Clinical study of add on effect of Triphaladaruadi Kwatha (Vangsenokta) along with antidiabetic drug in management of Prameha (Non-Insulin Dependent Diabetes mellitus- NIDDM)
Prameha (Diabetes mellitus) is an age long disease known from vedic period and now it is a leading lifestyle disorder. Diabetes mellitus is a group of metabolic syndromes of fat, protein and carbohydrate which is due to absolute or relative deficiency of insulin. The prevalence of diabetes in India has risen from 7.1% in 2009 to 8.9% in 2019.1 The estimates in 2019 showed that 77 million individuals had diabetes in India, which is expected to rise to over 134 million by 2045.2 In India, it is also 3rd leading cause of death (After heart disease and cancer). The number is projected to be twice by 2030. Type 2 diabetes makes up about 85-90% of all cases. Increase in the overall diabetes prevalence rates largely reflect an increase in risk factors for type 2, notably greater longevity and being overweight or obese. Hence preventive measures are essential and an ideal therapy is still obscure. It remains one of the baffling enigmas for clinical research. This is a randomized single blind standard controlled clinical trial conducted on 66 patients of Prameha (Type II DM & who are on antidiabetic treatment) to rule out Add on effect of Triphaladaruadi kwath of either gender between age group of 40-60 years and grouped into Group A & Group B. 33 patients in group A were on Antidiabetic drug (Metformin 500mg OD/BD and 33 patients in Group B are on same antidiabetic drug (Metformin 500mg OD/BD) with additional Triphaladaruadi kwath (Decoction) for 3 months. Results obtained in subjective and objective parameters were analyzed for the statistical significance by adapting paired ‘t’ test in the groups, Two-way ANOVA test between the groups, and Wilcoxon rank sum test. The study revealed that Group B was found to be more effective in bringing symptomatic relief and improving biochemical markers in the patients of Prameha
Proficiency of Agnikarma followed by excision in the management of Chronic Scalp Wart (Charmakila) – A Case report
Abstract
Background: Agnikarma is regarded as the best of the anushastras because of his quick activities and long-lasting cure. It is said to have the ability to cure ailments that cannot be cured by other therapeutic methods such as surgery, kshara, and pharmacological management. It is stated as more important than kshar because of non-recurrence of diseases burnt therewith. Ayurvedic classics advocated the use of Agnikarma in the management of various Kshudrarog, Charmakila is one among them. The pathophysiology of charmakeela is caused by vata and kapha vitiation over the skin, resulting in the formation of hard nailed structures known as charmakeela. In this present case study efficacy of Agnikarma followed by excision in managing a chronic recurrent scalp wart was evaluated.
Methodology: Agnikarma was done after proper excision of scalp wart followed by local application of Ghrutkumari pulp. Proper follow up was taken for about six months in order to see any recurrence.
Observation: It was observed that within a month the complete wound of excised scalp wart was healed without any sign of slough, discharge. The patient was previously treated for several times with different treatment modalities even with LASER therapy still was suffering from recurrent scalp wart but it was observed that after Agnikarma, wart didn’t recur even after 1 year of follow up.
Conclusion: It can be concluded that Agnikarma is not only a successful treatment in treating wart but also have noticeable results in treating chronic recurrent wart with no recurrence for a long term.

