Employment Growth and Inter-industry Job Reallocation:Spatial Patterns and Relatedness
Employment growth and inter-industry job reallocation: spatial patterns and relatedness, Regional Studies. The nature of employment reallocation between industries is assessed using rich register data for the Netherlands. It is found that employment decline in some industries is countered, in a communicating vessels fashion, by employment growth in other industries, which is primarilydriven by the availability of released skilled labour. These labour demand interactions are predominantly local and stronger between related industries. In addition, the inter-industry labour reallocation has a distinct geographical character in which the location of employment creation depends primarily on the residential location of the released employees rather than on the location of the job destruction

Conduct Problem Trajectories Between Age 4 and 17 and Their Association with Behavioral Adjustment in Emerging Adulthood
Individual heterogeneity exists in the onset and development of conduct problems, but theoretical claims about predictors and prognosis are often not consistent with the empirical findings. This study examined shape and outcomes of conduct problem trajectories in a Belgian population-based sample (N = 682; 49.5 % boys). Mothers reported on children's conduct problems across six waves (age 4-17) and emerging adults reported on their behavioral adjustment (age 17-20). Applying mixture modeling, we found four gender-invariant trajectories (labeled life-course-persistent, adolescence-onset, childhood-limited, and low). The life-course-persistent group was least favorably adjusted, but the adolescence-onset group was similarly maladjusted in externalizing problems and may be less normative (15 % of the sample) than previously believed. The childhood-limited group was at heightened risk for specifically internalizing problems, being more worrisome than its label suggests. Interventions should not only be aimed at early detection of conduct problems, but also at adolescents to avoid future maladjustment.</p
Prediction of outcome after subarachnoid hemorrhage:Timing of clinical assessment
OBJECTIVE Currently, early prediction of outcome after spontaneous subarachnoid hemorrhage (SAH) lacks accuracy despite multiple studies addressing this issue. The clinical condition of the patient on admission as assessed using the World Federation of Neurosurgical Societies (WFNS) grading scale is currently considered the gold standard. However, the timing of the clinical assessment is subject to debate, as is the contribution of additional predictors. The aim of this study was to identify either the conventional WFNS grade on admission or the WFNS grade after neurological resuscitation (rWFNS) as the most accurate predictor of outcome after SAH.METHODS This prospective observational cohort study included 1620 consecutive patients with SAH admitted between January 1998 and December 2014 at our university neurovascular center. The primary outcome measure was a poor modified Rankin Scale score at the 2-month follow-up. Clinical predictors were identified using multivariate logistic regression analyses. Area under the receiver operating characteristic curve (AUC) analysis was used to test discriminative performance of the final model. An AUC of > 0.8 was regarded as indicative of a model with good prognostic value.RESULTS Poor outcome (modified Rankin Scale Score 4-6) was observed in 25% of the patients. The rWFNS grade was a significantly stronger predictor of outcome than the admission WFNS grade. The rWFNS grade was significantly associated with poor outcome (p <0.001) as well as increasing age (p <0.001), higher modified Fisher grade (p <0.001), larger aneurysm size (p <0.001), and the presence of an intracerebral hematoma (OR 1.8, 95% CI 1.2-2.8; p = 0.002). The final model had an AUC of 0.87 (95% CI 0.85-0.89), which indicates excellent prognostic value regarding the discrimination between poor and good outcome after SAH.CONCLUSIONS In clinical practice and future research, neurological assessment and grading of patients should be performed using the rWFNS to obtain the best representation of their clinical condition.</p
In the centre of an epidemic:Fifteen years of LA-MRSA CC398 at the University Hospital Munster
Ten years after initial publications on livestock-associated methicillin-resistant Staphylococcus aureus (LA-MRSA) in 2005, we report on the course of the LA-MRSA CC398 epidemic among patients of the University Hospital Munster. This tertiary care facility is located in the Dutch-German border region (EUREGIO), which is characterized by a high density of livestock production and is hence a hotspot for the occurrence of LA-MRSA CC398. Taking advantage of the unique opportunity to track the emergence and spread of MRSA CC398 among humans from the very beginning of the epidemic until today, a total of 6555 non-duplicate MRSA isolates from all screenings and clinical specimens cultivated within the period from 2000 to 2014 were included in the analysis. Retrospectively, the first MRSA CC398 isolate (spa type t034) was obtained from a screening specimen of a patient in 2000, which represents one of the first human-associated LA-MRSA CC398 isolates reported in Europe. After sporadic detections between 2000 and 2004, this clonal lineage accounted for 9.6% of all local MRSA in 2005; a proportion which increased to 35% in 2013 and became stable since then. Considering the period from 2000 to 2014, the group of MRSA CC398 isolates comprised a total of 45 different spa types among which toll (48.3%), t034 (39.3%) and t108 (3.5%) were predominant and so far unreported types were found. Overall, LA-MRSA CC398 emerged rapidly during the past decade, developed enormous sublineage diversity and contributed substantially to the total burden of MRSA colonization and infection at the hospital. PEMODELAN KEPUASAN PASIEN MENGGUNAKAN KUALITAS PELAYANAN KESEHATAN PADA RUMAH SAKIT UMUM MUHAMMADIYAH KOTA METRO
