    Elucidation Of Hub Genes And Pathways Associated With Gut Microbiome Dysbiosis And Mild Cognitive Impairment

    Bidirectional communication route of brain and gut, gut-brain axis, was suggested to contribute to mild cognitive impairment (MCI) through inflammatory bowel disease (IBD) as gut microbiome dysbiosis occurs. Therefore, this study aimed to predict the gene signatures, and the potential molecular network associated between gut microbiome dysbiosis and MCI. To initiate the study, datasets (GSE13367, GSE140829, GSE36701 and GSE63063) were extracted from Gene Expression Omnibus (GEO) and the differentially expressed genes (DEGs) were selected based on the Log2 fold change >1.0 with p-value < 0.05 for both MCI and IBD. The shared DEGs between MCI and IBD were identified, and inputted into Database for Annotation, Visualization and Integrated Discovery database (DAVID) to perform gene ontology (GO) for functional enrichment analysis. The functional pathways were selected based on p<0.05 and FDR <0.05. The GeneMANIA was used to construct the protein-protein interaction (PPI) network. This study identified 1061 common shared DEGs between MCI and IBD. Based on the GO enrichment study, majority of the genes were involved in protein binding for molecular function, signal transduction biological process and cytosol for cellular component. The co-expression network showed that leukocyte cell-cell adhesion and Type 1 interferon function might contribute to IBD and MCI. Further elucidation studies on DEGs showed that leukocyte cell-cell adhesion signaling pathway would be the main pathogenesis pathway leading to both diseases, in which 8 genes involved were CD3E, PTPRC, GRAP2, CD86, VAV1, SEMA4D, FYN and LYN. Thus, further studies especially in the perspective of neuroinflammation is worth to be explored

It becomes an essential component of modern retailing, a key element of competitive strategy, as well as a sustainable competitive advantage. Today, modern consumers expect personalization throughout their entire interaction with a retailer – across all possible touch points (online and offline). The article examines the evolution of the concept of personalization from the traditional marketing approach to the interruption marketing and permission marketing; the relationship between personalization and permission marketing; the opt-in marketing; the relationship between personalization and interruption marketing; the e-marketing mix and the place of personalization in it.     Tracheal (accessory) bronchus – a case report

    Introduction: Tracheal bronchus (also known as accessory bronchus) is a congenital anatomical variant in which an additional bronchus originates directly from the supracarinal trachea. This type of anomaly usually arises about 2cm above the carina but can occur anywhere along the trachea, from the cricoid cartilage to the tracheal bifurcation.Case Presentation: We present a case from our practice of a 62-year-old male, who arrived at our Multidisciplinary Emergency Department with complaints of sudden onset of dyspnea and high blood pressure with values of 225/125mmHg. The patient was admitted to the Clinic of Pulmonology and Phthisiology. He also reported having mild haemoptysis for the last 2 to 3 months. It was decided that a contrast-enhanced computed tomography scan with contrast enhansement should be performed, for the evaluation of possible pulmonary thromboembolism (PTE). As a result of the imaging study an accessory right bronchus was identified as an incidental finding.Results: After excluding PTE as a possible diagnosis, the following changes were identified on the CT scan: accessory right bronchus located about 20mm from the carina; multilobulated retrotracheal gas structure (differential diagnosis - atypical accessory pulmonary segment; extraluminal mediastinal gas collections; tracheal diverticulosis); a perihilar tumour formation in the left lung, causing stenosis of the left main bronchus and its lobar branches; a combination of subsolid micronodules and ground glass opacities affecting most of the left lower lobe.Conclusion: Tracheal bronchus is a congenital anomaly of the bronchial tree, often remaining entirely asymptomatic. Follow-up of such a finding should be done by an interdisciplinary team

    Contemporary Challenges for Teaching Staff in Higher Medical Education

    INTRODUCTION: Since the beginning of the new millennium, humanity has been confronted with a variety of challenges. The search for effective solutions to address these challenges has led to the development of numerous strategic documents at both the European and national levels, including initiatives for the development and modernization of education.AIM: To highlight the challenges facing higher education, particularly teaching staff in higher medical schools, and to outline potential actions to address these challenges.MATERIALS AND METHODS: Literature sources, strategic and normative documents related to the development of higher education in Europe and the Republic of Bulgaria, as well as internal normative documents of the Medical University of Varna, were used. The following methods were applied: theoretical analysis, summarization, and drawing conclusions.RESULTS: The modernization of higher education in the Republic of Bulgaria, particularly in higher medical schools, aligns with the development guidelines of other EU member states and supports collective efforts to establish a unified European area for higher education. However, the creation of such an area inevitably raises the issue of developing and implementing uniform standards across different fields of education to ensure the comparability of national outcomes. It also highlights the need for a unified educational information system to facilitate the rapid dissemination of information through new educational technologies.CONCLUSION: The analysis of literature sources, strategic, and normative documents clearly highlights that the modernization and development of higher education depend significantly on teaching innovation. This requires effective interaction among peer entities within the institutionalized educational process and ongoing training for teaching staff
