The Relationship of Breast Cancer Subtypes with the Event of Metastasis in Dr. M. Djamil Hospital Padang
Background: One of the high mortality rates from breast cancer is related to the incidence of metastases. It is known that >90% of deaths in breast cancer are related to the incidence of metastases and the complications that follow. Breast cancer is divided into several subtypes based on the expression of receptor genes in breast cancer tissue, namely Luminal A, Luminal B, HER 2 and Triple Negative Breast Cancer (TNBC). This study aims to determine the relationship between breast cancer subtypes and the incidence of metastases in Dr. M. Djamil Padang.
Methods: This study used a retrospective case-control study to breast cancer patients with metastatic at Dr M Djamil Hospital, Padang from 2016-2021. The research subjects were 260 breast cancer patients who met the inclusion criteria. The study subjects were divided into 130 patients as the case group with metastases and 130 patients as the control group with no metastases. To determine the relationship between breast cancer subtypes and the incidence of metastases, the chi-square test was used. If the p value <0.05, it can be concluded that it is significant. Furthermore, analysis is continued to obtain an odds ratio (OR) in identifying risk opportunities with Cochran's and Mantle-Haenszel statistics common odds ratio estimate. The data were analysed using the Statistical Package for Social Sciences (SPSS) program.
Result: Characteristics of the subjects in this study can be seen that there is a relationship between hormonal contraception, T and N status with the incidence of metastasis (p <0.05). Patients with metastases were more common with breast cancer subtypes luminal B (61.5%), HER2+ (21.5%), TNBC (14.6%) and luminal A (2.3%). The most common locations for breast cancer metastases were lung (48.5%), bone (26.2%), liver (19.2%), brain (5.4%) and other places (0.8%). There was a relationship between breast cancer subtypes and the incidence of metastasis (p<0.038). The highest risk of metastases was in patients with TNBC subtype with OR = 7.74 (95% CI 1.72-34.79). There was no relationship between breast cancer subtypes with metastatic location (p>0.05) and breast cancer subtypes TNBC had a risk (OR) of 9.60 (95% CI 1.96-47.14) times increasing the risk of metastases in brain.

Analysis of Neuron Specific Enolase Serum Levels in Traumatic Brain Injury
Background : Traumatic brain injury is still the main cause of death and disability in productive age. Assessment the level of consciousness and imaging examinations after a brain injury can not always describe the severity of damage in the brain, this is because the pathological process is still ongoing due to secondary brain injury. Therefore, it is necessary to examine biomarkers that can describe the severity of the pathological process that occurs. The purpose of this study was to assess serum neuron-specific enolase (NSE) levels and their relationship to the severity and outcome of a traumatic brain injury.
Methods : A cross sectional design was conducted in the emergency department of DR M Djamil Hospital, Padang. There were 72 patients who met the inclusion criteria. A Glasgow Coma Scale examination was performed to assess the severity of brain injury and examination of NSE serum levels at 48 hours post- injury using ELISA technique and assess the Glasgow outcome scale (GOS) at 6 weeks post-injury. Data analysis using SPSS 22 program, the results are significance if the p value <0.05
Results : The average NSE level was higher in severe brain injuries than moderate and mild brain injuries and this difference was statistically significant (p<0.05). The NSE serum levels were higher in poor outcomes than in good outcomes and this difference was statistically significant (p<0.05).
Conclusion : High NSE serum levels in the acute phase were associated with the severity of the brain injury and poor outcome 6 weeks after the brain injury. 
The Efficacy of Genistein-Rich Edamame as a Prevention of Atherosclerotic Lesion in Abdominal Aorta: Study in Rats Model of Atherosclerosis
Background: Atherosclerosis is the main cause of ischemic heart disease which leads to death for people aged more than 40 years old. Genistein is an important isoflavone compound which may protect the blood vessels from endothelial injury. This research is to observe the efficacy of genistein rich edamame as a prevention for atherosclerotic abdominal aortic lesions that seen from abdominal aortic thickness and foam cells count.
Method: Thirty rats were divided randomly into five groups and treated for 28 days. The negative control group was given food and drink ad libitum. The positive control group was induced for atherosclerosis using adrenaline 0.006 mg / 200 gr BW injected on the first day and then the next day was given egg yolk 5 gr / 200 gr BW on next day for 28 days. All of the treatment groups were induced for atherosclerosis and treated with genistein-rich edamame extract 5 mg / 200 gr BW, edamame extract 38 mg / 200 gr BW and atorvastatin 1.5 mg / 200 grBW. Data were analyzed using One Way ANOVA - post hoc Bonferroni test, Kruskal Wallis - Mann Whitney test, and Pearson correlation test.
