しての三次リンパ組織形成および新規濾胞関連リンパ球サブセットの解明) 
 
 研究目的 
 腹部大動脈瘤（以下 AAA）の形成進展機序には瘤壁局所の慢性炎症が関与するとされる
が、その調節機序は不明である。近年炎症性疾患や悪性腫瘍など組織の慢性炎症が病態に関
与する疾患において、局所慢性炎症の調節および病態制御機構として三次リンパ組織（以下
TLS）の形成が注目されている。そこで本研究では、AAAの病態制御機構として TLSが果
たす役割に着目し検討を行った。 
 
 研究方法   
 研究同意を取得した AAA開腹手術症例を対象とし、血液および組織サンプルを採取した。
組織は一部を永久標本として TLS の形態解析を行い、もう一部は血液とともにリンパ球を
抽出してフローサイトメトリーを行った。TLS 形成および組織中のリンパ球と臨床データ
との相関を検討した。また公共データを用いたシングルセル解析を行い、AAA 組織中のリ
ンパ球に関する普遍性の評価およびエンリッチメント解析による機能的評価を行った。 
 
研究成績及び考察 
 研究機関において 37 例が対象となった。TLS は AAA 組織で有意に形成され、瘤径およ
び壁在血栓と正に相関した。また組織リンパ球は T/B細胞が中心であり、このうち濾胞ヘル
パーT細胞および Double negative B 細胞といった新規サブセットが瘤径・壁在血栓量と有
意に相関した。エンリッチメント解析ではこれら両細胞集団における T 細胞活性化経路や
炎症経路、脂質代謝経路が有意に亢進していることが示唆された。 
 
結論 
  AAA における TLS の形成は瘤径や壁在血栓と関連し、濾胞ヘルパーT 細胞や Double 
negative B 細胞と関連していた。これらの細胞群は AAA病態生理に寄与している可能性が
あり、AAA発症の新たな治療標的となりうる。

 研究同意を取得した腹部大動脈瘤（AAA）開腹手術症例 37例を対象とし、血液および
組織サンプルを採取した。組織は一部を永久標本として三次リンパ組織（TLS）の形態
解析を行い、もう一部は血液とともにリンパ球を抽出してフローサイトメトリーを行っ
た。TLS は AAA 組織で有意に形成され、瘤径および壁在血栓と正に相関した。また組
織リンパ球は T/B 細胞が中心であり、このうち濾胞ヘルパーT 細胞および Double 
negative B細胞といった新規サブセットが瘤径・壁在血栓量と有意に相関した。エンリ
ッチメント解析ではこれら両細胞集団における T 細胞活性化経路や炎症経路、脂質代謝
経路が有意に亢進していることが示唆された。以上より、AAA における TLS の形成は
瘤径や壁在血栓と関連し、濾胞ヘルパーT細胞や Double negative B細胞が AAA病態と
関連することが示唆された。これらの細胞群は AAA 病態生理に寄与し、AAA 発症の新
たな治療標的となりうる可能性が考えられた。本研究結果は、Journal of the American 
Heart Association 誌へ Acceptされた。

Effects of postprandial hyperglycemia on blood flow regulation during exercise in young adults  
with normal fasting blood glucose 
背景および目的：運動時には交感神経活動が増加し，交感神経性血管収縮が生じる．し
かし活動肢では，骨格筋への酸素供給を維持するために，交感神経性血管収縮を減弱さ
せる機能的交感神経遮断が生じる．内皮由来の血管拡張能は，機能的交感神経遮断のメ
カニズムにおいて重要な役割を果たすと考えられている．慢性の高血糖を示す 2 型糖尿
病患者では，血流依存性血管拡張反応（FMD）で評価される血管内皮機能が低く，機能
的交感神経遮断も減弱していることから，運動時に血流量（BF）の増加が小さいことが
示されている．急性の高血糖である食後高血糖も FMD を急性に低下させ，安静時に骨
格筋の BF を減少させる．しかし，食後高血糖が運動中に活動肢の血流調節に与える影
響は不明である．そこで本研究は，食後高血糖が機能的交感神経遮断を急性に減弱さ
せ，運動時に活動肢の BF 増加を抑制するかを明らかにすることを目的とした．  
方法：糖尿病と診断されたことがない若年成人男女 25 名をリクルートし，75 g ブドウ
糖負荷後 60 分以内に，血糖値が 140 mg/dL 以上を示した 14 名（男性 8 名，女性 6 名，
25 ± 6 歳，平均 ± 標準偏差）を対象とした．75 g の経口果糖負荷（Sham 試行）または
経口ブドウ糖負荷（Glucose 試行）を 2 日に分けて別日に実施した．糖負荷前と 60 分後
に左腕にて上腕動脈の FMD を測定した．運動時の測定は糖負荷後 40 分後から開始し，
2 分間の安静ののち，運動時の測定を行った．被験者は最大随意収縮（MVC）の 15%強
度にて 5 分間の動的な片腕（左腕）ハンドグリップ運動を行い，運動の後半 2 分間に交
感神経刺激として下半身陰圧負荷（LBNP）を実施した．上腕動脈の血管径と血流速度
は，FMD と運動時測定において，超音波診断装置を用いて測定した．FMD 測定は活動
肢のみで実施し，FMD は安静時の上腕動脈血管径から前腕カフを解放した後の最大拡張
径までの血管拡張率として算出した．運動時の測定は，活動肢と非活動肢の両方で行
い，BF および前腕血管コンダクタンス（FVC）を算出した． LBNP による FVC変化率
 
