The Profile of Ocular Fibrous Histiocytoma Tumors at Dr. M. Djamil General Hospital, Padang, Indonesia
Background: Ocular fibrous histiocytoma tumors originate from histiocytic cells or primitive mesenchymal cells. A fibrous histiocytoma is basically composed of a mixture of fibroblastic cells, histiocytes, blood vessels, and collagen in various proportions. This study aimed to determine the profile of ocular fibrous histiocytoma tumors at Dr. M. Djamil General Hospital, Padang, Indonesia.
Methods: This study was a descriptive observational study. A total of 8 research subjects participated in this study. Data analysis was carried out with the help of SPSS version 24 software, and univariate analysis was carried out.
Results: The majority of subjects were female, with an age range of 41-60 years. The clinical symptoms felt by patients are the presence of a mass. The majority of study subjects had disease onset < 6 months, and tumor sizes were 1-5 cm. The majority of tumor locations are on the eyelid, and the histopathologic overview shows xanthelasma and dermatofibromas. Excision therapy is the therapy for the majority of research subjects.
Conclusion: Most types of fibrous histiocytoma tumors at Dr. M. Djamil General Hospital, Padang, Indonesia, are benign type and locally aggressive
The Effect of Giving Human Umbilical Cord Mesenchymal Stem Cells (HUMSC) on the Expression of Collagen I in Menopause Model Mice
Background: Patients with pelvic floor dysfunction (PFD) who fail conservative treatment will undergo surgery. In recent years, various therapeutic approaches for PFD using stem cells have been investigated in vivo. One component of stem cells is the human umbilical cord mesenchymal stem cell (HUMSC). It is hoped that HUMSC can modify the collagen and elastin content in the extracellular matrix, leading to improvements in pelvic floor strength and PFD.
Methods: This research is an experimental study with a randomized posttest-only controlled group design with the aim of determining the effect of giving HUMSC on the expression of Collagen I in a female mouse model of menopause. There are inclusion criteria, namely female Balb/c mice, healthy, never mated, aged 8 - 12 weeks, and body weight 18 - 22 grams. The research will be carried out with statistical analysis using SPSS; a p-value <0.05 is considered significant.
Results: The results of the analysis of the distribution of sample means show that there are significant differences between the two groups. The treatment group had a higher mean expression of type I collagen compared to the control group. In addition, variation in the control group was lower compared with the treatment group, which could be interpreted as higher consistency in the response to HUMSC treatment.
Conclusion: Human umbilical cord (HUMSC) has a positive impact on type I collagen expression in menopausal model mice
Prognostic Factors for the Occurrence of Recurrent Major Adverse Cardiac Events after Isolated Coronary Arterial Bypass Graft
Background: Coronary heart disease (CHD), commonly known as coronary artery disease (CAD), is a heart disorder that occurs when the arteries supplying blood to the heart walls experience hardening and narrowing, leading to a reduced supply of oxygen and nutrients to the myocardial tissue due to limited coronary blood flow. The treatment of CHD requires revascularization or reperfusion procedures on the coronary blood vessels responsible for supplying the heart muscle. One recommended therapy is coronary artery bypass grafting (CABG). Patients post-CABG are at risk of experiencing major adverse cardiovascular events (MACE).
Methods: This design is a cross-sectional study involving all patients who underwent CABG at Siloam Hospitals in Lippo Village and Kebon Jeruk from January to June 2023. Data analysis using a regression approach aims to evaluate the relationship between various prognostic factors and MACE occurrences, thereby identifying a cross-sectional association between these variables and MACE events.
Results: Findings from this cross-sectional study indicate a correlation between overweight and the occurrence of recurrent MACE in patients with a history of CABG (p-value = 0.037). These results suggest that individuals with obesity undergoing CABG have a higher risk of experiencing recurrent MACE in CHD.
Conclusion: The prognostic factor for recurrent CHD-related major adverse cardiovascular events (MACE) is body mass index (BMI). Monitoring body mass index (BMI) in CHD patients who have undergone CABG is essential to reduce the risk of recurring MACE in the future
Anesthesia Approach for Nephrectomy in Full-Term Neonate with Suspected Wilms Tumor and Patent Foramen Ovale: A Case Report
Background: Nephrectomy for Wilms tumor presents a considerable challenge in pediatric anesthesia. This study aimed to describe the anesthesia approach for nephrectomy in a full-term neonate with suspected Wilms tumor and patent foramen ovale.
Case presentation: A 25-day-old male neonate was referred with a longstanding intraabdominal mass present since birth. Echocardiography revealed a patent foramen ovale with a diameter of 2.7 mm and a left-to-right shunt. The patient also had a diagnosis of partial ileus obstruction, likely caused by the tumor's pressure on the left kidney. Preoperative fasting adhered to a "6-4-3-1" regimen. Normoglycemia was maintained throughout the perioperative period. The case was managed under general anesthesia without the use of the rapid sequence induction technique. Caudal analgesia was administered using 1.14 ml of 0.175% bupivacaine. Vigilant monitoring of blood loss, prevention of hypothermia, and effective pain management are vital aspects of the surgical procedure. For postoperative pain management, analgesia and morphine infusion were employed. The patient was then transferred to the neonatal intensive care unit (NICU) for observation for any signs of complications related to anesthesia and surgery.
Conclusion: Nephrectomy in neonates is one of the challenges of pediatric anesthesia. Pre-anesthesia preparation in relation to hypertension control, analgesia plan and postoperative care site, transfusion strategy, and management of intravascular extension
The Jarisch-Herxheimer Reaction on Secondary Syphilis Patient with Roseola Syphilitica and Human Immunodeficiency Virus Co-Infection: A Case Report
Background: The Jarisch-Herxheimer reaction is an inflammatory reaction that occurs within 12 hours after administration of antibiotic therapy for spirochaeta species like Treponema pallidum. The reaction includes fever, headache, reappearing or worsening of skin lesions, lymphadenopathy, pharyngitis, malaise, and myalgias.
