Assessment of environmental radiological impact in former metallic mines in Extremadura (Spain): A case study
Metal mining in the Extremadura region was very important in the 19th and 20th centuries. However, due to different reasons the great majority of mines ceased operations, leading to plenty of abandoned mining sites, most of them with on-site waste dumps. Although metal extraction is not radioactive per se, it is considered a NORM activity. In this study, three former mining sites, in which Pb-V-Zn-Ag, Pb-Ag, and Pb-Zn were extracted, were selected to assess the radiological impact on the population and the environment. The external γ exposure was estimated by determining the effective dose and elaborating isodose maps of the sites. The presence of the mining sites increased up to 0.41 mSv/y the effective dose over the surrounding background, which is below the reference value of 1 mSv/y. In only one mining site, the uranium and radium activity concentration of waste dumps were higher than the surrounding soil. The soil to plant (wild grass) transfer factors were similar to other reported values without the influence of NORM activities. So, no enhanced transfer of radionuclides was observed. The radiological impact on the environment was assessed by the risk to non-human biota using the tiered approach developed in ERICA Tool. The sum of the risk quotients of all considered radionuclides in the most conservative Tier 1 was below 1. Total dose rates for several terrestrial Reference Animal and Plants (RAPs) were estimated using Tier 3, obtaining values below 40 μGy/h. Therefore, the impact on non-human biota can be considered as negligible. This study was funded by the Consejería de Economía, Ciencia y Agenda Digital of the Junta de Extremadura, through the project “Evaluación del impacto radiológico ambiental de la minería metálica en Extremadura” (Ref. IB20060). We are also grateful to the Junta de Extremadura for the financial support to the LARUEX research group (FQM001).peerReviewe
The effect of varus rearfoot wedges on hallux dorsiflexion resistance
Background The first metatarsophalangeal joint (MTPJ), which includes the first metatarsal and proximal phalanx, plays a crucial role in gait and impacts the windlass mechanism. Disruptions to this mechanism are implicated in various foot pathologies. Jack’s Test serves as a valuable tool for clinicians to assess the functionality of the MTPJ. Varus rearfoot wedges (VRFWs) are a common treatment employed in the management of lower limb pathologies. The impact of VRFWs on the resistance of the first MTPJ during Jack’s Test is currently unknown. This study aimed to measure the influence of VRFWs on the resistance of the first MTPJ during Jack’s Test. The secondary objective was to validate a new measurement method using a digital force gauge.
Methods Thirty participants (17 women and 13 men) were enrolled. A digital force gauge measured the weightbearing force needed for Jack’s Test, thereby evaluating the effects of VRFWs of different angulations. The Kolmogorov-Smirnov test confirmed that the data followed a normal distribution (p > 0.05). The nonparametric Friedman test (p < 0.001) showed that there were significant differences among all VRFWs, while the Wilcoxon test (p < 0.001) showed that there were differences between barefoot conditions and 3°, 5°, and 8° VRFWs. Results: The use of 8° VRFWs yielded a statistically significant reduction in the passive dorsiflexion force of hallux during Jack’s Test (12.51 N } 4.12, p < 0.001).
