Efektivitas Insecticide Treated Nets (ITNs) dan Intermitten Preventive Treatment (IPT) pada pencegahan malaria dalam kehamilan
Tujuan: Mengetahui efektivitas Insecticide Treated Nets (ITNs) dan
Intermitten Preventive Treatment (IPT) pada pencegahan malaria dalam kehamilan.
Bahan dan cara kerja: Literature review sejumlah kepustakaan dan
jurnal yang meneliti tentang efektivitas ITNs dan IPT dalam pencegahan malaria dalam kehamilan.
Hasil: Dilakukan penelaahan terhadap 16 buah kepustakaan dan jurnal mengenai malaria dalam kehamilan dan penelitian-penelitian mengenai penggunaan ITNs dan IPT dalam pencegahan malaria dalam kehamilan.
Dari semua metode mencegah gigitan nyamuk, tidur dengan
ITNs kemungkinan adalah yang paling efektif. Studi yang telah dilakukan menunjukkan bahwa di antara gravida 1 - 4 penggunaan ITNs, menurunkan kejadian parasitemia sebanyak 38% dan anemia berat sebanyak 47%. Pada saat persalinan, rata-rata kadar hemoglobin 0,6 g/dl lebih tinggi, dan prevalensi malaria plasental dan maternal diturunkan sebanyak 35%. Sedangkan bayi baru lahir, ITNs membantu dengan cara menurunkan insiden berat lahir rendah (prevalensi berat lahir rendah diturunkan sebanyak 28% pada gravida 1 - 4), menurunkan risiko anemia pada bayi baru lahir, menurunkan risiko kematian bayi baru lahir, meningkatkan pertumbuhan dan perkembangan selama kehamilan dan minggu-minggu pertama usia kelahiran. Untuk IPT, studi juga menunjukkan strategi ini efektif dalam pencegahan malaria dalam kehamilan karena dapat menurunkan risiko anemia berat pada primigravida yang
tinggal di area malaria. Bahkan perempuan yang baru mendapat dosis
satu kali oleh karena terlambat memeriksakan kehamilannya, secara
signifikan mendapat manfaat dari intervensi ini.
Kesimpulan: Perempuan hamil termasuk grup penduduk yang paling
be-risiko terkena infeksi malaria. Oleh karena beratnya komplikasi yang bisa terjadi pada malaria dalam kehamilan dan banyak perempuan hamil yang tidak memiliki akses pada intervensi yang efektif maka pencegahan malaria dalam kehamilan merupakan hal yang sangat penting. Malaria dalam kehamilan dapat dicegah dengan cukup efektif menggunakan ITNs dan pemberian IPT. Bila terjadi infeksi malaria dalam kehamilan, penting untuk menentukan apakah tanpa atau dengan komplikasi karena yang tanpa komplikasi dapat diobati dengan mudah, sedangkan yang dengan komplikasi perlu manajemen yang spesialistik di tempat pelayanan tersier.

Deteksi Human Papilloma Virus pada sediaan sitologi Papanicolaou Smear lesi serviks: suatu uji diagnostik
Tujuan: Untuk mengetahui akurasi diagnostik sitologik serviks dalam mendeteksi infeksi HPV pada lesi serviks.
Tempat: Pusat Kesehatan Masyarakat yang ada di Bangli (Kecamatan Tembuku dan Susut) dan Gianyar (Kecamatan Payangan), Bali.
Bahan dan cara kerja: Penelitian ini memakai uji diagnostik. Pemeriksaan Pap Smear dilakukan pada semua perempuan yang sudah menikah atau sudah pernah melakukan hubungan seksual. Hasil pemeriksaan
tes pap tersebut dikirim ke Laboratorium Patologi Anatomi
FK UNUD/RS Sanglah Denpasar untuk pemeriksaan sitologi dan ke Laboratorium Biologi Molekuler Universitas Leiden Belanda untuk pemeriksaan PCR.
Hasil: Akurasi pemeriksaan sitologi sebesar 75%, sensitivitas sebesar 67,8%, spesifisitas sebesar 77,3%, nilai prediksi positif sebesar 50%, nilai prediksi negatif sebesar 87,8%, nilai positif palsu sebesar 50%, dan nilai negatif palsu sebesar 12,2%.
Kesimpulan: Pemeriksaan sitologi dapat dipakai sebagai salah satu cara untuk mendeteksi infeksi HPV. [Maj Obstet Ginekol Indones 2009; 33-1: 41-7]

