Motor Imagery for Post-Stroke Upper Limb Recovery: A Meta-Analysis of RCTs on Fugl-Meyer Upper Extremity Scores

    Get PDF
Objectives: Motor imagery (MI) may enhance post-stroke recovery, but evidence of its benefit over conventional rehabilitation therapy (CRT) is inconsistent. This study evaluated the effect of MI combined with CRT on upper-limb recovery, accounting for methodological quality and publication bias. Methods: A systematic review and meta-analysis was conducted following PRISMA guidelines. Searches were performed in multiple databases up to July 2025. Methodological quality and risk of bias were assessed using the PEDro scale and Cochrane RoB 2 tool, respectively. Analyses included the calculation of effect sizes (ES), heterogeneity, sensitivity, publication bias, and GRADE-based certainty assessment. Results: From 4074 records, 10 randomized controlled trials (n = 255) were included. The initial pooled analysis showed a small-to-moderate effect of MI + CRT versus CRT alone (ES = 0.45; 95% CI: 0.16–0.74). However, the overall ES calculated with a robust variance estimator was −0.06 (95% CI: −0.21, 0.08). Most trials had methodological limitations (mean PEDro = 6.0; high risk of bias in 7/10 studies). The GRADE evaluation indicated a very low certainty of evidence. Conclusions: The initially observed positive effect of MI combined with CRT is not robust. When accounting for statistical dependencies and potential biases, the effect vanishes and is no different from zero. Current evidence does not support the use of MI as a standalone adjunct to CRT. Larger, high-quality RCTs with standardized protocols are required to establish any potential clinical relevance

    Assessing Ontology Quality with QASAR

Ontologies are foundational for enabling semantic interoperability, data integration, and knowledge sharing across a broad spectrum of domains, including biomedicine, engineering, and the social sciences. As their adoption continues to grow, so too does the need for robust methods to ensure their quality. Despite the development of a range of quality assurance frameworks and methodologies aimed at evaluating various aspects of ontology quality, the practical application of these methods remains fragmented. In this demonstration, we present QASAR, a novel, integrated quality assurance tool designed to bridge this gap. QASAR consolidates several QA methodologies developed by our team into a unified, user-friendly platform. The system supports a broad set of quality dimensions and alignment with best practices and community standards. By offering automated analyses, interactive reports, and guided recommendations, QASAR enables both novice and experienced ontology developers to iteratively improve their ontologies with greater ease and confidence. Through this tool, we aim to promote the creation of higher-quality ontologies and foster broader adoption of quality assurance practices within the ontology engineering lifecycle. QASAR represents a step forward in operationalizing ontology quality assurance by providing a streamlined and accessible environment for comprehensive ontology evaluation

Eating disorders (ED) are complex conditions that affect the relationship with food, generating high levels of anxiety and food rejection. Adjusting to hospitalization in the Eating Disorders Unit (UTCA) of the San Juan University Hospital in Alicante can be difficult, especially for patients with restrictive eating habits and fear of weight gain. To facilitate this process, the pre-admission therapeutic dining room has been developed as an intermediate resource for patients on the waiting list for admission, with the aim of improving their adaptation to treatment and reducing anxiety associated with meals. The service is offered from Monday to Thursday, from 12:30 to 15:00, in small groups of up to four people. The structure consists of three parts: reception and relaxation beforehand, supervised lunch, and rest with group work on goals. In addition, on Thursdays, nutritional guidelines are provided to families for managing their eating habits over the weekend. The intervention addresses nutritional, emotional, and behavioral aspects through cognitive-behavioral techniques and gradual exposure to food in a therapeutic setting. The dining room is supervised by a multidisciplinary team of two psychologists, an occupational therapist, and a nutritionist. To evaluate its effectiveness, a satisfaction survey is conducted at the end of the intervention. This survey aims to understand the user's experience and assess whether the therapeutic dining room has helped them adapt to hospitalization and, in some cases, avoid it

