callus observed during the 6−weeks healing period.
(Hughes et al., 2006)
Protein malnutrition, especially arginine−citrulline−nitric
oxide metabolism, can affect fracture healing through several
collagen − forming precursors, and affect biomolecular in-
flammation and local capillary growth through the nitric ox-
ide synthase cycle. (Meesters et al., 2020 ; Meesters et al.,
2018) The arginine − citrulline −nitric oxide, like essential
amino acids, has two important roles in the fracture healing
process. Firstly, the nitric oxide synthase (NOS) enzyme,
which is secreted in the arginine−citrulline−nitric oxide me-
tabolism, takes the role in one of the stages of the fracture
healing process. Secondly, the conversion of arginine into
ornithine by the arginase−1 enzyme is essential in bone re-
modeling. Through the formation of polyamines, ornithine is
a precursor for collagen synthesis, necessary for osteogenesis
(Fig. 1).
It has been reported that non−union or delayed union is
related to protein malnutrition especially insufficient ar-
ginine, citrulline, and ornithine levels. This is probably
caused due to an increasing amount of NOS enzyme in

chondrocytes and bone cells in normal fracture healing. On
the contrary, a decline in nitric oxide (NO) number sup-
pressing the callus formation through osteoclast − induced
bone resorption. This most likely triggers non−union devel-
opment. (Wijnands et al., 2012)
Under physiological conditions, the semi−essential amino
acid arginine is produced by the conversion of citrulline by
the cytoplasmic enzymes arginine succinate lyase and ar-
ginine succinate synthase. Arginine can be converted back to
citrulline by one of the nitric oxide synthase (NOS) en-
zymes. In the inflammatory phase of fracture healing, the
NOS 1 (inducible form ; iNOS) is active, mainly in the in-
tramembranous area along with the periosteum callus. In the
later stages of the healing process, constitutive and calcium−
dependent NOS 3 ( endothelial form ; eNOS ) and NOS 1
(neuronal form ; nNOS) are expressed in the lining of blood
vessels and in fibrous and cartilage tissues. During these
conversion processes, free radical NO is formed. NO stimu-
lates bone cells to regulate bone remodeling by affecting os-
teoblasts and osteoclasts, and on the other hand, affects vas-
cular reactivity on the fracture healing process. (Meesters et
al., 2020)
It has been shown that arginine-citrulline-nitric oxide me-
tabolism regulates critical signaling pathways in fracture
healing through in vitro and animal studies. However, hu-
man research is a major and interesting perspective for fur-
ther investigation. Although the bone healing metabolism
between rats and humans is nearly similar, there are small
differences in the biomechanical loading.

