Imatinib Resistance and Relapse in CML Patients with Complex Chromosomal Variants
The BCR-ABL tyrosine kinase inhibitor Imatinib is highly effective for chronic myeloid leukemia (CML). However, some patients gradually develop resistance to Imatinib, resulting in therapeutic failure. In the present study, we analyzed 192 CML patients, from which CML relapse was observed in 17 individuals with involvement of other chromosomes in addition to Philadelphia translocation and who were on treatment of Imatinib (400mg per day since last 3-4 years). Interestingly, all 17 individuals had only BCR/ABL fusion at the time of diagnosis and attained complete Cytogenetic and hematological remission (CHR) within 18 weeks of the therapy. Three individuals among these 17 were not regular in the uptake of Imatinib after attaining CHR and CCyR and could be probable reason for relapse. In addition, we have also recorded primary resistance to Imatinib in 4 individuals who were diagnosed with some complex chromosomal variants. Therefore, either involvement of other genes along with BCR/ABL fusion, or additional chromosomes and point mutation in the fusion BCR/ABL gene itself could be a reason for primary resistance and relapse to Imatinib
Occult Follicular Thyroid Carcinoma presenting as Primary Breast Tumor with Sternal and Skull Metastasis
Introduction: Follicular thyroid carcinoma (FTC) that initially presented as breast tumor with no previous medical history of malignancy of thyroid gland is relatively rare and may cause diagnostic confusion.Presentation of case: We report a 59-year-old Mexican woman with no prior history of malignant thyroid neoplasm that presents with pain and swelling in the upper outer quadrant of the left breast with a year of evolution. Subsequently, subcutaneous tumor was identified in left parietal region. Clinically it was thought in primary breast tumor metastasis to skull. Furthermore, computerized tomography scan identified a tumor in the deep portion of the left breast, infiltrating the sternum that subsequently was confirmed a follicular carcinoma of the thyroid gland.Conclusion: Metastatic FTC may mimic a primary breast tumor. The combined use of clinical information, histopathology and immunohistochemistry were important to establishing a correct cancer diagnosis
Primary Adenocarcinoma in a Seminal Vesicular Cyst: A Case Report
Introduction: Primary adenocarcinoma of the seminal vesicle (SVC) is very rare. Presentation to the case: Herein, we reported a case of SVA of SV cyst arising in a 51-year-old deaf mute male patient. Laboratory parameters including serum prostate specific antigen (PSA) level were normal. Contrast-enhanced magnetic resonance imaging demonstrated large reterovesical cystic lesion with mural nodules. The patient was managed by radical prostatectomy and seminovesiculolectomy. Microscopic examination revealed well-differentiated primary mucinous adenocarcinoma of left seminal vesicle cyst. Conclusion: To the best of our knowledge, this was the first case of SVA of SV cyst arising in deaf mute patient
ANTIBIOGRAM AND SUSCEPTIBILITY OF KLEBSIELLA SPP ISOLATED FROM DIFFERENT CLINICAL SPECIMENS IN HEALTH CARE CENTERS IN ETSAKO WEST LOCAL GOVERNMENT AREA OF EDO STATE, NIGERIA
Arising multidrug resistance to available antibiotics is a problem of deep scientific and research concern. One organism with increasing antibiotics resistance is Klebsiella spp. that had cause several outbreaks in different locations. This study was undertaken to investigate the antibiogram and susceptibility of Klebsiella spp. isolated from clinical samples in health care centers. The study was conducted in Auchi Township in Etsako West Local Government Area, Edo State, Nigeria. Following standard procedures samples were collected and Klebsiella spp. identified using standard microbiology techniques. Antibiotic susceptibility testing was carried out using standard methods. 109 samples were collected and comprised urine (44), wound (20), high vagina swabs (17), urethral (3), sputum (9), semen (6), stool (5), endo-cervical swab (2), throat (1) and ear (1). 10 (9.17%) samples were positive to Klebsiella organism with urine consisting 50.0% and 10.0% each from sputum, stool, wound, ear and high vaginal swabs. Klebsiella isolates were completely resistance to penicillin, Amoxiclav and Chloramphenic. Amoxicillin, Floxapen, Metronidazole, Ampicillin, Cotrimoxazole, Nalidixic acid, Streptomycin, Tetracycline, Cephalexin, Genticin, Tobramycin, Nitrofurantoin, Ciprofloxacine and Cefuroxime have less than 50% potency. On the other hand, Ofloxacine, Cefotaxime, Ampisulbactam and Perflacine were 50% and above susceptible. Klesiella spp. isolated from stool sample was more multi-drug resistance (87.75%) followed by those from wound (77.55%), sputum (73.47%), high vaginal swab (69.39%), urine (65.30%) and lastly ear sample (48.98). Based on our findings, Klebsiella spp. in the study area is multidrug-resistance thus suggesting antibiotic analysis prior to treatment
Exploiting Wavelet and Prosody-related Features for the Detection of Voice Disorders
An approach for the detection of voice disorders exploiting wavelet and prosody-related properties of speech is presented in this paper. Based on the normalized energy contents of the Discrete Wavelet Transform (DWT) coefficients over all voice frames, several statistical measures are first determined. Then, the idea of some prosody-related voice properties, such as mean pitch, jitter and shimmer are utilized to compute similar statistical measures over all the frames. A set of statistical measures of the normalized energy contents of the DWT coefficients is combined with a set of statistical measures of the extracted prosody-related voice properties in order to form a feature vector to be used in both training and testing phases. Two categories of voice samples namely, healthy and disordered are considered here thus formulating the problem in the proposed method as a two-class problem to be solved. Finally, an Euclidean Distance based classifier is used to handle the feature vector for the purpose of detecting the disordered voice. A number of simulations is carried out and it is shown that the statistical analysis based on wavelet and prosody-related properties can effectively detect a variety of voice disorders from the mixture of healthy and disordered voices
