Изменение объективных параметров аутофлуоресцентной картины кожи под влиянием инсоляции и возраста
There are several key factors, such as age and sun exposure, the effects of which leads to a change in the properties of the skin, detected by the method of autofluorescent dermoscopy. In this paper we used the principle of multispectral imaging of the skin in the light of autofluorescence at excitation wavelength of about 400 nm. It was found that, the background skin glow, the coefficient of heterogeneity and the integral index of autofluorescence significantly increases in a low insolated areas. The most pronounced increase of these parameters observed in the group of women older than 45 years. We can assume that this is due to changes in the properties of collagen and accumulation of advanced glycation end products, and the uneven distribution of absorbent skin chromophores. Insolation accelerates the development of age-related changes of skin AF. According to the literature, in people aged 50–70 years increased NADH (external NADH oxidase) and AGEs compared with age 20–40 years. The differences in the autofluorescence of the skin between high and low insolated sites saved in all age groups. Autofluorescence age-related features described above should be considered for diagnostic and therapeutic measures.
Флюоресцентная диагностика заболеваний шейки матки
Results of local fluorescence spectroscopy in 185 women with underlying and pre-invasive disease of cervix and high-risk HPV infection are represented. Fluorescence study was performed 2h after intravenous injection of fotoditazin in a dose of 1 mg/kg (wavelength 636.5 nm). Accumulation of the photosensitizer was estimated by diagnostic parameter (DP) value, calculated as mean value of fluorescence scaled to each type of tissue. For normal tissues DP accounted for 0.6±0.4, showing accumulation of the photosensitizer. According to the study the medication did not also accumulate in retention cysts (DP 0.3±0.1), explaining low efficiency of photodynamic therapy for this pathology. The accumulation of fotoditazin depends significantly on type of pathologic tissue. In patients with inflammation, leukoplakia and CIN I accumulation of the photosensitizer in pathologic foci was negligible: DP accounted for 1.7±0.2, 1.8±0.2 and 2.1±0.3, respectively. In sites of endometriosis and CIN II DP was significantly higher and accounted for 8.3±2.1 and 14.1±4.1, respectively. The greatest accumulation of the photosensitizer was registered in sites of CIN III, squamous cell carcinoma and adenocarcinoma. Though DP value for these pathologies had almost no difference and accounted for 23.1±4.7, 22.7±1.8 and 23.3±1.4, respectively. For fluorescence diagnosis of severe dysplasia in 48% of patients borders of fluorescence regions were beyond lesions detected for extended colposcopy with additional areas of fluorescence. Targeted biopsy of these regions proved pathology in all patients: CIN II, CIN III, mild dysplasia or CIS. Thus, local spectroscopy allows to diagnosis multifocal lesions on cervix, to define correctly borders of lesion and consider excisional biopsy in-time.
Новые технологии в диагностике преинвазивных заболеваний шейки матки
Results of efficiency assessment of different diagnosis for preinvasive cervical lesions are represented in the article. During investigation the retrospective analysis of 353 patient histories of women with cervical intraepithelial neoplasia (primary and recurrent), who have been observed in Ivanovskiy regional oncological dispensary from 2002 to 2011, was performed. The accuracy rates of diagnosis of cervical intraepithelial neoplasia for one or several methods including cytological study, colposcopy and fluorescence cystoscopy were compared. Fluorescence diagnosis was performed with drug fotoditazin (LLC «VETA-GRAND», marketing authorisation №LS 001246) at dose of 1,0 mg/kg body weight. The final diagnosis was determined by results of histological study. The accuracy of cytological study accounted for 67,5–80,7%. Using colposcopy the exact diagnosis was determined in 67,9–74,4% of cases. Combination of cytological study with colposcopy improved the accuracy of diagnosis up to 82.1–88.9%. The superior results were for concurrent use of cytological study with colposcopy and fluorescence diagnosis – in such case the diagnosis was correct in 92,0–96,7% of patients. The analysis of results showed that using all methods of diagnosis the accuracy of diagnosis for recurrent cervical intraepithelial neoplasia was lower than for primary lesions. The decrease of diagnosis accuracy is probably related to previous diagnosis and therapeutic manipulations on cervix. Thus, multimodal diagnosis investigation with methods of fluorescence spectroscopy allows to improve accuracy of diagnosis for preinvasive cervical lesions.