Insilico evaluation of pharmacological activities of Kalarchi Chooranam
The Indian traditional medicine practices such as siddha and ayurveda records various medicinal plants for the betterment of human health. Recent reports shows that this system of medicine has potential plants and formulation to treat various diseases and ailments. In the present study, we extracted and prepared the Kalarchi Chooranam of various ratios and chosen 4:1 ratio from our previous work. The aqueous extract of 4:1 ratio Kalarchi Chooranam was subjected to GCMS study and the compounds Inositol, Xanthosine (CAS), alpha.-D-Glucopyranoside, methyl 2,3-bis-O- were identified as lead 3 compounds. The Xanthosine (CAS), alpha.-D-Glucopyranoside, methyl 2,3-bis-O- have passed the drug likeness and ADME properties and further docked against various receptors involved in disease pathogenesis such as cancer (Bcl2, Bcl-X), PCOS (Cyp17, Follistatin), Inflammation & Analgesic (Cox1, Cox2), and Diabetics & Cardiovascular diseases (Er Alpha, Er Beta). The overall study reveals that the compounds have more binding affinities against the poly cystic ovarian syndrome receptors as the Xanthosine showed significant binding energies of -8.9 and -8.2 Kcal/mol and the Alpha.-D-Glucopyranoside, methyl 2,3-bis-O showed -8.2 and -9.7 Kcal/mol against CYP17 and Follistatin. Amaltas (Cassia fistula linn.) - A medicinal and pharmaceutical plant
Today, interest in medicinal plants has increased dramatically. The description of these plants in ancient texts is very detailed, but in scattered form. We only discovered a few details in the Nighantus (an ancient pharmacopeia about medicinal plants). One should go through all available Ayurvedic texts for information on any medicinal plant. Amaltas (Cassia fistula linn.) has been popular since ancient times as a frequent remedy of choice for Ayurvedic physicians due to its various therapeutic properties. These properties are laxative, antibacterial, antipyretic, anti-inflammatory, smooth muscle stimulating, hepato-protective, analgesic, hypoglycaemic, anticancer, abortifacient, anti-colic, anti-infertility, oestrogens, etc. useful in the treatment of diseases such as skin diseases, heart problems, gout, blood diseases, diabetes mellitus, herpes, feverish conditions, etc. It grows in Indian habitat abundantly, and deciduous woodlands of south India. Its concentrate mostly contains alkaloids, tars, flavonoids, rhein glucoside, fisulic corrosive. It also possesses the properties such as hypo-glycemic, laxative, anti-bacterial, antipyretic, anti-inflammatory, smooth muscle stimulant, hepato-protective, analgesic, anticancer, abortifacient, anti-colic, antifertility, estrogenic, etc. It has vast therapeutic importance, but not enough addressed by Ayurvedic point of view, even though many therapeutic potential references from classical Ayurvedic texts are available. Hence, an attempt was made to review its therapeutic potential through this article, which will guide the researchers and scholars for further study. Management of Uthiravathasuronitham (Rheumatoid arthritis) by Siddha traditional formulation Amirdha Gandhi Kukkil Vallathy – A case series
Aim and Objectives: Rheumatoid arthritis is an auto immune disease and its signs and symptoms are correlated with Uthira vatha Suronitham (UVS) in Siddha. Here we discussed a case series of Rheumatoid arthritis managed by a novel, classical Siddha formulation Amirdha Gandhi Kukkil Vallathy (AGKV). Methods: Three cases were diagnosed as Rheumatoid arthritis (RA) based on ACR/EULAR 2010 criteria, along with symptoms of UVS as per Siddha literature. All the three cases were treated with Siddha medicine AGKV 2 gm bid for 48 days in the General out patients department, Siddha Central Research Institute. Observations: The assessment was made by the signs and symptoms and serological parameters like C-Reactive protein (CRP), Erythrocyte Sedimentation Rate (ESR), Haemoglobin (Hb), Rheumatoid Arthritis factor (RA factor). Results: The RA Factor was reduced in all three patients from 60 to 13.9, from 133 to 106, from 60.7 IU/ml to 35.6 IU/ml ; CRP reduced in case 1 from 135.8 to 34.3, case 3 from 18.9 to 5.6, reduction in RAPS score and clinical symptoms were noticed in all three cases and were followed for 1 year and managed with diet advice. This case series highlights that RA can be successfully managed with Siddha herbo mineral preparation AGKV within a minimum period of 48 days. Hence in a long duration of treatment, it will certainly give good results to reduce the burden of RA patients
Lymphedema Praecox a subtle exigent case study through endogenic avenue W.S.R. to Urustambha
The lymphatic system has historically been disregarded, undervalued, and poorly understood. As it mimics chronic venous insufficiency, it leaves patients in a terrible condition. One such congenital condition where aberrant protein-rich fluid accumulates in the lower extremities is lymphedema praecox. In ‘Ayurvedic samhitas, there is a condition called Urustambha which can be correlated with Lymphedema praecox. Clinical characteristics and therapy are beautifully detailed in samhitas like Charak and Sushrut thousands of years ago, and modern science is leveraging them even today. Since this condition is uncommon, we decided to address and take up this case. A 25years old female was admitted in our opd with complaints of B/L lower limbs, huge swelling followed by long standing and feeling of heaviness. We diagnosed and treated the patient according to classical texts of Charak and Sushrut with baluka swedan, prachana karma, udwartan and few oral medications and got promising results. With ayurvedic intervention, their quality of life has been improved and no further medication was required.