    No full text
Since the birth of survival horror video games, a clear and undeniable similarity and direct inspiration from both classic and contemporary horror cinema have been observed. These video games of the genre have taken many essential elements from horror films, such as the use of atmosphere to create tension or the construction of an engaging narrative. This influence is most evident in the narrative, visual, and sound elements, which have become key aspects in both the cinematic visual language for horror and survival horror games. This academic work will analyze both the similarities and differences that arise when adapting these elements from a cinematic format to an interactive one, such as video games. As an essential part of this comparison between both visuals, the video game Silent Hill 2 (Konami, 2001) will be taken as a case study. Through Silent Hill 2, we will analyze how its designers drew inspiration from horror films and auteurs such as David Lynch, Stanley Kubrick, and Alejandro Jodorowsky to create a dense and claustrophobic atmosphere, developing characters with deep traumas, and how the music and sound effects reinforce feelings such as despair. This entire analysis will illustrate and help understand how survival horror shares and adapts the cinematic language of horror films to create unique immersive experiences

    No full text
Transient receptor potential (TRP) channels are a superfamily of multifunctional ion channels permeable to a wide variety of cations, whose activation can occur in response to physical (temperature, voltage, pressure, and tension) and chemical stimuli. In recent years, they have been studied extensively due to their involvement in numerous pathologies, such as cardiovascular disorders, cancer, and chronic pain. Within the TRPV subfamily, the ion channel TRPV1 is of great importance due to its polymodality and its involvement in widely distributed diseases, making it one of the most studied targets for potential therapeutic intervention. In this context, PROTAC compounds, which induce protein degradation, could represent one of the most promising treatments to inhibit the pathological activity of TRPV1. For this reason, the present project investigated the effect of eight PROTAC compounds on TRPV1, performing an initial screening to identify the most promising candidates. In addition, membrane permeability and cytotoxicity assays were conducted to rule out possible adverse effects. Finally, a protein detection and quantification assay, Western Blot, was performed, after first optimizing the protocol. Overall, under the conditions tested, it was not possible to demonstrate that the compounds induced channel degradation

    No full text
Cardiovascular diseases are one of the leading causes of death worldwide. This risk is especially pronounced in patients with severe mental disorders, particularly those with schizophrenia, whose cardiovascular mortality is two to three times higher than that of the general population. Various factors contribute to this vulnerability: early development of multiple cardiovascular risk factors, unhealthy lifestyle habits, lack of social and economic support, and barriers to accessing healthcare. In addition, the adverse effects of atypical antipsychotics further compound the problem. Objective: To determine how many patients with schizophrenia undergoing treatment with atypical antipsychotics meet the diagnostic criteria for metabolic syndrome, and to compare this prevalence with that of the general population. Additionally, to analyze the prevalence of each component of metabolic syndrome and identify additional risk factors. Materials and Methods: The study included 75 patients diagnosed with schizophrenia who have been undergoing treatment with atypical antipsychotics for more than 12 years, all of whom are from the General University Hospital of Elche. A descriptive analysis was conducted on clinical data, sociodemographic information, and data related to the disease and its treatment. Results: The study included 75 patients, of whom 72% were men and 28% women, aged between 36 and 82 years. The overall prevalence of metabolic syndrome was 44%, affecting men (66.7%) more than women (33.3%). When analyzed by sex, 11 of the 21 women (52%) and 22 of the 54 men (40.74%) met the criteria for metabolic syndrome. Because metabolic syndrome is more common at older ages, prevalence was broken down by age group: 50% in patients aged 36–44 years; 31.25% in those 45–54 years; 57.9% in the 55–64 age range; 60% among participants aged 65–74 years; and 50% in the oldest group (over 74 years). Separate analysis of the diagnostic criteria revealed an increase, compared with 12 years ago, in the use of medications for hypertension, diabetes, and dyslipidemia. In the current sample, 18.7% of patients were on antihypertensive therapy, 22.7% on antidiabetic agents, and 26.7% on lipid-lowering treatment—versus 4%, 5.3%, and 5.3%, respectively, recorded in 2013. The route of administration of antipsychotics was also examined: 34.7% received only oral formulations (51.9% of them had metabolic syndrome); 21.3% received long-acting injectable formulations (37.5% with metabolic syndrome); and 42.7% were on both. The most frequently prescribed drugs were paliperidone (32%), olanzapine (28%), and clozapine (22.7%). Regarding substance use, 57% of the sample were smokers (versus 52% previously), while consumption of cannabis, cocaine, and amphetamines fell markedly; alcohol and opioid use remained stable. Concerning obesity, most participants were obese (38.7%), 28% were overweight, and only 29.3% had a BMI below 25. Three cardiovascular events were recorded between 2013 and 2025: two cases of coronary artery disease and one of cerebrovascular disease. The SCORE and REGICOR scales were also applied to estimate 10-year cardiovascular risk, yielding similar results: the majority of patients were classified as low risk. Conclusions: Patients with schizophrenia treated with atypical antipsychotics show a higher prevalence of metabolic syndrome, especially in those under 65 years of age. Obesity is a key predictor, and clozapine and olanzapine have the greatest metabolic impact