[Purpose] Due to the good sealing ability and biocompatibility, MTA cement is widely used for retrograde 
fillings and treatment of root perforations. MTA cement possess poor handling properties, especially when 
used as a retrograde filling because of prolonged setting time. The study aimed to improve mechanical, 
chemical and biological properties of MTA cement using strontium (Sr) nitrate as a mixing liquid.  
[Method] Commercial White TMR-MTA (Yamakin Ltd., Osaka, Japan) cement powder and distilled 
deionized water (DDW) were mixed at 20% water rate as a control group, and as experimental groups, powder 
mixed with 1, 5, 10, and 30% strontium nitrate solution. Mixtures were cast in the ring molds, and incubated 
with 95-100% humidity at 37°C, up to set. Initial (working time) and final (curing time) setting times were 
determined with a Vicat needle apparatus weighing at 400±2 gr. Compressive strength was measured using 
universal testing machine (AG-IS, SHIMADZU Corporation, Japan) at 1 and 7 days. Ions concentrations 
(ICP-OES Optima 5300 DV, PerkinElmer precisely, Shelton, USA) and pH value (pH/ion/cond meter F-55, 
Horiba, Japan) were measured after soaking in 5 ml of DDW in a sealed bottle at 37 °C for 1, 3 and 7 days. 
Cell proliferation was assessed with WST-1 assay at 24, 48, 72, and 96 hours in the cell culture with MTA 
extract divided into 6 groups (1, 10, 100, 1,000, and 10,000 ppm of Sr groups and the control, which was not 
contained Sr). Mineralization of MC3T3-E1 was evaluated with ALP staining and Alizarin Red S staining in 
the 1 – 1,000 ppm of Sr groups and the control. The statistical analysis was carried out with one-way ANOVA, 
adjusted by the post-hoc Tukey’s test at a significance level of 0.05. 
[Results and Discussions] The final setting time of the cement with all concentrations of Sr-MTA were 
significantly shorter than that of the control. Among all groups, the 30% Sr-MTA was cured in the shortest 
time. These results suggested that the Sr2+ accelerated setting reactions of MTA. The compressive strength of 
the 1 and 5% Sr-MTA significantly lower than (p<0.05) that of the control within 1 day. The 10 and 30% Sr-
MTA were comparable to the control throughout the 1 and 7 days of immersion in DDW. The pH of the test 
solutions prepared from the 1, 5, 10, and 30% Sr-MTA specimens at 1, 3 and 7 days, were around 11.0, similar 
to the pH of the solution prepared from MTA. The amount of Sr2+ and Ca2+ released from various 
concentrations of Sr-MTA immersed in DDW decreased with time. In the result of in vitro experiment, 1 – 
1,000 ppm of Sr hasn’t affected to the cell proliferation, but 10,000 ppm of Sr significantly decreased. However, 
the calcification of MC3T3-E1 cells decreased in 100 and 1,000 ppm of Sr within 4 days, increased in 21 days. 
The mineralized nodule formations in the 100 and 1,000 ppm of Sr, were relatively larger than that of the other 
groups, described in the Alizarin Red S staining images. 
[Conclusion] In conclusion, Sr successfully extended the working time and reduced the curing time without 
decreasing the compressive strength of MTA. The cement, was mixed with 30% of Sr(NO3)2 solution, was the 
most effective according to the results of the setting but it is limit of the safe dose to the cell, confirmed with 
the results of mineralization assay.

Abstract  
Introduction: 
Psychological stress is an important factor in the etiology of various oral diseases, such as 
burning mouth syndrome (BMS), atypical odontalgia, lichen planus, and recurrent aphthous 
stomatitis. The pharmacotherapeutic outcomes of psychotropic agents in BMS have been recently 
demonstrated (Paudel et al. Oral Diseases 24, 2020). The outcomes were found to vary among 
individuals, which may be due to differences in the psychological backgrounds and stress levels. 
Alterations in salivary proteins and the oral microflora as a result of chronic psychological stress 
might be associated with the etiopathogenesis of these diseases. However, there is little 
information on this association so far. Recently, it was demonstrated that chronic psychological 
stress elevates IL-1β levels in the saliva and submandibular glands of mouse (Paudel et al., 
Medical Molecular Morphology 53 (4), 2020). IL-1b might be involved in stress-related diseases 
and can act as a potential biomarker in response to chronic stress. Global analyses are required to 
fully understand the associations between stress and both salivary proteins and the oral 
microflora. Therefore, proteomic analysis of salivary proteins and metagenomic analyses of oral 
and intestinal microflora were carried out using a rat model of chronic restraint stress. 
 
