    Familial Lamellar Ichthyosis in 6 Years Old and 2 Years Old Children: A Rare Case Report

    Background: Lamellar ichthyosis (LI) is a rare non-syndromic congenital ichthyosis and is autosomal recessive. LI patients are generally born with abnormalities of keratinization, then develop into lamellar scales and persist into adulthood. This study aimed to describe cases of lamellar ichthyosis and provide appropriate management to prevent worsening patient prognosis. Case presentation: A 6-year-old girl and her younger brother, 2-year-old have been complaining of scaly skin all over their bodies since birth. Both patients were born with a history of being covered by a collodion membrane. On physical examination, thick and wide lamellar scales were seen with an erythematous base and palmoplantar hyperkeratosis. Ectropion of the eyelids, eclabium in the mouth area, and microtia in the ears were seen. The results of the dermoscopy examination showed brownish quadrilateral structures with white scales forming a lamellar pattern. Laboratory examination showed vitamin D insufficiency. Both patients were referred to the Pediatric Polyclinic regarding insufficiency and developmental complaints. Patients received therapy in the form of moisturizers, artificial eye drops, topical urea as a keratolytic, and oral vitamin D 2,000 IU per day. There was a clinical improvement after 3 months of treatment. Conclusion: Lamellar ichthyosis is an autosomal recessive congenital ichthyosis with clinical manifestations limited to the skin. In a minority of cases, LI can be inherited in an autosomal dominant manner. Thorough management is necessary to prevent a poor prognosis in patients

    Use of Personal Protective Equipment and Length of Work as Risk Factors for Occupational Contact Dermatitis on the Hands of Woodworkers

    Background: Occupational contact dermatitis (OCD) is an inflammatory response caused by contact with allergens or irritants in the work environment, and this disease is most often found as an occupational disease. Personal protective equipment (PPE) is a safety tool that must be used by workers to protect the body from possible potential hazards in the workplace. Construction workers, woodworkers, batik workers, and farmers can take preventive measures to avoid the occurrence of OCD. This study aimed to determine the relationship between the length of work and the use of PPE on the incidence of OCD on the hands of woodworkers in Serenan, Klaten. Methods: Cross-sectional analytic observational study. A total of 30 research subjects participated in this study. The research subjects were woodworkers in Serenan, Klaten, Indonesia. Univariate and bivariate data analysis was carried out with the help of the SPSS program. Results: The majority of research subjects with OCD have worked for more than 3 years and do not use PPE. Studies show that there is a relationship between the length of work and the use of PPE with the incidence of OCD. Conclusion: There is a significant relationship between the length of work and the use of PPE on the incidence of OCD on the hands of woodworkers in Serenan, Klaten, Indonesia

    The Role of Podocyte Cells in Diabetic Nephropathy: A Narrative Literature Review

    The main cells affected in diabetic nephropathy are podocytes and proximal tubular cells. One of the main functional proteins in the podocyte slit diaphragm is podocin which podocytes need to express together with several specific proteins in the correct way for their differentiation, as well as to maintain their complex anatomy. Podocytes are highly specialized epithelial cells. They line the urinary surface of the glomerular capillary bundles. Podocytes are part of the filtration barrier along with capillary endothelial cells and GBM. Podocytes ensure selective permeability of the glomerular capillary walls. Podocin is a membrane protein with a molecular weight of 42kD which is located in the foot processes, and also forms part of the SD with the nephrin protein. Urinary podocin levels more specifically indicate ongoing glomerular damage because they were significantly increased in the normoalbuminuria group compared with the control group