Colchicine and DMARDs for chronic calcium pyrophosphate crystal arthritis

    No full text
Background: There is limited data on the treatment of chronic calcium pyrophosphate (CPP) crystal arthritis, and no previous reports have focused on the potential role of combining anti-rheumatic agents. Objectives: To compare treatment outcomes between combination therapy and monotherapies in chronic CPP crystal arthritis management in clinical practice, along with assessing the impact of sex and advanced age. Methods: A retrospective cohort study was conducted by seven European centers. Patients diagnosed with chronic CPP arthritis (either persistent or recurrent forms) were selected and monitored at months 3, 6, 12, and 24 to evaluate treatment response and safety. This subanalysis evaluates differences in effectiveness, safety, and drug retention between monotherapy with colchicine, with disease-modifying antirheumatic drugs (DMARDs), and combination of colchicine with DMARDs. Results will also be stratified by sex and age (≤65 vs. >65 years). Linear mixed models were built for comparisons. Results: A total of 102 treatment lines were analyzed: 71 (69.6%) for colchicine monotherapy, 20 (19.6%) for DMARD monotherapy, and 11 (10.8%) for combination therapy of colchicine plus DMARD. Combination therapy led to a higher chance of obtaining a response > 2/4 (ꞵ=+6.22, 95%CI +0.98 to +39.6, p=0.053) than individual agents, although this tendency was observed only in the physician’s assessment. Combination therapy was significantly associated with deeper pain reduction, compared to colchicine alone (ꞵ=- 0.83, 95%CI -1.56 to -0.09, p=0.029). By the end of the study, combined therapy reached 100% response (>2/4), followed by monotherapy with colchicine (95%) and with DMARD (72%). Adverse events were similar, but the colchicine-only group reported more gastrointestinal disorders. Discontinuation rates were similar between groups. Superior outcomes were noted in men, with no notable variations by age. Conclusions: Combination therapy showed superior results in pain reduction and physician-assessed effectiveness. By the end of the follow-up, combination therapy achieved complete response in all patients, colchicine monotherapy was nearly as effective, while DMARD monotherapy showed the lowest response rate. No significant differences were found in discontinuation and safety.

    No full text
Introduction: Hemophilia A is an inherited coagulopathy caused by a deficiency in the activity of coagulation Factor VIII. The levels of this factor determine the severity of the disease, allowing for a correlation between laboratory findings and clinical presentation. Joint and muscle bleeds are the main clinical manifestations. To prevent hemorrhagic complications, prophylaxis is mainly administered using Factor VIII or emicizumab. In cases of poor disease control, hemophilic arthropathy may develop as a complication of joint bleeds. Objectives: To evaluate joint damage in hemophilic patients and its relationship with age, type of prophylaxis, and overall hemostatic capacity. Methods: Retrospective observational study conducted at HGU Dr. Balmis, collecting clinical data from patients using authorized databases. Results: A high prevalence (65%) of joint damage was observed in patients with severe hemophilia despite prophylactic treatment. Within the group of patients with moderate or mild hemophilia, a potential benefit of prophylactic treatment over on-demand therapy is suggested. A significant association was found between age and joint damage, but no association was found between global hemostatic capacity studies and joint damage. Conclusions: Despite prophylactic treatment, patients with severe hemophilia show a high prevalence of joint damage. Meanwhile, patients with moderate and mild hemophilia may benefit from prophylaxis. Personalized treatment strategies could improve patients’ quality of life by addressing their individual needs