Materials and Methods: 
A. Stress protocol and saliva collection 
Six-week-old Sprague Dawley rats were randomly divided into a control group and a stress 
group. The rats in the stress group (n = 10) were maintained under restraint stress for 1 month 
following an established protocol (Yun et al., Journal of Neurochemistry, 2010). Briefly, the rats 
were enclosed in a plastic tube with the ends covered by a wire mesh thereby restricting 
movement for 4 hours daily. After a month, behavior analysis (elevated plus maze test) was done 
to confirm the stress level. The mice were anesthetized with pentobarbital (50 mg/kg, 
intraperitoneal) and pilocarpine (5 mg/kg, intraperitoneal) was injected to stimulate the saliva, 
which was collected in a 1.5 ml centrifuge tube using a pipette. 
 
 ii 
B. Identification of altered proteins 
i. Gel electrophoresis and identification of protein spots 
Two-dimensional gel electrophoresis was performed for each sample to determine the 
alterations in the salivary proteins. The salivary proteins were focused into two dimensions: the 
isoelectric point with an IpG strip (Bio-Rad, USA) and the molecular weight with sodium 
dodecyl sulfate-polyacrylamide gel. The protein spots on the gels were recorded using the 
ProExpress 2D Proteomic Imaging System (PerkinElmer Inc.,USA). The expression levels of the 
protein spots were quantified using the Progenesis SameSpot software (Nonlinear Dynamics Ltd, 
UK), and differences in expression among samples were analyzed statistically using the analysis 
of variance (ANOVA test). A p-value of ＜0.05 was considered significant.  
 
ii. Mass spectrometry for protein identification 
The significantly altered spots were subjected to LTQ linear ion trap mass spectrometry 
(ThermoElectron, San Jose, USA) equipped with a nano-electrospray ion source (AMR, Japan) 
for protein identification. The Mascot software (v2.3.01; Matrix Science, UK) was used to search 
for peptide mass ion peaks against the SWISS-PROT database. 
 
iii. Western blotting for protein confirmation 
The candidate proteins were subjected to Western blotting for confirmation. Sodium 
dodecyl sulfate-polyacrylamide gel electrophoresis was performed in precast gels, which were 
then transferred onto polyvinylidene difluoride membranes. The membrane was then stained with 
Ponceau S stain for total protein normalization. The membrane was blocked with 5% skimmed 
milk and incubated with primary and secondary antibodies. 
 
C. Identification of altered oral and intestinal microflora 
i. Sample preparation and sequencing 
Oral swabs were collected using a polystyrene swab. Genomic DNA was extracted from 
the swabs using the DNeasy Blood and Tissue kit (Qiagen, Germany). Stool was collected from

the intestine and genomic DNA was extracted using the DNeasy Powersoil Kit (Qiagen). The 
DNA was then used for 16S metagenomic sequencing library preparation for the Illumina MiSeq 
System. The V3-V4 regions of the bacterial ribosomal RNA were targeted using a two-step 
polymerase chain reaction (PCR) process. 
 
ii. Data analysis 
  The obtained data were analyzed with Quantitative Insights into Microbial Ecology. 
Bacterial taxonomy, alpha diversity, and beta diversity were evaluated in the saliva and stool; 
significant differences in alpha diversity, beta diversity and relative abundances were assessed 
using the Kruskal-Wallis test, Permutational multivariate ANOVA and Analysis of Composition 
of Microbiomes (ANCOM), respectively. 
  
Results and Discussion: 
1. Stress markers 
The mean body weight of the stress group rats was significantly lower than that of the 
controls (p < 0.05). The number of entries and time spent in the open arm of the elevated plus 
maze were significantly lower in the stress group compared to the controls (p < 0.05). This 
confirmed that rats were sufficiently stressed after 1 month of the stress protocol.  
 
2. Alterations in the salivary proteins 
The Progenesis SameSpot software detected 33 significantly altered protein spots on the gel. 
The gels were further stained with a silver stain from which eight spots were visible and cut out 
for mass spectrometry. Proteins such as the BPI fold-containing family A member 2 (BPIFA2; 
2.6-fold change), von Ebner’s gland protein (VEGP: 1.9-fold change), ɑ-amylase (1.8-fold and 
1.5-fold change), carbonic anhydrase 6 (1.6-fold change), common salivary protein (1.7-fold 
change) and cystatin D (1.6-fold change) were identified. The two most significantly altered 
proteins, BPIFA2 and VEGP, were confirmed by Western blotting.  
 iv 
BPIFA2 is a parotid secretory protein known to colonize bacteria and Candida albicans in the 
oral cavity. Recently, it has been demonstrated to play a physiological role in maintaining the 
surface tension of the saliva. VEGP belongs to the Lipocalin family and is known to play a role in 
bitter taste perception. The results suggest that BPIFA2 and VEGP affect the physical properties 
of saliva, taste perception, and bacterial and fungal colonization under stress conditions. 
 