 
（Δ%FVC）は両側で算出され，その差を機能的交感神経遮断の大きさ（Δ%FVC active-
inactive）として算出した．また，運動による BF 増加量（ΔBF）をその差分として算出
した．静脈血サンプルは右正中皮静脈から，糖負荷前，糖負荷の 30 分後， 40 分後，47
分後，60 分後にそれぞれ採血を行い，血糖値とインスリンを分析した． 
結果：Sham 試行では，血糖値とインスリンが糖負荷後わずかに増加したが，FMD は低
下しなかった（糖負荷前 vs. 糖負荷 60 分後, 7.1 ± 2.2% vs. 8.1 ± 2.5%，P = 0.20）．一方，
Glucose 試行では，血糖値もインスリンも Sham 試行より大きく増加し，糖負荷の 40 分
後時点には，血糖値の平均値は 140 mg/dL 以上であった（両者ともに交互作用 P < 0.01，
試行の主効果 P < 0.01）．その結果，Glucose 試行では FMD が低下した（糖負荷前 vs. 糖
負荷 60 分後，7.5 ± 2.3% vs. 5.6 ± 1.7%，P = 0.01）．運動による血流増加量である活動肢
の ΔBF は Glucose 試行で小さかった（Sham vs. Glucose，101 ± 53 mL/min vs. 70 ± 49 
mL/min，P = 0.03）．また，機能的交感神経遮断の大きさを示す Δ%FVC active-inactive
は，Glucose 試行で Sham 施行より低値を示した（15 ± 14% vs. 8 ± 14%，P = 0.03）． 
考察：Glucose 試行では，食後高血糖および内皮機能の低下が観察された．この内皮機
能の低下の要因の 1つとして，酸化ストレスの増加が推測される．また，本研究の被験
者は経口ブドウ糖負荷後に高インスリン状態であり，インスリン抵抗性を有していた可
能性がある．先行研究では，インスリン抵抗性を有する者において，インスリンによる
一酸化窒素（NO）介在性血管拡張が抑制され，エンドセリン-1介在性血管収縮が優位
になることが示唆されている．このように食後高血糖・高インスリン状態に伴う内皮由
来の NO介在性血管拡張能の低下および収縮作用の増加が，機能的交感神経遮断を減弱
させ，運動時に活動肢の血流量増加を抑制した可能性がある． 
結論：本研究は，空腹時血糖値が正常な若年成人において，食後高血糖は血管内皮機能
の低下を引き起こし，機能的交感神経遮断を減弱させ，運動時における活動肢の血流量
増加を抑制することを明らかにした．本研究の結果は，食後高血糖が運動耐容能低下の
一因となる可能性を示唆するものである．

Introduction: Exercise increases sympathetic vasoconstrictor outflow. However, this sympathetic 
vasoconstriction is normally attenuated in the active limb to facilitate oxygen delivery to the 
skeletal muscle, a process termed functional sympatholysis. Endothelial-dependent vasodilation is 
speculated to play a crucial role in the mechanism of functional sympatholysis. In patients with 
type 2 diabetes experiencing chronic hyperglycemia, vascular endothelial function, as indicated by 
brachial flow-mediated dilation (FMD), is reduced. In these patients, functional sympatholysis is 
attenuated, resulting in a smaller increase in blood flow (BF) in the active limb during exercise. 
Moreover, postprandial hyperglycemia, an acute rise in blood glucose that reduce brachial FMD, 
also decreased brachial BF at rest. However, the effect of postprandial hyperglycemia on BF 
regulation in the active limbs during exercise has not yet been elucidated. Therefore, this study 
aimed to investigate whether postprandial hyperglycemia acutely attenuates functional 
sympatholysis and suppresses the increase in brachial BF in the active limbs during exercise. 
Methods: We recruited 25 young adult men and women who had never been diagnosed with 
diabetes. Fourteen participants (8 men and 6 women, aged 25 ± 6 years, mean ± SD) whose blood 
glucose levels exceeded 140 mg/dL within 60 min of a 75-g glucose load were included. 
Participants underwent either a 75-g oral fructose load (Sham trial) or a 75-g oral glucose load 
(Glucose trial) on separate days. Brachial FMD in the left arm was measured 60 min before and 
after the load. Exercise measurements began 40 min after the load, following an initial 2 min of 
rest. Subjects then performed 5 min of dynamic handgrip exercise of the left arm at 15% of 
maximal voluntary contraction. Lower-body negative pressure (LBNP) was applied as a 
sympathetic stimulus during the last 2 min of exercise. The diameter of the brachial artery and BF 
velocity were measured using a Doppler ultrasound during both FMD and exercise measurements. 
The brachial FMD measurement was conducted only in the active limb and calculated as a 
percentage increase in artery diameter following forearm cuff release compared to baseline values. 
Exercise measurements were obtained in both the active and inactive limbs, and BF and forearm 
vascular conductance (FVC) were calculated. The change in FVC during LBNP (Δ%FVC) was 
calculated for both sides, and the difference between them was calculated to identify the 
magnitude of the functional sympatholysis (Δ%FVC active-inactive). The exercise-induced 
increase in BF (ΔBF) was calculated as the difference between rest and exercise. Venous blood 
samples were collected from the median antecubital vein of the right arm before (Pre) glucose 
loading and at 30, 40, 47, and 60 min after the glucose load to analyze blood glucose levels and 
insulin. 
Results: Blood glucose and insulin levels slightly increased after the fructose load, but FMD did 
not decrease in the Sham trial (FMD, Pre vs. 60 min, 7.1 ± 2.2% vs. 8.1 ± 2.5%, P = 0.20). As 
anticipated, both blood glucose and insulin levels increased significantly in the Glucose trial 
 
 
compared with the Sham trial, with the mean blood glucose level of >140 mg/dL by 40 min post-
load (interaction, P < 0.01; main effect of trial, P < 0.01 for both). Consequently, FMD decreased 
in the Glucose trial (Pre vs. 60 min, 7.5 ± 2.3% vs. 5.6 ± 1.7%, P = 0.01). The exercise-induced an 
increase in BF (ΔBF) was smaller in the Glucose trial than the Sham trial (Sham vs. Glucose, 101 
± 53 mL/min vs. 70 ± 49 mL/min, P = 0.03). Moreover, Δ%FVC active-inactive (the magnitude of 
functional sympatholysis) was lower in the Glucose trial compared with the Sham trial (15 ± 14% 
vs. 8 ± 14%, P = 0.03). 
Discussions: In the Glucose trial, postprandial hyperglycemia and endothelial dysfunction were 
observed. An increase in oxidative stress is speculated to be one of the factors contributing to 
endothelial dysfunction. Moreover, the participants in this study exhibited hyperinsulinemia 
following oral glucose loading, suggesting the possibility of insulin resistance. Previous studies 
have indicated that individuals with insulin resistance experience suppressed nitric oxide (NO)-
mediated vasodilation and a predominance of endothelin-1-mediated vasoconstriction by insulin. 
Consequently, the reduced endothelium-derived NO-mediated vasodilatory effects along with 
increased vasoconstrictive effects following postprandial hyperglycemia and hyperinsulinemia 
may attenuate functional sympatholysis, thereby inhibiting the increase in blood flow of the active 
limb during exercise. 
Conclusions: Postprandial hyperglycemia attenuated endothelial function and functional 
sympatholysis, leading to a suppression of the increase in BF to the active limb during exercise in 
young adults with normal fasting glucose. This study provides preliminary evidence that 
postprandial hyperglycemia may contribute to reduced exercise tolerance.