A comparative study on the effectiveness of Pathadi churna and Protein powder in Karshya (Undernutrition) among preschool children
Introduction: Ayurveda depicts a disease named ‘Karshya’ which goes hand in hand with the clinical entity ‘Under-nutrition’ in contemporary science. Aim & Objectives: The present study aimed to compare the efficacy of ‘Pathadi churna’ and Protein Powder as oral administration in preschool children suffering from Under-nutrition. Methodology- The trial was a randomized parallel group carried out in 30 children aged 3 to 6 years presenting with the clinical features of under-nutrition who were divided into two groups equally. In Pathadi churna (group A-trial) and protein powder (control, group B), Pathadi churna and protein powder respectively was given in dosage according to age as per Young’s formula for 60 days. Results were analyzed according to frequency %, PEM (protein energy malnutrition) gradation shift, changes in assessment parameters, and the level of significance were noted by using Wilcoxon signed rank test and the Student’s paired t-test. Results- Significant results were noted in Daurabalya (~Fatigue) in both the groups whereas the Pathadi churna group showed significant results in the Agnimandya (~Lack of appetite & poor digestion), weight, weight for height, BMI, PEM gradation, and the values of hemoglobin, serum proteins & calcium. On comparing both groups, statistically significant differences were seen in Pathadi churna than Protein powder. Conclusion- Significant results were seen in Daurbalya in both the groups but on comparison more significant effects were noted for all subjective and objective criteria in the Pathadi churna group. No ADR was reported in the study. Further large sample studies with standard control would render better evidence in results with justification
Screening of Phytoresources from the Romanian Flora with medical applications against Covid - Review
Plants are an important means of combating numerous harmful influences on humans (microorganisms, viruses, fungi, etc.) and have always been used to treat various diseases. In the context of the Covid pandemic, interest in the use of plants has increased. Great importance has been given to screening plants\u27 potential against Covid (antiviral, anti-inflammatory, immunostimulatory, and antioxidant). According to recent research, many of the plant species used against Covid are of Asian origin. In this review, we aim to discuss the plant species with these medicinal potentials with a focus on Romanian flora. We have listed a total of 50 Phyto-resources from Romanian flora with different potentials: 26 containing the confirmed anti-covid compounds, and 9 species having antiviral, anti-inflammatory, immunostimulant, and antioxidant potentials
Network pharmacology and Molecular docking-based activity of Hemidesmus indicus (L.) R.Br. in Acute myeloid leukemia : A Computational Study
ABSTRACT
Background
Acute myeloid leukemia (AML) is malignancy of the stem cell precursors of the myeloid lineage occurs due to variations in genetics. Incidence rate of childhood AML in Asian population is 8.4 per million. There is no exact description of AML is Ayurveda, it can be considered into Raktapitta (~bleeding disorder) disease. Hemidesmus indicus (L.) R.Br. (~H. indicus) is described for treatment of Raktapitta. The main treatment for AML is chemotherapy, and patient are searching for Ayurvedic medicines. Hence attempt is made for evaluating activity of H. indicus in AML.
Objective
To establish link for therapeutic activity of H. indicus in AML using Network pharmacology and molecular docking study.
