Clinical assessment of ASD in adults using self- and other-report:Psychometric properties and validity of the Adult Social Behavior Questionnaire (ASBQ)
The aim of this study was to develop and validate the Adult Social Behavior Questionnaire (ASBQ), a multidimensional Autism Spectrum Disorder (ASD) questionnaire that contains both a self report version and a version to be completed by someone close. Psychometric qualities, convergence between self report and other report ratings, and scores in a group diagnosed with ASD and multiple comparison groups were examined.Principal Component Analyses yielded a structure with six dimensions (reduced contact, reduced empathy, reduced interpersonal insight, violation of social conventions, insistence on sameness, and sensory stimulation/motor stereotypies) for both self- and other-report versions. Reliability estimates and correlations between self- and other-ratings were good and the score profile on the 44-item ASBQ differentiated a group with ASD from a non clinical group and patients with depression, schizophrenia and ADHD.We conclude that the ASBQ is a short and easy to apply questionnaire that captures the heterogeneous nature of ASD. It yields a score profile among six ASD problem domains both from the perspective of the patient and from a significant other. (C) 2016 Elsevier Ltd. All rights reserved.</p
Regulation of cell wall plasticity by nucleotide metabolism in Lactococcus lactis
To ensure optimal cell growth and separation, and to adapt to environmental parameters, bacteria have to maintain a balance between cell wall (CW) rigidity and flexibility. This can be achieved by a concerted action of peptidoglycan (PG) hydrolases and PG synthesizing/modifying enzymes. In a search for new regulatory mechanisms responsible for the maintenance of this equilibrium inLactococcus lactis, we isolated mutants that are resistant to the PG hydrolase lysozyme. We found that 14% of the causative mutations were mapped in theguaAgene, the product of which is involved in purine metabolism. Genetic and transcriptional analyses combined with PG structure determination of theguaAmutant enabled us to reveal the pivotal role of thepyrBgene in the regulation of CW rigidity. Our results indicate that conversion of L-aspartate (L-Asp) to N-carbamoyl-L-aspartate by PyrB may reduce the amount of L-Asp available for PG synthesis and thus cause the appearance of Asp/Asn-less stem peptides in PG. Such stem peptides do not form PG cross-bridges, resulting in a decrease in PG crosslinking and, consequently, reduced PG thickness and rigidity. We hypothesize that the concurrent utilization of L-Asp for pyrimidine and PG synthesis may be part of the regulatory scheme, ensuring CW flexibility during exponential growth and rigidity in stationary phase. The fact that L-Asp availability is dependent on nucleotide metabolism, which is tightly regulated in accordance with the growth rate, providesL. lactiscells the means to ensure optimal CW plasticity without the need to control the expression of PG synthesis genes.</p

Is YouTube useful as a source of information for Sjogren's syndrome?
ObjectivesTo quantitatively and qualitatively assess the characteristics of YouTube videos dealing with Sjogren's syndrome.Materials & MethodsA comprehensive electronic search was performed for Sjogren's syndrome' in YouTube. After excluding duplicates, irrelevant videos, and non-English-language domains, 70 videos were included for analysis. Videos were classified as useful, misleading, or personal experience. The overall quality of videos was scored according to the Global Quality Scale (GQS). Useful videos were assessed for reliability and comprehensiveness based on two 5-point scales. Key points of the misleading videos were explored and patients' personal experiences were further investigated.ResultsThirty-six videos (51.4%) were classified as useful, 6 (8.6%) as misleading, and 28 (40%) as patient's personal experience. Independent users tend to upload videos with personal experience, while university channels/professional organizations share useful videos with evidence-based information. Significant difference was observed in GQS among useful, misleading videos, and patient's experiences. The mean reliability, comprehensiveness, and GQS scores of useful videos were 2.5 (s.d.:1.2), 2.6 (s.d.:1.4), and 3.4 (s.d.:1.0), respectively, whereas only six videos (16.7%) were rated as complete vis-a-vis content. The most frequently misleading topics were etiology and treatment.ConclusionsSpecialists should refer their patients to validated e-information resources and actively participate in the development of video-sharing platforms.</p
Gray matter volume and white matter lesions in chronic kidney disease:exploring the association with depressive symptoms
Objective: Chronic kidney disease (CKD) is associated with structural brain damage and with a high prevalence of depression. We therefore investigated structural brain alterations in both gray and white matter in CKD patients, focusing on depression-related (frontal-subcortical) regions.Method: This cross-sectional MRI study in 24 CKD patients and 24 age- and sex-matched controls first tested whether CKD was associated with regionally lower gray matter (GM) volumes and more severe white matter lesions (WMLs). In exploratory subanalyses, we examined whether differences were more pronounced in CKD patients with depressive symptoms.Results: CKD patients showed lower global GM volume (P=.04) and more severe WMLs (P=.04) compared to controls. In addition, we found substantial clusters of lower GM in the bilateral orbitofrontal-cortex for CKD patients, which were however nonsignificant after proper multiple-comparison correction. In exploratory analyses for depressed CKD patients, reduced GM clusters were mainly detected within the frontal lobe. WML severity was unrelated to depression.Conclusion: CKD was characterized by differences in brain structure. Although subthreshold, lower GM volumes were observed in depression-related brain areas and were more pronounced for depressed patients. There is a need for replication in larger and longitudinal studies to investigate whether WMLs and regional GM reductions may render CKD patients more susceptible for depression. Prevalence, risk factors and molecular epidemiology of highly resistant gram negative rods in hospitalized patients in the Dutch region Kennemerland
