　今年も皆さまの研究成果を投稿いただき，原著論文３編，研究報告８編，資料５編を掲
載する学部学会誌第17巻１号の発行が出来ました．今年度（2020年度）は世界中がコロナ
禍の中にあり，多くの不条理と向き合うことも多くありました．
　その社会状況下において，フランスの作家であるアルベール・カミュが第二次世界大戦
まもない1947年に発表した小説『ペスト』が，70年後に生きる私たちに多くのことを教え
てくれるとして世界中で読まれていると言われています．
　この小説はさまざまな困難を抱える社会の中で生きる私たちに，社会にある不条理と向
き合い，誠実に前向きに生きること，冷静になること等を気づかせてくれる作品であると
評されています．
　カミュの小説同様に学会員の皆さまの研究が，社会に何らかの示唆を与えるものとなる
ことを願ってやみません．

Abstract : An increase in the number of foreign healthcare workers in Japan has made second language 
workplace communications more likely. This research investigated whether pre-service nurses would be 
willing to engage in English communication with foreign co-workers. It also surveyed the reasons for 
and impediments to those interactions given by the students. The results of this study will be of use to 
those involved in both healthcare education and hospital administration.

Regarding English Communication With Foreign Co-workers 
Introduction
 Faced with an aging population, along with a 
declining workforce, Japan is engaged upon a program of 
rapid expansion in the number of foreign healthcare workers it 
admits to the country. An increase in foreign employees will 
undoubtedly lead to linguistic challenges within the healthcare 
sector as first language (L1 ) Japanese speakers interact with 
those workers. This research focussed on L1  Japanese pre-
service healthcare workers studying in Japan. It surveyed the 
intentions of these students regarding second language (L2 ) 
use in the workplace and their reasons for or impediments to 
engaging in L2  interactions.
 Due to the designation by the Japanese government 
of nursing care as an area requiring proactive acceptance of 
visa applications (Immigration Services Agency of Japan, 
2019), there are plans to admit up to 60,000 foreign healthcare 
workers within the current 5-year period. Working towards 
the successful integration of these workers into the Japanese 
system will require considerable effort on both sides, and the 
role of linguistic interactions in assimilating those workers 
will be crucial. Therefore, the opinions towards L2  
communication among workers in this field need to be 
investigated in order to bring to light any potential issues. 
Information regarding these opinions, and the reasons for and 
impediments to L2  interactions perceived by pre-service 
nurses, will be useful for both those involved in educating 
healthcare workers and those administering the institutions 
they work at. 
Methods
 A questionnaire was administered to a group of 
second-year nursing students at a medical university in Japan. 
The questionnaire was given via the use of Google Forms to 
ease data collection and to reduce the risk of viral transmission 
during the COVID-19 pandemic. 
 Participants were asked about their intention to 
communicate in English with potential future co-workers 
using a six-point Likert scale ranging from strongly disagree 
to strongly agree. They were then asked to select all the 
reasons for and impediments to communicating with co-
workers in English that applied to them from four options. 
Then they also had the opportunity of adding other reasons 
and impediments in a free-form answer section.
 The items on the questionnaire were written in 
English before being translated into Japanese and checked by 
two L1  Japanese speakers. Participation in the study was 
completely voluntary. In total, 108 students provided 
responses. The results obtained are detailed below.

Results and Discussion
 The first set of results relate to the participants’ 
intentions regarding L2  communication with co-workers. 
When asked if they would speak in English with a coworker, 
if given the chance, the following results were obtained.