ABSTRACT This study aims to (1) determine the effect the reliability of customer satisfaction, (2) the effect of responsiveness to customer satisfaction, (3) identify and analyze the influence of empathy towards customer satisfaction, (4) identify and analyze the influence of the reliability of the empaty, (5 ) identify and analyze the influence responsiveness to empaty. The method used is quantitative method, test hypotheses and path analysis. Based on research that has been done can be concluded that the results of the study found that the variable reliability, positive effect on customer satisfaction. Variable responsiveness positive effect on customer satisfaction.Variable empaty positive effect on customer satisfaction.Variable reliability, a positive direct effect on empathy.Variable responsiveness positive effect on empathy. The implication of this study is an attempt to repair the infrastructure, improve the reliability of employees when serving patients, responsive to the wishes of patients, providing assurance of service time, and give attention personally sincere empathy and understand consumer desires.Keywords: reliability, responsiveness, empathy, customer satisfaction
HUBUNGAN DAYA TANGGAP TERHADAP LOYALITAS PELANGGAN (Studi Kasus Pada Nasabah Tabungan Supa PT. BPR Sumber Pangasean Bandar Jaya)
Penelitian ini menggunakan daya tanggap sebagai variabel independen dan dan loyalitas sebagai variabel dependen. Tujuan dari penelitian ini adalah untuk mengetahui hubungan daya tanggap terhadap loyalitas pelanggan pada PT. BPR Sumber Pangasean Bandar Jaya.Metode penelitian menggunakan survey, dengan jumlah sampel sebanyak sebanyak 60 nasabah dengan tingkat ketelitian sebesar 10%. Alat analisis yang digunakan adalah analisis rank spearman untuk mengetahui hubungan variabel daya tanggap terhadap loyalitas pelanggan dengan menggunakan alat bantu program komputer.Berdasarkan dari perhitungan dan pengujian hipotesis diperoleh hasil bahwa daya tanggap mempunyai korelasi atau hubungan yang signifikan dengan variabel loyalitas pelanggan. Artinya semakin tinggi daya tanggap, maka loyalitas pelanggan juga akan semakin tinggi. Kata Kunci: Daya tanggap, loyalitas pelangga
Does an Agency-Type of Audit Model Fit a Stewardship Context? Evidence from Performance Auditing in Dutch Municipalities
The degree of auditor independence is an important issue in the performance auditing literature. However, little attention has been paid to the influence of the context in which an audit body operates. This paper investigates how an audit model with a high degree of auditor independence, which is consistent with agency theory’s rather formal view of relationships in organizations, functions in a context with more informal relationships, as implied by stewardship theory. Based on two case studies and a survey, the paper concludes that Dutch councilors are more satisfied with audit reports if in their municipality there is a fit between context and audit model
Feedback-giving behaviour in performance evaluations during clinical clerkships
CONTEXT: Narrative feedback documented in performance evaluations by the teacher, i.e. the clinical supervisor, is generally accepted to be essential for workplace learning. Many studies have examined factors of influence on the usage of mini-clinical evaluation exercise (mini-CEX) instruments and provision of feedback, but little is known about how these factors influence teachers' feedback-giving behaviour. In this study, we investigated teachers' use of mini-CEX in performance evaluations to provide narrative feedback in undergraduate clinical training.METHODS: We designed an exploratory qualitative study using an interpretive approach. Focusing on the usage of mini-CEX instruments in clinical training, we conducted semi-structured interviews to explore teachers' perceptions. Between February and June 2013, we conducted interviews with 14 clinicians participated as teachers during undergraduate clinical clerkships. Informed by concepts from the literature, we coded interview transcripts and iteratively reduced and displayed data using template analysis.RESULTS: We identified three main themes of interrelated factors that influenced teachers' practice with regard to mini-CEX instruments: teacher-related factors; teacher-student interaction-related factors, and teacher-context interaction-related factors. Four issues (direct observation, relationship between teacher and student, verbal versus written feedback, formative versus summative purposes) that are pertinent to workplace-based performance evaluations were presented to clarify how different factors interact with each other and influence teachers' feedback-giving behaviour. Embedding performance observation in clinical practice and establishing trustworthy teacher-student relationships in more longitudinal clinical clerkships were considered important in creating a learning environment that supports and facilitates the feedback exchange.CONCLUSION: Teachers' feedback-giving behaviour within the clinical context results from the interaction between personal, interpersonal and contextual factors. Increasing insight into how teachers use mini-CEX instruments in daily practice may offer strategies for creating a professional learning culture in which feedback giving and seeking would be enhanced.</p