Results: There were significant differences (p<0,001) in abdominal aortic thickness and foam cells count between positive control group and treatment genistein-rich edamame extract, edamame extract and atorvastatin. There was a significant correlation between the abdominal aortic wall thickness and foam cells count (correlation coefficient value 0,753; p<0,001).
Conclusion: The administration of genistein-rich edamame extract can prevent the thickening of abdominal aorta and foam cell formation. Genistein-rich edamame can prevent foam cells formation better than atorvastatin.
 
Diagnosis, Treatment, and Prognosis of Syphilis in HIV Patient
Syphilis is a sexually transmitted infection caused by the bacterium Treponema pallidum (T. Palladium). Human Immunodeficiency Virus (HIV) is a virus that attacks the body's immune system, decreased the number of CD4 cells that makes the immune system decrease, and facilitates syphilis infection. Syphilis increased the risk of spreading HIV infection with skin barrier destruction. The diagnosis of syphilis is made based on a complete history, including sexual history, clinical manifestations, and examination. The treatment of syphilis with HIV and without HIV similar, penicillin is still the gold standard for syphilis treatment with HIV. Prevention of syphilis with HIV are educated, screening for syphilis and other sexually transmitted infections, as well as early detection and therapy of partners. The prognosis for syphilis in HIV is worse than for syphilis without HIV
Coronary Microvascular Dysfunction
Coronary microvascular dysfunction (DMK) is a condition of patients who are accompanied by complaints of chest pain where the results of coronary angiography examination are normal and this is almost 49% with 60% of patients diagnosed with DMK. Another study said that about 40% of patients with DMK showed coronary flow reserve (CFR <2) of about 40% and the WISE study (Women's Ischaemia Syndrome Evaluation) showed that about 47% of patients with chest pain had normal coronary arteries. DMK can be divided into 4 groups; DMK with no coronary arterial disease (CAD) obstruction and myocardial disease, DMK with myocardial disease where this occurs due to remodeling of intramural coronary arteries, DMK with CAD (coronary arterial disease) or acute myocardial infarction with or without ST segment, iatrogenic typhoid DMK occurs after coronary recanalization caused by vasoconstriction and distal embolization. The mechanism of action of DMK can be caused by several factors, namely endothelial dysfunction, smooth muscle dysfunction, decreased diastolic perfusion time, damage to blood vessels and damage to the vascular and microvascular remodeling. And to enforce this DMK, there are several tests carried out in diagnosing the disease, some of which are invasive and non-invasive so that by enforcing the diagnosis of this disease, treatment for DMK can be done immediately and optimally
Abnormal Uterine Bleeding in Adolescent
Abnormal uterine bleeding (AUB) is defined as the abnormal bleeding from the uterine corpus in term of duration, volume, frequency and/or regularity. This condition occurs in 37% of adolescents and may affect the quality of life and increased hospitalization. Etiology is divided into structural and non-structural causes, known as PALM-COEIN. The most common etiology in adolescents is anovulatory menstruation due to immature hypothalamus-hypophysis axis. Diagnostic evaluation should include investigation in the etiology of AUB, anemia signs, and hemodynamic status. Treatment of AUB consists of hormonal and non-hormonal therapy. Therapy in adolescent is given based on the severity of bleeding, grading of anemia, and hemodynamic stability. Follow-up is required after therapy. Understanding AUB in adolescents can help clinicians deliver appropriate and comprehensive treatment. A Case of Whistle as Foreign Body in Trachea
Background. Most of foreign body aspiration cases are found in children under the age of fifteen. Pediatric patients often presents with non-food foreign body aspiration, such as toys. The most common clinical manifestation are history of choking following foreign object insertion into the mouth (85%), paroxysmal cough (59%), wheezing (57%) and airway obstruction (5%).
Case presentation. Main principle of airway foreign body extraction is to do it immediately in the most optimal condition with slightest possible trauma. Rigid bronchoscopy is a suitable choice for tracheal foreign body extraction. We reported a case of seven years old male with tracheal foreign body presented with history of whistle ingestion five hours prior to admission. This patient was discharged from hospital after third days of rigid bronchoscopy procedure.