 Table 1  above shows that over 80 percent of the 
respondents agreed, to some extent, that they would 
communicate with a foreign co-worker in English if given the 
chance. These results are very encouraging and suggest that 
the pre-service nurses in this study are open to L2  
communication in the workplace. 
 Intention to communicate in an L2  has often been 
equated with the willingness to communicate (WTC) construct 
in second language acquisition research (e.g. MacIntyre, 
Dörnyei, Clément, & Noels, 1998). Previous studies have 
found significant correlations between WTC and actual 
communication behaviours in the Japanese context (e.g. 
Yashima, Zenuk-Nishide & Shimizu, 2004). 
 These results also indicate that the students in this 
study tend to not hold negative feelings towards L2  
communication with future co-workers. Under 20 percent of 
those surveyed expressed any disagreement whatsoever to the 
statement that they would speak to a co-worker in English if 
they had the chance. This suggests that, on the whole, most of 
the participants from this group are amenable to L2  
interactions in the workplace.   
 The second set of results detail the reasons why the 
respondents would undertake L2  communication with co-
workers. Participants were given four options within the 
questionnaire to select, they were also given the opportunity 
to input other reasons in a free-form answer section. The 
results for the four options are given below in Figure 1.
 As can be seen in the chart above, the most common 
reason selected for engaging in L2  communication with 
foreign co-workers was to improve their English ability. This 
tallies with previous studies carried out in this context that 
found that instrumental factors, such as skill development, are 
often a key motivator for Japanese students (e.g. Irie, 2003). 
The next most selected reason was to make friends with their 
foreign co-workers. This suggests a more integrative form of 
motivation, where the learner views language as a medium for 
connecting with other groups (Gardner, 2001). A majority of 
the respondents, 60 percent, selected looking cool as a reason 
to communicate. While just under half, 48 percent, considered 
that increasing their understand of foreign cultures was a 
reason they would choose to communicate in English with a 
foreign co-worker. 
 There were some interesting reasons proffered by 
the students in the free-form section of the questionnaire. A 
translated selection are listed below.

It's fun to talk to various people.
English is used all over the world, so it's useful.
I want to go beyond the ‘Japanese’ framework and 
speak with lots of people.
I don't have any English skills, but I think we can 
communicate as we speak. 
Being able to speak English allows me to know the 
way of thinking and customs of people from other 
countries. If I can speak English when I work as a 
nurse, I will be able to take care of not only 
Japanese people but also people from other 
countries. 
If I do not speak I will not be able to perform my 
work eff ectively.
 As can be seen above, most of the reasons given 
relate to either the usefulness of English as a lingua franca, or 
the necessity of it for carrying out their professional duties.

communication has often been mentioned as a reason for 
developing English language skills (Ushioda & Dörnyei, 
2012). In this study it is interesting to note how some 
participants extended this to include L2  communication as a 
skill that will improve their ability to carry out their nursing 
duties. 
 The next section will look at the results derived from 
the part of the questionnaire that surveyed the participants’ 
views on what would impede their L2  communication with 
co-workers. First of all,  Figure 2  below shows the 
impediments selected by the participants from the four options 
listed.  
 In contrast to the balanced results displayed within 
the reasons selected, the distribution of the impediment 
responses was much more pronounced. Almost all of the 
respondents, 90 percent, selected a lack of confi dence in their 
English abi l i ty  as  an impediment  to  ini t ia t ing L2  
communication with foreign co-workers. Previous research in 
similar contexts has also found low levels of self-perceived 
confi dence a hindrance to L2  communication (e.g. Tsuchiya, 
2006). A thorough discussion of the underlying factors that 
combine to create these low levels of confi dence is a complex 
topic beyond the scope of this study. However, on the basis of 
the limited data available in this case, it is clear that this is the 
overriding impediment to L2  communication, as self-assessed 
by the participants.
  After lack of confi dence, the second most selected 
impediment was that the respondent would feel embarrassed 
communicating in English with a co-worker. Embarrassment 
has also been found to be a factor in previous studies in the 
Japanese context (e.g. King, 2013).  However, only 20 percent 
of the participants in this study considered it to be an issue. 
Even fewer, 16 percent, said that they would find L2  
interactions with a foreign co-worker troubling. Finally, just 
one person selected the option that they were not interested in 
this type of communication. 
 The free-form section of the questionnaire again 
produced some thought-provoking responses regarding the 
impediments to L2  communication suggested by the 
participants. A translated selection is listed below.
Impediments to communicating in English with a 
foreign co-worker
I cannot express myself well in English.
My English is not good enough.
Even if I have the ability to read English on my own, 
I'm not confident in my ability to converse with 
native speakers without diffi  culty. I'm worried that 
people might think my English is not good enough.
I'm not sure my English will be understood.
I get anxious when I think that the English I speak 
may not be understood by the other person. I would 
leave it to someone who is better at English than 
me.
I'm not very good at English and can't speak it. But 
I really want to communicate with foreign people. 
I'm not good at English.
I think it's necessary and I'd like to speak it, but I 
don't have enough knowledge of English to do so.
I don't have any foreign acquaintances.
Foreigners are scary.
 Most of these comments refl ect the fact that a lack 
confidence was the most commonly selected impediment to 
English communication with foreign co-workers. The nursing 
students in this study tend to have a low self-perceived 
confi dence in their own language skills. It is also notable that 
they are concerned that, even if they should they attempt to 
converse in English, they will not be understood. Finally, it is 
noteworthy that some students expressed reservations towards 
L2  communication due to their lack of foreign acquaintances, 
or belief that foreigners are ‘scary’. Xenophobic comments 
such as the second one are somewhat troubling. However, 
only one participant out of over a hundred made such a 
remark, and it is also possible that it was made in a light-
hearted, jokey kind of way.