・日本語要旨 
【はじめに】ハンドセラピィでは，早期運動療法の実践や useful hand を獲得するために，
急性期の手の疼痛に対するアプローチが重要となる．近年，振動刺激を用いた運動錯覚が
着目されており，橈骨遠位端骨折術後患者例において，術後早期の疼痛を改善させること
が報告されている．しかし，客観的指標による疼痛の改善効果が検証されていないこと
や，先行研究では運動錯覚の惹起率にばらつきを認め個々の対象への効果は均一ではな
いことが課題として報告されている．惹起率にばらつきが生じる要因について，個人因子
や疼痛の情動的側面を検討する必要性が示唆されているが検証はされていない． 
【目的】手の疼痛に対して振動刺激を効果的に用いるために，(1)客観的効果を知覚・痛
覚分析装置を用いて明らかにすること，(2)個人因子の素因的な要因である性格傾向，疼
痛の情動的側面である不安から効果的な対象を明らかにすることを目的とした． 
【方法】対象は，成人健常者(18〜70 歳) 46 例で右利きとした．はじめに，性格傾向を日
本語版 Ten Item Personality Inventory(以下，TIPI-J)，疼痛の情動的側面を State-Trait Anxiety 
Inventory(以下，STAI)を用いて評価した．次に，対象が感じる疼痛の大きさを電流刺激に
置き換えることで疼痛を客観的に評価できる知覚・痛覚定量分析装置の PainVision®を用
いて，指標となる痛み度の測定を行った．測定部位は，右橈骨手根関節背側とし，振動刺
激介入実施後に痛み度の評価を行った．そして，振動刺激を今井らの方法に基づき実施
し，運動錯覚強度を Visual Analogue Scale(以下，VAS），運動錯覚角度を右手関節背屈の自
動運動により再現させ測定した．データ分析は，振動刺激前後における痛み度の比較につ
いて Wilcoxon の順位和検定を行った．次に，運動錯覚強度と運動錯覚角度をそれぞれ目
的変数とし，TIPI-J と STAI を説明変数として重回帰分析を行った．そして，対象を運動
錯覚あり群となし群に分け，年齢と TIPI-J，STAI(状態不安)に対応のない t 検定，各群の
 
 
振動刺激前後の痛み度の変化に Wilcoxon の順位和検定，性別に χ 二乗検定を行った．各
統計分析は，IBM SPSS Statistics version 28.0 を使用し統計学的有意水準は 5%未満とした．
本研究は本学倫理委員会の承認を得ている(承認番号 4-1-79)． 
【結果】振動刺激前/後の痛み度は 476.0±403.6/401.3±325.7 で有意差(p=0.01)を認めた．運
動錯覚強度(VAS)は 8.1±13.8，運動錯覚角度は 21.1°±23.6°であり，重回帰分析の結果，目
的変数の運動錯覚強度に対し TIPI-J(協調性)のみが選択され，決定係数は R2＝0.110 であ
った．また，運動錯覚角度ではいずれの説明変数も最終変数に選択されなかった．運動錯
覚あり群(22 例)と運動錯覚なし群(24 例)において，年齢と性別，TIPI-J の下位尺度，STAI(状
態不安)に有意差は認めなかった．振動刺激前/後の各群の痛み度は，運動錯覚あり群で
585.0±495.6/415.0±317.9 で有意差(p＝0.002)を認め，運動錯覚なし群で 367.0±237.6/387.7± 
332.7 で有意差(p＝0.732)は認めなかった． 
【考察】本研究では振動刺激前後において痛み度が有意に改善し，また，運動錯覚の有無
による比較では，運動錯覚あり群に痛み度の有意な改善を認めた．このことから，振動刺
激を用いた運動錯覚による介入は，客観的にも除痛効果を有する可能性が示唆された．重
回帰分析の結果，目的変数の運動錯覚強度に対し説明変数として協調性が選択されたが
R2＝0.110 と予測精度は低く，性格傾向や不安などの個人因子から振動刺激の効果的な適
応となる対象の特徴は明確にできなかった．そのため，運動錯覚の惹起率や強化を図るた
めに，適切な周波数や振動刺激を行う筋腱の検証，併用する治療方略を検討することも必
要と考える．本研究の限界として対象者を健常者としているため，疼痛を有する上肢運動
器疾患患者に適応した場合に，同様の結果が得られるかについて明らかにできない． 
【結論】振動刺激の疼痛に対する効果について，知覚・痛覚定量分析装置を用いて検討し
た結果，振動刺激は客観的に疼痛を改善させることが明らかになった．一方，性格傾向や
疼痛の情動的側面である不安といった個人因子は振動刺激の効果的な対象を選択する際
の要因とはならないことが示唆された．

【Introduction】In hand therapy, approaches to acute hand pain are crucial for implementing early 
mobilization therapy and achieving a “useful hand”. Recently, movement illusions induced by 
vibration stimulation have gained attention, with reports of their effectiveness at improving early 
postoperative pain in patients following distal radius fracture surgery. However, objective 
evaluation of pain improvement is lacking, and previous studies have reported variability in the 
elicitation rate of movement illusions, indicating that the effects on individual patients are not 
uniform. Factors contributing to this variability have been suggested to include personal traits and 
emotional aspects of pain, but these factors have not been thoroughly investigated. 
 