Perbedaan prevalensi anemia defisiensi besi pada perempuan hamil di daerah pantai dan pegunungan di wilayah Semarang
Tujuan: Mengetahui karakteristik prevalensi anemia defisiensi besi pada perempuan hamil di daerah pantai dan pegunungan di wilayah Semarang serta membuktikan perbedaan prevalensi anemia defisiensi besi pada perempuan hamil di daerah pantai dan pegunungan di wilayah Semarang.
Rancangan/rumusan data: Studi potong lintang yang melibatkan 50 perempuan hamil trimester kedua di daerah pantai dan 50 perempuan hamil trimester kedua di daerah pegunungan di 7 Puskesmas Semarang Kota dan 5 Puskesmas Kabupaten Semarang pada kurun waktu bulan April - Juni 2008.
Bahan dan cara kerja: 50 perempuan hamil trimester kedua di daerah pantai dan 50 perempuan hamil trimester kedua di daerah pegunungan dilakukan pengambilan darah untuk kemudian dilakukan pemeriksaan hemoglobin dan leukosit dengan metode spektrofotometer menggunakan alat Nihon Cohden, serta serum feritin dengan metode Ellisa menggunakan alat Elx 800 Universal Microplate Reader di laboratorium Gaky RS. Dokter Kariadi Semarang. Hasil laboratorium didapatkanprevalensi anemia defisiensi besi pada perempuan hamil trimester kedua di dearah pantai dan pegunungan kemudian dilakukan analisis. Analisis statistik yang dilakukan berupa analisis univariat untuk menyajikan distribusi frekuensi berupa rerata dan simpang baku, serta analisis bivariat dengan melakukan uji beda X2, uji t dan uji Mann-Whitney.
Hasil: Rerata kadar hemoglobin perempuan hamil trimester kedua di daerah pegunungan (11,3 mg/dl) lebih tinggi dibanding dengan daerah pantai (10,6 mg/dl), dan secara statistik bermakna (p = 0,009). Rerata kadar serum feritin di daerah pegunungan (13,6 ng/dl) cenderung lebih tinggi dibanding dengan daerah pantai (11,9 ng/dl) namun secara statistik tidak bermakna. Prevalensi anemia di daerah pegunungan (24%) lebih rendah dibanding daerah pantai (52%), dan secara statistik bermakna (p = 0,007). Prevalensi anemia defisiensi besi di daerah pegunungan (18%) lebih rendah dibanding dengan daerah pantai (42%), dan secara statistik bermakna (p = 0,016).
Kesimpulan: Prevalensi anemia defisiensi besi pada perempuan hamil trimester kedua di daerah pantai lebih tinggi dibandingkan daerah pegunungan di wilayah Semarang, berturur-turut 42% dan 18%.

Effectiveness of psychoeducation of postpartum depression (a study in health services in subprovince Nias, North Sumatra)
Objective: To evaluate the effectiveness of psycho-education intervention
which is given to postpartum mothers in preventing the
existence of postpartum depression and its relation to age, parity,
occupation, education and family support.
Methods: This research used a quasi experimental with post test
only design. Research was implemented at public hospital of
Gunung Sitoli and five community health centres in Sub-Province of
Nias taken by random on since May - July 2006. There were 85
mothers with normal childbirth as samples. They were accompanied
by their husband, condition of mothers and their babies were
healthy without any complication. This research used an Edinburg
Pascasalin Depression Scale questionnaire as instrument and psycho-
education intervention was given with a booklet as media.
Results: 85 subjects were included, 40 were intervention group
and 45 control group. This research showed that psycho-education
intervention was effective significantly (p= 0,001), Odds Ratio
(OR): 5,924 (95 % CI : 2,081-16,868) to prevent the postpartum depression
occurrence. The most dominant variable in effectiveness of
psycho-education intervention is family support variable with pvalue
is p = 0,001, OR : 80 (95 % CI : 6,069-1054,570).
Conclusion: Psycho-education intervention is very effective for
preventing the postpartum depression occurrence after controlled
by family support factor.

The correlation between serum protein Bcl-2 and caspase 3 levels as a risk factor in abortion
Objective: To determine the correlation between serum protein
Bcl-2 with caspase-3 levels on normal pregnancy and abortion, and
the strength of serum protein Bcl-2 with caspase-3 levels as means
for diagnostic test.
Methods: A case control study was carried out. Samples were
obtained from the serum of incomplete abortion patients and normal
pregnancy as control. Serum protein Bcl-2 and caspase-3 levels
were examined with ELISA method.
Result: Mean Bcl-2 level in subjects with abortions is 7.263 (SD
1.71) ng/ml, lower than normal pregnancies, which is 7.490 (SD
3.938) ng/ml, with significant different (p=0.050). Mean caspase-3
in subjects with abortions is 112.74 (SD 152.73) mg/ml, higher than

The Rise of Inhibin A Serum Level in Preterm Labor
Objective: The purpose of this study was to measure inhibin A serum level in women with preterm delivery, thus determining any correlation between inhibin A serum and gestational age in preterm delivery.
Method: The design of our study was cross sectional with 36 subjects who came to Dr. Hasan Sadikin Hospital, Bandung and six satellite hospitals in July-August 2011 and met inclusion-exclusion criteria. Inhibin A serum level was measured with ELISA. Inhibin A serum level in preterm labor and was compared using independent t test, and correlation between inhibin A serum level and gestational age in patients with preterm labor was calculated using Pearson correlation test.
Result: Characteristics test in both groups showed that both are homogeneous and comparable. The mean inhibin A serum level in preterm labor was higher (845.733 pg/ml) compared with preterm gestation (568.203 pg/ml) (p = 0.025). There was a significant correlation between inhibin A serum level and gestational age in preterm labor (p = 0.023) with a correlation coefficient of 0.38, indicating a moderate positive relationship.
Conclusion: Inhibin A serum level in preterm labor was higher than preterm pregnancy. In preterm labor, inhibin A serum level increases with gestational age.