Conclusion
 This study was intended to provide a preliminary 
investigation of nursing students’ opinions regarding the L2  
interactions they may encounter with foreign co-workers in 
their professional careers. The results suggested that they were 
open to such interactions and would be willing to engage in 
them if given the opportunity. Reasons given for this 
willingness included the desire to improve their English 
ability, a keenness to make foreign friends, and the belief that 
doing so would make them more effective healthcare 
providers.
 Although the results and opinions were largely 
positive, impediments to L2  communication were also 
surveyed. By far the most common impediment suggested was 
the students’ self-perceived lack of confidence. This is an area 
that could benefit from some pedagogical interventions. 
However, fostering confidence in language learners is an 
intractable problem that is not easily solved.
 The relatively low number of participants limit the 
strength of the conclusions that can be drawn from this study. 
In the future, a larger study, containing groups of both pre-
service and in-service nurses, could provide a more in-depth 
investigation of the topic. Additionally, if foreign healthcare 
workers could also be added to the study, a comparison 
between the two groups would prove insightful.

Oral mucosal diseases with chronic courses present as ul-
cers, blister and erosions on the oral mucosa, of which cause
is occasionally unknown. These diseases of unknown etiol-
ogy are often associated with underlying psychological fac-
tors. The medical fields of skin and gastro−intestine diseases
associated with psychological factors are termed as psycho-
dermatology and psychogastroenterology, respectively (Jaf-
ferany M, 2007 ; Keefer et al., 2018). However, no specific
medical fields of oral mucosal diseases associated with psy-
chological factors had been proposed. Recently, the term
“psychostomatology” was proposed for the oral mucosal dis-
eases associated with psychological factors, since psychoso-
matic diagnosis and approaches were critically needed for
the patients (Abiko et al., 2021).
Recurrent aphthous stomatitis ( RAS ) and oral lichen
planus (OLP) were the two most common oral inflammatory
mucosal disease with unknown etiology. Psychological fac-
tors such as stress, anxiety and depression are often involved
in causing and progression of the oral inflammatory mucosal
diseases. Moreover, the symptoms of these diseases such as
spontaneous pain, discomfort and strangeness affects the
quality of life which may lead to additional psychological
distress. Therefore, the psychological states have to be ex-
amined in the oral inflammatory mucosal diseases, and psy-
chological approach is often needed for the care of those pa-
tients.
The psychological status of the patients with the oral mu-
cosal diseases can be determined using various psychologi-
cal assessment tools. The psychological questionaries such
as State−Trait Anxiety Inventory, Beck Anxiety/Depression
Inventory, Perceived Stress Scale, Neuroticism−Extraversion
−Openness Five−Factor Inventory, Hamilton Anxiety /De-
pression Scale, and many others have been used (Abiko et
al., 2021). Other biomarkers such as salivary cortisol, dehy-
