Efficacy of Yashtimadhu ghrit matra basti in post-operative management of Ano-rectal Cases – A case Series
Background- Exploitation of analgesics is becoming an advent scenario in the current practice of both medical and surgical cases. Adopting a less complicated and a wholesome beneficial modality for post operative analgesia is the need of the hour. In this case series, focus has been placed on yashti ghrit matra basti as the intervention for pain and wound management of post-operative cases of chronic fissure in ano associated with internal haemorrhoids. Objective- The current study was aimed at post operative analgesia and wound management with yashti ghrit matra basti in ano-rectal cases. Material & Methods-The method adopted here was Lord’s anal dilatation followed by excision of sentinel tags or ligation haemorrhoidectomy according to the case presentation, followed by yashti ghrit mastra basti as primary ayurvedic intervention for post operative analgesia and wound healing. Results -It is found that the planned treatment gave a positive response as post operative analgesia and also played an adjuvant role in wound healing. Conclusion-This case series provides the successful pain and wound management of post-operative ano-rectal cases with a promising ayurvedic intervention.
Variations in the severity of symptoms, DAS28, and Disability Index among different Vata predominant Deha-Prakriti individuals of Amavata (~Rheumatoid Arthritis) - An Analytical Observational Study
Background: Subgrouping of diseased population, based on Deha-Prakriti (DP), phenotypic classification of Ayurveda will explore the variation in the disease severity, which further aid in the management of the disease more precisely and selectively. But there is a paucity in the data generation. Amavata (~Rheumatoid Arthritis) is one of such diseases, resulting from the conglomeration of Ama and aggravated Vata gets lodged in the joints. Aim & Objective: To evaluate variations in the severity of symptoms, duration of morning stiffness, DAS28 (Disease Activity Score in 28 Joints), and Disability Index (DI) among different Vata predominant Deha-Prakriti (DP) individuals of Amavata (~Rheumatoid Arthritis). Materials & Methodology: The study was conducted among 155 clinically diagnosed patients of Amavata, who were screened for DP assessment through the CCRAS-PAS scale. Those who were found to have Vata predominant DP were sub-grouped into Single Vataja (V), Vata-Pittaja (VP), and Vata-Kaphaja (VK) DP, based on the proportion of the doshas in the output of scale. Demographic profile, the severity of the symptoms, duration of the morning stiffness, DAS28, and DI were assessed and calculated using SPSS software version 20.0, p≤0.05 was considered statistically significant. Results: Kruskal-Walli’s test revealed that Single Vata predominant DP patients were found to have severe Sandhi-shula (Joint pain) (p<0.0001), Shunatanga (Numbness of the joint) (p-value at 0.0069), Nidra-Viparyaya (Irregular sleep patterns) (p-value at 0.0012) than VP and VK DP individuals. Sandhi-Shotha (Joint swelling) (p-value at 0.0299), Angamarda (generalized body pains) (p-value at 0.0130), Alasya (laziness) (p<0.0001), Gaurava (Heaviness) (p<0.0001), and Apaka (delayed digestion) (p-value at 0.0254) were found severe in VK DP, whereas Trishna (Thirsty) (p<0.0001), Nidraviparyaya (p-value at 0.0012) were more severe in VP DP individuals. One-way ANOVA revealed that the duration of morning stiffness (2.03±0.2 hours, p-value at 0.446), DAS28 (6.85±0.175, p-value at 0.0035), and DI (1.87±0.54, p-value at 0.0003) were found significantly high in single Vataja than VP and VK Deha-Prakriti. Conclusion: The disease severity in terms of duration of morning stiffness, DAS28, and DI were found more severe in Single Vataja DP. The severity of the signs and symptoms also varied according to DP
Open-Labeled, Randomized, Prospective Controlled Clinical Study to Assess Efficacy of Rasayana Churna and Pranayama in the Management of Rajonivritti Lakshanas w. s. r. to Postmenopausal Syndrome