 
【Purpose】The purposes of this study were to: (1) clarify the objective effects of vibration 
stimulation using a perception/pain analysis device, and (2) identify effective candidates for 
treatment based on personality traits, which are intrinsic personal factors, and anxiety, which is an 
emotional aspect of pain. 
【Methods】The subjects were 46 healthy right-handed adults aged 18 to 70. First, personality 
traits were evaluated using the Japanese version of the Ten Item Personality Inventory (TIPI-J), and 
the emotional aspect of pain was assessed using the State-Trait Anxiety Inventory (STAI). Next, 
pain intensity was objectively measured using the PainVision® device, which evaluates pain by 
converting the subject’s perceived pain into an electrical stimulus. The measurement site was the 
dorsal aspect of the right radiocarpal joint, and pain intensity was evaluated before and after the 
application of vibration stimulation. Vibration stimulation was applied based on the method 
described by Imai et al. Movement illusion intensity was measured using the visual analogue scale 
(VAS), and the movement illusion angle was reproduced through voluntary dorsiflexion of the right 
wrist. Data analysis involved comparing pain intensity before and after vibration stimulation using 
the Wilcoxon signed-rank test. Additionally, multiple regression analysis was performed with 
movement illusion intensity and movement illusion angle as the dependent variables, and TIPI-J 
and STAI as explanatory variables. The subjects were divided into two groups: those with and 
without movement illusions. An independent Student’s t-test was conducted to compare age, TIPI-
J scores, and STAI (state anxiety), while a chi-squared test was used to analyze gender differences. 
Changes in pain intensity before and after vibration stimulation were compared within each group 
using the Wilcoxon signed-rank test. All statistical analyses were conducted using IBM SPSS 
Statistics version 28.0, with a significance level set at less than 5%. This study was approved by the 
ethics committee of our institution (approval number 4-1-79). 
【Results】Pain intensity before and after vibration stimulation was 476.0 ± 403.6 and 401.3 ± 
325.7, respectively, showing a significant difference (p = 0.01). Movement illusion intensity (VAS) 
was 8.1 ± 13.8, and the movement illusion angle was 21.1° ± 23.6°. In the multiple regression 
analysis, only “agreeableness” from the TIPI-J was selected as an explanatory variable for 
movement illusion intensity, with a coefficient of determination R² = 0.110. None of the explanatory 
variables were selected for the movement illusion angle. In the comparison between the group with 
movement illusions (22 cases) and the group without (24 cases), no significant differences were 
found for age, gender, TIPI-J subscales, or STAI (state anxiety). Pain intensity in the group with 
movement illusions significantly decreased from 585.0 ± 495.6 to 415.0 ± 317.9 (p = 0.002), while 
no significant difference was observed in the group without movement illusions (367.0 ± 237.6 to 
387.7 ± 332.7, p = 0.732). 
【Discussion】This study demonstrated a significant improvement in pain intensity before and after 
vibration stimulation. Moreover, in the comparison between the groups with and without movement 
 
 
illusions, a significant improvement in pain intensity was observed in the group with movement 
illusions. This suggests that interventions using movement illusions induced by vibration 
stimulation may have an objective pain-relieving effect. In the multiple regression analysis, 
“agreeableness” was selected as the explanatory variable for movement illusion intensity, but with 
an R² of 0.110, the predictive accuracy was low. Therefore, personal factors, such as personality 
traits and anxiety could not be clearly identified as characteristics of effective candidates for 
vibration stimulation. Further investigation is needed regarding the appropriate frequencies, the 
muscles and tendons where vibration stimulation should be applied, and combined therapeutic 
strategies to improve the elicitation rate and strength of movement illusions. One limitation of this 
study is that the subjects were healthy individuals, so it is unclear whether the same results would 
be obtained when applied to patients with upper-limb musculoskeletal disorders accompanied by 
pain. 
【Conclusion】This study investigated the effects of vibration stimulation on pain using a 
perception/pain analysis device. The results showed that vibration stimulation objectively improved 
pain. However, personality traits and anxiety, which are emotional aspects of pain, were not 
identified as factors for selecting effective candidates for vibration stimulation.

研究の目的 
心臓大血管手術におけるせん妄は他の手術と比較して発症率は高く、周術期合併症の増加
や入院期間の延長などの術後の転帰にも影響があるため適切な評価や介入に基づく予防が
必要である。当科では待機手術を受ける患者に対して原則全例で術前評価として、認知機
能検査と前頭葉機能検査を実施しているが、これまでの術後せん妄の予測因子として年齢
や既往歴などの因子についての検討はなされているが、このような高次脳機能と心臓血管
手術後のせん妄発症との関係について述べられている研究は見られなかった。そこで、本
学における心臓血管外科術患者の術前高次脳機能と術後せん妄の発症との関係を明らかに
することを目的とした。 
 
方法 
2023 年 4 月 1 日から 2024 年 3 月 31 日までの 1 年間に札幌医科大学附属病院の
心臓血管外科で開胸開腹手術を受けた患者を対象とし、術後に重度な脳血管疾患を合併し
た患者を除外した。患者背景因子として年齢、性別、術式を調査し、術前の高次脳機能評
価は、MMSE（Mini-Mental State Examination）、FAB（Frontal Assessment Battery）、
IQ（Intelligence Quotient）を行った。術後せん妄の評価には CAM-ICU を使用し、抜管
後 3 日間連続で評価を行い、1 度でも陽性となった患者を術後せん妄発症患者として定義
した。術後せん妄発症群と未発症群の比較として術前高次脳機能の MMSE 、FAB、IQ に
患者背景因子である年齢を加えた 4 要因において一元配置分析を用いた。得られた値は平
均値±標準偏差で表示し、有意水準は 5% 未満とした。副次的評価としてせん妄発症群と
未発症群の入院期間においても比較を行った。 
 
結果 
本研究における術後せん妄の発症率は 13% であった。せん妄群と非せん妄群において統
計学的解析をした結果、非せん妄群の平均年齢はせん妄群と比較して有意に低く、術前高
次脳機能 FAB、IQ においてはせん妄群の成績は非せん妄群と比較して有意に低値を示した。
しかし、MMSE においては両群に有意差を認めなかった。入院期間に関しても術後せん妄
発症群の方が約 2 週間程度長く、両群に有意差を認めた。

考察 
本研究の術後せん妄発症率は 13% であったが、先行研究と比較しても大きくは変わらな
い結果であった。本研究において術後せん妄発症群と非発症群の前頭葉機能である FAB と
知能指数である IQ に有意差を認めたが、前頭葉は習得した意図的な行動の計画及び遂行に
不可欠であり、同時に多くの抑制機能を有する部位である。すなわち前頭葉機能が低下し
ているということは自身の意図した行動の遂行が困難であり、抑制機能も損なわれている
状態であると考えることができる。これが術後せん妄として臨床上、露わになると思われ
る。すなわち、私たちが臨床上で目にする術後せん妄にも遂行機能や抑制機能、注意機能
などが関わっている可能性もあると考えられる。