droepiandrosterone and peroxidase have also been used but
have not been a reliable marker. The combination of psy-
chological assessment tools is recommended to identify the
psychological state in the patients with the oral mucosal dis-
eases.
Psychological approaches including hypnosis, cognitive
behavior therapy, relaxation therapy and psychotropic medi-
cations including anti−anxiety and antidepressant have been
commonly used in the fields of psychodermatology and psy-
chogastroenterology ( Jafferany M, 2007 ; Keefer et al. ,
2018). Although psychological statues of the oral mucosal
diseases have been evaluated by several different psycho-
logical assessment tools, a little information about the psy-
chological approach for the patients have been shown. A re-
laxation/imagery therapy for 30 min/week for 12 weeks sig-
nificantly reduced the frequency and recurrence of RAS
(Andrews & Hall, 1990). Combination of psychiatric drug
therapy and topical corticosteroids significantly improved in
reducing the size of OLP (Delavarian et al., 2010) An anti−
anxiety agent, benzodiazepine when combined with routine
treatment for OLP for 30−40 days showed significant im-
provement and healing of lesion than the routine treatment
alone (Guarneri et al., 2014). These findings suggest that
psychotherapies and psychotropic drugs may be applied for
the treatment of psychostomatology related diseases.
In conclusion, psychostomatology was proposed as an ap-
proach for oral inflammatory mucosal diseases. Further stud-
ies are needed to establish common psychosomatic ap-
proaches for the oral inflammatory mucosal diseases.

第1章 総 則
（名 称）
第1条 本会は北海道医療大学歯学会（The Dental Soci-
ety of Health Sciences University of Hokkaido）
と称する．
（目 的）
第2条 本会は北海道医療大学歯学部（以下本学部と略
す）を中心に，会員相互の緊密な協力により，
学術研究の推進・専門技術の錬磨を計り，歯学
の進歩・発展に寄与するとともに，会員の親睦
を図ることを目的とする．
第2章 会 員
（会 員）
第3条 本会は以下の会員よりなる．
1．正会員
歯学の研究に従事し，本会の目的に賛同す
る者，本学部教職員・大学院性・研究生・
臨床研究生・歯科臨床研修医・卒業生およ
び本学部元教育関係者で理事会の承認を得
た者．
2．名誉会員
本会の設立または発展に，特に功労のあっ
た者で，常任理事会が推挙し，理事会，評
議員会の議を経た者．なお，名誉会員には
名誉会員証を送るほか会員の権利を保有
し，年会費一切の費用を徴収しない．
3．準会員
歯学教育・診療関係者で理事会の承認を得
た者．
4．学生会員
本学部専門課程の学生で理事会の承認を得
た者．但し，学生会員は卒業後正会員に移
行するものとする．
5．賛助会員
本会の目的および事業に賛同し，協力・支
持する個人・団体等で，理事会の承認を得
た者．
（入 会）
第4条 本会に入会を希望する者は，所定の申し込み書
に必要事項を記入の上本会事務局に申し込むも
のとする．
（退 会）
第5条 会員で退会を希望する者は，速やかにその旨を
本会事務局に通知すること．ただし，納入済み
の会費の返還は行わない．
（会員資格喪失）
第6条 会員は以下の事由により資格を喪失する．
1．2年以上会費の未納．所在不明または連絡
のつかない者．
2．本会の名誉に反する言動のあった者につい
ては，会長は理事会，評議員会の議を経て
退会を勧告または除名することがある．
（再入会）
第7条 会費未納により会員資格を喪失したものが再入
会を希望する場合，2年分の未納会費を納入後
入会手続きを取るものとする．
第3章 役員および運営
（役 員）
第8条 本会に以下の役員をおく．
会長1名，専務理事1名，常任理事 若干名，
理事 若干名，監事2名，評議員 若干名及び常
任委員 若干名
1．会長は本学部教授の中より，理事会が推薦
し，評議員会の議を経てこれを決める．会
長は本会を代表し，会務を総括する．
2．専務理事は理事会の議を経て会長が委嘱す
る．専務理事は会務の運営処理を推進す
る．
3．常任理事は理事の中より選出し，会長が委
嘱する．常任理事は常任理事会を組織し，
会務を分担し，執行する．分担する会務
は，庶務，会計，編集，企画，その他とす
る．
4．理事は本学部教授ならびに3名以上の理事
の推薦を受け理事会の承認を得た者とす
る．理事は理事会を組織し役員の推薦など
会務に関する重要事項を審議する．
5．監事は理事会の議を経て会長がこれを委嘱
する．監事は会計およびその他の会務を監
査する．また必要に応じ，理事会に出席す
る．
6．評議員は本学部教授，准教授，専任講師で
構成するほか，会長の推薦により理事会の
承認を得た者とする．評議員は評議員会を
組織し，会長の諮問に応じて必要事項を審
議する．
7．常任委員は理事会の議を経て，会長がこれ
を委嘱する．常任委員は常任理事を補佐
し，会務の分掌処理にあたる．
（会議の成立条件）
第9条 理事会，評議員会は構成員の2分1以上の出席
（委任状を含む）をもって成立し，議事は出席
者の過半数によりこれを決する．
（任 期）
第10条 各役員の任期は2年を原則とする．ただし，再
任を妨げない．
第4章 事 業
第11条 本会は第2条の目的を達成するために以下の事

