The Effect of Snakehead Fish (Channa striata) Extract on Dry Socket Wound Healing: TGF-β1 Expression in Rats Model
Background: Dry socket or alveolar osteitis is a condition characterized by the breakdown of blood clots due to excessive fibrinolysis activity, which disrupts the wound healing process. Consumption of nutrition-rich foods such as snakehead fish (Channa striata) rich in albumin, omega-3 fatty acids, vitamins, minerals, and striatin may accelerate delayed wound healing. This study aimed to analyze the role of snakehead fish extract on the expression of TGF-β1 in the healing of dry socket wounds after tooth extraction in Wistar rats.
Methods: This study was an in vivo study with a posttest-only control group design. Forty-five Wistar rats were divided into five groups: snakehead fish extract groups with doses of 1/2, 1, 2, 4 times human dose, and a negative control. The lower left incisor teeth of the rats were extracted, then dry socket was induced with adrenaline using a paper point. On days 3, 7, and 14, TGF-β1 expression was assessed using immunohistochemical staining.
Results: This study showed significant difference in TGF-β1 expression on days 3, 7, and 14. TGF-β1 expression was higher in the treatment groups than the control (p<0.05).
Conclusion: Administration of snakehead fish extract increased TGF-β1 expression. These findings underline the complex interaction between snakehead fish extract and TGF-β1 expression
Senescence-Induced Atherosclerosis: The Potency of Senolytic Therapy
The aging process is an inevitable occurrence that involves physiological changes at the cellular level. The presence of intrinsic and extrinsic stressors can cause cellular damage, leading to senescence and premature aging. Senescent cells undergo activation of the p53/p21 and p16INK4a pathways, induce cell cycle arrest, increased expression of senescence-associated beta-galactosidase (SA-β-Gal), and secretion of SASP (senescence-associated secretory phenotype), leading to "inflamm-aging" or chronic inflammation associated with senescence. These premature aging and “inflamm-aging” accelerates the occurrence of age-related diseases, one of which is atherosclerosis. The relationship between premature aging, senescence, and atherosclerosis has been a focus of research on pathogenesis, prevention, and therapy. Recent research has emphasized the crucial role of senolytics, compounds or agents capable of eliminating senescent cells, in inhibiting the progression of atherosclerosis and slowing down premature aging. Obtaining a more comprehensive understanding of the processes and effectiveness of senolytics in premature aging and atherosclerosis should facilitate the development of more potent medicines to mitigate side effects in the management of cardiovascular disease and extend longevity
Idiopathic Pulmonary Fibrosis: A Narrative Literature Review
Idiopathic pulmonary fibrosis is a chronic progressive interstitial lung disease (ILD) with unknown causes. Male gender, age > 60 years, history of smoking with symptoms of dry cough and progressive chronic shortness of breath are typical clinical findings in this patient. The diagnosis is made based on a combination of radiological findings in the form of a pattern consistent with usual interstitial pneumonia (UIP) and/or histopathology with the exclusion of other causes of ILD. Treatment includes pharmacological and non-pharmacological therapy in the form of pulmonary rehabilitation, psychosocial support, and lung transplantation. Anti-fibrosis pharmacological therapy, namely nintedanib and pirfenidone, has been proven to slow the progression of pulmonary fibrosis and reduce mortality. The relatively low average survival rate of 3-4 years after the diagnosis is made makes this disease have a poor prognosis and requires adequate identification and treatment in order to reduce morbidity, mortality and improve the quality of life of sufferers
Efficacy of Garlic Extract (Allium sativum L.) as a Therapeutic Modality of Condyloma Acuminata: A Meta-Analysis
Background: Condyloma acuminata (CA) is a sexually transmitted infection caused by human papillomavirus (HPV). Current standard treatment has limitations, so effective and safe alternative therapies are needed. Garlic (Allium sativum L.) has long been used in traditional medicine and has potential as an antiviral and antitumor agent. This study aims to evaluate the efficacy of garlic extract as a CA therapy modality through meta-analysis.
Methods: A literature search was conducted on PubMed, Scopus, and Web of Science databases to identify randomized controlled clinical studies (RCTs) evaluating the efficacy of garlic extract in CA patients. Extracted data included study characteristics, interventions, sample size, clinical outcomes, and side effects. Statistical analysis was performed using Review Manager 5.4 software.
