Management of Chronic Renal Failure (CRF) through Ayurveda –A Case Report
Chronic renal failure (CRF)is one of the clinical entities which occurs as a result hampering of renal function due to many medical or systemic illness developed due to current fast lifestyle. The contemporary science has quite expensive modalities having non confirming results in advanced stages of diseases. Ayurveda, an ancient science, prefers to prevent or treat the disease in initial condition to avoid such complications. The present case study was taken up to evaluate the cumulative effect of Shodhana & Shamana Chikitsa in CRF based on the collaborative application of two fundamental principles of Ayurveda i.e., root cause analysis and treatment according to underlying pathophysiology involved in specific clinical conditions. This is a case report of a male patient having age 69 years who was diagnosed as CRF before five years & on regular conservative treatment in modern science. However, due to persistent increase in values of renal profile, he approached to Panchakarma OPD MGACH& RC. He underwent Dashmooladi Sasneha Niruha Vasti in addition to some proprietary as well as traditional Ayurvedic medicines (Syp. Neeri KFT, Chandrapbha Vati, Punaranavdi Qwath, Gomutra Arka). After one month, there was significant reduction in values of serum creatinine & Serum Blood urea from 4.89 to 1.21 & 64 to 50 respectively with a substantial decrease in systolic & diastolic blood pressure along with improvement in patients\u27 quality of life. This case study will become helpful in planning further research studies with a large sample size based on cost-effective standard regime in Ayurveda for the management of CRF.
An observational study of hetus of musculoskeletal pain in IT (Information Technology) professionals
Background & Objective: The IT-BPO industry employees often suffer the musculoskeletal disorders (MSD). Hence the objective of this entitled study was to observe the Ayurvedic hetus (causes) for these MSDs. Methods : A cross sectional survey study was conducted on IT employees using a questionnaire on 223 subjects along with pilot study on 23 subjects. On the basis of the pilot study, validity and reliability of questionnaire was checked. Result : High incidence of low back trouble was observed (75.33%) as compared to shoulder and neck troubles. The presence of causes – Vishamasana (wrong posture), lack of exercise and Atishrama (physical and mental exertion) were very high. Higher observations were also found for the – ratri jagarana (night vigil) and kshudha vegadharana (postponing the meals). A comparative study of lifestyle factors among males & females showed that, more haphazard and faulty lifestyle was present in males. Conclusion : Vishamasana and Atishrama are major causes of these MSDs while high AC exposure, lack of exercise, ratri jagarana and kshudha vegadharana acts as Viprakrishta hetus (long term causes) for MSDs
A scoping review on the tools for assessment of Agni and its derangements including Grahani dosha
Abstract:
Objective: The purpose of this study was to review and analyze the tools available for the assessment of Agni and its derangements, including grahani dosha.
Methods: A review of the published literature was conducted online using PubMed, Science Direct, and Google scholar. The keywords for the search included, "Agni,", "Agni Bala,” “Agni vaishamya," "Grahani dosha”, “Assessment tool “, and “Questionnaire”. The initial search retrieved 37 articles, of which seven were screened, classified, and coded for further analysis including their content, methodology as well as utility in clinical practice.
Results: The review discovered four tools aimed at assessing Agni and three for diagnosing grahani roga. All were different with regards to the domains assessed, mode of administration, and methodologies in which they were developed. Only two of the tools for Agni assessment were prepared following proper methodology including the evaluation for their reliability and validity. The absence of a gold standard against which these tools can be corroborated is a major drawback, because of which these tools were neither subjected to stringent scientific validation nor evaluated for their clinical utility.