業を行う．
1．総 会
総会は会長の招集により年1回学術大会を
開催し，会務等について報告する．また，
必要に応じ会長は臨時総会を開催すること
がある．
2．学術大会
学術大会は年1回以上開催し，会員の研究
発表，その他学術発展に関する行事も行
う．
3．学術講演会，研修会
4．会 誌
本会は機関誌“北海道医療大学歯学雑誌
（The Dental Journal of Health Sciences Uni-
versity of Hokkaido）”を年2回発行し，会
員に配布する．会誌は逐次増刊することが
出来る．北海道医療大学歯学雑誌の投稿規
定ならびに論文査読規定については別に定
める．
5．研究活動の奨励
詳細は内規に定める．
6．委員会
本会に委員会を置くことが出来る．委員会
は委員を持って組織する．構成，任務等必
要事項は別に定める．
7．その他
本会の目的達成に必要と認めた事業．
第5章 会 計
（運営経費，会計）
第12条 本会の運営経費は会員の納入する会費，寄付
金，その他の収入を持ってこれにあてる．
1 各会員の会費は以下の通りとする．
イ 正会員
入会金 3，000円 年会費 5，000円
ロ 準会員
年会費 3，000円
ハ 賛助会員
入会金 10，000円 年会費30，000円
ただし新入会員（正会員，賛助会員）
で，会費3年以上を前納した者に対して
は入会金を免除する．
なお，事業の目的に応じ，臨時会費を徴
収することがある．
2 本会の会計年度は1月1日より12月31日と
する．
（会計報告）
第13条 本会の収支決算については，理事会，評議員会
の承認を得て，総会において会員に報告しなけ
ればならない．
第6章 雑 則
（事務局）
第14条 本会の事務局は本学部内におく．
（会則の改廃）
第15条 この会則に定めるもののほか，本会則の実地に
必要な内規は理事会の議を経て別に定めるもの
とする．
第16条 本会則の改廃は理事会，評議員会の承認を得
て，会長は会員に報告しなければならない．
附 則
1．本会則は昭和61年8月1日より施行する．
2．本会則は平成7年3月1日より施行する．
3．本会則は平成8年4月1日より施行する．
4．本会則は平成17年4月1日より施行する．
5．本会則は平成27年4月1日より施行する．
6．本会則は平成29年4月1日より施行する．
