The Extraction of Affecting Processes on the Management of Health, Safety and Environment (Case Study Mapna Group MD 2 in 2016)
Nowadays, the change and dynamism of organizations are the basic principles; therefore, managers should modify the goals, plans and work procedures so that they can be compatible with the environment condition. The management of HSE is not excepted from this principle. The aim of this study was to evaluate the processes affecting HSE.
This study was a cross-sectional and applied study using the review of relevant documentation, interviews and brainstorming with experts, and the reviews of Best Practice to identify the HSE's processes.
After checking the HSE activities, the 5 processes of HSE training, managing the risks, events, emergencies and contractors and any changes in the other processes of the organization were also suggested.
The results showed that a process approaching the form of an intervention programed can be provided in order to have a better interaction between the HSE activities and organizations processes and workplace health promotion for the HSE activities
Fuzzy-based Prioritization of Health, Safety, and Environmental Risks: The Case of a Large Gas Refinery
The main objective of this study was to develop a fuzzy–based framework for the prioritization of health, safety and environment related risks posed against employees, working conditions, and process equipment in large gas refineries. The First Refinery at Pars Special Economic Energy Zone in South of Iran was taken as a case study. For this purpose, health, safety and environment related risks were determined based on the three criteria of impact severity, occurrence probability, and detect-ability using a questionnaire of 33 identified failures. The values obtained were processed by a so-called ‘contribution coefficient’. The results were then subjected to fuzzification and fuzzy rules were defined to calculate the risk level indices as the model outputs, which was then employed to facilitate the management decision-making process by prioritizing the management options. The prioritization values were then classified in six categories in the order of risk severity. Results revealed that failure in a combustion furnace had the highest rank while failure in the slug catcher ranked the lowest among the risk sources. It was also found that about 0.4% of the identified risks prioritized as “intolerable”, 79% as “major”, 20% as “tolerable”, and 0.7% as “minor”. Thus, most of the risks (more than 79%) associated with the refinery has the potential of significant risks. The results indicated that the risk of the pollutant emissions from the combustion furnaces is the highest. Exposures to harmful physical, chemical, psychological, and ergonomic substances are the other risks, respectively
Investigation of Relationship between Level of Awareness around Health, Safety and Environment Management System and Its Effects on Safety Climate and Risk Perception by Employees in an Iran Oil Refinery, 2015
Special attention of and oil, gas and petrochemical large corporation in the world to the HSE management system is due to its importance in the designing and development of products, services and processes by considering its health, safety and environment requirements. Staff's perception of the existing job risks has a significant impact on their safe behavior at work. This study was conducted to determine the relationship between safety climate and staff's perception of risk with an awareness level among employees of the HSE management system in an oil refinery in Kermanshah.
The study population was employees in one of the oil refineries in Iran. After designing and questionnaire psychometric assessment of staff knowledge of HSE management system (Cronbach's alpha was 0.9 and its validity was assessed by certified professionals), Loughborough safety climate questionnaire and Flin risk perception questionnaire were used. Data analysis was performed using SPSS V22 software.
Results showed that the relationship between safety climate and awareness level of the HSE management system; also the relationship between safety climate and perception of risk was also getting significant. However, the relationship between perception of risk and awareness level of the HSE management system was not significant.
The results of this study showed a moderate awareness of HSE Management System in refinery workers. In this regard, appropriate and proper management policy should be committed to improving the situation. The results of this study is a profile the situation of safety climate in the refinery, which can be used as an indicator for the development of preventive policies and evaluate the performance of the organization's safety and the results of the safety improvement organization
Awareness Regarding Zoonotic Diseases among the Butchers of Proddatur, Kadapa Dist., A.P., India
Zoonotic diseases present an ongoing public health concern. Most human infections with zoonoses come from livestock, including pigs, chickens, cattle, goats, sheep and camels. Butchers constitute the high risk group in the transmission of zoonotic diseases. The educational status and awareness regarding the information, identification and transmission of zoonotic diseases among this high risk group, is critical in controlling the spread of zoonotic diseases. Hence, a study has been designed to evaluate the educational status, awareness among the butchers regarding the various zoonotic diseases and their modes of transmission and the effluent treatment methods followed by the butchers in and around Proddatur, Andhra Pradesh. The survey revealed that 86 percent of the butchers had high school education and most of them acquired this profession from their ancestors. 60 percent of the butchers were having awareness regarding the commonly occurring zoonotic diseases, 81 percent of butchers identified the disease by observing the lesions and were familiar with their control. Some of the butchers follow the hygienic practices and majority of them (81.4 %) were interested in attending training programmes pertaining to zoonotic diseases
Assessing the Risk Factors for Musculoskeletal Disorders in Construction Workers Using PATH, Case Study: Construction Project
Construction workers experience a high risk of developing musculoskeletal disorders due to the nature of their jobs. This article aimed to evaluate the risk factors of musculoskeletal disorders among construction workers using Posture, Activity Tools and Handling (PATH). This is a sectional, descriptive-analytical study in a construction site in Tehran, Iran. Certain factors were identified namely body posture, weight of carried tools and objects, activities and tools, and their tasks and activities using PATH. PATH sheets were coded for a certain job. Descriptive data and Chi-square test were employed to analyze the data using SPSS.19. Identification and evaluation were performed in three most important stages of construction: foundation, carcass, finishing (elaborate work). The mean age was 33.08±8.97. Body posture included gentle bending posture (21.2%), severe bending (8.2%), bending and twisting (7% and 1.9%, respectively). Evaluating the Knowledge, Attitude and Safety Performance in the Prevention of Occupational Accidents among Workers in One of Iran Khodro's Piece Making Companies
