Pancreatic Adenocarcinoma with Stomach Wall Invasion: Can it be Foreseen?
Introduction: The association of adenocarcinoma found in the pancreas and stomach wall is a relatively rare condition as is any metastasis from the pancreas to the stomach. Pancreatic carcinoma associated with additional gastric cancer is also relatively uncommon. The most frequent sites of metastasis reported for pancreatic adenocarcinoma are the lymph nodes, lung, liver, adrenal glands, kidney, and bone. We report a case of adenocarcinoma of the pancreas associated with a separate adenocarcinoma of the stomach wall. Not enough research has been placed on properly distinguishing these two forms of malignancies.Presentation of Case: In this case report we discuss the presentation of an 87 year old female who presented with abdominal pain and was found to have multiple sites of metastasis of Adenocarcinoma of pancreatic origin with direct spread to the stomach wall.Conclusion: The characteristics of this combination of gastric and pancreatic adenocarcinoma, whether there is a direct correlation or through local metastasis, needs to be further investigated to shed light on this condition
Mechanical and Water Absorption Properties of Hybrid Sisal/Glass Fibre Reinforced Epoxy Composite
Our environment is being pullulated due to the great use of synthetic fibre as reinforcement for polymer composites. Natural fibres may be better choice for replacement of synthetic fibre for polymer composite to reduced environment burden. Hybrid sisal/glass fibre reinforced epoxy composites are prepared by hand lay-up technique using different weight fractions (10, 20, 30 and 40 %) with 10 mm length of fibres. Mechanical and water absorption properties of prepared composites are investigated. This study shows that the addition of glass fibre into sisal fibre reinforced composite has increased its mechanical properties. Statistical analysis is also carried out using T-test and ANOVA and found significant variation among composites
Design and Study of Epoxy Composites based on Polycaprolactone and Nanodiamond Functionalized Carbon Fibers
In this study, the carbon fibers were coated with polycaprolactone (PCL) through in-situ polymerization. During this process, different content (1-10 wt. %) of functional nanodiamonds (NDs) were added to form PCL-CFs-NDs. The affect of NDs on the polymerization and molecular weight of PCL was deliberated through gel-permeation chromatography. The weight average molecular weight of PCL was decreased from 42×103 gmol-1 to 18×103 gmol-1 when the polymerization was carried out in the presence of 10 wt. % NDs. The fractographic study showed the formation of polycaprolactone and nanodiamonds coating over the surface of modified carbon nanofibers. The fiber pullout was found to decrease with the filler loading due to better interface bonding and interaction with epoxy. DSC results revealed an increase in the melting temperature with higher NDs content. Epoxy/PCL-CFs-NDs 10 with 10 wt. % filler acquired the tensile strength of 1226 MPa and tensile modulus of 891 MPa
Mechanical and Thermal Properties of Polyamide 1010 Composites Filled with Nanodiamond/Graphitized Carbon Black Nanoparticles
A series of polyamide 1010 (PA1010) composites were prepared by melt technique. A combination of two ball milled nanfillers were used to reinforce the composites i.e. nanodiamond (ND) and graphitized carbon black (GCB). The structure of nanocomposites was characterized by Fourier transform infrared spectroscopy (FTIR). The mechanical tests showed that the tensile strength and Young modulus increased as the ND/GCB content was increased. Compared with pure PA1010, the Young’s modulus and tensile strength of PA1010/ND/GCB 20 were significantly improved by 52 % and 22 %, respectively. On the other hand, the elongation at break of PA1010/ND/GCB composites was decreased with the increasing proportion of nanobifiller. For the composites containing 20 wt.% ND/GCB, the elongation at break was decreased to 2 % compared with 5 wt.% loaded PA1010 (25 %) prepared under the same experimental conditions. Scanning electron microscopic (SEM) images showed that the nanobifiller was uniformly dispersed on the fractured surfaces of the nanocomposite and exhibited strong interfacial adhesion with the polyamide matrix. Thermal stability of PA1010/ND/GCB 20 was improved significantly (Tmax = 530 ºC) relative to 5 wt. % PA1010 composite (Tmax = 489 ºC) and neat polyamide
Can docetaxel concomitant administration with bisphosphonates be considered as a risk factor for osteonecrosis of the jaw in metastatic breast cancer patients? A preliminary study.
