    Ayurvedic management of Asthimajja gata vata w.s.r to AVN of head of femur – A case study

    Avascular necrosis (AVN) of the bone is a condition characterized by the death or necrosis of bone tissue, accompanied by structural changes. It is also referred to as osteonecrosis or ischemic necrosis of the bone. It is mostly seen in patients having a history of excessive alcohol and high-dose steroid medications. In the initial stage, it is painless but as the disease progresses there is a gradual increase in pain which affects the patient’s daily routine as well as his lifestyle. It is the most challenging condition in orthopedics. The available treatments in modern medicine for this condition are not considered satisfactory in the current era. Here we present a case of Avascular Necrosis (AVN) of the Femoral Head that was treated with Panchakarma procedures, including Udvartana, Abhyanga, Swedana, Basti karma, Raktamokshana, and Physiotherapy exercise. In Ayurveda, AVN Head of Femur can be co-related to Asthi Majjagata Vata, based on similar signs and symptoms. Marked improvements were observed in terms of pain reduction, range of motion, and overall quality of life. The study shows that Avascular Necrosis of the Femoral head may be successfully managed with Panchakarma treatment, Shaman chikitsa, and Physiotherapy Exercises

    Role of Ayurveda in the management of Post-traumatic chronic inflammatory synovitis of knee joint- A Case Report

    Knee is a complex joint. It is the most commonly injured joint in the body. Trauma may injure soft tissues such as ligaments or bones of the joint. Inflammatory synovitis is one of the conditions developed after trauma. This article encompasses a diagnosed case of Post traumatic chronic inflammatory synovitis. Treatment protocol was opted considering its samprapti (Pathophysiology) as janu sandhi marmaghata. Treatment was planned considering Sandhi marmaghata. Symptomatic treatment was also considered. Significant improvement was observed in parameters like range of motion, knee joint circumference, walking distance and VAS scale within a week. No recurrence observed even after 1 year of suspended treatment. This multimodular Ayurveda treatment appears to be safe and efficient for early recovery and better outcomes.

    Ayurveda management of Recurrent Ischiorectal Abscess with Horseshoe connection - A case report

    Anorectal abscesses are one of the potentially debilitating diseases among them perianal abscess and ischiorectal abscess are common. Lateral spread of the ischiorectal abscess through the conjoint tendon leads to a horseshoe abscess. Surgical treatment is incision and drainage. Complications include sepsis, faecal incontinence, and fistula in ano. Kshara sutra is proved effective for fistula in ano, which helps in preventing recurrence. Here we report a case of ischiorectal abscess with horseshoe connection came with swelling and pain over the right perianal region. The condition was diagnosed as guda vidradi (perianal abscess) as per Ayurveda classics, and Bhedana karma is considered as primary treatment. Considering the recurrence and chronicity, kshara sutra ligation was planned after Bhedana karma. The case was completely cured with minimal follow-ups.     A Siddha Management of Chronic Tonsillitis - A Case Report

    Aim and objective: In the present scenario the medicine system from the west has started acknowledging the importance and wisdom of the ancient medicine system such as Siddha, Ayurvedha, Unani. There are many diseases that need surgical procedures till date, so in order to find a probable cure now a paradigm shift is really important. Tonsillitis is an infection of the tonsils associated with a sore throat, difficulty in swallowing, fever, etc. Still, surgical removal of tonsils is one of the most frequent procedures during childhood. In the Siddha system of medicine tonsillitis can be compared with Virana silatham. To evaluate the effect of Siddha medicine in the management of tonsillitis. Materials and Methods: The subject approached Out Patient Department of Siddha Clinical Research Unit(SCRU),under Central Council for Research in Siddha(CCRS), Safdarjung Hospital Campus, New Delhi with symptoms of sore throat, ear pain, difficulty in swallowing, sleep apnoea, which involved problems of breathing at night and foul smell from the mouth for past four months systemically. Discussion: The manifestation of symptoms and their severity depends on the extent of involvement of Uyir thathukkal (Vali,Azhal, Iyyam). Though the disease is Iyyam and Azhal predominant, there is derangement of Iyyam and Azhal Dosham in Virana silatham. Results: In this case, the subject showed a marked improvement as depicted in the photographs taken at different levels of treatment, also there was a significant improvement noted in both subjective and objective parameters. By adopting the Siddha treatment approach with an external treatment modality an attempt is made to bring about satisfactory results