3. Alteration in oral and intestinal microflora 
Metagenomic analysis of the oral microbiome revealed significant reductions in alpha 
diversity (observed operational taxonomic units) in the stress group when compared to the 
controls (Kruskal-Wallis test; p < 0.05). Metagenomic analysis of the intestinal microbiome 
demonstrated results that were consistent with those of previous studies, thus validating our stress 
protocol. 
 
Conclusion: 
This study identified novel salivary proteins and reduced alpha diversity of the oral 
microbiome that was caused by psychological stress in an animal model. These findings may help 
identify potential biomarkers for psychological stress and aid in understanding the pathogenesis 
of stress-related oral diseases.

「北海道医療大学歯学雑誌」投稿規程（2019年12月1日現在）
1．投稿資格
著者は，原則として共著者を含め，本会会員に限る．
（臨時会員は除く）
2．生命倫理への配慮
1）臨床研究は，ヘルシンキ宣言の主旨にそったもの
で北海道医療大学の各学部・大学院研究科倫理委
員会の承諾を得たものとする．
2）人の遺伝子解析を含む場合は，本学の「ヒトゲム
・遺伝子解析研究の計画および実地に関する倫理
規程」に基づき，「ヒトゲノム・遺伝子解析研究
に関する倫理審査委員会」の審査を経て学長の許
可を得たものとする．
3）動物実験は，「北海道医療大学動物実験規程
（Regulations for the Care and Use of Laboratory
Animals in Health Sciences University of Hok-
kaido）」に基づき，「北海道医療大学動物実験委
員会（Animal Ethics and Research Committee）の
審査を経て，北海道医療大学長の承認を得たもの
とする．
なお，本学以外の研究機関で行われた研究について
は，該研究機関等の倫理委員会等で承認を得たもの
とする．
3．利益相反（COI）について
投稿する論文については，本学会の定める「研究等
の利益相反に関する指針」に基づき，投稿時に指定
の申告書を提出するとともに，利益相反の有無を本
文の最後もしくは謝辞の前に明記すること．企業な
どの経済的，人的，専門的な利益相反がある場合は
企業名を記載する．ない場合は，「申告すべきCOI
状態はない」と文言を記す．

4．論文の種類及び内容
1）論文の種類は，原著論文（Original），症例報告
（Clinical report），総説（Review），解説（Com-
ment），システマティックレビュー（Systematic
review），臨床統計，（Clinical statistical survey）と
する．
2）論文の内容は，他の刊行物に未発表のものに限る．
3）本誌はその他に，ミニレビュー，最近のトピック
ス，歯学情報，本学会講演抄録，学会関係記事，
学位論文などを掲載する．
5．査読および採否
1）投稿論文は，編集委員会および編集委員会の依頼
する専門家により査読される．
2）採否については，査読の結果に基づき編集委員会
が決定する．
6．投稿論文の作成
1）投稿論文は，投稿規程ならびに別に定める「投稿
の手引き」に準拠して作成すること．
2）投稿論文は，表紙，論文指導責任者（correspond-
ing author），およびその連絡先（住所，電話番
号，E−mailアドレス），チェックリストシート，
英文抄録（300語以内），本文，表，図および図表
説明文の順番にまとめる．
3）投稿原稿は，2部（正1部，コピー1部）とす
る．また，投稿原稿と著者プロフィールを電子フ
ァイルにて編集長宛にe−mailで提出する．
4）和文論文の本文については，原則として，緒論
（緒語），方法（材料および方法），結果，考察，
結論（結語），謝辞（必要な場合のみ），文献の順
に記載するものとする．
5）英文論文の本文については，原則として，Ab-
stract（300語以内）， Introduction，Materials and
Methods，Result，Discussion，Conclusion，Ac-
knowledgment（必要な場合のみ），Referencesの順
に記載するものとする．
6）投稿論文のヘッダーに右詰めで，名前，所属，さ
らに初稿なのか修正論文なのかがわかるように記
載する．
7）投稿時，著者全員が編集委員会に当該論文の共著
者である旨の承諾許可をメールで送信するものと
する．
7．最近のトピックスの作成
1）最近のトピックスは，投稿規程ならびに別に定め
る「投稿の手引き」に準拠して作成すること．
2）最近のトピックスは，作成した文書ファイル
（Microsoft Word）をe−mailでの添付文書として編
集委員会まで送付すること．