数ある高次脳機能評価のなかで FAB と IQ を用いた理由をもう少し述べても良いかもしれ
ない．発表を聞くと理解できるが，本研究の解釈に必要な評価のクリアデータや作業療法
について具体的に述べたほうが良い．FAB で有意差を認めたことは大変良いことであるの
でもっとプラスに捉えて良い． 
 
太田教授 
FAB の下位項目についてもう少し詳しい考察と，FAB と MMSE の相関についても述べた
ほうが親切だと思う．IQ については今後余裕があればレーブン色彩マトリックス検査を用
いて調べてみても面白いかもしれない． 
 
川原田教授 
せん妄群の方が入院期間が長く予後が悪いことが改めてわかったので，術前に予測するこ
とができるようになると良い．今回は幅広い術式を取り込んでいるが手術侵襲に差がある
ため今後はよりデータを統一していくとまた違った結果が見れるかもしれない．

【緒言】 
近年、わが国では精神疾患患者の増加に伴い、メンタルヘルス不調の母親が増加して
いる。メンタルヘルス不調の母親に対する母乳育児支援には、多様な視点と高度なアセ
スメントが必要とされ、助産師は試行錯誤しながら支援を行っている。熟練助産師が行
う母乳育児支援は、母乳育児支援に困難を抱える助産師の示唆となり、支援の質の向上
に不可欠であると考える。 
【研究目的】 
 母乳育児を希望するメンタルヘルス不調の母親に対して熟練助産師が行う母乳育児支
援を明らかにする。 
【研究方法】 
 研究デザインは質的記述的研究である。助産師経験 10年以上の助産師を対象に、非構
造化面接を実施した。面接では、母乳育児を希望するメンタルヘルス不調の母親に対し
て、母親の希望を叶えるために行った母乳育児支援の経験を具体的に語ってもらった。
得られたデータから母乳育児支援に関する箇所を抽出し、意味が類似・共通するものを
まとめ、カテゴリーを生成した。本研究は札幌医科大学倫理委員会の承認を得て実施し
た。 
【結果】 
研究協力者は、承諾が得られた 3施設に勤務する 8名の助産師であり、助産師の年代は
40代が 6名、50代が 2名、助産師経験年数は平均 20.3年であった。分析の結果、4つの
カテゴリーと 13のサブカテゴリーが生成された。熟練助産師が行う母乳育児支援として、
【母親が無理せず続けることができる母乳育児の方法を妊娠中から見極め提案する】は、
自ら助けを求めづらい〔母親が困ったときにいつでも頼ることができるように先回りす
る〕、注意深く母親の言動を観察しながら〔母親にとっての必要な休息と育児のバランス
 
 
を取ることができているか判断する〕といった 5 つのサブカテゴリーから生成された。
【向精神薬を内服しながらできる母乳育児の方法を模索する】は、内服治療と母乳育児を
両立できるよう〔薬の安全性と児への影響を確認して伝える〕といった 3つのサブカテゴ
リーから生成された。また、母親の母乳育児の希望を叶えることは、母親の自信やメンタ
ルヘルスの安定に繋がると考え、メンタルヘルスの状況をアセスメントしながら【母親の
児に対する愛着を観察し、母乳育児の希望を叶える姿勢で関わる】ことが語られ、2つの
サブカテゴリーから生成された。更に、母親が母乳育児を含めた育児全般をメンタルヘル
スの悪化なくできるよう【母親が退院後に安心して育児ができる環境を見据え、妊娠中か
ら調整する】ことが語られ、〔妊娠中から母親のメンタルヘルスの状況を把握し、周囲の
サポートを調整する〕、〔退院後の母児と家族を地域で継続的に支援できるように、多職種
と連携する〕といった 3つのサブカテゴリーから生成された。 
【考察】 
 助産師は、助けを求めづらい母親の言動を注意深く観察し、休息とのバランスを取りな
がら育児ができているか見極めていた。メンタルヘルス不調の母親に対して医療者は、
「授乳を頑張らせないように」という認識を持ちやすい。しかし本研究で助産師は、母親
が無理せず母乳育児を続けることができる母乳育児の方法を検討していた。向精神薬を
内服している母親については、母乳育児が断念される事例が報告されているが、本研究に
おいて助産師は、内服をしながらできる母乳育児の方法を模索していた。また助産師は、
自身のケア経験から、母親が希望する母乳育児の実現は母親としての自信やメンタルヘ
ルスの安定につながると考え、母親のメンタルヘルスを考慮しながら母乳育児の希望を
叶える姿勢で関わっていた。更に助産師は、母親のメンタルヘルスの状況やサポート状況
を妊娠中から把握し、多職種と連携することで、母親が退院後も安心して育児ができる環
境を調整していた。このような支援には、熟練助産師の観察力や判断力、先々を予測する
力が反映されていると考える。 
 
【Introduction】 
In recent years, along with an increase in the number of patients with mental illnesses, the number 
of mothers with mental health problems has also been increasing in Japan. Breastfeeding support 
for mothers with mental health problems requires diverse perspectives and advanced assessment, 
and midwives provide support through trial and error. We believe that midwives skilled in 
breastfeeding support can provide suggestions to midwives who have difficulty in this, thus 
improving the quality of support provided. 
【Research Objective】 
 To identify the breastfeeding support strategies used by skilled midwives to assist mothers with 
mental health problems who wish to breastfeed. 
 
 
 