Background: In modern science, hormonal replacement therapy (HRT) is the only alternative for postmenopausal syndrome whereas non-hormonal pharmacologic therapies have been shown to be less effective than hormonal therapy. So, it was assumed that Rasayana Churna along with Pranayama would be effective in relieving postmenopausal symptoms. Objective: to assess the clinical efficacy of Rasayana Churna and Pranayama in the management of Rajonivritti Lakshanas w. s. r. to Postmenopausal Syndrome. Methods: The present open-labeled, randomized, prospective controlled clinical study was conducted after IEC approval. Total 60 female patients suffering from Postmenopausal Syndrome were enrolled in the study and randomized into two groups i. e. Group A & Group B. 30 patients in Group A were given Rasayana Churna orally with daily practice of Pranayama was advised. 30 Patients in Group B were advised daily practice of Pranayama for consecutive 12 weeks. The results were assessed by using Menopausal Rating Scale (MRS) and Menopause-Specific Quality of Life Questionnaire (MENQOL). Results: The significant difference was observed in MRS-Total score and MENQOL scores after treatment in both the groups. Out of four scales of MENQOL, the vasomotor, physical and psychological scales showed significant improvement in the group received regimen of Rasayana Churna along with Pranayama as compared with only Pranayama. Only change in sexual scale score was found to be similar in both the groups’ patients. Conclusion: It can be inferred that regimen of Rasayana Churna along with Pranayama was more efficacious in treatment of post-menopausal syndrome as compared with only Pranayama
Importance of Swa Anguli Pramana in Health Assessment: Insights from Cross-Sectional Study on Aayama Relationship
Introduction: The anthropometry techniques used today share similarities with the method described in Ayurveda by the acharyas. Ayurvedic science adopts a more personalized approach to measurement, taking into account individual variations in the body proportions for a greater precision in assessing each body part. In contrast to the modern approach, which relies on a generalized view, ayurvedic science offers significant advantages. Method: Survey conducted among 400 individuals to determine the correlation between swa anguli pramana (measurement of the width of the proximal phalanx of the middle finger in the palmar aspect) and aayama (height). Result: The study found that there was a change in the average swa anguli pramana values, which caused a noteworthy difference in the aayama of the individuals. The study clearly indicates that, swa anguli pramana is positively associated with height of individual. Discussion: However, the study does not invalidate the swa anguli pramana measurement technique or elevate the contemporary day standard metric system above it. The existing metric system has limitations, such as variations with changes in gender, race, and area, whereas swa anguli standards are individualistic and not impacted by these aspects. The study suggests that revalidation of the pramanas is required, but considering it as a standard system of measurement remains relevant
Single Blind Randomized Control Clinical Study to Assess the Effect of Cow-Ghee On Lipid Profile and Blood Glucose in Healthy Volunteers
Ayurveda consider cow ghee as healthiest source of dietary fat and have rejuvenating property, improves memory and strengthens nervous, eyes and protects the body from various diseases by balancing all three Doshas (Vata, Pitta and Kaph) and advocates it’s use in daily diet to maintain health. Though, today concept of healthy eating has become synonym to avoid ghee. Aim: To assess the efficacy of Cow Ghee on Lipid Profile and Blood Sugar levels in Healthy Volunteers. Materials and methods: This was a Single blind randomized control clinical study. 60 healthy volunteers (30 in each group) between the age group 25-45 years were randomly selected. FBS was assessed based on WHO Guideline and Complete lipid profile was assessed based on ATP III Guideline. Statistical analysis used: result was assessed using Paired and unpaired t test with the help of Graph Pad InStat-3 software. Results: The slight increase in FBS, TC, TGs, VLDL, and HDL was recorded with mean difference 2.054, 2.91 (p = 0.4376), 21.568 (p = 0.0024), 4.141 (p = 0.0039), and 2.068 (p=0.0483) respectively compare to mean difference 10.260, 5.835 (p=0.2348), 16.88 (p=0.0297), 3.39 (p = 0.0274), and no change in HDL in placebo control group. While decrease in LDL by mean difference 3.291 (p = 0.3786) was recorded in cow ghee group compare to increase in mean difference 1.497 (p = 0.7844) in placebo control group. Conclusion: study reveals no adverse effect of consumption of 15-gram cow ghee daily for 45 days on lipids and FBS.