Materials and methods
Active compound from root of H. indicus was retrieved from phytochemical based IMPPAT database. ADME (absorption, distribution, metabolism and excretion) study of retrieved active compound done with SwissADME database, and ADME qualified active compound target were obtained with having probability >0.7 from SwissTargetPrediction database. Target of AML retrieved from GeneCard database having relevancy score ≥5.0. To take the common, target of active compound and AML targets from GeneCard are imported into the Venny2.1 database, and the resulting targets used for the analysis. Cytoscape3.9.1 software was used to construct the "drug-active components-target" network diagram from common targets. The PPI (protein-protein interaction) network between proteins was constructed by STRING and result exported to Cytoscape3.9.1 for network analysis to get subnetwork with key target of subnetwork and core targets of overall PPI. GO (gene ontology) and KEGG (Kyoto Encyclopedia of Genes and Genomes) pathway analysis of key target from subnetwork done with g-profiler database. Core targets were docked with their corresponding active compound to get docking score.
Results
Total of 66 active compound were obtained from H. indicus, On ADME screening 49 active compounds qualified. ADME qualified active compound screened for target and 149 target obtained, after removal of duplicates 81 target were remained. 806 targets of AML were screened from GeneCard database. 19 common targets were obtained between target of active compound and target of AML from GeneCard. These screened 19 targets imported in STRING database to construct PPI network, and obtained result imported into Cytoscape3.9.1 for network analysis, on analysis 1 sub networks with 11 key targets were obtained. Top five H. indicus core targets for AML obtained using “CytoHubba” plug-in. GO and KEGG enrichment analyses were performed on the above-mentioned 11 key targets from sub network, 44 MF (Molecular function), 166 BP (Biological process), 12 CC (Cellular component) and 55 pathways were obtained from the KEGG pathway analyses. Molecular docking results showed that the active component quercetin could spontaneously bind to the core targets EGFR, SRC, AKT1, KDR and IGF1R. EGFR has the best combination with quercetin.
Conclusion
All core targets identified through network analysis of PPI network were linked to common active compound quercetin, and on molecular docking study all core targets showed good docking score to quercetin. Hence, based on this study conclusion can be drawn that the activity of H. indicus is AML might be due to presence of quercetin active compound in it. This study generated link for usefulness of H. indicus is AML.  
Probable mode of action of Sida Cordifolia in Dyspareunia an In-Silico network Pharmacological approach
Dyspareunia, a painful sexual disorder can be correlated to Paripluta yonivyapath (disorders of the genital tract) as per Ayurveda classics, which comes under the category of Female sexual dysfunction, the less explored clinical field in Ayurveda. Bala, Sida cordifolia Linn is the single drug that is directly indicated in this disease condition. This is the era in which the whole medical world is running behind in standardizing Evidence-based medicine (EBM) in clinical application. Network pharmacology, a branch of science that combines Network biology and polypharmacology, which predicts the mode of action of therapeutic drugs on both the interactome and the diseasome level. Here is an attempt to put forth the therapeutic efficacy of Bala in Dyspareunia through network pharmacology and bonding energy was demonstrated by molecular docking. Methods: Phytochemicals of Bala were collected from the IMPPAT database and related research articles. The disease targets were obtained from databases namely the Therapeutic target database, ChEMBL, Human protein atlas, and DisGeNet. The therapeutic efficacy of these gene targets in dyspareunia is confirmed and the conclusion is obtained from the network pharmacological ligand-target interaction methodology. The ligands and targets were retrieved from the PubChem, Protein Data Bank and docked using PyRx software. Results and Conclusion: The current study identified important Phyto-constituents like Resin Acid, Malvalic Acid, 5,7-Dihydroxy-3-Isoprenyl Flavones, Peganine, Β-Phenethylamine, Ψ-( Pseudo) -Ephedrine, Coronaric Acid, Potassium Nitrate, Phenethylamine, Ephedrine, Choline which were highly modulating CYP19A1, ESR2, MAPK1, AR, MAPK3, ESR1, CYP19A1, PGR proteins related with Dyspareunia.
