Predictors of longitudinal changes in body weight, muscle and fat in patients with and ever-smokers at risk of COPD
Background and objective:
Weight and muscle loss are predictors of poor outcomes in chronic obstructive pulmonary disease. However, to our knowledge, no study has investigated the predictors of longitudinal weight loss or its composition from functional and morphological perspectives.
Methods:
This longitudinal observational study with a median follow-up period of 5 years (range: 3.0-5.8 years) included patients with COPD and ever-smokers at risk of COPD. Using chest computed tomography (CT) images, airway and emphysematous lesions were assessed as the square root of the wall area of a hypothetical airway with an internal perimeter of 10 mm (√Aaw at Pi10) and the percentage of low attenuation volume (LAV%). Muscle mass was estimated using cross-sectional areas (CSAs) of the pectoralis and erector spinae muscles, and fat mass was estimated using the subcutaneous fat thickness at the level of the 8th rib measured using chest CT images. Statistical analyses were performed using the linear mixed-effects models.
Results:
In total, 114 patients were enrolled. Their body mass index remained stable during the study period while body weight and muscle CSA decreased over time and the subcutaneous fat thickness increased. Reduced forced expiratory volume in 1 s and peak expiratory flow (PEF) at baseline predicted the future decline in muscle CSA.
Conclusion:
Severe airflow limitation predicted future muscle wasting in patients with COPD and ever-smokers at risk of COPD. Airflow limitation with a PEF slightly below 90% of the predicted value may require intervention to prevent future muscle loss.journal articl
Myostatin expression in lung cancer induces sarcopenia and promotes cancer progression
Objectives:
Many studies have demonstrated that sarcopenia among lung cancer predicts poor prognosis due to cancer progression. However, the cytokines that link sarcopenia and lung cancer progression remain unidentified. This study aimed to investigate whether lung cancer producing myostatin, which induces skeletal muscle atrophy, leads to sarcopenia and promotes cancer progression in patients with resected lung cancer.
Methods:
Tumor tissues were obtained from 148 patients who underwent curative resection for lung cancer. Tumor cells were stained with myostatin and tumor-associated macrophages (TAM) in the tumor microenvironment were stained with CD68. We assessed the association between myostatin expression and the clinicopathological features.
Results:
High myostatin expression in lung cancer was significantly associated with low skeletal muscle mass. The 5-year overall survival and relapse-free survival were significantly worse among patients with high myostatin expression than those with low expression. A multivariate analysis showed that TAM count was positively correlated with high myostatin expression.
Conclusion:
Sarcopenia may be induced by myostatin secreted by lung cancer cells. Moreover, myostatin may promote TAM migration into the tumor microenvironment, leading to advance lung cancer. As a result, patients with high myostatin expression had poor prognosis.journal articl
Nutritional knowledge attitudes and practices towards the prevention and control of COVID-19 among educated young adults in Bangladesh
Background:
To strengthen the immune system and reduce the risk of infectious diseases like novel coronavirus disease (COVID-19), a sound knowledge, attitude, and practice about healthy eating patterns and well-balanced diet ensuring proper nutrition is highly important. However, the relevant information is quite limited and still unknown in Bangladesh. The objective of this study was to assess the levels of nutritional knowledge, attitudes, and practices towards the prevention and control of COVID-19 among the educated young adult population in Bangladesh.
Methods:
This cross-sectional study was carried out among 166 educated young adults (aged 20– 40 years) recruited conveniently using online social media platforms. A Google form link of the structured questionnaire was distributed to the potential respondents through Facebook and Messenger, and also invited to participate. Participant’s self-reported responses were recorded. Both descriptive and comparative (Chi-square) statistics were used.
Results:
Of all [mean ± standard deviation (SD) age 27.4±3.5 years], majority were men (54.8%). Around two-third of the respondents had an average level of nutritional knowledge (65.4%), attitudes (68.1%) and practices (68.6%) about prevention and control of COVID-19, while the good levels were found in around one-fifth of them. And, the level of nutritional practices was found to be significantly related to sex (P=0.012).
Conclusions:
Majority of the study population had an average level of nutritional knowledge, attitudes, and practices towards the prevention and control of COVID-19. It demands nutrition-related health promotion and health educational programs for the population.journal articl
Evaluation of amniotic fluid neutrophil gelatinase-associated lipocalin and L-type fatty acid-binding protein levels during pregnancy
Objective:
We aimed to examine amniotic fluid neutrophil gelatinase-associated lipocalin (NGAL) and L-type fatty acid-binding protein (L-FABP) levels during pregnancy.
Study design:
This study included singleton pregnancies. Amniotic fluid samples were collected at the time of vaginal delivery, cesarean section, amniocentesis, amnioreduction, and amnioinfusion. We analyzed changes of the NGAL and L-FABP levels during pregnancy and the factors affecting these values and their association with clinical outcomes.
Results:
Three hundred and one pregnancies were analyzed. Respective Pearson correlation coefficients for the NGAL and L-FABP levels and gestational age at inspection were - 0.351 and - 0.819 (p <0.001 and p < 0.001, respectively); weak and strong negative correlation were observed. The NGAL level was significantly higher in the intra-amniotic infection group than in the control group (p < 0.001). The L-FABP level was significantly higher in the fetal blood flow abnormalities group than in the control group (p < 0.001). The NGAL and L-FABP levels were significantly higher in the adverse outcomes group than in the control group (p = 0.019 and p < 0.001, respectively), and the respective areas under the concentration-time curve, with optimal cutoff values, for the NGAL and L-FABP levels were 0.693 (14,800 µg/gCr) and 0.864 (378 µg/gCr).
Conclusions:
Amniotic fluid NGAL and L-FABP levels reflect fetal and neonatal immaturity. Additionally, the NGAL level is a useful predictive factor of intra-amniotic infection, and the L-FABP level is a useful predictive factor of fetal condition and short- and long-term prognoses.journal articl
