NICU入院児の母親が母乳育児を行う体験と思いに関する文献レビュー
Get PDF〈目的〉NICU入院児の母親が母乳育児を行う体験と思いに関する国内の研究動向について文献レビューし，今後の課題を明らかにする．
〈方法〉医中誌webを用い「NICU」「低出生体重児」「早産児」「母親」「母乳育児」「体験」「思い」をキーワードとして検索された45件から，分析対象論文14件を抽出した．研究目的，対象をマトリックス方式でまとめ，母親の体験と思いに関する結果をカテゴリー分類した．
〈結果〉対象論文は全て質的研究で，目的は母乳育児あるいは搾乳の体験や思いであった．対象はほとんどが疾患合併のない早産児の母親で，子どものNICU退院後についての調査は少なかった．体験と思いの結果から生成された9カテゴリーは，搾乳に関するもの4つ，直接授乳に関するもの2つ，母乳育児全体に関するもの3つに分類された．
〈考察〉NICU入院児の母親が行う母乳育児の継続を支えるためには，対象の選定条件を広げ，直接授乳の思いや体験を探求すると共に，子どものNICU退院後における多様な母乳育児の実態を明らかにしていく必要がある．departmental bulletin pape
緊急手術を必要とする大動脈疾患
Get PDF1999年に大学の医局に戻り、2000年から助手となって2015年に教授に就任し、本年無事退職を迎えました。2013年から2年間を札幌中央病院で勤務しましたが、皆様のおかげをもちまして無事教授の任期を全うさせていただきました。
わたしの専門は大動脈疾患になります。よく胸部大動脈瘤とか解離性大動脈瘤とか病名を耳にするかと思いますが、この大動脈疾患の治療は通常の心臓手術と違い、機能を改善する手術ではなく、破裂を予防する手術になります。実は最終講義を「胸腹部大動脈瘤の外科治療―術後の脊髄障害」について話そうと思っていたのですが、とても30分では講義しきれず中途半端になりそうでしたので、比較的理解しやすい破裂性腹部大動脈瘤と急性A型大動脈解離について話すこととしました。したがって本日講義する内容は、専門外でも理解できる内容にしており、医学部に入学した1年生に「医学入門セミナー」で講義しておりますし、研修医にも話しております。救命のために早期診断と治療が必要であり理解しておかなければならない基本的なことをお話しします。
（a） 破裂性腹部大動脈瘤
腹部大動脈瘤とは、腹部大動脈が部分的に大きくなる病気で、通常は20mm程度の大動脈が30mm以上に膨らんだ状態です。その原因の90％以上は動脈硬化です。
50～70歳が発生のピークで、平均年齢は65歳前後であり、男女比は6～8：1程度と圧倒的に男性に多いとされています。腹部大動脈瘤は胸部大動脈瘤と同じく自覚症状はほとんどありません。痩せている方が腹部の拍動性（“ドクドクする”）の腫瘤として気づくこともありますが、ほとんどの方は、健康診断や他の病気の検査や治療中に、偶然、腹部大動脈瘤が見つかっています。したがって、この病気の1番の問題点は無症状であることです。破裂するまで病気に気が付かない患者さんがほとんどです。腹部大動脈瘤を治すための薬はありません。治療は外科手術のみです。
（b） 急性大動脈解離（StanfordＡ型）
大動脈は内膜、中膜、外膜の3層に分かれています。中膜がなんらかの原因で裂けて、もともとは大動脈の壁であった部分に血液が流れ込むことで大動脈内に二つの通り道ができる状態が大動脈解離です。特に上行大動脈に解離が及ぶA型では、1時間に1%ずつ死亡率が上昇すると言われています。つまり、48時間以内におよそ半分の患者さんが亡くなることになります。したがって上行大動脈に解離があれば緊急手術を行うことがほとんどです。急性大動脈解離（StanfordＡ型）の術後在院死亡率などの成績は、じつは日本が世界一であり、もっとも死亡率が低い国が日本になります。

Mechanical interactions between the biceps femoris long and short heads: Implications for T-junction hamstring injuries
No full textAlthough force transfer during elongation occurs longitudinally and transversely, the influence of transverse force transfer between the biceps femoris long head and short head remains unclear. This study aimed to investigate whether separating the intermuscular connections between the biceps femoris long head and short head alters tension in the biceps femoris long head. Eight human cadaver legs were used, and ultrasonic shear wave elastography measurements were performed under four conditions: (1) intact, (2) removal of all tissues from the skin to the deep fascia, (3) intermuscular dissection, and (4) biceps femoris short head detachment. Measurements were taken in four limb positions, defined by hip and knee joint angles, under each tissue condition. The shear modulus of the biceps femoris long head significantly increased by 62.2% after intermuscular dissection compared to fascia removal, and further increased by 174.7% after biceps femoris short head detachment. In contrast, the shear modulus of the biceps femoris short head significantly decreased by 36.0% following intermuscular dissection and by 75.1% after detachment. In conclusion, reducing biceps femoris short head tension while increasing biceps femoris long head tension may influence muscle stress distribution, particularly during movement.journal articl
Review of studies on host and environmental factors associated with successful aging and aging in place
Get PDFHost and environmental factors for successful aging and aging in place have been investigated. At this moment, those factors would be categorized into the following groups: economic status, households, lifestyle habits, social capital, cognitive and physical function, disease status, and use of medical and nursing care. It must be mentioned that these factors are overlapped and are mutually related each other. We found that almost all of these categories have been previously reported to be significantly associated with successful aging and aging in place.