Conclusion: None of the tools available for review could be deemed best at accurately detecting Agni and its dysfunctions. Two of the tools were developed adopting proper methodology, however, the feasibility of these in a clinical scenario has to be established with further studies. Additionally, there was no tool directed at assessing grahani dosha in specific disease states, which necessitates further research to develop and validate tools, especially in specific disorders
The Ayurvedic management of Ardhavbhadaka with special reference to migraine : A single case study
Migraine most often begins at adolescence and most affects those aged between 35 and 45 years. It is caused by the stimulation of a mechanism deep in the brain that leads to release of pain-producing inflammatory substances around the nerves and blood vessels of the head. Migraine is recurrent, often life-long, and characterized by recurring attacks. About 15% of the population suffers migraine. Prevalence of migraine is approximately 5.7% in men and 17.0% in women and accounts 2% years of life lost due to a disability in women of all ages. Modern science enforced several medications to manage migraine. additionally, these drugs have adverse effects like GI upset, hypotension, lack of concentration etc It is more common in women, because of hormonal influences. Ardhavabhedaka can be scientifically correlated with Migraine due to its cardinal feature ‘half sided headache’ and also due to its spasmodic nature. All the three doshas are involved in the pathogenesis of the Ardhavabhedaka with the predominance of Vata or Vata Kapha. Head is the ruler of indriya and Kapha dosha. The medicine given through Nasya reaches up to the brain and thus it pulls out all the disease causing doshas impurities Manasamitra Vatakam and Shir Shuladi Vajra Ras are is mentioned in Sahasrayogam and Bhaishajya Ratnavali book with special indication to Shirha Shoola
A Literary review on Buddhi(IQ) and Smriti(Memory)–An Ayurveda approach
Mana (mind) is one of the important things in life which leads to overall health. Smriti (memory) is the remembrance of what has been experienced directly earlier. It is described as one among the eight Aishwaryas (desire of power) which only happen through the concentration of the mind. Buddhi (Intellect) is also known as Mahata (greatness), Upalabidhi (accomplishment), and Gyana (intellectual). Grahana (grasping) and Dharana (retaining) capacity are the most important properties of intellect because of this knowledge can be retrieved by Smriti (memory). In the modern era due to stress, and disturbing emotions there is an increase in incidences of psychosomatic disease. Aim and objectives: The main aim is to explore and review IQ and memory through the Ayurveda approach. Materials and methods: The source for this study is collected from the classical Ayurvedic books and commentaries, modern medical science textbooks and different articles from Pubmed, DHARA, Google scholar etc. Result: There is an elaborate description of various mental faculties like Buddhi (intellect), Manas (mind), and Smriti (memory) in the ancient Indian texts including Ayurveda texts. Conclusion: Exploration of IQ and memory through the Ayurveda approach helps better understanding of IQ and memory which can improve the practical utility and clinical applicability of these Ayurvedic concepts. Clinically, the anatomic and physiologic knowledge of both IQ and memory will not only facilitate choosing the right medicinal formulations but also definite Panchakarma procedures for the best outcome in the prevention of diseases and cure the neuromotor or neuro-behavioural ailments in which both IQ and memory get defected
Photocolorimeter – Can it be a novel application to quantify Avila Mutrata?
Prameha has been described as one of the Mahagadas and Anushangi vyadhi. It is also a disease which is associated with derangement in Ojus (~Immunity). The disease is manifested as Prabhootavilamootrata (Polyuria with turbidity in urine). Madhumeha can be closely correlated to Diabetes mellitus. Glycosuria is a striking feature of the disease, which alerts the patient. It happens when the glomerulus filters more glucose than the proximal tubule can reabsorb. It can be due to elevated plasma glucose or renal glucose absorption impairment, or both. Excessive Kleda in the condition of Madhumeha gets mixed with other doshas and dhatus and forms Vikrita avastha. Kleda has two routes of expulsion from the body viz; Sweda and Mutra. This can lead to the Avilata mutrata in Madhumeha. Avilata is a condition related to light absorption, diffusion and Chroma/Chrome. liquid. Hence avilata is related with transparency of an object and its optical density (OD). Therefore, COLORIMETER instrument was chosen for the mutra pariksha, which works on the principles of OD. Colorimetric analysis can become a simple, OPD based, cost effective, and specific quantitative analysis of Urine which can be used to translate the principles of Ayurveda into practice and for better management and monitoring of Madhumeha.