Acute toxicity study of hydroalcoholic extract of two Indian medicinal plants Alternanthera ficoidea and Ludwigia octovalvis
Background: The ethano-pharmacological survey on two Indian medicinal plants Alternanthera ficoidea and Ludwigia octovalvis revels that both plants are biologically potential and effective in the management of various diseases. Despite the both the plants as whole are used traditionally by the local people in Indian subcontinent but the toxicity studies on whole plant extract of both plant has not explored scientifically. Objective: The toxicity study was designed to explore the toxicity potential associated to hydroalcoholic extract of whole plant of Alternanthera ficoidea and Ludwigia octovalvis according OECD guidelines. Material and methods: Healthy Charles Foster albino female rats of 8-12 weeks’ old were divided in 3 groups. Group I, group II, and group III received vehicle (0.5% CMC, p.o., single dose), hydroalcoholic extract of whole plant of Alternanthera ficoidea (HAF) (Single dose of 2000 mg/kg, given orally), and hydroalcoholic extract of whole plant of Ludwigia octovalvis (HLO) (Single dose of 2000 mg/kg, given orally), respectively. After 14 days, all rats were anesthetized with chloroform and blood were collected by retro-orbital capillary puncture for the study of haematological and biochemical parameters. After blood collection, rats were sacrificed; organs were carefully collected, weighted and examined for changes. Results: The acute toxicity study on Charles foster female rats showed that there was no sign and symptoms of toxicity were seen in rats of all group. Haematological parameters, biochemical parameters and histopathological architectures of various organs of treated groups were found to be non-significant and normal in range when compared with normal control groups. Conclusion: The study found that no mortality occurred at a dosage of 2 g/kg of HAF and HLO, and it came to the conclusion that the IC50 of HAF and HLO was higher than that level
Optimization and Evaluation of Anti-Inflammatory Herbal Emulgel loaded with Vitex Negundo for Enhanced Permeation
The use of conventional drugs for the treatment of pain and inflammation has largely resulted in various side effects. These challenges have triggered the search for alternative therapy such as herbal medication that may serve as a safe, effective, and alternate treatment approach to managing pain and inflammation. Vitex negundo, a potential lipophilic molecule, possesses limited solubility and permeability. Emulgels are either water-in-oil or oil-in-water type emulsions, mixed with a gelling agent to form a gel for the delivery of lipophilic active ingredients. Therefore the current study is undertaken for the effective delivery of vitex negundo locally in the form of oil-in-water emulgel for improving solubility and permeability. The emulgel was formulated from carbopol 934 with vitex negundo in mineral oil emulsion using tween 80 (surfactant), and triethanolamine (co-surfactant). The formulations were optimized using the design of experiment software. The optimized emulgel formulation containing Smix (tween 80 and triethanolamine) ratio of 61.90, mineral oil 50.58%, and carbopol 934 3% showed a pH of 6.5±0.3, a viscosity of 17793 – 939 cps, and spreadability of 7.07 g.cm/s. Data from in-vitro and ex-vivo diffusion studies demonstrated improved permeability properties. The stability of the optimized emulgel formulation was additionally examined at ambient temperature for 28 days. The emulgel remained stable for 28 days. Based on the overall results we can conclude that emulgel can be a promising and suitable formulation for the application of vitex negundo
A Systematic Review on Panchakarma-Based Ayurveda Treatment in Janu Sandhigata Vata
Janu Sandhigata Vata or knee osteoarthritis (KOA) causes chronic pain, stiffness, immobility and affects daily life. Panchakarma-based Ayurvedic treatment is popular in arthritis. The present study has done a systematic review on effectiveness and safety of panchakarma-based treatment in chronic KOA. The study searched for double-arm randomized comparative trials (RCoTs) with human subjects (age ≥ 18 years) explicitly diagnosed with KOA in fourteen electronic databases including MEDLINE, PubMed Central, Cochrane, Ayush Research Portal for indexed research and clinical trials published till September 30, 2022. At least one patient-centred primary outcome, namely pain, physical function, global improvement according to OMERACT III were eligible. Risk-of-bias analysis was done independently by the two authors using the revised Cochrane risk-of-bias tool for randomized trials. Effect size as standardized mean differences between treatments were estimated for pain and stiffness. The inclusion criteria screened 10 RCoTs. The RCoT which used manual therapies in a multimodal ayurvedic care against conventional care had overall low risk of bias. Remaining 9 RCoTs compared the treatment outcomes of different panchakarma-based treatment with and without pharmacological interventions. In 3 RCoTs, effect size on pain and stiffness were medium to large and significant (P<0.01). Only 3 RCTs reported safety concerns and no serious adverse outcomes related to intervention were noted. Sudation techniques and enema therapy used as standalone or in multimodal ayurvedic care showed benefit in KOA. Future studies on the long-term efficacy and safety goals aligned to psychosomatic record of patients are required for personalized Ayurveda multimodal care.
