The objective of this study was to find out the barriers of sentinel events reporting at tertiary hospital and then formulate recommendations to ameliorate based the findings of this study. The study was carried out in 2015 and conducted as two parts; the first part is a retrospective data, review of Occurrence Variance Reports forms according to the reporter. The second part was through self-administered questionnaires, which was randomly distributed to the hospital staff. A total of 120 completed questionnaires out of 135 were received.  In this study, we found that the commonest reasons that prevent the hospital staff from reporting the sentinel events are poor communication of policy and procedure of sentinel events reporting, lack of motivation among the staff to report sentinel events in 28 % and 26 % respectively. Staff fear of punitive actions from the hospital administration was rated as the first and second priority reason in preventing employee reporting sentinel events in about half of the cases. No feedback from reporting sentinel events was the reason for underreporting in about one fifth. Sentinel events definition was not clear and sentinel cases reporting form was not available in 14 % and 15 % respectively. This study highlights the common factors that may contribute to under-reporting of sentinel events in tertiary hospital. The findings may be useful in formulating beneficial strategies to improve reporting which will have great value on quality of care and patient safety

Internet of vehicles (IoV), or vehicles to everything (V2X), is a relatively new concept for all vehicles connected to the internet. In this paper, we propose a solution to the problems of traffic congestion. Road traffic congestion has continued to be a major problem in many developed countries. This concept shows the place or location traffic congestion occurs at the time of rainfall, the position of the data based on GPS, and the sensors in the vehicle. The sensors in vehicles are used to collect information for the purpose of evaluating traffic congestion through cloud server via protocols such as CoAP and MQTT. At the beginning of the study, the authors introduced the problems and then proposed a solution

A Tree Story: Narratives of Paraserianthes Falcataria L. Nielsen (Falcata) Farmers

    Get PDF
The study narrates the plantation to industrialization processes of Falcata trees through the lens of the Falcata farmers. It recounts the: a) pre-planting story, b) planting story, c) harvest story, d) market story, and e) industry story. This is a narrative research, using post-structuralist approach, and putting forth three philosophical stances: logical positivism, narrative fidelity, and aesthetic finality. Pre-planting story progresses from the motivation of the farmers to plant and his challenges prior to planting season, soil and land preparation, and seedling choice. Planting Story is about the soil, financial matters, trees and the government support to the farming activities. Harvesting Story depends on the land area to be harvested, harvesting mode, harvesting activities and the owner’s return of his investment. Market Story encompasses the search and selection of buyers; and the projected income if the trees are sold. Industrialization Story from the hind sight of the farmers. They only presume that these trees are made into particular products; but the method of making them is unknown to the farmers. The stories of assuring and distressful outlooks of tree farmers reveal much about the Falcata tree industry. Maintaining the stand point of narration, it is revealed that marketing comes before harvesting; and thus alters the order of narrative structure and content. Keywords: Falcata trees, Farmer’s Perspectives, Narrative Research, Tree Farming, Timber Species, Philippines, Asi

    Get PDF
This research was conducted to evaluate the teaching practice program practiced every second semester in the College of Education (CoE) found in Eritrea Institute of Technology, Mainefhi. It explored the challenges and strengths of the program. To conduct this qualitative study, a questionnaire and a focused group discussion was used. A total of 104 participants (cooperating teachers, student teachers and supervisors)were selected for the study. The findings reveal that the teaching program in the CoE is practiced in a short time span with difficulties in materializing the proposed budget by the CoE and other challenges that vitiate its quality. However, the involvement of the cooperating teachers (CTs) and supervisors is satisfying. Majority of the student teachers (STs) felt that the practice plays a great role in preparing them for the teaching profession. Therefore, generally solving the budget implementation issue and the minor drawbacks of the program will definitely make it contribute a lot in producing qualified teachers. Finally, these findings are expected to mitigate the limitations of the program and raise the outcome of the practice. Besides, other initial teacher education institutes may benefit from the study findings

Classification of Chemicals According to UN-GHS and EU-CLP: A Review of Physical Hazard Classes and Their Intricate Interfaces to Transport and Former EU Legislation

