当院における心肺蘇生法教育の現状と課題
Get PDF本邦における心肺蘇生法教育は，米国の二次救命処置（ACLS；Advanced Cardiovascular Life Support）コースが輸入されたのち，日本救急医学会においてICLS（Immediate Cardiac Life Support）コースが開発され，本格的な普及が始まった．当院では2003年に第１回ACLS 基礎コースを開催した．2005年にICLS 小委員会が発足し，委員会活動としてICLS コースを開催しながら，全職員を対象にBLS（Basic Life Support）コースも開始した．その後も種々の教育プログラムを取り入れながら工夫を重ね，現在まで継続している．今回，当院における心肺蘇生法教育の発展と変遷についてまとめた．これまでの活動は院内の医療安全に大きく貢献しており，地域貢献も見据えながら今後も活動を継続したい．The Advanced Cardiovascular Life Support（ACLS）course was imported to Japan from the United States, and the Immediate Cardiac Life Support（ICLS）course was developed by the Japanese Association for Acute Medicine. In 2003, the first ACLS basic course was held at our hospital, and in 2005, an ICLS subcommittee was established. While conducting the ICLS course as a committee activity, a Basic Life Support（BLS）course was also introduced for all staff members. The committee has since continued to devise and implement various educational programs, which remain active in the present. This report summarizes the development and evolution of CPR education at our hospital. Our activities to date have greatly contributed to medical safety at our hospital,and we aim to continue our efforts in the future with a goal of extending services to the local community.journal articl
当院におけるMSI-High/dMMR 消化器癌患者の治療経験

石灰化した縦隔リンパ節の瓦解により形成された炎症性気管支ポリープの１例
Get PDF症例は70歳代，男性．胸部CT で７年前と比較して気管分岐部の小結節影，右主気管支から中間気管支幹にかけての壁肥厚，石灰化した気管分岐下リンパ節の瓦解・縮小を認めた．気管支鏡検査では気管分岐部に赤色調で表面平滑なポリープと，右主気管支から中間気管支幹にかけての内腔も発赤調で腫脹し，一部舌状のポリープ様病変を認めた．抗酸菌は検出されなかったが，炎症細胞浸潤を伴う肉芽組織を認め，炎症性気管支ポリープと診断した．結核性リンパ節炎が疑われ，抗結核薬治療を行い気管支病変は改善した．石灰化したリンパ節が瓦解し炎症性気管支ポリープを認めた場合は，結核性リンパ節炎の合併を考慮する必要がある．technical repor
Prehospital emergency care family satisfaction scale for care provided by emergency medical technicians: Scale development and validation
Get PDFRationale: To date, family satisfaction with emergency medical technicians (EMTs) has only been reported through narrative statements in subjective evaluations. Although a quantitative assessment of healthcare professional satisfaction is desirable, no specific measures have been devised to assess family satisfaction with the healthcare and care provided by EMTs.
Aims and objectives: This study aimed to develop and validate an EMT care family satisfaction scale to measure patient satisfaction with prehospital emergency care.
Methods: The study population comprised 216 family members (N = 216) of patients who used ambulances between November 2020 and May 2021 in a single region in Japan. Questionnaires were distributed to the participants who provided informed consent. An exploratory factor analysis of construct validity was performed to validate the Family Satisfaction Scale. The Cronbach's alpha was used to validate the internal consistency reliability of the scale.
Results: The exploratory factor analysis results revealed a four-factor structure: 'explanation and communication,' 'physical treatment,' 'psychological support,' and 'environment in the ambulance.' The Cronbach's range (0.80-0.93) for the total score for each of these four factors and the overall total score confirmed favorable internal reliability of this study.
Conclusions: The family satisfaction scale developed in this study was constructed and validated to highlight the role of EMTs and needs of the families in the prehospital care settings. Moreover, this scale can be applied in the evaluation and consideration of interventions to improve family satisfaction with EMTs.journal articl

続発性リンパ浮腫を有する女性患者の弾性着衣による影響と改善の工夫
Get PDF目的：研究目的は乳がん・婦人科がん治療後の続発性リンパ浮腫患者が体験している弾性着衣の使用に伴う影響と改善の工夫を明らかにすることである．
方法：乳がんまたは婦人科がんの治療後，続発性リンパ浮腫と診断され弾性着衣を使用している女性患者６名に対し半構造化面接を用いて質的帰納的分析を行った．
結果：弾性着衣による影響は６つのカテゴリが抽出され，着用による【手指や関節への負担】【皮膚への負担】といった身体的影響，【日常生活への支障】【着用継続による経済的負担】【着用による安心感】等といった日常生活への影響が挙げられた．改善の工夫は７つのカテゴリが抽出され，【着用時の引き上げを楽にする】【疼痛を緩和する】【皮膚を保護する】【暑さに対処する】【睡眠時の負担を軽減する】等であった．
考察：弾性着衣は身体的苦痛や心理社会的苦痛に繋がる可能性があるため，患者の改善の工夫を尊重した個別対応が重要である．departmental bulletin pape