Клиническое наблюдение сочетанной фотодинамической терапии и дистанционной гамматерапии метастазов меланомы кожи
Results of follow-up of patient with melanoma metastases to inguinal lymph nodes (two tumor foci) are represented. The patient underwent a course of combined photodynamic therapy and distant gamma-ray therapy. Single intravenous injection of fotosens as photosensitizer at dose of 0.3 mg/kg was administered to the patient. Distant gamma-ray therapy was performed daily during 12 days (single dose of 3 Gy, total dose of 36 Gy), beginning in a day after injection of photosensitizer. Two hours after gamma-ray therapy session contact photodynamic therapy on both metastatic tumors was performed. There were 7 sessions of photodynamic therapy in all, total light dose on each tumor accounted for 1800 J. Three weeks after combined modality treatment the second step of radiotherapy on inguinal region including 12 daily irradiation sessions (single dose of 3 Gy, total dose of 36 Gy) was performed. One year after treatment the patient had complete tumor resorption proved in 6 months. There were no additional treatment during this period in this patient. The patient is still under follow-up.
Эффективность фотодинамической терапии базальноклеточного рака кожи с местным введением радахлорина
The results of evaluation of the efficiency of photodynamic therapy with photosensitizer radachlorine for basal cell carcinoma are represented. The study included patients with primary and recurrent cancer, solitary and multiple foci of different histological subtypes. All tumors corresponded stages T1-2N0M0. The radachlorine solution was injected into pathological focus at dose of 1.75-3.50 mg/ cm2 of tumor 15 min before the onset of irradiation (wavelength of 662 nm, light dose of 300 J/cm2). The evaluation of efficiency by means of short-term and long-term outcomes was performed on the basis of clinical and cytological data. According to shortterm outcomes evaluation, the total tumor regression was in 43 (95,5%) patients for 47 (95,9%) tumors. The partial regression was achieved in 2 (4,5%) patients, who subsequently had one repeated course of photodynamic therapy with short-term outcome as total tumor regression. All patients with multiple, superficial and nodal forms of basal cell carcinoma had total tumor regression in 100% of cases, with ulcerated form – in 94,4%, with morphea-like form – in 83,3%. During follow-up in subjects, 44 (97,7%) patients had 5-year recurrence-free period. The relapse of tumor was detected in 1 (2,3%) patient after PDT for primary cancer of nasal ala stage Т2N0M0 of solid and adenoid histological subtype. Thus, photodynamic therapy with intralesional injection of radachlorine showed high efficiency for treating all existent clinical forms and histological subtypes of basal cell carcinoma.
Флюоресцентная диагностика с аласенсом у больных раком кожи
Results of fluorescence diagnosis (FD) with alasens in 237 patients with skin cancer are represented. The average age of patients accounted for 63±11 y.o. The indications for FD were the borders adjustment for skin cancer of different localizations prior to special treatment and also the search for occult foci of malignant lesion. There were two groups of patients: patients with locally advanced head and neck carcinomas, who had FD prior to surgical treatment, and patients with cancer of different localization, who had FD prior to photodynamic therapy (PDT). FD permitted to adjust borders of detected tumor foci in 100% of patients that impacted on extent of resection during surgery in group 1 of patients and helped planning fields of laser irradiation for PDT session in group 2 of patients. Due to FD with alasens 506 foci of additional fluorescence were detected in 118 (53.1%) of 222 patients in group 2, skin cancer was diagnosed morphologically in 63 patients (28.4%) of them. Приведены результаты флюоресцентной диагностики (ФД) с препаратом аласенс у 237 больных раком кожи. Средний возраст пациентов составил 63±11 года. Показаниями к ФД служили уточнение границ рака кожи различной локализации перед проведением специализированного лечения, а также поиск скрытых очагов злокачественного поражения. Были выделены две группы пациентов: больные с местно-распространенными формами рака кожи головы и шеи, которым ФД проводили перед хирургическим лечением, и больные раком кожи различной локализации, которым ФД проводили перед фотодинамической терапией (ФДТ). ФД позволила уточнить границы ранее выявленных опухолевых очагов у 100% больных, что повлияло на объем резекции тканей во время оперативного вмешательства в 1-й группе пациентов и помогло спланировать поля лазерного облучения при проведении сеанса ФДТ во 2-й группе больных. В результате ФД с препаратом аласенс у 118 (53,1%) из 222 пациентов 2-й группы было выявлено 506 очагов дополнительной флюоресценции, из которых у 63 (28,4%) больных при морфологическом исследова- нии диагностирован рак кожи.