    Formulation and development of Oral herbal liquid dosage form with its quality evaluation parameters

    The present work deals with the formulation and development of oral herbal liquid dosage and its comparative study with marketed dosage for quality evaluation parameters. We have used different Poly-herbs for the formulation of syrup because they expressed high effectiveness in a vast number of diseases. As aforementioned the therapeutic effect of herbal medicines is exerted due to the presence of different constituents and the effects are further potentiated when compatible herbals are formulated together in PHFs (Poly-herbal formulations). PHF show compatible effect with those of standard allopathic drugs. We had tested its Quality control pre and post-formulation evaluation parameters. The Quality control tests for developed herbal syrup include: Physical appearance (Color, Odor, and Taste), Clarity tests, solubility tests, pH, Density, Viscosity, Specific gravity, Refractive index, FTIR study of active constituent used in herbal syrup. This syrup is also tested for acute oral toxicity study (LD50), Antioxidant activity by using Nitric oxide free radical scavenging method, and hepato-protective activity by using paracetamol-induced hepatotoxicity in rats. From the obtained results, we concluded that formulated poly-herbal syrup passed all quality control tests and also proved its effectiveness. The formulation F2 and F3 were the best poly-herbal formulation and it was selected for further study. In hepato-protective activity, F2 formulation when compared with the standard formulation of silymarin have shown good results

    Antidiabetic potential of Guizotia Abyssinica cass seed oil in Wistar Albino rats - An experimental approach

    Guizotia abyssinica Cass. widely grown plant of southern India, seed oil of which are said to be healthy source of oil. Diabetes mellitus is a metabolic disorder affecting major population worldwide. Nutritive healthy source of oils and fats are essential in current scenario. Hence antidiabetic property of Guizotia abyssinica Cass seed oil is tested in Streptozotocin induced diabetes in Wistar albino Rats. Materials and methods: Test drug collected, authenticated, seed oil prepared. Study was conducted in Wistar albino Rats inducing diabetes with Intra peritoneal injection of Streptozotocin. The animals under different test groups received Glibenclamide as standard drug; single and Double dose of G. abyssinica Cass seed oil as test drug and the effect was observed by analysing serum glucose, lipid profile, weight of the Liver and histopathological changes in liver and pancreas after 21 days of experiment. Results: Test drug disclosed decrease in the Serum Sugar level. It has also shown decrease in total cholesterol, LDL and not showed the significant increase in HDL. Histopathological observation of Liver and pancreas proved safety aspect of the drug by preventing the cytotoxic effect of STZ.

    Study on the effects of sports induced injuries on Janu sandhi (knee joint) marma - A retrospective observational study

    In Ayurvedic samhita, it is explained that marma viddha lakshana are caused due to aghat (trauma). Sports injuries are concerned with traumatic injuries. Comparative study of marma sharira vigyan in shalyatantra of ayurveda with modern aspects of sports medicine can aware a sportsmen about the injuries. Janu sandhi (knee joint) is the mostly used joint in various sports leading to a wide range of injuries. The aim of this study was to evaluate sports injuries especially in Janusandhi Marma and structural damage occurring in it. The viddh lakshannas(traumatic injuries) of Janu Marma are limping  (Khanjata) and the objective of this study is to determine the specific injury at the knee joint that might cause limping. Total 30 patients were selected as per the inclusion and exclusion criteria mention in proforma. Observation & Results were noted with the help of Objective criteria mentioned in the proforma which included sports wise distribution, types of injury, mode of trauma, structural deformities clinical examination score and final diagnosis in study group. The result was Maximum patients had Medial Collateral Ligament involvement followed by Anterior Cruciate Ligament & Medial Meniscus injuries. Patellar injuries and Lateral Collateral Ligament injuries were comparatively less.  So it was concluded that with the assessment of clinical examination score it is possible to judge the severity of trauma to janu sandhi marma and the structures involved in the traumatic injuries amongst sports athletes. This study can be helpful for deciding the treatment regime of the patient.