Conclusions The use of VRFWs has been observed to reduce dorsiflexion resistance in the proximal phalanx of the first MTPJ during Jack’s Test. Additionally, the digital force gauge was proven to be a valid tool for conducting Jack’s Test, thus offering a reliable measurement method.peerReviewe
The development and implementation of public health surveillance programmes that include social media monitoring as a priority action is being considered, as we have seen in the analysis of a conversation without an international ostomy specific campaign day. The creation of 'observatories' to assess social media conversations on public health issues will enable a rapid response to misinformation. The later this happens, the more complex it will be to get the right information to the public. However, these actions should be led by public and private health organisations

Noise Pollution Studies in the Arab World: A Scientometric Analysis and Research Agenda
This review analyzed scientific publications on noise pollution in 22 Arab countries from 1983 to 2022. The objective of this study was to assess the current state of research on noise pollution in the Arab world, identify trends and gaps in the literature, and subsequently formulate a thorough research agenda. A comprehensive search of the Scopus database was conducted using relevant search terms and inclusion criteria. Out of 249 results, 104 studies were selected. The data extraction from the 104 studies includes the country where the study was conducted/designed; the publication year; the journal of publication; sponsorship details; and the research methodology used. VOSviewer software (VOSviewer 1.6.18) was used to visualize the literature data and co-occurrence networks, collaborations, and research fronts. The analysis revealed that Saudi Arabia, Jordan, and Kuwait were the most active countries in noise pollution research, while Sudan and Libya had the lowest number of publications in this field. Additionally, countries like Somalia, Syria, Yemen, and Mauritania had no publications on this subject. Research on noise pollution in this region mainly focuses on noise in urban zones and workplaces, particularly in Saudi Arabia, Kuwait, and Jordan. Topics vary across other Arab countries. The results highlight the increasing significance of noise pollution research in the Arab world, particularly evident in recent periods. However, scientific output from Arab countries remains significantly limited compared to other global contexts. This study emphasizes the pressing requirement for increased investment in noise pollution research, aiming to achieve knowledge of the acoustic situation in Arab countries in order to develop strategies to mitigate population exposure to noise pollution, improve the well-being of citizens, and protect public health. This research was funded by the Vice Presidency for Graduate Studies, Research and Business (GRB) at Dar Al-Hekma University, Jeddah, Saudi Arabia. The authors, therefore, acknowledge with thanks GRB for technical and financial support.peerReviewe

A multicenter prospective evaluation of the benefits of two advanced hybrid closed-loop systems in glucose control and patient-reported outcomes in a real-world setting
Versión aceptada de artículo publicado en: Diabetes Care 19 January 2024; 47 (2): 216–224. https://doi.org/10.2337/dc23-1355OBJECTIVE: Advanced hybrid closed-loop systems (AHCL) have been shown to improve glycemic control and patient-reported outcomes in type 1 diabetes. The aim was to analyze the outcomes of two commercially available AHCL in real life. RESEARCH DESIGN AND METHODS: A prospective study was performed, including adolescents and adults with type 1 diabetes, AHCL naïve, from 14 centers, who initiated the use of MM780G with SmartGuard or Tandem t:slimX2 with Control-IQ. Baseline and 3-month evaluations were performed, assessing HbA1c, time in different glycemic ranges, and patient-reported outcomes. The primary outcome was the between-group time in range 70–180 mg/dL difference from beginning to end of follow-up. RESULTS: One hundred fifty participants were included, with 75 initiating each system (age: 39.9 ± 11.4 years [16–72]; 64% female; diabetes duration: 21.6 ± 11.9 years). Time in range increased from 61.53 ± 14.01% to 76.17 ± 9.48% (P < 0.001), with no between-group differences (P = 0.591). HbA1c decreased by 0.56% (95% CI 0.44%, 0.68%) (6 mmol/mol, 95% CI 5, 7) (P < 0.001), from 7.43 ± 1.07% to 6.88 ± 0.60% (58 ± 12 to 52 ± 7 mmol/mol) in the MM780G group, and from 7.14 ± 0.70% to 6.56 ± 0.53% (55 ± 8 to 48 ± 6 mmol/mol) in the Control-IQ group (both P < 0.001 to baseline, P = 0.819 between groups). No superiority of one AHCL over the other regarding fear of hypoglycemia or quality of life was found. Improvement in diabetes-related distress was higher in Control-IQ users (P = 0.012). Sleep quality was improved (PSQI: from 6.94 ± 4.06 to 6.06 ± 4.05, P = 0.004), without differences between systems. Experience with AHCL, evaluated by the INSPIRE measures, exceeded the expectations. CONCLUSIONS: The two AHCL provide significant improvement in glucose control and satisfaction, with no superiority of one AHCL over the other.peerReviewe
Initial training in civic education. A multiple case study in Costa Rica
The article is a multiple case study in three Initial Training in the Teaching of Social Studies and Civic Education of two Costa Rican public universities, where the purposes of the courses in the Civic Education component are described. In order to understand the initial formation and citizenship, we start from critical theories applied to the didactics of social sciences, based on formation models that include school culture and conflict as part of the factors that allow problematizing current citizenships.
