Relationship between a Myokonin-Like Way of Life and Subjective QOL in a Patient Requiring High-Level Nursing Care‐A Case of a Patient with Chronic Renal Insufficiency Treated at Home‐
本研究では,一貫して高水準の主観的QOLを維持したクライアントの行動及びナラティヴを,援助者(訪問リハビリテーションを担当した作業療法士)の視点から分析し,妙好人としての生き方と主観的QOLとの関連性を検討した.妙好人とは,浄土真宗の在俗の篤信者で,いかなる困難に見舞われても心が安寧で感謝の念を頻繁に示すという特徴がある.
クライアントは,慢性腎不全で入院し要介護4で退院した後,訪問リハビリテーションの受療を開始し,要介護2まで改善した.その後,再び要介護4へと悪化し,改善することはなかった.また,週3回の人工透析と合併症との共存を余儀なくされた.しかし,いかなる状況でも常に感謝と他者への気遣いを貫いた.そうした妙好人的な生き方が,安定的に高い主観的QOLを示した要因であったことが示唆された.一見,セラピストがクライアントを癒す立場にあると思われがちだが,実際は,クライアントとセラピストは互いに癒し合う立場にあると考える.departmental bulletin pape