Фотодинамическая терапия экпериментальной саркомы М-1 крыс с фотосенсибилизатором борированным хлорином
On model of experimental sarcoma М-1 the efficiency of photodynamic therapy (PDT) has been studied with using as photosensitizer boronated chlorine. The doses of the last was 5 mg/kg of mass of an animal. A laser irradiation was spent at densities of energy 150 J/cm2 and power 0,25 V/cm2. Effect PDT was estimated by percent of animals with full regress of the tumor, to quantity of relapses, and in situation of the tumor growth by dynamic of coefficient of absolute it increase. Experiences have been lead on 6 pubertal rats. As the control not treated animals with tumor served. Researches have shown, that efficiency of PDT depends on a dose boronated chlorine and conditions of a laser irradiation. Представлены результаты изучения динамики накопления в опухоли и здоровых тканях и фотодинамической активности борированного хлорина у беспородных крыс с саркомой М-1. Показано, что индекс контрастности опухоль/здоровые ткани достигает своего максимального значения через 3,5 ч после внутрибрюшинного введения борированного хлорина в дозе 5 мг/кг. При про- ведении облучения (плотность энергии 150 Дж/см2, плотность мощности 0,25 Вт/см2) через 3,5 ч после введения фотосенсибилизатора в дозах 1,25; 2,5 и 5 мг/кг к 48-му дню наблюдения у всех животных зарегистрирована полная регрессия опухоли, что свидетельствует о высокой фотодинамической активности борированного хлорина.
Оптимизация параметров лечения при фотодинамической терапии предрака и рака шейки матки
Comprehensive study for optimization of parameters of photodynamic action with fotoditazin in patients with tumor and pre-tumor cervical diseases was conducted. The study included 52 female patients: pre-invasive cervical diseases were diagnosed in 34 (CIN I – in 9, CIN II – in 13, CIN III – in 12), cervical cancer – in 11 (8 had squamous cell cancer, 3 – adenocarcinoma of cervical canal), chronic cervicitis – in 7. The study agent in the form of 0,5% gel was applied on cervix in dose of 1 ml. To detect optimal interval between gel application and conduction of photodynamic therapy dynamics of accumulation and elimination of photosensitizer by means of its fl uorescence was studied. Fotoditazin was shown to have good accumulation in pathological tissues. The maximal agent accumulation was noticed in 30 min, continued about 15 min, and then gradually decreased. Maximal fl uorescence of photosensitizer was observed in foci of malignant tumor and severe intraepithelial neoplasia. To detect optimal light dose for irradiation cytological study of cell smear from specimen after light exposure with different light doses was performed. The minimal light dose necessary for activation of photochemical reaction pathway was 100 J/cm2, and optimal – 250 J/cm2. This dose allowed to destroy all atypical cells in the area of light exposure after application of gel fotoditazin. According to obtain data we suppose that the most effi cient regimen of photodynamic therapy with local application of fotoditazin-gel for treating dysplasia and pre-invasive cervical cancer was a dose of laser irradiation of 250 J/cm2 with duration of application of 30–45 min.