Case presentation: We present a 27-year-old man complaining of high fever 4 hours after the injection of benzathine penicillin. The patient also complained of headaches, body aches and aches. There were no complaints of shortness of breath, itching, skin blisters, red eyes, or sores on the genitals. History of take any medication was denial by the patient. After treatment by giving antipyretics and corticosteroids and getting enough rest, in the 24 hours, the patient was improved.

Bronchoscopic Lung Volume Reduction as Therapy in Chronic Obstructive Pulmonary Disease (COPD) Patients with Emphysema
BLVR is a combination of non-surgical techniques for lung volume reduction performed via bronchoscopy as an alternative to LVRS in emphysema patients. The techniques most often used in BLVR are bronchial valves (EBV/IBV), coils (lung coil), and thermal vapor (BTVA), while the BioLVR and ABS techniques have begun to be abandoned. BLVR is generally beneficial in improving lung function, exercise capacity, and quality of life in patients with emphysema. There is a need to monitor post-procedure side effects and long-term follow-up to assess the effectiveness of the procedure and reduce complications
Diagnosis and Current Management of Hepatocellular Carcinoma
Hepatocellular carcinoma (HCC) is a primary malignant liver tumor originating from hepatocytes which has a very poor prognosis and is ranked the sixth most common cancer disease in the world and is ranked third in deaths caused by cancer worldwide. Symptoms of underlying liver diseases such as hepatitis and cirrhosis often disguise the diagnosis of HCC so that most cases are discovered at an advanced stage. The examination modalities commonly used in surveillance are liver ultrasound (USG) examination and measurement of alpha levels fetoprotein (AFP) with sensitivity diagnostic up to 90%. Non-invasive imaging plays an important role in objective recognition and staging enforcement diagnosis as early as possible so that the patient's prognosis is better. Treatment for early-stage HCC can be given through curative therapy such as resection, liver transplantation, and local ablation, but disease at an advanced stage causes limited options in management where governance The current focus is on systemic therapy with a focus on a combination strategy of immunotherapy or a combination of targeted therapy with immunotherapy as the first line
Renal Amyloidosis: A Narrative Literature Review
Amyloidosis is a disorder in which soluble proteins aggregate and are deposited extracellularly in tissues as insoluble fibrils, causing progressive organ dysfunction. Amyloid fibril formation begins with misfolding of amyloidogenic precursor proteins. The fibrils have a characteristic appearance by electron microscopy and produce double refraction under polarized light when stained with Congo red dye. Classification of amyloidosis is based on the precursor proteins that form amyloid fibrils and the distribution of amyloid deposition both systemically and locally. The main form of systemic amyloidosis; AL amyloid, AA amyloidosis, ATTR amyloid. The kidney is the organ most frequently involved in systemic amyloidosis. Systemic amyloidosis may originate from anomalous proteins, such as immunoglobulin light chains or serum amyloid protein in chronic inflammation or may arise from hereditary disorders. The clinical manifestations of renal amyloidosis vary with the type of amyloid protein and the location and extent of amyloid deposition. Treatment of amyloidosis should be a two-part process; managing symptoms and reducing or stabilizing amyloid protein. Treatment of amyloidosis is focused on reducing the production of amyloidogenic proteins and inhibiting their aggregation
Delayed Bochdalek Diaphragmatic Hernia Appearance and Coincidence with Pneumonia in One-Year-Old Baby: A Case Report
Background: Congenital diaphragmatic hernia (CDH) is a developmental defect in the diaphragm leading to protrusion of abdominal contents into the thoracic cavity. The incidence rates showed that this disease happened to one in 2,200-3,000 births. This study aimed to describe a case of delayed Boschdalek diaphragmatic hernia and coincidence with pneumonia in a one-year-old baby.
Case Presentation: A 12-month-old girl came to the emergency room of Dr. M. Djamil General Hospital with a chief complaint of breathlessness for 2 days before admission to the hospital. She had difficulty gaining weight, only about 200 grams per month. The symptoms are breathlessness, cyanosis, asymmetry of lung sounds, change in the position of the heart sound, and scaphoid abdomen. The chest X-ray showed the opacity loops, which are fluid-filled, suggestive left diaphragmatic hernia.
Conclusion: Pneumonia-like symptoms and signs could be an initial or coincidence with delayed presentation of Bochdalek hernia diaphragmatic
Effect of Oxytocin Massage on Uterine Involution in Postpartum Mothers at Rengasdengklok Health Center, Karawang Regency
Background. After giving birth, the uterus will normally experience involution, which is the process of shrinking and returning to its pre-pregnancy size and shape. The process of involution involves uterine contractions, decreased blood flow, and the expulsion of unnecessary tissue. However, in case of failed uterine involution, the uterus cannot return to its normal size and shape in a timely manner, and leads to bleeding. This study aimed to determine the effect of oxytocin massage on uterine involution of postpartum mothers at the Rengasdengklok Health Center, Karawang Regency, Indonesia.
Methods. This study was an experimental study, where as many as 30 research subjects participated in this study. Data analysis was performed bivariate to assess the effect of oxytocin massage on uterine involution.
Results. The results of the study showed that the control group had a slower process of uterine involution compared to the intervention group, p-value 0.003 (p<0.05).
Conclusion. It can be concluded that there is an influence between oxytocin massage and the uterine involution process