    Get PDF
The Globally Harmonized System of Classification and Labelling of Chemicals (UN-GHS) is being implemented in more and more countries all over the world; the EU has done so with the CLP-Regulation (EU-CLP). Compared to the undeniably important questions on health and environmental hazards, the classification of physical hazards of chemicals often has not been in the focus, although their implementation can be challenging and there are traps and pitfalls to be avoided. The following overview of the classification systematics for physical hazards aims at a principle understanding without detailing all criteria or test methods. Similarities and differences between the classification systems of the UN-GHS and EU-CLP, the transport of dangerous goods and the former EU system are reviewed with regard to the physical hazard classes. Available physical hazard classifications for the transport of dangerous goods and according to the former EU system can be used as available information when classifying according to the GHS. However, the interfaces of these classification systems and their limitations have to be understood well when concluding on GHS/CLP classifications. This applies not only to industry when applying CLP but especially to legislators when adapting legislation that in one way or another refers to the classification of chemicals

Pronunciation Acquisition of the Inflectional Morpheme –ed in English by Nicaraguan Spanish Speakers

    Get PDF
This descriptive non-experimental quantitative study investigated if the pronunciation of the –ed morphemeim proves as the English proficiency level develops. A random sampling design was used to recruit research participants.  A target sample of 48 Nicaraguan English as a foreign language students were chosen from an accessible population (N= 91) to participate in this research.  The participants’ age ranged from 18 to 24, with a mean age of 21 years.  They belonged to three English proficiency levels: high beginners, intermediate, and high intermediate. Findings showed that more advanced English as a Foreign Language learners had a significantly higher pronunciation accuracy on the production of the allomorphs, /t/ and /d/. Their error rate on these two allomorphs was as low as 9% and 8%, respectively. Concerning the /əd, ɪd/ allomorph, no significant differences were found among proficiency levels.  These results were interpreted in view of the Markedness Differential Hypothesis (Eckman, 1977) and the Similarity Differential Rate Hypothesis (Major & Kim, 1996). The trends in the data definitely suggested support for the Similarity Differential Rate Hypothesis which postulates that markedness by itself cannot explain the development of L2 learners as they improve their L2 proficiency level. This study offers implications for the teaching of the three phonological realizations of the -ed past tense inflection

    Get PDF
Infertility is one of the major health concerns in the world whose globally incidence rate is on the rise. Several hormones are effective in this process, such as testosterone, estrogen and Osteocalcin (OCN). The Osteocalcin has Gprc6a receptors on leydig cells, which promotes the production of testosterone when mounted on these receptors. Therefore, the aim of this study was to investigate the role of Osteocalcin on the improvement of spermatogenesis and expression of Gprc6a receptors on leydig cells in the azoospermic mouse model.Twenty-five mice (4 to 6 weeks) were randomly divided into five groups: control, sham I Group that was initially injected by busulfan solvent (DMSO) at 5 weeks old and then by Osteocalcin solvent (PBS) after 5 weeks for one month, azoospermia Group that received busulfan (40 mg/kg/ip) at 5 weeks old. Sham II Group received busulfan 40mg/kg/ipandt hen after 5 weeks received PBS and experimental group, including azoospermic mice, was administered by Osteocalcin (3ng/g/d) for one month. After the last injection, the tests were dissected and then exposed to the tissue passage. To measure morphological changes, H & E staining was performed on a number of sections to measure the diameter of the seminiferous tubule, thickness of the germinal layer, count the spermatogonial cells, spermatocyte, round spermatid, long spermatid, Sertoli, leydigandmyoid cells. Image J software was used to conduct quantitative studies. Immunohistochemical method was employed to examine the expression of the specific receptor of Osteocalcin, Gprc6α, in the Leydig cell among the groups. The H & E staining showed a significant difference in the count the spermatogonial cells, spermatocytes, round spermatids, long spermatids, the thickness of the germ layer, the seminiferous tubule diameter between the studied groups (P<0.05).There was also no significant difference in the count of Sertoli, leydig, myoid cells and seminiferous tubule diameter between the groups (P<0.05). In the immune histochemistry, no significant difference was found in the count of GpRC6α-positive leydig cells between the groups (P< 0.05).According to the current results, the OCN plays an important role in spermatogenesis, which has a positive effect on the count of spermatocytes and spermatids, and can be further explored as an appropriate therapeutic strategy proposed for the infertility.