【Research Methods】 
This qualitative descriptive study was conducted with the approval of the Sapporo Medical 
University Ethics Committee. Informed consent was obtained from the participants. Unstructured 
interviews were conducted with midwives with ≥10 years of midwifery experience. In the 
interviews, the midwives described their specific experiences with the breastfeeding support they 
provided to fulfill the wishes of mothers with mental health problems who wished to breastfeed. 
From the data obtained, passages related to breastfeeding support were extracted, and those with 
similar or common meanings were grouped together to generate categories . 
【Results】 
The study participants included eight midwives working at three facilities. Six midwives were in 
their 40s, and two were in their 50s; the average midwifery experience was 20.3 years. Four 
categories and 13 subcategories were generated on analysis. Breastfeeding support provided by 
skilled midwives included 【identifying and suggesting breastfeeding methods that the mother can 
continue without strain from the time of pregnancy】. This category was generated from five 
subcategories, including understanding the mother’s reluctance to ask for help on her own and 
〔anticipating the mother’s need for help whenever she needs it〕, carefully observing the mother’
s words and actions to 〔determine whether she is adequately balancing rest and childcare〕. The 
second category, namely 【exploring breastfeeding methods while taking psychotropic drugs】, 
was generated from three subcategories, including 〔confirming and communicating the safety of 
drugs and their effects on the child〕 so that breastfeeding can be compatible with oral psychotropic 
treatment. The midwives also believed that fulfilling the mothers’ breastfeeding wishes would 
lead to an increase in their confidence and mental health stability. They then assessed mental health 
and said that they would 【observe the mother’s attachment to her child and engage her with an 
attitude of fulfilling her breastfeeding wishes 】 . This category was generated from two 
subcategories. Furthermore, midwives believed that it was necessary to 【observe the mother’s 
mental health status during pregnancy and coordinate support for those around her】so that the 
mother could raise her child, including breastfeeding, without worsening her mental health. This 
category was generated from three subcategories, including 〔monitoring the mother’s mental 
health status during pregnancy and coordinating support for those around her〕 and 〔providing 
continuous support for the mother, child, and family in the community after discharge from the 
hospital by collaboration with other professionals〕. 
【Discussion】 
Midwives carefully observed the words and actions of mothers who had difficulty asking for help 
and determined whether they were able to balance childcare with rest. Medical providers perceive 
that mothers with mental health problems should not be made to work harder at breastfeeding. 
However, the midwives in this study explored breastfeeding methods that would allow mothers to 
 
 
continue breastfeeding without strain. There have been reports of mothers giving up breastfeeding 
while taking psychotropic drugs; however, the midwives in this study were looking for 
breastfeeding methods that would allow mothers to continue breastfeeding while taking 
psychotropic drugs. Based on their own experience of caring for mothers, the midwives believed 
that the realization of the mothers’ desire for breastfeeding would lead to an increase in their 
confidence as mothers and stabilize their mental health; thus, they were involved in fulfilling the 
mothers’ wishes for breastfeeding while taking their mental health into consideration. Furthermore, 
the midwives were aware of the mothers’ mental health and support status throughout pregnancy, 
and by multidisciplinary collaboration, they adjusted the environment so that the mothers could 
continue to care for their children with peace of mind after discharge from the hospital. We believe 
that such support reflects the skilled midwife’s ability to observe, judge, and anticipate future 
outcomes.

ADL at discharge: Analysis of electronic medical information 
目的：入退院時の日常生活動作（Activities of daily living: ADL）やリハビリテーションの
実施状況を把握する方法のひとつとして，診断群分類（Diagnosis Procedure Combination）
システムを使用する病院から収集された DPC データが用いられている．DPC データを使
用した研究において，様々な疾患について転帰先や治療効果の検討がなされているが，収
集されたすべての疾患を対象とした，詳細な ADL 項目と転帰先との関連は十分に明らか
にされておらず，入院した時点の ADL が転帰先の予測に役立つか等の検討はなされてい
ない．また，入院後早期のリハビリテーション介入が，退院時のどのような ADL 項目に
影響を与えるのかを検討した報告はない．本研究では，DPC データを用いて，リハビリ
テーションを施行した患者における入院時 ADL と転帰先との関連性と，入院後の早期リ
ハビリテーションの実施が退院時 ADL に与える影響を明らかにすることを目的とした． 
方法：解析データは，匿名加工情報として集められた DPC データを用いた．対象期間は
2019 年 12 月-2020 年 8 月とし，対象期間中に一回以上の入院，かつ，一度でもリハビリ
テーションを実施した 65 歳以上の患者 13,495 名を対象とした．ロジスティック回帰分析
によって転帰先と入院時 ADL の自立によるオッズ比（OR）と 95%信頼区間（95%CI）を
算出した．加えて，リハビリテーションを開始するまでの期間と退院時 ADL の OR を算
出した．調整変数は年齢，性別，BMI，入院時 ADL の合計点，入院期間とした．サブグ
ループ解析として年齢や性別，BMI 及び国際疾病分類，入院時 ADL の合計点ごとに解析
を行った．また，各群に対して傾向性の検定を行った．有意水準は 0.05 とした． 
結果：リハビリテーションを実施した 65 歳以上の患者の入院時 ADL と転帰先との OR は
すべての項目で有意な関連性を示した．また，リハビリテーション開始までの期間と退院
時 ADL との関連は，すべての ADL 項目で，リハビリテーションの開始が遅くなるほど，
退院時 ADL が自立していない傾向が強くなった（p<0.05）．しかしながら，年齢層の違い
や疾患の違いによって，関連性がみられなくなった ADL 項目もあった． 
 
 
考察：本研究では，リハビリテーションを施行した 65 歳以上の患者において，入院時の
ADL が自立していないことは，家庭へ退院する転帰と比較して，他病院・診療所への転
院や施設への入所といった転帰となるリスクが高いことが明らかとなった．リハビリテ
ーションを施行する患者において，入院時の ADL が自立しているかどうかが，将来的な
転帰を予測する可能性を示唆していると考えられた．また，リハビリテーションの開始ま
での期間と退院時 ADL との関連性について，入院からリハビリテーションの開始までの
期間が長くなるほど，退院時の ADL が自立していない傾向が強くなることが示されたが，
層別解析を行うと多くの項目で傾向性がみられなくなった．高齢であることや，疾患の違
いによっては，早期リハビリテーションは退院時 ADL と関連性が低い可能性が示唆され
た．DPC データを用いた本研究においても，臨床データを用いた先行研究と同様の結果
となり，大規模データを用いた転帰先の予測やリハビリテーションに関する効果判定の
予測が可能となることを示唆していると考えられた． 
結論：本研究の知見は DPC データを用いて入退院時の ADL に関連する要因を検討した
研究の一つであり，転帰先予測や治療開始に関して，エビデンスの蓄積に寄与する可能性
がある．

