    Ketone bodies : A double-edged sword for mammalian life span.

    application/pdfAccumulating evidence suggests health benefits of ketone bodies, and especially for longevity. However, the precise role of endogenous ketogenesis in mammalian life span, and the safety and efficacy of the long-term exogenous supplementation of ketone bodies remain unclear. In the present study, we show that a deficiency in endogenous ketogenesis, induced by whole-body Hmgcs2 deletion, shortens life span in mice, and that this is prevented by daily ketone body supplementation using a diet containing 1,3-butanediol, a precursor of β-hydroxybutyrate. Furthermore, feeding the 1,3-butanediol-containing diet from early in life increases midlife mortality in normal mice, but in aged mice it extends life span and prevents the high mortality associated with atherosclerosis in ApoE-deficient mice. By contrast, an ad libitum low-carbohydrate ketogenic diet markedly increases mortality. In conclusion, endogenous ketogenesis affects mammalian survival, and ketone body supplementation may represent a double-edged sword with respect to survival, depending on the method of administration and health status.Journal Articlejournal articl

    IDH wild-type lower-grade gliomas with glioblastoma molecular features: a systematic review and meta-analysis

    The WHO 2021 classification defines IDH wild type (IDHw) histologically lower-grade glioma (hLGG) as molecular glioblastoma (mGBM) if TERT promoter mutation (pTERTm), EGFR amplification or chromosome seven gain and ten loss aberrations are indicated. We systematically reviewed articles of IDHw hLGGs studies (49 studies, N = 3748) and meta-analyzed mGBM prevalence and overall survival (OS) according to the PRISMA statement. mGBM rates in IDHw hLGG were significantly lower in Asian regions (43.7%, 95% confidence interval [CI: 35.8-52.0]) when compared to non-Asian regions (65.0%, [CI: 52.9-75.4]) (P = 0.005) and were significantly lower in fresh-frozen specimen when compared to formalin-fixed paraffin-embedded samples (P = 0.015). IDHw hLGGs without pTERTm rarely expressed other molecular markers in Asian studies when compared to non-Asian studies. Patients with mGBM had significantly longer OS times when compared to histological GBM (hGBM) (pooled hazard ratio (pHR) 0.824, [CI: 0.694-0.98], P = 0.03)). In patients with mGBM, histological grade was a significant prognostic factor (pHR 1.633, [CI: 1.09-2.447], P = 0.018), as was age (P = 0.001) and surgical extent (P = 0.018).     Immunohistochemical Analysis of Mitochondrial Ferritin in the Midbrain of Patients with Parkinson's Disease

    Mitochondrial ferritin (FtMt) is an endogenous iron-storage protein localized in the mitochondria. FtMt is mainly observed in restricted tissues, such as those in the testis, islets of Langerhans, and brain. Further, it may protect cells from oxidative stress in neurodegenerative diseases, including Alzheimer's disease and progressive supranuclear palsy. However, the role of FtMt in Parkinson's disease (PD) remains unclear. Therefore, the current study investigated the localization and expression level of FtMt in the midbrain of patients with PD and healthy controls using immunohistochemical techniques. FtMt immunoreactivity was mainly detected in dopaminergic neurons in the substantia nigra pars compacta (SNc) in both healthy controls and patients with PD. In addition, FtMt-positive particles were observed outside the dopaminergic neurons in patients with PD. Based on a quantitative comparison, patients with PD had a significantly upregulated FtMt immunoreactivity in dopaminergic neurons than healthy controls. Our result might be helpful in future studies on the role of FtMt in PD.journal articl

    症例は45歳、女性。深夜に突然、左下腹部痛を認め近医受診し、当院紹介となった。造影CTを行ったところ、腹水および小腸ループの拡張が認められた。絞扼性腸閉塞の診断のもと、緊急手術を施行した。腹腔鏡下に腹腔内を観察すると、血性腹水を認め、S状結腸間膜に直径約3cmの異常裂孔が存在し、小腸が貫通して脱出しており、S状結腸間膜裂孔ヘルニアによる腸閉塞と診断した。小腸の整復は比較的容易で、小腸の色調も問題なく、腸切除をすることなく裂孔を体内結紮縫合閉鎖し手術を終了した。術後経過は良好で、術後5日目に退院となった。A 45-year-old woman admitted with left abdominal pain. Abdominal computed tomography revealed ascites and mechanical obstruction of small intestine. Urgent laparoscopic surgery was performed. Laparoscopy showed bloody ascites and an oval hernial orifice about 3 cm diameter in the mesentery of sigmoid colon. Small intestine invaginated through this orifice. The reduction in small intestine was easy relatively, and the color tone of the small intestine was also no problem. Intraabdominal ligation suture closes this hiatus, and an operation has been ended without resection of small intestine.
