    Adult-onset autoinflammation caused by somatic mutations in UBA1:A Dutch case series of patients with VEXAS

    Background: A novel autoinflammatory syndrome was recently described in male patients who harbored somatic mutations in the X-chromosomal UBA1 gene. These patients were characterized by adult-onset, treatment-refractory inflammation with fever, cytopenia, dysplastic bone marrow, vacuoles in myeloid and erythroid progenitor cells, cutaneous and pulmonary inflammation, chondritis, and vasculitis, which is abbreviated as VEXAS. Objective: This study aimed to (retrospectively) diagnose VEXAS in patients who had previously been registered as having unclassified autoinflammation. We furthermore aimed to describe clinical experiences with this multifaceted, complex disease. Methods: A systematic reanalysis of whole-exome sequencing data from a cohort of undiagnosed patients with autoinflammation from academic hospitals in The Netherlands was performed. When no sequencing data were available, targeted Sanger sequencing was applied in cases with high clinical suspicion of VEXAS. Results: A total of 12 male patients who carried mutations in UBA1 were identified. These patients presented with adult-onset (mean age 67 years, range 47-79 years) autoinflammation with systemic symptoms, elevated inflammatory parameters, and multiorgan involvement, most typically involving the skin and bone marrow. Novel features of VEXAS included interstitial nephritis, cardiac involvement, stroke, and intestinal perforation related to treatment with tocilizumab. Although many types of treatment were initiated, most patients became treatment-refractory, with a high mortality rate of 50%. Conclusion: VEXAS should be considered in the differential diagnosis of males with adult-onset autoinflammation characterized by systemic symptoms and multiorgan involvement. Early diagnosis can prevent unnecessary diagnostic procedures and provide better prognostic information and more suitable treatment options, including stem cell transplantation.</p

    The Tonic Immobility Scale in adolescent and young adult rape victims:Support for three-factor model

    OBJECTIVE: A substantial number of sexual assault victims report experiencing some form of peritraumatic tonic immobility (TI). A self-report questionnaire that is widely used to assess TI retrospectively is the Tonic Immobility Scale (TIS). This study explored the factor structure of the TIS in a clinical sample of adolescent and young adults.METHOD: The sample comprised 131 female rape victims, aged 13-25, who were referred for specialized trauma-focused treatment. An exploratory factor analysis (EFA) was performed.RESULTS: The EFA showed support for a three-factor model, with factors TI, Fear, and Detachment. Item correlations ranged from .32 to .57 for TI, from .14 to .35 for Fear, and .29 for the two Detachment items.CONCLUSIONS: We found support for a three-factor solution distinguishing TI, fear, and detachment, suggesting the need to further develop the TIS with different subscales in varying age groups and clinical samples.     Varieties of Public Attitudes toward Immigration:Evidence from Survey Experiments in Japan

    What factors shape attitudes toward immigration? Previous studies have typically debated whether citizens oppose immigration more for economic or cultural reasons. We broaden this debate by exploring how different segments of the citizenry feel about immigration. Our original surveys conducted in Japan reveal two separate axes along which many citizens view immigration: (1) its cultural and economic effects, and (2) its positive and negative effects. Even in Japan, whose relatively closed policy toward immigration is conventionally believed to reflect widespread public intolerance of outsiders, over 60 percent of our respondents favor widening the doors to immigrants for economic or cultural reasons or both

    Carbon intensity and the cost of equity capital

    he transition from high- to lower-carbon production systems increasingly creates regulatory and market risks for high-emitting firms. We test to what extent equity market investors demand a premium to compensate for such risks and thus might raise firms' cost of equity capital (CoE). Using data for 1,897 firms spanning 50 countries over the years 2008–2016, we find a distinct and robust positive impact of carbon intensity (carbon emissions per unit of output) on CoE: On average, a standard deviation higher (sector-adjusted) carbon intensity is associated with a CoE premium of 6 (9) basis points or 1.7% (2.6%). This effect is primarily explained by systematic risk factors: high-emitting assets are significantly more sensitive to economy-wide fluctuations than low-emitting ones. The CoE impact of carbon intensity is more pronounced in high-emitting sectors, EU countries, and firms subject to carbon pricing regulation. Our results suggest that carbon emission reduction might serve as a valuable risk mitigation strategy

    Perceived Fit and User Behavior in Activity-Based Work Environments

    While activity-based working is gaining popularity worldwide, research shows that workers frequently experience a misfit between the task at hand and their work setting. In the current study, experience sampling data were used to examine how perceived fit in activity-based work environments is related to user behavior (i.e., the use of work settings and setting-switching). We found that workers’ perceived fit was higher when they used closed rather than open work settings for individual high-concentration work. Furthermore, more frequent setting-switching was related to higher perceived fit. Unexpectedly, however, this relation was observed only among workers low in activity-switching. These findings indicate that user behavior may indeed be relevant to creating fit in activity-based work environments. To optimize workers’ perceived fit, it seems to be particularly important to facilitate and stimulate the use of closed work settings for individual high-concentration work

    Teacher-Student Interaction Patterns Change During an Early Science Teaching Intervention

    This study aimed to explore the interaction between teachers and young students in terms of their question and answer patterns during science lessons and to investigate whether this changes over the course of an intervention called ‘Language as a Tool for learning science’ (LaT). It also compared experienced teachers with novices. A total of 16 teachers—of which 8 experienced and 8 novice teachers—and their students participated in this study. The teachers’ utterances were coded with regard to the use of questioning strategies, and the students’ reasoning skills were categorized by the use of three types of scientific reasoning skills (observations, predictions, explanations). Consistent with the complex dynamic systems orientation, we analyzed the interactions among these dimensions by means of state space grids (SSG). The results showed that before the intervention, the teacher-student interaction often took the form of a relatively rigid pattern in which teachers did not ask questions and students did not respond with reasoning expressions. In the course of the LaT intervention, a richer repertoire and a greater amount of interactions emerged in which knowledge was “co-constructed” by means of open-ended questions of the teacher and reasoning by the students. The results also suggest that the patterns of experienced and novice teachers were quite similar to each other