Introduction: One method to determine the status of activities of daily living (ADL) and 
rehabilitation at the time of hospital admission and discharge is to use data collected from hospitals 
using the Diagnosis Procedure Combination (DPC) system. Although studies using data from the 
DPC system have reported outcome destinations and treatment effects for various diseases, the 
relationship between detailed ADL items and outcome destination has not been fully clarified for 
all the collected diseases, and ADL status at admission has not been analyzed for its usefulness in 
predicting outcome destination. In addition, no studies have determined what ADL items at 
discharge are affected by early rehabilitation intervention after hospitalization. This study aimed to 
clarify the relationship between ADL status at admission and outcome destination in patients who 
underwent rehabilitation and determine the impact of early rehabilitation after admission on ADL 
status at discharge using data from the DPC system. 
Methods: Data from the DPC system, collected as anonymized processed information, were 
analyzed. The study period was between December 2019 and August to 2020. Data from 13,495 
patients, 65 years or older who were hospitalized at least once and who underwent rehabilitation at 
least once during the study period were included. Logistic regression analysis was used to calculate 
the odds ratios (ORs) and 95% confidence intervals for outcome destination and independence in 
ADL at admission. In addition, the ORs for the time to rehabilitation and ADL status at discharge 
 
 
were calculated. Adjustment variables were age, sex, body mass index (BMI), total ADL status at 
admission, and duration of hospitalization. Subgroup analyses were performed by age, sex, BMI, 
International Classification of Diseases code and ADL status at admission. A propensity test was 
performed for each group. The significance level was set at p<0.05. 
Results: All ORs between ADL status at admission and outcome destination were significantly 
associated in patients 65 years and older who received rehabilitation. The relationship between time 
to rehabilitation and ADL status at discharge was stronger for all ADL; the later the start of 
rehabilitation, the less independent the ADL at discharge (p<0.05). However, some ADL items were 
no longer related to the start of rehabilitation because of differences in age groups or diseases. 
Discussion: The study found that patients 65 years and older who receive rehabilitation are at a 
higher risk of being transferred to another hospital/clinic or admitted to a facility if their ADL are 
not independent at the time of admission, compared to patients discharged to home. For patients 
undergoing rehabilitation, independence in ADL at the time of admission may predict future 
outcomes. Regarding the relationship between the time from admission to the start of rehabilitation 
and ADL status at discharge, the longer the time from admission to the start of rehabilitation, the 
more likely the patient was not independent in ADL at discharge. However, for many items in the 
stratified analysis this relationship was not evident. Depending on the advanced age and stage of 
disease of patients, early rehabilitation may be less-well associated with ADL status at discharge. In 
this study, using data from the DPC system, it was found that large datasets can be used to predict 
patient outcomes and determine the effectiveness of rehabilitation. 
Conclusion: Our findings have identified factors related to ADL status at admission and discharge 
using data from the DPC system, which contributes to the body of evidence regarding prediction of 
outcomes and initiation of treatment.

を実施した 65歳以上の患者 13,495名を対象とした．ロジスティック回帰分
析によって転帰先と入院時 ADLの自立によるオッズ比（OR）と 95%信頼区間
（95%CI）を算出した．加えて，リハビリテーションを開始するまでの期間と
退院時 ADLの ORを算出した． 
結果として，リハビリテーションを実施した患者の入院時 ADLと転帰先と
の ORはすべての項目で有意な関連性を示した．また，リハビリテーション開
始までの期間と退院時 ADLとの関連は，すべての ADL項目で，リハビリテー
ションの開始が遅くなるほど，退院時 ADLが自立していない傾向が強くなっ
た． 
本研究の知見は DPCデータを用いて入退院時の ADLに関連する要因を検討
した研究の一つであり，転帰先予測や治療開始に関して，エビデンスの蓄積
に寄与する可能性がある．

要旨 
背景：重症心身障害者は重度の知的障害と運動障害を併せ持ち，筋緊張異常や脊柱・四
肢の変形を有することで，安定した姿勢保持が困難であるものが多い．そのため 24時間
体制の姿勢管理が必要とされる．その中でも HOB（Head of Bed）挙上姿勢は，消化促進
や誤嚥予防などの効果が期待される一方，仙骨部の褥瘡リスク増大や姿勢の崩れによる影
響も指摘されている．しかし，重症心身障害者における HOB挙上姿勢の効果やリスクに関
する詳細な検討は不足している．  
目的：本研究は，背臥位と HOB挙上姿勢が健常成人および重症心身障害者に与える影響
を体圧分布および心拍変動解析（HRV）を用いて解析し，両群内，および両群間の違いや
特徴を明らかにすることを目的とした．  
方法：健常成人 23名と重症心身障害者 23名を対象に，背臥位と 45°HOB挙上位の 2
つの姿勢条件を検証した．実験は各姿勢保持を 10分測定し，その内後半の 5分間を解析
対象とした．体圧分布は圧力分布測定装置 FSAを用い，全身の平均圧と高圧領域（30mmHg
以上）の割合，体部位別の高圧領域のエリア比率を解析した．HRV解析はウェアラブル心
拍センサ myBeat WHS-1を胸部に貼付し，心拍変動解析ソフト RRI Analyzer 2を用いて正
規化 LF（交感神経指標）および正規化 HF（副交感神経指標）の解析範囲における変化量
を算出した．統計解析には Rの ARToolパッケージを用い，整列ランク変換後に混合計画
における二元配置分散分析を実施した． 
結果：健常成人では，体部位別のエリア比率から，HOB挙上時に頭部・背部のエリア比
率は有意に狭くなり，殿部でエリアが有意に広くなることが示された．重症心身障害者で
は健常成人と比較して両姿勢条件における頭部と背部のエリア比率が有意に広く，脚部の
エリア比率が有意に狭いことが示された．HOB挙上時のエリア比率の移行は健常成人と同
 
 
様に，頭部・背部のエリア比率は有意に狭くなり，殿部でエリアが有意に広くなることが
示された．HRV解析データからは，両被験者条件ともに HOB挙上で瞬時心拍が有意に上昇
すること，重症心身障害者群では健常成人よりも瞬時心拍が有意に高いことが示された．
自律神経系指標の変化量には差を示さなかった． 
考察：重症心身障害者において体圧分布は姿勢の非対称性や筋緊張の影響が関連してお
り，それを反映した特徴を示すことができたと考える．HRV解析による自律神経系指標の
傾向に差がなかったことは，瞬時心拍は有意に高かったことを踏まえると一概に健常成人
と同様であるといえない可能性があり，先行研究の知見も踏まえると重症心身障害者の自
律神経系の反応特性が関連していると推察された．これらの結果から，重症心身障害者に
HOB挙上姿勢を適用する際は，適切なポジショニングと慎重なバイタルチェックが必要で
あると考える． 
結論：本実験のように全身の体部位別に高圧領域のエリア比率を測定し，その姿勢によ
る変化を捉えることで，被験者条件の特徴と HOB挙上の特徴を示すことができ，これは姿
勢のサポート手法に関連する知見となると考える．HRV解析による自律神経系指標からは
重症心身障害者の自律神経系の反応が通常とは異なる可能性が示され，HOB挙上姿勢の導
入にはバイタルチェックを欠かさないことは重要であると考える． 
 