Background: This paper describes (1) the Highly Resistant Gram Negative Rod (HR-GNR) prevalence rate, (2) their genotypes, acquired resistance genes and (3) associated risk factors of HR-GNR colonization among the hospitalized population in the Dutch region Kennemerland.Methods: Between 1 October 2013 and 31 March 2014, cross-sectional prevalence measurements were performed in three regional hospitals as part of each hospitals infection control program. Rectal swabs were analyzed at the Regional Public Health Laboratory Kennemerland by direct culturing. Genotypes and acquired resistance genes of positive isolates were determined using Whole Genome Sequencing with the MiSeq instrument (Illumina). Association between several independent variables and HR-GNR positivity was examined using logistic regression models.Results: Out of 427 patients, 24 HR-GNR positive isolates were recovered from 22 patients, resulting in a regional HR-GNR colonization prevalence (95 % CI) of 5.2 % (3.6-7.9). Of these 22 positive patients, 15 were Extended Spectrum Beta-Lactamase (ESBL) positive (3.5 % (2.1-5.7)), 7 patients were positive for a Fluoroquinolones and Aminoglycosides (Q&A) resistant Enterobacteriaceae (1.6 % (0.8-3.3)) and from one patient (0.2 % (0-1.3)) a Stenotrophomonas maltophilia resistant towards co-trimoxazole was isolated. No carbapenemase producing Enterobacteriaceae (CPE), multi-resistant Acinetobacter species or multi-resistant Pseudomonas aeruginosa were isolated. The ESBL genes found were bla(CTX-M-1) (n = 4, 25.0 %), bla(CTX-M-15) (n = 3, 18.8 %), bla(CTX-M-27) (n = 2, 12.5 %), bla(CTX-M-14b) (n = 2, 12.5 %), bla(CTX-M-9) (n = 2, 12.5 %), bla(CTX-M-14) (n = 1, 6.3 %), bla(CTX-M-3) (n = 1, 6.3 %), bla(SHV-11) (n = 1, 6.3 %) and bla(SHV-12) (n = 1, 6.3 %). Being known HR-GNR positive in the past was the only significant associated risk factor for HR-GNR positivity, odds ratio (95 % CI): 7.32 (1.82-29.35), p-value = 0.005.Conclusions: Similar ESBL prevalence rates and genotypes (3.5 %) were found in comparison to other Dutch studies. When previously HR-GNR positive patients are readmitted, they should be screened for HR-GNR colonization since colonization with GR-GNRs could be prolonged. We recommend for future studies to include all defined HR-GNRs in addition to ESBLs in prevalence studies, in order to obtain a more comprehensive overview of colonization with HR-GNRs.</p
First combined intestinal and abdominal wall transplantation in the Netherlands
AchtergrondAls totale parenterale voeding (TPV) niet mogelijk is, is een dunnedarmtransplantatie de enige kans op langetermijnoverleving voor patiënten met darmfalen. Maar bij 20-33% van de patiënten treden na een geïsoleerde dunnedarmtransplantatie complicaties van de buikwand op.CasusWij zagen een 24-jarige vrouw met een kortedarmsyndroom dat werd veroorzaakt door een ernstig gecompliceerd beloop van de ziekte van Crohn. Nadat ze ruim 5 jaar was behandeld met TPV, stelden we de indicatie voor een dunnedarmtransplantatie. Een buikwandtransplantatie was ook nodig vanwege een buikwanddefect, vele littekens van de buikwand en het gebrek aan ruimte in de buik. Patiënte onderging een gecombineerde dunnedarm-buikwandtransplantatie. 6 maanden hierna had ze een sufficiënte buikwand en een niet-afwijkende BMI en had ze geen voedingslijnen meer nodig.ConclusieDeze casus beschrijft de eerste gecombineerde dunnedarm-buikwandtransplantatie in Nederland en in de Eurotransplant-regio bij een patiënte met een eindstadium van darmfalen en met ernstige problemen van de buikwand.BACKGROUND: When total parenteral nutrition (TPN) is not an option, intestinal transplantation is the sole treatment for patients with end-stage intestinal failure to increase the chance of long-term survival. However, in 20-33% of patients, abdominal wall-related complications occur after isolated intestinal transplantation.CASE DESCRIPTION: The patient is a 24-year-old woman with ultra-short bowel syndrome, caused by a severely complicated history of Crohn's disease. After 5 years of TPN, the patient was referred for intestinal transplantation. In addition, an abdominal wall transplant was required due to an abdominal wall defect, extensive scarring of the abdominal wall and lack of free space within the abdomen. Therefore, a combined intestinal and abdominal wall transplantation was performed. Six months after transplantation the patient has a sufficient abdominal wall, a normal body mass index and no longer requires any feeding lines.CONCLUSION: This case report describes the first combined intestinal and abdominal wall transplantation in the Netherlands and in the Eurotransplant region in a patient with end-stage intestinal failure and loss of abdominal domain.</p

Siemon (zó geschreven!) Reker was streektaalfunctionaris van de Provincie Groningen vanaf 1984 en werd in 2001 de eerste hoogleraar Groninger taal en cultuur aan de Rijksuniversiteit Groningen.