    Applications of the Poincare-Hopf Theorem:Epidemic Models and Lotka-Volterra Systems

    This paper focuses on properties of equilibria and their associated regions of attraction for continuous-time nonlinear dynamical systems. The classical Poincare–Hopf Theorem is used to derive a general result providing a sufficient condition for the system to have a unique equilibrium. The condition involves the Jacobian of the system at possible equilibria, and ensures the system is infact locally exponentially stable. We apply this result to the susceptible-infected-susceptible (SIS) networked model, and a generalised Lotka–Volterra system. We use the result further to extend the SIS model via the introduction of decentralised feedback controllers, which significantly change the system dynamics, rendering existing Lyapunov-based approaches invalid. Using the Poincare–Hopf approach, we identify a necessary and sufficient condition under which the controlled SIS system has a unique nonzero equilibrium (a diseased steady-state), and monotone systems theory is used to show this nonzero equilibrium is attractive for all nonzero initial conditions. A counterpartcondition for the existence of a unique equilibrium for a nonlinear discrete-time dynamical system is also presented

    Dealing with disagreement:The depolarizing effects of everyday diplomatic skills face-to-face and online

    In online text-based discussions, people behave less diplomatically because they are more outspoken and less responsive. This can feed impressions of polarization. This article uses a new methodology to isolate the influence of outspokenness and responsiveness in shaping perceptions of polarization in online chat and face-to-face discussions. Text-based online and face-to-face discussions were reproduced in a face-to-face format (Study 1) and in a text-based chat format (Study 2). Uninformed observers (N = 102 and N = 103, repeated measures) evaluated these. The results showed that responsiveness was generally considered indicative of agreement and good social relationships but the interpretation of outspokenness (or lack of ambiguity) depended on the medium format. This suggests that what counts as diplomacy is not the same for each medium. Moreover, the experiences of the actors reproducing the chats in a face-to-face format highlighted the differences between media. We conclude that online conversational dynamics may play an important role in societal polarization

    Perceptual snoring as a basis for a psychoacoustical modeling and clinical patient profiling

    Purpose The perceptual burden and social nuisance for mainly the co-sleeper can affect the relationship between snorer and bedpartner. Mandibular advancement devices (MAD) are commonly recommended to treat sleep-related breathing such as snoring or sleep apnea. There is no consensus about the definition of snoring particularly with MAD, which is essential for assessing the effectiveness of treatment. We aimed to stablish a notion of perceptual snoring with MAD in place. Methods Sound samples, each 30 min long, were recorded during in-home, overnight, automatic mandibular repositioning titration studies in a population of 29 patients with obstructive sleep apnea syndrome (OSAS) from a clinical trial carried out to validate the MATRx plus. Three unspecialized and calibrated raters identified sound events and classified them as noise, snore, or breathing as well as providing scores for classification certainty and annoyance. Data were analyzed with respect to expiration-inspiration, duration, annoyance, and classification certainty. Results A Fleiss' kappa (&gt;0.80) and correlation duration of events (&gt;0.90) between raters were observed. Prevalence of all breath sounds: snore 55.6% (N = 6398), breathing sounds 31.7% (N = 3652), and noise 9.3% (N = 1072). Inspiration occurs in 88.3% of events, 96.8% contained at least on expiration phase. Snore and breath events had similar duration, respectively 2.58s (sd 1.43) and 2.41s (sd 1.22). Annoyance is lowest for breathing events (8.00 sd 0.98) and highest for snore events (4.90 sd 1.92) on a VAS from zero to ten. Conclusion Perceptual sound events can be a basis for analysis in a psychosocial context. Perceived snoring occurs during both expiration as well as inspiration. Substantial amount of snoring remains despite repositioning of the mandible aimed at the reduction of AHI-ODI.</p

    Effectiveness of cognitive remediation in depression:a meta-analysis

    BACKGROUND: Preliminary evidence suggests beneficial effects of cognitive remediation in depression. An update of the current evidence is needed. The aim was to systematically assess the effectiveness of cognitive remediation in depression on three outcomes.METHODS: The meta-analysis was pre-registered on PROSPERO (CRD42019124316). PubMed, PsycINFO, Embase and Cochrane Library were searched on 2 February 2019 and 8 November 2020 for peer-reviewed published articles. We included randomized and non-randomized clinical trials comparing cognitive remediation to control conditions in adults with primary depression. Random-effects models were used to calculate Hedges' g, and moderators were assessed using mixed-effects subgroup analyses and meta-regression. Main outcome categories were post-treatment depressive symptomatology (DS), cognitive functioning (CF) and daily functioning (DF).RESULTS: We identified 5221 records and included 21 studies reporting on 24 comparisons, with 438 depressed patients receiving cognitive remediation and 540 patients in a control condition. We found a small effect on DS (g = 0.28, 95% CI 0.09-0.46, I2 40%), a medium effect on CF (g = 0.60, 95% CI 0.37-0.83, I2 44%) and a small effect on DF (g = 0.22, 95% CI 0.06-0.39, I2 3%). There were no significant effects at follow-up. Confounding bias analyses indicated possible overestimation of the DS and DF effects in the original studies.CONCLUSIONS: Cognitive remediation in depression improves CF in the short term. The effects on DS and DF may have been overestimated. Baseline depressive symptom severity should be considered when administering cognitive remediation.</p
