Get PDF【目的】高血圧治療ガイドライン2019 において降圧目標が厳格化され，高血圧症患者には，より一層の疾患管理が期待される。本研究の目的は，降圧目標厳格化以降の高血圧症患者の自己健康管理の実態とその関連要因を明らかにすることである。【方法】2020 年8 月～ 9 月に，高血圧症の診断，治療を受けている患者を対象に，自己健康管理に関する無記名自記式質問紙調査を実施した。【結果】回答数は64 名（回収率73.6％），有効回答数は61 名（有効回答率95.3％）であった。「高血圧症患者の日常生活行動自己管理尺度」得点の平均値は，食事22.8±4.6 点，運動19.5±5.6 点，ストレス26.8±4.3 点，服薬管理24.7±3.3 点，飲酒10.2±3.4 点，喫煙4.5±1.8 点であった。家庭血圧測定習慣のある者は，20 名（32.8％），ガイドライン改訂前後で自己健康管理に変化があった者は17 名（27.9％）であった。年齢，性別，職業，家族形態，高血圧による結果の重大性の認識，ガイドライン改訂に関する知識，医療的支援と，自己健康管理に有意な関連
が認められた。老年期，女性，職業のない者，独居の者，高血圧による結果の重大性を認識している者，知識のある者，ガイドライン改訂に伴う医療的支援の変化があった者の自己健康管理の実践状況がより良い状態を示した。【考察】自己健康管理の実態は，ガイドライン改訂前と近似しており，十分な自己健康管理が実践できていなかった。患者・家族への支援の提供，支援システムの構築が求められる。departmental bulletin pape