ファイル名：最近のトピックス，講座名，
著者名
3）最近のトピックスは，原則1トピックスにつき1
頁での掲載とする．
4）最近のトピックスは，全角文字1800字程度にまと

表を添える際は，以下の例に従って，片段サイズ
の図・表1つにつき本文の文字数を500文字程度
に削減すること．

8．投稿論文の校正
1）投稿論文に対する著者校正は原則として1回とす
る．
2）校正論文は，特別な事情がない限り一週間以内，
校正時間は48時間以内に返却するものとする．（返
却，連絡がない場合は，投稿を取り下げたものと
判断する）．
9．証明書等の発行
1）投稿原稿の受付日は，編集委員会に到着した日付
とする．
2）受理証明が必要な場合には，掲載が決定した後に

11．優秀論文賞
その年の本誌に掲載された原著論文の中から「最優
秀論文賞」（1編），「優秀論文賞」（2編）を選び表彰
する．
選考は編集委員会が行う．
12．著作権の帰属
本誌に掲載された著作物の著作権は北海道医療大学
歯学会に帰属する．本会はこれら著作物の全部また
は一部を，ネットワーク媒体を含む媒体に掲載・出
版することが出来る．ただし，論文の内容について
は，著者がすべての責任を負う．
13．著者のプロフィール（6．投稿論文作成 3）参照）
巻末に著者のプロフィールを記すので，著者のスナ
ップ写真と経歴を提出すること．

患者のプライバシー保護ならびに研究倫理に関する指針（2014年2月26日）
北海道医療大学歯学雑誌に掲載される症例報告等を含
む臨床研究論文では，患者のプライバシーを保護するた
め，以下の指針を遵守しなければならない．また，臨床
研究等においては，患者ならびに被験者の尊厳と人権に
配慮し，世界医師会によるヘルシンキ宣言と我が国が定
めた下記の指針ならびに法的規範を遵守しなければなら
ない．
1．患者のプライバシー保護に関する指針
1）氏名，カルテ番号，入院番号，イニシャル等，患
者個人の特定が可能となる情報は記載しない．
2）患者の住所は記載しない．ただし，疾患の発生場
所が病態等に影響する場合は，区域（県，市など）
までに限定して記載する．
3）診療日等の記載は，年月までとする．
4）診療科名と他の情報を照合することで患者が特定
され得る場合，診療科名は記載しない．
5）他施設でも診断・治療を受けている場合，その施
設名と所在地は記載しない．ただし，救急医療など
で搬送元の記載が不可欠の場合は，この限りではな
い．
6）顔写真には目隠しをする．
7）生検，剖検，画像情報などに含まれる番号等，症
例を特定できる情報は削除する．
8）以上の配慮をしても個人が特定される可能性のあ
る場合は，発表に関する同意を患者本人（または遺
族もしくは代理人，未成年者では保護者）から得
る．
9）前項の手続きが困難な場合は，筆頭著者または責
任著者（corresponding author）の所属する施設の倫
理委員会の承認を受ける．