Occupational accidents are one of important events in the world. Individual affecting factors on accidents are knowledge, attitude and perception of risk. So this study aimed to determine knowledge, attitude and safety performance in prevention of occupational accidents at one of Iran Khodro's piece making companies' workers. In this cross-sectional study, 212 workers from Mehrcam Pars Company selected by stratified random method at each working site in 2015. By simple random sampling, subjects were enrolled. Questionnaire was completed by workers through self-reporting. Questionnaire consisted of demographic and underlying information and questions about safety knowledge, attitude and performance in the workplace. Data were analyzed using SPSS 21 software. The average workers' age was 34.3 year with a standard deviation of 4.7%. 35.8% were not trained in terms of performing their job. 48.1% of participants had no history of injuries in site, but others reported suffers from mild to severe intensity. There was significant relationship between educational situation with attitude and safety performance. There was significant relationship between job experience with safety knowledge, attitude and performance. Age and workers' safety attitude was a predictive of safety performance. The results showed that there is significant relationship between safety knowledge, attitude and performance in the workplace with some variables. So in order to improve the overall situation of safety, some measures can be effective such as raising the knowledge of personnel, safety staff's participation management, balancing work and time
Evaluation and Management of Human Errors in Critical Processes of Hospital Using the Extended CREAM Technique
Medical errors result in serious and often-preventable problems for patients. Human errors can be used as an opportunity for learning as well as a key factor for patients’ safety improvement and quality of patients' surveillance in hospitals. The aim of the present study was to identify and evaluate human errors to help reduce risks among personnel who render health services during critical hospital processes. This cross-sectional study was done in the Besat hospital in Hamedan in 2016. At first, the critical processes were selected via given scores in Delphi method and by multiplying the scores of each of the five criteria including the severity of the consequences caused by error incidence, probability of error, capability of the error detection, task repeatability and type of hospital ward with each other. Determining the risk numbers of each process, three ones were chosen with the largest scores. At the end, the selected processes were analyzed by the method of extended CREAM. The results showed that the highest CFP is associated with the CPR process, particularly in the sub-stage of command of starting CPR by anesthesiologists (0.0891), the one in the giving medicine process is in the sub-stage of calculating of medicine dozes and determining prescription methods (0.0796) and also the one in the tracheal intubation process is in the sub-stages of pulmonary and respiratory monitoring of patients and observing the vocal cords and larynx of patients (0.0350). Regarding the critical consequences of human errors in the selected processes, reviewing the qualities of roles and responsibilities of each of the medical group members and providing specialized introduction for hospital processes seem necessary
A Management and Economic Survey in Implementation of Blown Bitumen Production Using Acidic Sludge Recycling (A Case Study)
Many units of blown bitumen and used-motor oil reprocessing industries have been currently implemented and many modern technologies and additives are being developed and requested continuously. The objective of current study was a management and economic survey in implementation of blown bitumen production using acidic sludge recycling as a new additive that encompasses financial views and other success factors so carried out using a Kanji’s Business Excellence Model, empirical equations, professional experiences and observations during establishment an industry. Using acidic sludge had promoted the product quality in parallel with the TQM parameters. Current study had shown the indices values such as value-added percent, profit, annual income, breakeven point, value-added, output value, data value, variable cost of commodity unit and production costs were found to be about 44%, 5252251.65, 5401935.47, 12150000, 6897748.4 respectively. Monitoring of THMs Concentration in Isfahan Water Distribution System and Zoning by GIS, a Case Study in the Center of Iran
Trihalomethanes (THMs) formation in treated water is a consequence of a reaction between the chlorine used for water disinfection and some natural organic matters. The objectives of the present study were monitoring of THMs concentration in Isfahan (A metropolis city in center of Iran) water distribution network (IWDN), evaluation factors that affect the THMs formation potential and identification of critical points by using geographical information system (GIS). The study was performed in summer months of 2014. For sampling point's selection, city divided into 30 zones and water quality parameters such as pH, Electric Conductivity (EC), residual Chlorine, Total Organic Carbon (TOC) and THMs of IWDN measured. Multi regression analysis was used to estimate the correlation between THMs formation and these variables. While the statistical analysis with Spearman non-parametric correlation coefficients showed a positive correlation between distance from treatment plant and THMs concentration(r=0.45, P =0.01) and negative strong correlation(r=-0.95, p>0.001) between THMs and TOC concentrations, there was no strong significant relationship between THMs formation in IWDN and some variables including pH, temperature and residual Chlorine.
The results reveal that the average value of the THMs at sampling points for summer attained 42.56 ppb which was lower than the EPA and WHO standards. It is recommended that the distance from the treatment plant was used as an effective parameter for estimation of THMs formation potential
A Study of The Relationship Between The Components of The Five-Factor Model of Personality and The Occurrence of Occupational Accidents in Industry Workers
Accidents are among the most important problems of both the developed and the developing countries. Individual factors and personality traits are the primary causes of human errors and contribute to accidents. The present study aims to investigate the relationship between the components of the five-factor model of personality and the occurrence of occupational accidents in industrial workers.
The independent T-test indicated that there is a meaningful relationship between the personality traits and accident proneness. In the two groups of industry workers injured in occupational accidents and industry workers without any occupational accidents, there is a significant relationship between personality traits, neuroticism (p=0.001), openness to experience (p=0.001), extraversion (p=0.024) and conscientiousness (p=0.021). Nonetheless, concerning the personality trait of agreeableness (p = 0.09), the group of workers with accidents did not differ significantly from the workers without any accidents.
The results showed that there is a direct and significant relationship between accident proneness and the personality traits of neuroticism and openness to experience. Furthermore, there is a meaningful but inverse correlation between accident proneness and the personality traits of extraversion and conscientiousness, while there was no relationship between accident proneness and the personality trait of agreeableness