Conclusion. History of foreign body aspiration in children should be suspected as a tracheobronchial foreign body. Rigid bronchoscopy is preferred to extract foreign bodies present in the trachea. The prognosis for tracheobronchial foreign body aspiration is good if the foreign body is treated early and without complications
Screening for Cervical Cancer
Screening is a public health intervention administered to a target population without symptoms. Screening is not performed to diagnose a disease, but to identify individuals with a higher likelihood of developing the disease itself or a precursor to the disease. Not all diseases are suitable for screening programs. The following criteria help determine whether a disease is suitable for a screening program: (1) The disease is bound to have serious consequences. (2) The disease must have a detectable preclinical and asymptomatic stage. (3) Treatment at the preclinical stage should influence the long-term course and prognosis of the disease being screened. (4) Care must be available and accessible to those who have a positive screening test. History, screening tests and treatment options for cervical pre-cancer meet these criteria
Comparison of Germ Patterns and Antimicrobial Susceptibility in Chronic Suppurative Otitis Media with Cholesteatoma and without Cholesteatoma in Dr. Mohammad Hoesin Hospital.
Introduction: Chronic suppurative otitis media (CSOM) is a chronic inflammation of the middle ear and mastoid cavity. It is necessary to identify bacterial patterns and antibiotics susceptibility in CSOM. Appropriate administration of antibiotics is essential to treat infection and prevent widespread resistance.
Objective: The aim of this study is to compare the pattern of germs and antibiotic susceptibility between CSOM with cholesteatoma and without cholesteatoma.
Methods: Observational study with cross-sectional design. Data collection was carried out using medical record data on 102 subjects who underwent middle ear swab and bacterial culture from January to December 2019.
Results: From 102 subjects and 122 bacterial culture results. it was found that Pseudomonas aeruginosa (36.1%) was one of the most common organisms to cause CSOM. followed by Staphylococcus haemolyticus. Staphylococcus epidermidis and Staphylococcus aureus. There was a significant difference between the germ pattern and the type of CSOM (p = 0.002). Amikacin. meropenem. gentamicin. cefepime. ciprofloxacin and ceftazidime were found to be the most effective antibiotics for Pseudomonas aeruginosa. The rate of resistance to tigecycline and cefazoline were highest. Age. sex. nutritional status and comorbidities were not factors that significantly influenced the incidence of cholesteatoma in CSOM.
Conclusion: There is a significant difference between the germ pattern in both CSOM groups. Patients that show Gram negative have a significant effect on the incidence of CSOM with cholesteatoma
Correlation of Serum IL-6 Levels with Muscle Mass, Muscle Strength, and Physical Performance in Elderly Gymnastics Community in Mohammad Hoesin General Hospital Palembang
Introduction. Old age is related to geriatric syndrome where there are several health problems that often occur related to a decrease in body function and an increase in the inflammatory process. Decrease in muscle mass, muscle strength and physical performance will cause a condition of sarcopenia and frailty syndrome in the elderly. The importance of prevention so that sarcopenia does not occur in the elderly by evaluating the factors that can cause it, one of which is controlling interleukin 6 (IL-6) as proinflammatory. The purpose of this study was to determine the correlation between serum IL-6 levels with muscle mass, muscle strength and physical performance in the elderly gymnastics-community in Mohammad Hoesin General Hospital, Palembang.
Methods. This research is an observational analytic study with a cross-sectional approach which was conducted at the Integrated Geriatric Clinic Internal Medicine RSMH Palembang from November 2019 to November 2020. A sample of 31 people aged > 60 years was examined for muscle mass, muscle strength, physical performance and IL-levels. 6 serum. All data processing and analysis in this study used the SPSS version 25 for Windows program.
Results. of the 31 subjects, 27 were female (87.1%) and 4 male (12.9%). The mean value of muscle mass is37.9 kg/m2, muscle strength 23 kg, and physical performance 6.1 m/sec.There were 28 (90.3%) samples with elevated serum IL-6 levels, while 3 (9.7%) samples had normal levels. There was a significant negative correlation (p = 0.000) with a strong correlation strength (r = -0.722) between serum IL-6 levels and muscle mass and a significant negative correlation (p = 0.050) with a weak correlation strength (r = -0.354) between IL levels -6 serums with physical performance.
Conclusions. There was a significant negative correlation between serum IL-6 levels and muscle mass and physical performance. So, it is very important to prevent an increase in serum IL-6 levels so that the elderly do not fall into a state of sarcopenia