Background: Individuals with severe motor and intellectual disabilities (SMID) often struggle to 
maintain stable posture because of abnormalities in muscle tone and deformities in the spine and 
limbs. Consequently, 24-h postural management is essential for this population. Among the 
various postural interventions, the head-of-bed (HOB) elevation position is expected to promote 
digestion and prevent aspiration. However, it is also associated with potential risks, such as an 
increased risk of sacral pressure ulcers and postural instability. Despite these concerns, detailed 
investigations into the effects and risks of HOB elevation for individuals with SMID remain 
insufficient. 
Objective: This study aimed to analyze the effects of the supine and HOB elevation positions in 
healthy adults and individuals with SMID using body pressure distribution and heart rate 
variability (HRV) analysis, clarifying differences and characteristics both within and between 
these groups. 
Methods: This study included 23 healthy adults and 23 individuals with SMID. Two postural 
conditions were evaluated: supine and 45° HOB elevation. Measurements were conducted for 10 
min in each posture, with the last 5 min used for analysis. Body pressure distribution data were 
collected using the body pressure mapping system, analyzing mean pressure across the body, the 
percentage of high-pressure areas (≥30 mmHg), and the area ratio of high-pressure regions by 
body part. HRV analysis was conducted using the wearable heart rate sensor myBeat WHS-1 
attached to the chest, and changes in normalized LF (sympathetic nervous system index) and 
 
 
normalized HF (parasympathetic nervous system index) were calculated using the heart rate 
variability analysis software RRI Analyzer 2. Statistical analyses were performed using the 
ARTool package in R, implementing a mixed-design two-way analysis of variance following an 
aligned rank transformation. 
Results: In the healthy adult group, the area ratio of high-pressure regions by body part showed 
significant reductions in the head and back and a significant increase in the hip area during HOB 
elevation. In the SMID group, both postures showed significantly larger area ratios in the head and 
back and significantly smaller ratios in the legs compared with those in healthy adults. Changes in 
the area ratio during HOB elevation in the SMID group mirrored those in the healthy adults group, 
with significant reductions in the head and back and a significant increase in the buttocks. HRV 
analysis demonstrated a significant increase in instant heart rate during HOB elevation for both 
groups, with the SMID group showing significantly higher heart rates than those in the healthy 
adults group. However, no differences were observed in changes in autonomic nervous system 
indices. 
Discussion: In the SMID group, body pressure distribution reflected postural asymmetry and 
muscle tone. The lack of differences in autonomic nervous system indices, despite higher heart 
rates in the SMID group, suggests that their autonomic responses may differ from those of healthy 
adults. This could be attributed to specific autonomic response characteristics in individuals with 
SMID. These findings emphasize the need for appropriate positioning and careful monitoring of 
vital signs during HOB elevation for individuals with SMID. 
Conclusion: By assessing the area ratios of high-pressure regions by body part and capturing 
postural changes, this study highlighted the distinctive features of both participant groups and 
characteristics of HOB elevation. These findings offer valuable insights into the development of 
posture support strategies. HRV analysis suggested that autonomic responses in individuals with 
SMID may differ from typical patterns, underscoring the importance of vigilant vital sign 
monitoring during HOB elevation.

SOX10はA375悪性黒色腫細胞株において免疫チェックポイント分子PD-L1の発現を誘導する
ことによりT細胞認識を阻害する． 
 
研究目的 
悪性黒色腫は致死的な皮膚がんであり，悪性黒色腫分化抗原やネオアンチゲンを発現
する免疫原性の高い腫瘍の一つである．SOX10は転写因子であり，神経堤分化マーカーと
して知られる転写因子の一つであり，悪性黒色腫の病理学的診断マーカーとして使用さ
れている．また，SOX10-MITF axisの活性化を通じて，悪性黒色腫の発生に関与している
とされる．最近の研究ではSOX10は免疫チェックポイント分子である，HVEMとCEACAM1の
発現を誘導することにより，悪性黒色腫の発生に関与することが示唆されている．本研
究では，免疫チェックポイント分子であるPD-L1の発現とSOX10の関与を明らかにするこ
とを目的とした． 
 
研究方法 
A375悪性黒色腫細胞株にSOX10遺伝子を過剰発現，ないしノックダウンし，フローサイ
トメトリーおよびウェスタンブロッティングによりPD-L1の発現を評価した．T細胞応答
は，NY-ESO-1特異的TCR-T細胞を用いて，IFNγ ELISPOTアッセイにより評価した． 
 
研究成績及び考察 
SOX10の過剰発現はPD-L1の発現を増加させ，一方で，siRNAを用いたSOX10ノックアウト
により，PD-L1の発現は減少した．IFNγ ELISPOTアッセイにより，SOX10の過剰発現は
A375細胞株のNY-ESO-1特異的TCR-T細胞に対する感受性を低下させることが明らかとなっ
た． 
 
結論 
SOX10はA375細胞株におけるPD-L1の発現に関与しており，PD-L1の発現を通じて，内在
性の免疫制御機構に関与する可能性が示唆された．

結果の要旨 
 
 SOX10の過剰発現，ノックダウンにより PD-L1はmRNAレベル, 蛋白レベルで正の
相関を示し，IFN-γ-JAK-STAT-IRF1 を介した PD-L1 上昇を増幅している可能性が考
えられた．SOX10 過剰発現 A375 細胞では PD-L1 の発現が上昇し，細胞障害性が低下
した．悪性黒色腫における SOX10の発現は PD-L1 を制御し免疫逃避をもたらすと考え
られる．SOX10は悪性黒色腫における PD-L1の発現に関与しており，PD-L1の発現を
通じて，内在性の免疫制御機構に関与する可能性が示唆された． 
論文審査後に腫瘍免疫学に関する質疑応答を行い、専攻学術に関して学識を有するこ
とを確認した。学位認定試験後、博士の学位授与に値すると審査委員全員に認められた。