    Male Breast Cancer in Dr. Hasan Sadikin General Hospital: A Serial Case

    Background: Male breast cancer is a rare case. It accounts for 1% of all breast cancer cases. Both female and male breast cancers were influenced by the same risk factors. Male breast cancer patients are typically associated with advanced stages, higher grades, higher prevalence of hormone receptor-positive, and a worse prognosis. Many factors can explain the late diagnosis in men: absence of screening programs, lack of awareness about the condition by the male population, embarrassment due to a stigmatization of the disease, and misjudgment by primary care physicians. The aim of study is to improve the knowledge about the biology tumor and characteristics, risk factor, and management of male breast cancer. Case presentation: Two patients who were studied were a man with 50 years old, without malignancy on their family and testicles disease. First patients was diagnosed with mucinous adenocarsinoma grade II on right breast luminal A subtype T4bN0M0 and second patient was diagnosed with Ductal carcinoma in situ on right breast luminal B Her2 negative subtype TisN0M0 as preoperative diagnosis and Invasive solid papillary carcinoma grade III on right breast T2N0M0 as post operative diagnosis. Both of them were operated with modified radical mastectomy. First patient was given neoadjuvant chemotherapy, adjuvant chemotherapy and tamoxifen as endocrine therapy. Second patient was given adjuvant chemotherapy and tamoxifen as endocrine therapy.   Conclusion: Both of patients on serial cases are male breast cancer with hormone receptor positive and they were given endocrine therapy. The majority characteristic of male breast cancer was hormone receptor-positive. Management of male breast cancer is similar overall to management of female breast cancer in modality surgery, chemotherapy, endocrine therapy, and radiotherapy

    Intensive Management of Spontaneous Basal Ganglia Hemorrhage: A Case Report

    Background: Simultaneous non-traumatic bilateral basal ganglia haemorrhage is an infrequent entity. ICH accounts for 10 to 20 % of all cerebrovascular events in the US, with 30-day mortality up to 40%. The predisposing factors and pathophysiological processes leading to simultaneous bilateral basal ganglia development are not well known. Case presentation: A 43-year-old female patient was diagnosed with spontaneous intracerebral haemorrhage in the left basal ganglia and intraventricular haemorrhage. The patient underwent an urgent ICH evacuation craniotomy. After the procedure, the patient was treated in the intensive care unit. The patient was intubated for 4 days. Then, on the fifth day of stay in the ICU, a tracheostomy was performed on the patient. Conclusion: Spontaneous ICH has high mortality and morbidity. ICH management is mainly on timely targeted blood pressure management, effective and rapid reversal of coagulopathy to prevent hematoma expansion, and uncompromising intracranial pressure management

    The Relationship Between High-Sensitivity C-Reactive Protein (hs-CRP) and Diastolic Dysfunction in Geriatrics Patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia

    Background: Systemic inflammation associated with high hs-CRP levels constitutes one pathway through which diastolic dysfunction develops in older adults. This study aims to determine the relationship between high-sensitivity c-reactive protein (hs-CRP) and diastolic dysfunction in older adults at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: This research is an observational analytical study using a cross sectional design. The research was conducted at the geriatrics polyclinic at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia from June to August 2023. A total of 65 people took part in this study. The relationship between hs-CRP and diastolic dysfunction was analyzed using SPSS version 25, univariate and bivariate. Results: Hs-CRP had a significant relationship with diastolic dysfunction on a numerical scale (p=0.012), with a median value of 1.6 mg/dL in subjects with diastolic dysfunction and 0.9 mg/dL in subjects without diastolic dysfunction. On the categorical scale, the hsCRP less than 1 mg/dL group found nine subjects (23.7%) with diastolic dysfunction and 14 subjects (51.9%) without diastolic dysfunction, the hsCRP 1-3 mg/dL group found 21 subjects (55.3%) with diastolic dysfunction and 12 subjects (44.9%) without diastolic dysfunction, in the hsCRP more than 3 mg/dL group (p=0.026) there were eight subjects (21.1%) with diastolic dysfunction and one subject (3.7%) without dysfunction diastolic. Conclusion: There is a significant relationship between hsCRP and left ventricular diastolic dysfunction in geriatrics patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia

    Background: Fluor albus, also known as leucorrhoea, vaginal discharge, or leukorrhea, is the name given to fluids that are secreted from the genitals that are not blood. Pathological fluor albus can be caused by an infection in the lower genitalia or in areas that are more proximal. This study aimed to determine the pattern of microorganisms that cause pathological fluor albus at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: This study was a descriptive observational study and used primary data from a direct examination of research subjects at the obstetrics and gynecology polyclinic of Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. A total of 63 research subjects participated in this study. Analysis of microorganism patterns was carried out univariately using SPSS software. Results: The majority of microorganisms that cause pathological fluor albus are Gardnerella vaginalis. Meanwhile, in non-pathological conditions of fluor albus, the majority of causative microorganisms are E. coli. Conclusion: Gardnerella vaginalis is the most common causative microorganism of pathological fluor albus disorders at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia

    Management of Anesthesia in Patient with Osteogenesis Imperfecta Type IV and Fractures on Tibia and Fibula: A Case Report

    Background: Osteogenesis imperfecta (OI), known as brittle bone disease, is a rare inherited and genetic skeletal disorder characterized by a significantly heightened risk of fragility fractures. About 1 in 15 to 20.000 births are affected with OI. these defects may moreover be combined with an airway that's troublesome to oversee, platelet dysfunctions, and other issues. The perioperative administration of anesthesia speaks to a significant challenge. Subsequently, it is critical to be able to anticipate the dangers during the perioperative period and to define and actualize particular high-quality anesthesia administration plans for such patients when they involve injury. Case presentation: We present a case of a female patient 26 years old with known OI type IV who underwent Osteotomy and Open Reduction Internal Fixation with plate and screw (ORIF PS) on her left Tibia and Fibula. In this case, we present the anesthesia technique with general anesthesia using a metal wire-enforced endotracheal tube to prevent partial obstruction due to kyphoscoliosis. Conclusion: It is crucial to assess the severity of the disease, identify risk factors in advance, and optimize the preoperative health status of individuals with OI

    Trends in Tongue Cancer in Tertiary Hospital in West Java for 5 Years: A Retrospective Study

    Background: Oral cavity malignancy is the most common carcinoma of the head and neck. In the Western world, the tongue and floor of the mouth are the most common sites of origin for primary squamous cell carcinoma in the oral cavity. However, data and reports about tongue cancer is still limited, especially in Indonesia. The aim of this study is to get the epidemiological profile of tongue cancer in tertiary hospital in West Java from 2017 – 2021. Methods: This study was a retrospective review of all patients with tongue cancer. The study used data from hospital information system from 2017 to 2021. Data collected were gender, age, histology, lymph node metastases. All data were analyze using statistical analysis program. Results: Of the 120 cases analyzed. Sixty-eight of the 120 cases (56.67%) were found on female. The age group with the most cases was found in group of 51-60 years (35%). Histological examination showed 59 cases (49.17%) were squamous cell carcinoma well-differentiated, 28 cases (23.33%) showed lymph node regional metastases, and one case showed distant metastases to lung. Conclusion: Tongue cancer profile in tertiary hospital in West Java mostly found in female with squamous cell carcinoma well-differentiated showed the most common. Of all cases, the spread to regional lymph nodes is only a few, not reaching thirty percent

    Management of Pulmonary Tuberculosis in Liver Cirrhosis: A Case Report

    Background: Anti-tuberculosis drugs (OAT) are metabolized in the liver and have a high potential for hepatotoxicity, and will increase the risk of drug-induced liver injury (DILI). This study aimed to describe the management of pulmonary tuberculosis in patients with cirrhosis of the liver. Case presentation: 45 years old male patient with cough with phlegm for 5 months, weight loss. The patient is also known to have cirrhosis of the liver. From the results of the bacteriological examination, it was found that TCM mtb was detected as high. The patient has risk factors for low BMI, a history of alcohol, hypo albumin, and comorbid hepatic cirrhosis. This patient was given a hepatoprotection. After liver function improved, the patient was started on anti-tuberculosis drugs with a regimen of 2 hepatotoxic drugs, namely isoniazid, rifampin, and ethambutol, given gradually and planned to be given for 9 months, with regular monitoring of liver function in the first 2 months. Conclusion: Effective management of pulmonary tuberculosis infection depends on giving OAT at the appropriate dose and duration. The selection of regimen and duration of therapy can be considered based on the condition of chronic liver disease. Careful assessment and periodic evaluation of liver function are required