Results: A total of 8 RCTs involving 542 CA patients met the inclusion criteria. Meta-analysis showed that garlic extract was significantly more effective in clearing CA lesions compared with placebo (OR 2.87; 95% CI 1.95-4.23; p < 0.00001). Garlic extract also demonstrated comparable effectiveness to standard therapies such as podophyllin and imiquimod. There was no significant difference in the incidence of side effects between the garlic extract group and the control group.
Conclusion: Garlic extract is an effective and safe therapeutic modality for CA. Further studies with more robust designs are needed to confirm these findings and explore the mechanism of action of garlic extract on CA
Giant Gartner’s Duct Cyst Mimicking Pelvic Organ Prolapse: A Rare Case Report
Background: Gartner’s duct cysts are benign vaginal cysts originating from embryological remnants. While usually asymptomatic, larger cysts can mimic pelvic organ prolapse, leading to misdiagnosis and delayed treatment. This case emphasizes the importance of accurate diagnosis and appropriate surgical intervention in managing large GDCs.
Case presentation: A 43-year-old woman presented with a protruding vaginal mass, initially mistaken for pelvic organ prolapse. Examination revealed a large Gartner’s duct cyst on the posterior vaginal wall. Surgical excision and marsupialization were performed, confirming the diagnosis.
Conclusion: This case highlights the importance of thorough evaluation in women presenting with vaginal masses. Large Gartner’s duct cysts, though rare, can mimic pelvic organ prolapse, necessitating accurate diagnosis and appropriate surgical intervention
Meta-Analysis of Corticosteroids in the Management of Sepsis: Evaluating Efficacy and Safety
Background: Sepsis remains a significant cause of morbidity and mortality worldwide. Corticosteroids have been used in sepsis management, but their efficacy and safety remain debated. This meta-analysis aims to evaluate the effectiveness of corticosteroids in sepsis management.
Methods: A comprehensive search of PubMed, Embase, and Cochrane Library was conducted to identify randomized controlled trials (RCTs) published from 2018 to 2024 evaluating corticosteroid use in sepsis. The primary outcome was mortality. Secondary outcomes included length of hospital stay, duration of mechanical ventilation, and adverse events. Data were pooled using a random-effects model, and heterogeneity was assessed using the I² statistic.
Results: Twenty-one RCTs (n=12,350 patients) were included. Corticosteroid therapy was associated with a significant reduction in mortality (risk ratio [RR] 0.87, 95% confidence interval [CI] 0.79-0.96, p=0.004). There was also a significant reduction in the length of hospital stay (mean difference [MD] -1.5 days, 95% CI -2.3 to -0.7, p<0.001) and duration of mechanical ventilation (MD -1.2 days, 95% CI -1.9 to -0.5, p<0.001). No significant increase in adverse events was observed.
Conclusion: This meta-analysis suggests that corticosteroid therapy is associated with a significant reduction in mortality, length of hospital stay, and duration of mechanical ventilation in patients with sepsis. The benefits appear to outweigh the risks. Corticosteroids should be considered as part of the standard of care in sepsis management
Factors Affecting the Response of Triple-Negative Breast Cancer (TNBC) to Neoadjuvant Chemotherapy: A Meta-Analysis
Background: Triple-negative breast cancer (TNBC) is an aggressive subtype with limited treatment options. Neoadjuvant chemotherapy (NAC) is often used to downstage tumors before surgery, but response rates vary. This meta-analysis aims to identify factors that influence TNBC’s response to NAC.
Methods: PubMed, Embase, and Cochrane Library databases were searched (2018-2024) for studies evaluating factors associated with TNBC response to NAC. Data on patient demographics, tumor characteristics, NAC regimens, and response rates were extracted. Pooled odds ratios (ORs) and 95% confidence intervals (CIs) were calculated using random-effects models.
Results: Twenty-three studies (n=4,512 patients) were included. Younger age (OR 1.82, 95% CI 1.35-2.46), smaller tumor size (OR 0.58, 95% CI 0.41-0.82), lower clinical stage (OR 0.39, 95% CI 0.27-0.56), and absence of lymph node involvement (OR 0.42, 95% CI 0.31-0.57) were associated with improved pathological complete response (pCR) rates. NAC regimens containing platinum agents (OR 2.15, 95% CI 1.54-2.99) and immune checkpoint inhibitors (OR 1.78, 95% CI 1.23-2.58) also showed higher pCR rates.