Distention Media in Hysteroscopy for Diagnostic and Operative Procedure
Objective: Hysteroscopy is the "gold-standard" procedure used
to describe the morphology of uterine cavity and the presence of intrauterine lesions and it is a minimally invasive intervention that
can be used to diagnose and treat many intrauterine and endocervical
problems. Hysteroscopy requires uterine distention for the effective
visualization of the uterine cavity and the clearing of blood
and tissue debris. Options for uterine distention include insufflation
with carbon dioxide (CO2) gas, and instillation with electrolytic and
nonelectrolytic liquid distention media. In this review, we would like to
review known available distending media and its characteristics for diagnostic and operative hysteroscopy.
Method: Literature review.
Conclusion: Carbon dioxide and normal saline are the most
preferable distention media for diagnostic hysteroscopy. There is no
significant difference between these medium in terms of visualization
quality, but most practitioners prefer to use normal saline because
of it’s availability and acceptability, quick performance,
fewer additional procedures, more satisfaction rate, and good visualization.
Low viscosity fluids are the most preferable media for operative
hysteroscopy. Low viscous-electrolytic fluids, mostly normal
saline is recommended in operative cases using mechanical, laser
or bipolar energy that requires no electricity. Nonelectrolytic
low-viscosity fluids are most preferable for extensive operative procedures using electrosurgery. Mannitol are chosen over glycine or
sorbitol when using monopolar electrosurgery.

Comparison between Vacuum and Forceps Extraction to Neonatal outcome on Prolonged Second Stage of Labor
Objective: To assess the effectiveness of vacuum and forceps extraction in prolonged second stage in Dr. Mohammad Hoesin Hospital Palembang.
Methods: A retrospective study for 5 years (2005-2009) by doing a prognostic test on prolonged second stage of labor.
Results: The subject age most commonly ranges from 20 to 35 years (84.8% in the forceps extraction group and 86% in the vacuum group). The common parity was nulliparity (58.1% in the forceps extraction group and 61.58% in the vacuum extraction). In the forceps
extraction group, 76.7% scores >7 in the 1 minute APGAR score with the mean score of 7.14±1.62, whilst in the vacuum extraction group, 79.1% scores >7 in the 1 minute APGAR score, with the mean score of 7.16±1.78. And in the forceps extraction group, 94.2 scores >7 in the 5 minute APGAR score with mean score of 8.62±1.11, whilst in the vacuum extraction group, 93% scores >7 in the 5 minute APGAR score with the mean score of 8.65±1.08. It is concluded that there is no significant differences on both 1 and 5 minute APGAR scores (p value is 0.713 and 0.755, respectively). Maternal complication of extended
episiotomy and perineal rupture were found more often in forceps extraction, but with no statistically significant difference (p=0.324).
Conclusion: There is no difference of effectiveness between forceps extraction and vacuum extraction in the case of prolonged second stage. Physicians are free whether to use the vacuum or foceps according to their own desirability and skill.

Multiple Factors Affecting Length of Hospital Postpartal Care in Eclampsia Patients
Objective: To identify any factors that could cause longer care at hospitals in eclampsia patients. Methods: It was a retrospective, cross sectional study design and conducted in Dr. Mohammad Hoesin Hospital Palembang from January 2005 to December 2009. This research conducted in 176 samples divided into 2 groups namely short-stay group (1-5 days) with 92 patients and longer stay group (≥ 5 days) with 87 patients, with mean length of hospital stay 5.41±3.43 days. Results: The most common mode of delivery in the group who stayed ≥ 5 days was cesarean section (44.8%), while the most common delivery route in the group who stayed < 5 days was forceps extraction (43.5%). The mean thrombocytes count in the longer stay group was lower than in the short-stay group (213459.77±108391.81 vs 258945.65±92213.24 mm3), which was significantly different (p=0.003). Ureum and LDH concentration in longer stay group was higher, and statistic test was found to be significantly different (p<0.05), while renal failure complication was mostly experienced by the longer stay group (20.7%). The Level of Ca125 in Preand Postoperative of Endometriosis
Objective: To determine the Ca-125 level in pre- and post-operative
of endometriosis and its correlation to endometriosis stage and severity
of dysmenorrhea.
Method: This was a cross-sectional study design conducted at Dr.
Wahidin Sudirohusodo dan some affiliated hospitals. We took the
patients undergoing laparoscopy or laparotomy consecutively.
Result: The mean value of preoperative Ca-125 level in stage I-II
was 21.53 (SD 12.64) IU/ml vs 72.52 (SD 8.52) IU/ml in stage III-IV.
The mean value of postoperative Ca-125 level was 14.82 (SD 10.00)