The Effect of Parent Education on the Extent of Paternal Adaptation in First-Time Fathers
Entering the stage of parenthood can be a stressful time and can significantly affect the mental health and well-being of parents by causing lifestyle changes. Thus, this research was created with the aim of assessing the effect of parent education on the extent of depression in fathers, after their first experience with parenthood. Members of the experimental group included 100 husbands of pregnant women visiting Ardebil’s healthcare medical centers, who received counseling and training about accepting the role of fatherhood. Members of the control group included 100 husbands of pregnant women in the same centers, who received the usual pregnancy care. The analysis of the data was conducted on two descriptive and deductive levels. The Kolmogorov-Smirnov test was done to evaluate the normality of the data, and the Mann-Whitney U non-parametric test and the Wilcoxon Signed-Rank test were used for comparing the experimental group and the control group. The findings of the present study showed that the average paternal adaptation score of the control and experimental groups does not have a statistically significant difference before and after the experimental interference. However, the average paternal adaptation score of the experimental and control groups had a statistically significant difference after the experimental interference. Therefore, it can be concluded that education increases paternal adaptation and quality of life. The outcome of this study showed that education and increasing personal information, especially during the childbearing period, increases the quality of life and paternal adaptation
Analytical standardization of Shleshmantaka Agad
Background: To counter-act the action of visa,(poison) ancient Acharyas have mentioned the different types of Agada. Though Acharyas have mentioned various types of potent Agadas in various poisoning conditions, still these Agadas have neither undergone analytical study nor animal experiment. Shleshmantaka Agad is one of the drug remedy mentioned in Shushrut Samhita, kalpa sthan used for treatment of snake bite poisoning. Aim: In present work an attempt has been made to standardize Shleshmantaka Agad. Material & Methods: Physicochemical parameter was tested by Loss on drying, Acid insoluble Ash, Total ash value, Water & Alcohol soluble Extractive Value, HPTLC was used for phytochemical analysis. Result: In analytical study of Shleshmantaka Agad obtained value of Loss on drying, Total ash value, Acid insoluble Ash ,Water & Alcohol soluble Extractive Value are 12%, 10%, 2%, 13%, 1% respectively and in HPLC major constitute phytoamines was detected. Conclusion: Analytical findings of present study can be considered as reference standard for SA. As Shleshmantaka Agada is a combination of many phyto-constitutes but major constituent are phytoamines which may be possesses anti-snake venom property
Simultaneous method development and validation of 1, 8-Cineole and Betulinic acid in Shati Vati - An Ayurvedic formulation by HPTLC
A simple, accurate and precise HPTLC method was developed and validated for simultaneous estimation of 1, 8-Cineole and Betulinic acid in Shati Vati - An Ayurvedic formulation by HPTLC. Shati Vati contains Shati rhizome powder have 1-8 cineole and Badar ripe fruit powder have Betulinic acid reported medicinal claims and uses. In the analytical method, toluene: ethyl acetate (93:07 v/v) was finalized as the mobile phase and a clear separation was achieved. The wavelength for 1-8 cineole and Betulinic acid was 665nm and 580nm respectively. The Rf value of 1-8 cineole and betulinic acid was obtained 0.647 and 0.213 respectively. The developed analytical method was validated for Linearity, Accuracy, Precision, LOD, LOQ and Robustness as per ICH Q2 (R1) guideline. Results for the validation process were within the range of acceptance criteria of ICH guideline. HPTLC method have been applied successfully for estimation of 1-8 Cineole and Betulinic acid in Shati Vati an ayurvedic formulation
In-silico and in-vitro toxicological evaluation of Tiktakam kashayam
“TIKTAKAM KASHAYAM” is evaluated by in-silico study using PASS online software. Its having anti-inflammatory activity and also having anti-neoplastic activity especially colon cancer. In-vitro heavy metal analysis shows lead and mercury below the detection limit, cadmium not detected and arsenic is present below the approved range. Some important toxicities like hepatotoxicity shown by the compound azadirachtin present in Azadiracta indica, neurotoxic shown by the compound biflorne present Inoldenlandia corymbosa, Ototoxicity shown by the compound sweroside, reproductive dysfunction produced by the compound luteolin present in Gentiana kuro. Neurotoxic by kobusone, respiratory failure by swertiamarin, ototoxicity by sweroside present in the plant swertia chirayata. Parkinsonism like effect produced by the compound piperine and pipperitin by Piper longum. The ayurvedic formulation “TIKTAKAM KASHAYAM” possess anti-inflammatory activity and will be beneficial for the treatment of inflammatory bowel disease