Conclusion: This meta-analysis identified several factors associated with improved TNBC response to NAC, including younger age, smaller tumor size, lower clinical stage, absence of lymph node involvement, and use of platinum-based or immunotherapy-containing NAC regimens. These findings can inform patient selection and treatment optimization for NAC in TNBC
Ultrasound vs Nerve Conduction Studies: A Comparative Analysis in Carpal Tunnel Syndrome Diagnosis
Background: Carpal tunnel syndrome (CTS) is a prevalent entrapment neuropathy. While nerve conduction studies (NCS) remain the gold standard for diagnosis, ultrasound (USG) offers a non-invasive alternative. This study aimed to compare the diagnostic accuracy of USG and NCS in CTS at Dr. Mohammad Hoesin General Hospital Palembang, Indonesia.
Methods: A cross-sectional study was conducted on patients presenting with CTS symptoms. Demographic and clinical data were collected. NCS and USG assessments were performed, blinded to each other's results. USG measurements included the cross-sectional area at the carpal tunnel inlet (CSAc), proximal to the carpal tunnel (CSAp), and the difference between them (∆CSA). Diagnostic accuracy was calculated, and agreement was assessed using Cohen's kappa.
Results: A total of 86 wrists from 49 patients were included. The mean age was 52 ± 11 years, with a female predominance (86%). The majority had mild CTS based on NCS (55.8%). USG measurements showed mean CSAc of 13.1 ± 3.5 mm², CSAp of 10.6 ± 3.0 mm², and ∆CSA of 2.5 ± 0.9 mm². ∆CSA had the highest sensitivity (92.2%), specificity (88.9%), and accuracy (91.9%), with substantial agreement with NCS (Kappa = 0.65).
Conclusion: USG, particularly using ∆CSA, demonstrates high diagnostic accuracy in CTS, comparable to NCS. It can serve as a valuable tool, especially in settings with limited NCS availability
Aspirin's Antiplatelet Effects Promote Arteriovenous Fistula Maturation in Hemodialysis Patients: A Systematic Review
Background: Arteriovenous fistula (AVF) is the preferred vascular access for hemodialysis, offering superior long-term patency and lower infection rates compared to other options. However, AVF maturation remains a significant challenge. This systematic review aims to evaluate the efficacy of aspirin in promoting AVF maturation and preventing failure in hemodialysis patients.
Methods: A comprehensive search of electronic databases, including ScienceDirect, SpringerLink, PubMed, Cochrane, and ProQuest, was conducted from 2014 to 2024. Studies investigating the impact of aspirin on AVF maturation and failure were included. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were followed.
Results: Five studies met the inclusion criteria, encompassing a total of 982 participants. Four studies indicated that aspirin significantly enhanced AVF maturation by increasing flow volume and promoting the development of a robust fistula. However, one study found that aspirin did not significantly reduce the risk of thrombosis or AVF failure.
Conclusion: Aspirin appears to be a promising adjunct therapy for promoting AVF maturation in hemodialysis patients. Its antiplatelet effects, primarily through cyclooxygenase inhibition and reduced platelet activation, contribute to improved flow volume and fistula maturation. While the evidence for aspirin's role in preventing AVF failure is mixed, its potential benefits in enhancing maturation warrant further investigation
Challenges in the Management of Pulmonary Contusion with Multiple Rib Fractures and Associated Injuries: A Case Report
Background: Pulmonary contusion, a common consequence of chest trauma, presents significant management challenges, especially when associated with multiple rib fractures and other injuries. These cases often require intensive care due to the high risk of complications and mortality. This report presents a case of a patient with pulmonary contusion, multiple rib fractures, and cerebral contusion to illustrate the complexities involved in managing such cases.
Case Presentation: A 44-year-old male was admitted to the intensive care unit (ICU) following a motor vehicle accident. He presented with decreased consciousness, multiple rib fractures, severe lung contusion, and cerebral edema. The patient's condition was further complicated by the development of respiratory distress and hemodynamic instability.
Conclusions: The management of pulmonary contusion with multiple rib fractures and associated injuries requires a multidisciplinary approach with careful attention to respiratory support, hemodynamic management, pain control, and early intervention for complications