A narrative review on genotoxic potential of medicinal plants used in Ayurveda
Genotoxic substances are those which are capable to induce a destructive effect on a cell’s genetic material. It can be either carcinogen, mutagen, teratogen or cytotoxic depending upon the dose, duration and mode of usage. Mutations may manifest in many forms as duplication, deletion or insertion. However, all mutagens are genotoxic, not all genotoxins induce mutations. Currently traditional medicines and plant drugs are used across the globe without strict medical supervision. In a country like India, folklore medicines play a pivot role in health sector. Irrational long-term usage of any plant-based drug are capable to induce adverse reactions. Till date there is no single hand information about the genotoxic effects of medicinal plants used in Ayurveda. Many of the drugs reported for toxicity are potent drug candidates in Ayurveda. From the available literature 184 articles reported the genotoxicity of medicinal plants. After shortlisting with the inclusion and exclusion criteria’s 57 articles are found to be on genotoxic potential of medicinal plants. Present review reports genotoxic effect of 32 drugs used either as single or combination. It contains various array of drugs, for example poisonous drugs like Arka, Dhattura, Mandookaparni – which is a medhya rasayana, Guduchi – which is a potent immunomodulator and so on. Among them a few drugs like Palandu, Tanduleeyaka, Misreya, Chandrasoora, Sariba, Manjishta, Dadima, Guduchi etc. need special attention. Traditional Indian Medicine Improves Clinical Outcome in Non-alcoholic fatty Liver diseases - A Systematic Review and Meta-analysis
Background: Non- Alcoholic Fatty Liver Disease (NAFLD) is a rapidly emerging liver disease linked strongly to metabolic syndrome. In the absence of specific pharmacological agents for the effective treatment of NAFLD, current treatment mainly focuses on lifestyle and dietary modifications. Several agents referenced in Ayurvedic texts have shown promising effects in patients over centuries of use. But the outcomes reported by various randomized clinical trials (RCTs) for the efficacy of herbs in non-alcoholic fatty liver disease (NAFLD) are conflicting. Objective: This study aims to systematically review and conduct meta-analysis of the available evidence to evaluate the efficacy and safety of Ayurveda-referenced drugs. Methodology: The electronic databases PubMed, Web of Science, Embase, Cochrane library, and ARP were searched up to May 2022 to identify relevant studies. Quality of studies was evaluated using modified Jadad Scale. Risk of bias was assessed using Cochrane risk of bias tool. Result: The search retrieved 1352 studies, but only 18 studies were finally evaluated. Statistical analysis showed significant reduction in grades of fatty liver (RR: 2.42, 95% CI; 1.52, 3.86), AST (SMD: -0.91, 95% CI; -1.04, 0.08;) and ALT (SMD = − 0.91; 95% CI; − 1.53, − 0.28; P < 0.00001). Conclusion: Ayurveda drugs have shown positive results in managing NAFLD, as evidenced by the limited available data. More quality-based RCT’s using standardized Ayurveda drugs with large sample sizes and taking histological outcomes into consideration must be conducted to generate stronger evidence