Severe Intracranial Hypertension Associated with Tetracycline Use in Non-Small Cell Lung Cancer
Introduction: Erlotinib has become an established treatment for patients with non-small cell lung cancer harbouring an epidermal growth factor receptor (EGFR) mutation. Tetracycline antibiotics are commonly prescribed for erlotinib-induced acneiform rash. A rare but morbid complication of tetracycline use is intracranial hypertension, an association which has not been reported in the oncology literature.Presentation of Case: We report a case of severe intracranial hypertension in a patient with non-small cell lung cancer. Risk factors were prolonged tetracycline use and leptomeningeal carcinomatosis. Initial investigations were unhelpful, necessitating a high index of suspicion.Conclusion: Tetracycline antibiotics, which are commonly prescribed for erlotinib rash, are an important risk factor for intracranial hypertension. Our patient developed severe vision loss from papilloedema, despite normal neuroimaging and relatively low opening pressure on lumbar puncture. Continuous intracranial pressure monitoring can be a valuable investigation in such circumstances
Synthesis and Evaluation of the Properties of Chitosan-Cellulose Whisker Microparticles for Controlled Release of Isoniazid
Cellulose whisker (CW) was synthesized from filter paper by acid hydrolysis. Chitosan –CW microparticles were prepared by microemulsion technique using isoniazid as the model drug and glutaraldehyde as the crosslinker. The microparticles were characterized by Fourier Transmission Infra-red Spectroscopy (FTIR), X-ray diffractometry (XRD), Scanning electron microscopy (SEM), Transmission emission microscopy (TEM). The effect of cellulose whisker and glutaraldehyde on the microparticles were assessed with regard to swelling, encapsulation efficiency and consequently on the release of isoniazid in different mediums. The drug release mechanism was studied for different time periods by UV-visible spectrophotometer. Cytotoxicity test was performed by MTT assay analysis. The results entailed that the microparticles can be exploited as potential drug carrier for controlled release applications
Silicone softener synthesis and application on knit and woven white cotton fabrics
Silicone Softener have wide spread applications in textile dyeing and finishing. In this paper three different samples of Silicone Softener softeners were synthesized on the basis of a new recipe with different chemical ratio and each of them have been used on knit and woven cotton white fabrics. Several tests have been done on Silicone Softener like ionic surfactant test, solid content test, stability test etc. Although, several tests were done on the Silicone Softener treated knit and woven white cotton fabrics e.g. fabric whiteness test, Tensile strength test, absorbency test. From these test results, observation and comparison it has come out that, the best silicone softener among the three, increases the fabric softness, flexibility but imparts relatively less absorbency property. It can endow the textiles with soft, fluffy style. Proper stability and durability can reduce the problem of the roller sticky, floating oil. It is colorless and transparent thick liquid and easy to dilute. It has the excellent affinity to all kinds of fabrics including cotton knit fabrics, woven cotton fabrics, jute etc. The overall results come out with great prospect for Silicone Softener “Sample C” in textile finishing
A Rare Incidence of Malignant Eccrine Spiradenoma in a Male’s Breast
Introduction: Malignant eccrine spiradenoma is a rare cutaneous tumor of sweat gland origin. It is even more rare to develop malignant eccrine spiradenoma located in the breast. Eccrine spiradenoma is a benign sweat-gland tumor which can arise on skin surfaces of the head, neck, limbs, and trunk [1].Kersting and Helwig were the first to describe spiradenoma as a skin adnexal neoplasm with a very slow growth pattern [2].Malignant eccrine spiradenoma (MES) is one of the rarest sweat-gland tumors and can arise from benign eccrine spiradenoma [3]or, less frequently, develop de novo.Case Presentation: We encountered a 74 year old male with a two centimeter malignant eccrine spiradenoma located at the six o’ clock position three centimeters from the right nipple with no skin changes present.Conclusion: The development of MES in a male’s breast is a very rare condition. A high index of clinical suspicion is required to reach a diagnosis. Furthermore, no gold standard management option exists and treatment should be individualized
Disease Control of Pulmonary Inflammatory Pseudotumor by Multidisciplinary Approach
Introduction: Inflammatory pseudotumor is a rare tumor of the lung, which is usually misdiagnosed as malignancy. Most of early cases are treated with complete excision, but advanced cases: radiation therapy, steroids and chemotherapy will be a part of treatment plan. Multidisciplinary approach can save many cases of inflammatory pseudotumor and other rare diseases.Case Report: we reporting a case of advanced inflammatory pseudotumor with had a long disease control with multidisciplinary approach in the form of chemotherapy, steroids, radiation therapy and surgery.Conclusion: Advanced IP of the lung is rare disease with unknown etiology, where multidisciplinary team is needed for diagnosis and treatment: maximum safe debulking, chemotherapy, radiation therapy and steroids can be the suitable combined treatment for many advanced case
