Pediatric Preoperative Warming Protocol to Prevent Hypothermia

Purpose: The purpose of this project was to discover the incidence of intraoperative hypothermia in pediatric patients undergoing spine surgery at Children’s Hospital of Michigan and create a preoperative warming protocol to reduce the incidence/time of intraoperative hypothermia.
Background: Intraoperative hypothermia (< 36°C) occurs in roughly two thirds of patients who undergo anesthesia. Thermoregulatory responses are impaired during a general anesthetic leading to complications such as morbid cardiac outcomes, delayed drug metabolism, increased surgical site infections, increase in surgical blood loss and the need for blood transfusions, prolonged post anesthesia recovery time and an increased length of stay. Pediatric patients are particularly at risk for hypothermia during anesthesia due to their higher body surface area to body mass ratio, increased radiant heat loss from large heads, and lower subcutaneous fat stores. Pediatric spine surgery patients are particularly vulnerable to hypothermia due to a surgical field that requires extensive exposure of body surfaces. The use of forced air warming has been utilized in the preoperative setting to warm patients prior to surgery and has been shown throughout multiple studies to reduce the incidence of intraoperative hypothermia.
Methods: A retrospective chart review was conducted on 44 pediatric spine surgery patients at Children’s Hospital of Michigan. The patient's temperature was collected in the preoperative area and every 15 minutes intraoperatively. The severity of temperature drop after induction of anesthesia as well as the duration of hypothermia was analyzed.
Results: Intraoperative hypothermia occurred in 90.1% of the cases. Patients dropped 0.8 °C immediately after induction and continued to become progressively hypothermic until 60 minutes post induction where they averaged 35.5°C. The average length of time patients remained below 36 degrees celsius was 150 minutes. A bootstrapped linear regression analysis revealed that the average length of hypothermia was increased based on the severity of post induction hypothermia.
Recommendation: After reviewing the literature and the incidence of hypothermia at Children’s Hospital of Michigan, it is our recommendation that the use of pre-operative forced air warming for at least 30 minutes prior to surgery would reduce the incidence of hypothermia in the pediatric spine surgery patient population

No full textIn this paper, I will examine how Algeria presented these newfound goals and identities to the world at the UN in the early years of its independence. I will start by completing a review of the current literature that relates to the topic of newly independent nations and Algerian foreign policy. I will then base my central hypotheses on the current scholarship to see if Algerian leaders use the same type of rhetoric which former scholars have said newly independent nations and Algeria respond to independence. I believe, based in the literature review, that Algeria will be a voice for change in the international community, and particularly focus on ideas of equality, justice, and nonalignment. I also believe that they will argue that colonialism continued, despite political independence, in the economic dependency of former colonized nations on their former colonizers. Lastly, I believe that countries will minimize their discussion of colonialism when they focus on issues of security and stability.
I found that the first hypothesis is consistent with evidence and that Algerian leaders talked many times. The existence of the need to end colonialism and fight for equality, justice, and nonalignment was evident in these documents. I also found that Algerian leaders focused the perpetuation of colonial hierarchy in the international economic system and the need to create new economic relations while not focusing on economic dependency. I lastly found that while Algerian leaders did talk of colonialism while discussing stability and security as they saw the two ideas tied to each other
Dosage Based Antibiotic Cycling: Exploring a New Solution to Antimicrobial Resistance
No full textAntimicrobial resistance is a well-documented public health crisis that continues to escalate and threatens our ability to successfully treat infections. Multiple solutions have been suggested, researched, and implemented to fight the development of antimicrobial resistance, but no current solution has proved adequate. Promising treatment methods against antimicrobial resistance are multidrug approaches promoting antibiotic heterogeneity. Dosage based antibiotic cycling is a novel treatment approach proposed in this study that combines the individual strengths of two existing multidrug approaches: traditional antibiotic cycling and combination therapy. This experiment utilizes a Kirby Bauer Disk Susceptibility test and a Colony Biofilm Assay to explore the effectiveness of dosage based antibiotic cycling as a solution against antimicrobial resistance by evaluating its ability to minimize the development of drug resistance in strains of S. aureus and E. coli. Antibiotic resistance results from dosage based antibiotic cycling will be compared to the results from corresponding monotherapy and combination therapy trials to determine if one strategy is more or less effective in preventing antimicrobial resistance than another. Results from this study, though inconclusive, open the door for the necessary further research
Problem-Solving Through Design: How Can Graphic Design Reconstruct American Art Museums?

The project reviews past research and literature on art museums and proceeds to explain the design process for this project. This project hopes to influence peoples’ perceptions on art museums and artists overall while demonstrating the impact of art in daily life

Intersection of Class and Gender
No full textNorth and South, by Elizabeth Gaskell, explores the differences in society during the Industrial Revolution as classes were redefined based on a new source of wealth, and gender roles were adapted to fit the new economy. The protagonist, Margaret Hale, finds herself confronted with these changes as she sees it from the perspective of the Higgins family, who are lower class factory workers, and the Thornton family, who own a factory. In her novel, Gaskell uses these multiple perspectives to explore gender roles in all parts of society.Gaskell proves the connection of gender roles and class, and how they only function in the upper-classes. Because of this, there is an inherent flexibility in gender roles since the lower classes cannot strictly adhere to them. This erases the argument that they are a result of the limited abilities of one gender, since they can preform those actions in a different context. Gaskell uses this to expand the opportunities available to both genders in her novel in a way that could translate into society. While completely abandoning the traditional roles would be immediately rejected by Victorian society, Gaskell starts by bending the roles. She still shows each gender, especially women, doing things outside of their sphere, but she frames it in such a way that it does not seem radical. She gives freedom to women by using tradition to their benefit