    A Randomised Control Trial comparing the efficacy of Jati Ghrita and 1% Chlorhexidine Gluconate on Aphthous Stomatitis

    Background:  Aphthous ulcer is seen to be one of the most commonly occurring ailments found among patients that dentists may come across on a daily basis. Taking into consideration the agents that have been used for treatment of aphthous stomatitis, it would seem advisable to apply the agent that is the safest and has the least toxic properties that can bring about symptomatic relief and that can be applied topically. These days, there are few reliable natural remedies that are emerging as a therapy for aphthae. Herbal and polyherbal medicines will alleviate pain, induce healing, prevent infection, and lessen inflammation in aphthous ulcers without any adverse effects. Aim:  To evaluate the efficacy of Jati Ghrita and 1% chlorhexidine gluconate and its comparison on aphthous stomatitis patients.Materials and methods: A randomized control trial is carried out in the Department of Oral Medicine and Radiology, Sharad Pawar Dental College and Dattatray Ayurvedic Rasashala.The study population consisting of 30 patients with Aphthous stomatitis is divided randomly into two groups i.e.,Jati Ghrita and 1% Chlorhexidine gluconate, each group consisting of 15 patients.The intergroup comparison is done by independent sample t test. Results: Jati Ghrita is found to be effective in faster healing of aphthous stomatitis with reduction in pain. Jati is found to be more effective than 1% chlorhexidine gluconate. Conclusion: Jati has good medicinal properties and ghee accelerates wound healing so combination of this was found to be effective for treating aphthous ulcers. It is advised to use these natural treatments in place of conventional medicine method in the treatment of recurrent aphthous stomatitis due to the affordability, accessibility, and lack of potential side effects of these herbal treatments

    In vitro antioxidant and nephroprotective properties of Polyalthia longifolia: A traditional medicinal plant

    Polyalthia longifolia (Sonn.) Thwaites is a traditional medicinal plant with diverse medicinal properties including kidney protection. The present investigation was aimed to assess the phytochemical content, antioxidant activity and evaluate nephroprotective activity of Polyalthia longifolia (PL) 70% ethanolic leaf extract against cisplatin and gentamicin-induced toxicity on HEK293 cell line. Phytochemical analysis confirmed the presence of flavonoids, reducing sugars, tannins, coumarin, saponins, alkaloids, and glycosides, as well as the abundant total phenolic and total flavonoid content in the PL leaf extract. The extract exhibited significant free radical scavenging activity (p<0.001) in ABTS (IC50 224 μg/mL), DPPH (IC50 150.1 μg/mL) and H2O2 (IC50 271.1 μg/mL) assays. In vitro MTT assay validated remarkable cytoprotective activity (p<0.001) of PL leaf extract at three concentrations (125 μg/mL, 250 μg/mL, 500 μg/mL) against cisplatin and gentamicin treated HEK293 cell line in dose dependent manner with 70% cell viability (IC50 312.6 µg/mL) and 72% cell viability (IC50 307.3 µg/mL), respectively, at PL dose 500 μg/mL. The cytotoxicity assay also confirmed no adverse effect of PL leaf extract on cell viability in the HEK293 and considered as nontoxic when compared with normal control group. The present study revealed that Polyalthia longifolia leaf extract has remarkable antioxidant activity and nephroprotective properties against cisplatin and gentamicin induced nephrotoxicity in in vitro model which might be due the presence of free radical scavenging phytochemicals.

    Fuzzy Set Qualitative methodology for identifying the Critical Process Parameters and Quality attributes in the manufacturing of Arogyavardhini Rasa

    The traditional drug manufacturing process involves numerous qualitative attributes that directly impact product quality. The Process Analytical Technology approach considers that to enhance process control, identifying critical process parameters and critical quality attributes that affect the manufacturing process is very much necessary. The Ayurvedic drug manufacturing process is more driven by fuzzy qualitative attributes. The present study was executed to identify the Critical Process Parameters and Critical Quality Attributes in the manufacturing process of a herbo-mineral formulation, viz. Arogyavardhini Rasa. Fuzzy set Qualitative Comparative Analysis (fsQCA) methodology was adopted to observe and identify the critical parameters. The study was executed in three steps, viz. Data collection, Data arrangement, and Analysis. The raw data collected was arranged and analyzed in the software R studio using the package QCA in four steps, viz. Calibration, Analysis of Necessity and Sufficiency, Truth Table construction, and Minimization. The results obtained show that Size reduction, Mardana, and Drying are the identified Critical Process Parameters that, in combination, lead to the outcome, i.e., good product quality. Thus, this study proves that Fuzzy Set Qualitative Comparative Analysis can be used as an efficient tool for the identification and measurement of the Critical Process Parameters that affect the Critical Quality Attributes and, thereby, the product quality in the manufacturing of Arogyavardhini Rasa