Osteonecrosis of the jaws (ONJ) is rare complication of bisphosphonates use which was described for the first time by Marx in 2004. Bisphosphonates (BP) are stable derived of inorganic pyrophosphate which had an antiresorbant effect in the bone making them very useful in bone metastasis, especially with the third generation of bisphosphonates. Many risk and trigger factors have been described as predisposing to the development of ONJ. The aim of this work is to show if docetaxel administration can be considered as a predisposing factor to develop ONJ. We report a series of 11 patients treated during 5 years in the maxillofacial surgery department in HU Habib Bourguiba Sfax – Tunisia for ONJ due to BP use for breast cancer bone metastasis. Those patients were divised into two sub-groups: with and without concomitant docetaxel administration. All patients’ data (age and delay of appearance of ONJ, cumulative doses and trigger factors) were collected and analyzed. It was found that patients treated with docetaxel in association to bisphosphonates developed ONJ in earlier age, with a lower cumulative doses of zoledronic acid within a shorter delay period. ONJ occurred in these patients even in the absence of trigger factor and intrinsic risk factors
Apparent Efficacy of Low-Dose Constant-Infusion Crotalidae Antivenom for Control of Defibrinogenation Recurrence Syndrome Following Envenomation by the Eastern Diamondback Rattlesnake (Crotalus adamanteus)
Background: Eastern Diamondback rattlesnake bites produce coagulopathy in the bite victims. The syndrome is treated with antivenom called Crotalidae Polyvalent Immune Fab. However following resolution of coagulopathy with the antidote, and sometimes days later, fibrinogen drops to very low levels. Rare bleeding may occur due to this recurrent defibrinogenation syndrome. No guidelines exist at this time of how these patients should be treated. However, many centers repeat the initial treatment protocol using another 6-18 vials of antidote acquiring significant treatment costs. Methods: We analyzed all cases treated at the University of Florida between 2013 and 2014 for the rattlesnake bites. All cases were treated with Crotalidae Polyvalent Immune Fab as recommended per manufacturer recommendations upon initial presentation. However, upon the recurrence of defibrinogenation, we administered the antivenom 1 vial diluted in 250 ml of normal saline and administered by continuous IV infusion over 2 to 12 hours tapered over 2 to 3 days. The patients and their coagulation labs were monitored at least daily during the recurrence of the defibrinogenation. Results: We identified 5 cases treated for Eastern Diamondback snake bites. All cases had recurrent defibrinogenation syndrome and treated as described above. All patients had normalization in their fibrinogen levels with the reinstitution of the antidote. Additionally, none of them had any further bleeding. No chills, fevers or cloudiness of solution reported during the continuous antidote administration
Renal Cell Carcinoma with IVC Thrombus Extending up to Right Atrium and Triple Vessel Coronary Artery Disease - One-stage uro-cardiac Procedure: A case Report
Introduction: The aim of this report is to prove the possibility of simultaneous difficult cardiac and urologic operation. Important point to make in our report concerns the fact that the oncologic treatment was not delayed despite severe heart disease. There is also an advantage in avoiding second operation and hence anesthesiaCase Presentation: A 72 year old male presented to us with right renal mass lesion with tumour thrombus extending up to right atrium. He had undergone Percutaneous Transluminal Coronary Angioplasty 5 years ago and had two coronary stents in situ. Coronary angiography revealed triple vessel coronary re obstruction. After proper planning he underwent right radical nephrectomy with tumour thombectomy along with Coronary Artery Bypass Grafting in the same sitting.Conclusion: One-stage cardiac and uro-oncologic operation can be a safe and beneficial procedure, if performed in selected patients
Metastatic Clear Cell Hidradenocarcinoma: Response to Chemotherapy and Long Disease Control
Introduction: Hidradenocarcinoma is a rare aggressive tumor arising from sweat glands that may occur from preexisting hidradenoma or most frequently appears de novo. It is often located in the head and neck. It is also characterized by a high potential of local recurrence, metastasis and poor outcome. This tumor is often radiotherapy and chemotherapy resistant.Presentation of case: We report a case of a 77-year-old man with a metastatic hidradenocarcinoma to the bone and lymph nodes who was operated 7 years ago for an hidradenoma of the right foot. He was treated with capecitabine-based chemotherapy with good tolerance and obtention of partial response during 10 months. After that, his disease progressed and he died 1 year after diagnosis.Conclusion: Hidradenocarcinoma is a cancer with a poor prognosis . Because of its low prevalence and high rate of recurrence, its treatment is still controversial. Our good result demonstrates that adjunction of chemotherapy in metastatic forms is effective for a better control and long overall survival
Acute Pancreatitis and Diabetes Mellitus Caused by Dengue
In this article, an aramid of 4,4′-(1,3-phenylenedioxy)dianiline and terephthaloyl chloride was prepared. Afterwards, Nylon 6/6 and aramid were used to prepare sol-gel coated and non-coated blend fibers reinforced with nanodiamond (ND). In this way, four types of fibers were fabricated i.e. aramid/Nylon 6/6 fibers (Ar/N66) and aramid/Nylon 6/6/nanodiamond fibers (Ar/N66//ND) and sol-gel coated s-Ar/N66 and s-Ar/N66//ND fibers. The fibers were fabricated via Brabender single screw extruder at 200 °C. The Ar/N66 and Ar/N66//ND fibers were coated by means of simple dip-coating technique. Fourier transform infrared spectroscopy was used for structural characterization. Scanning electron microscopic images of non-coated and sol-gel coated fibers were scanned for morphological comparison. Glass transition temperature of the sol gel s-Ar/N66 fibers increased up to 232 °C, while that of s-Ar/N66//ND was enhanced to 240 °C relative to neat fibers. Moreover, the sol-gel coated s-Ar/N66//ND (8.08%) fibers had higher water absorbing tendency than non-coated Ar/N66//ND (7.66 %)
Study of the Altered Anisotropy of Erythrocyte Ghost Membrane upon Interaction with Phytoreduced Negatively Charged Gold Nanoparticle from Celosia cristata and Vigna radiata
Synthesis of negatively charged gold nanoparticles (GNP) was done from seed extract and seed electrolyte of two locally available plant sources Celosia cristata and Vigna radiata without using any toxic chemicals and it is a rapid reduction way. Standard techniques of spectroscopy and advanced microscopic study was done in characterizing the particles and simultaneously the phytochemicals present in the electrolyte and extract were also spotted using established process. The effects of nanoparticles on live red blood cells were analyzed by calculating hemolysis percentage. Evaluations of GNP activity when interacted with erythrocyte ghost membrane and their impact on membrane fluidity and anisotropy were also studied as negatively charged GNP are less delineated in previous history. The results showed that negatively charged gold nanoparticles were reduced by flavonoids and phenols present in seed extract and electrolyte of plants and they are hemocompatible and can help to fluidize the membrane of erythrocyte ghost cells.The whole experiment suggested a simple, ecofriendly approach of synthesizing negatively charged nanogold which are safe to live cells that can change the fluidity parameter of membrane constituent by decreasing the anisotropy which can further help in curing health problem related with cell membrane issue
