    A single blind randomized controlled clinical trial of Munditika churna adjuvant Chinna kwath in the management of vatarakta with special reference to Gout

    Kayachikitsa is First branch of Ashtang ayurveda described by Vagbhatacharya. Samhitas like Charak Samhita, Shushrut, Vaghbhat Samhita etc. Vatarakta is described as a disease and symptoms also.             Vatarakta is a Sammurchana Janit (combined) Vyadhi of Vata and Rakta. The aggravated Vata is being obstructed in its passage by Dushit Rakta. This Vata again vitiates the whole Rakta. This condition is known as Vatarakta. It can be corelated to the Gout disease in modern science due to similar sign and symptoms. Munditika Churna has been mentioned in Chakradutta and Bhaishajyaratnavali is effective in the management of Vatarakta and thus selected as a trial drug. Amrutadi Kwath Previously tested drug is selected for control group. Aim: To evaluate the clinical efficacy of Munditika Churna Adjuvant Chinna Kwath in Management of Vatarakta w.s.r. to Gout. Materials and Method: A total 70 patients of the age group 20-70 years presenting with signs and symptoms of Vatarakta w.s.r Gout were selected randomly from OPD of the department of Kayachikitsa. The 35 patients of trial group were treated with Munditika Churna Adjuvant Chinna Kwath and 35 patients of control group were subjected to Amrutadi Kwath. Results: Munditika Churna Adjuvant Chinna Kwath is effective on Vatarakta. Conclusion: Munditita Churna is an effective, safe and potent treatment of Vatarakta w.s.r. to Gout

    A pilot study to observe the progression of Vyadhi Samprapti in Srotas in the context of Hypothyroidism

    In chronic conditions, the progression of samprapti (pathogenesis) of Hypothyroidism in various Srotasas (Body Channels) can be observed due to the lack of Nidanparivarjanam (restraining of etiological factor) and Shuddha-Chikitsa (Comprehensive treatment). Assessment of the progression of the pathogenesis of Hypothyroidism in the Srotasas would help to decide the treatment plan for Hypothyroidism according to the stage of the disease, which prevents the further progression of the disease and drug dependency. Objectives: Observe the progression of the pathogenesis of Hypothyroidism in the Srotasas by assessing signs and symptoms of the Dhatupradoshaja Vikara (diseases due to vitiation of dhatu) along with their severity. Methods: In a cross-sectional study, 50 patients with chronicity of hypothyroidism for more than 1 year and on therapy were selected randomly and categorized into 5 groups, according to their chronicity. A separate case proforma was designed to focus on the assessment of the progression of the pathogenesis in patients of Hypothyroidism. The signs and symptoms described as Dhatupradoshaja Vikara, chronicity of hypothyroidism, findings of Thyroid Function Test (total serum thyroid-stimulating hormone, serum, total serum triiodothyronine, total serum thyroxine,) and drug dosage were recorded in each patient. The data was subjected to statistical tests.  Results: It is observed that the extent of signs and symptoms of Dhatupradoshaja Vikaras increased with the chronicity of Hypothyroidism in study participants. Progression of Samprapti of hypothyroidism in further Srotasa was observed in patients with chronicity of Hypothyroidism, especially in individuals with Avar Agni (weak state of digestive and metabolic factors). It is observed that in the study participant drug dosages were escalated with chronicity and Avara Agnibala (weak state of Agni).  Rasadhatu and Rasavaha Srotas along with Medodhatu and Medovaha Srotas are priorly and predominantly get vitiated in the Samprapti while other Dhatus and Dhatuvaha Srotasa are involved in Samprapti over the period. Conclusion: It can be concluded that the vyadhi samprapti of Hypothyroidism progresses in further Srotasa with the chronicity of hypothyroidism.

    Development of a machine learning-based Parkinson\u27s disease prediction system through Ayurvedic dosha analysis

    The development of a machine learning (ML) based prediction model for Parkinson\u27s disease (PD) using Ayurvedic literature is very rare in practice. In modern era of digitization and artificial intelligence this lack leads us for developing a significant and useful predictive model for PD in light of Ayurveda. As in contemporary Ayurvedic literature, PD has not been fully classified according to all of its motor and non-motor symptoms, with the exception of Kampavata or tremor, one of the cardinal motor symptoms of PD, we have taken the help of MDS-UPDRS-II and MDS-NMSQ scaling system as an initial input for this purpose. Based on the available literature, we determined our tridosha score using these scales, which become our main inputs to the ML algorithm, along with other general health attributes such as age, sex, and BMI. We applied various ML algorithms and ranked our best ML model based on their performance. For training and testing purposes, we used the Fox Insight dataset with n = 80,916 records including PD and control. Finally, we found that Kernel-SVM, SVM, Logistics Regression (LR), and XGBoost are our four most accurate algorithms, with an accuracy more than 92.5% with no dimensionality reduction applied. Here we chose the LR model as our best ML model, depending on the lower false positive rate of 0.045 with an accuracy of 92.6%. The designed LR model is statistically significant with χ2(6) = 70703.137, p =0.000. The LR coefficient was also calculated for probability analysis and future implementation of digital Ayurveda-based PD prediction applications

    A Critical Review on the Scope of Management of Academic Stress in Adolescents with Sattvavajaya Chikitsa (Ayurved Psychotherapy)

    The adolescent stage of life is unique having various physical and psychological changes occurring in the body and in the brain. Academic stress (AS) is largely responsible for adolescents developing mental illnesses. A competitive environment, the pursuit of high scores in marks, peer comparisons, and parental expectations are the most common stressors for an adolescent to develop AS. Early identification of the stressed person, his sensitivity toward stress, and appropriate stress coping can help a person to develop the strength for coping with various stressful situations. Reports and articles published online from the year 1995 to 2022 were taken into account among them 95 articles were identified of which 25 articles had abstracts only were excluded and 70 articles including textbook references were reviewed and discussed. Ayurveda has the unique concept of Prakriti (~somatic constitution) helps a person to recognize himself and to realize his strengths and weaknesses, through which one can ignore or manage stressful situations. This stress coping can be more emphasized with the practice of Sattvavajaya Chikitsa(~Ayurvedic psychotherapy). Sattvavajaya Chikitsa works based on Manonigraha (control of the mind). Sadvrittta (~code of conduct for mental health), Swasthavritta (~measure to uplift general health), Achar Rasayana (~Ayurved code of conduct), Yoga, Surya Namaskar (~sun salutation), and meditation are different ways to practice mindfulness. Proper practice of Manonigraha (~control of mind) creates a connection between the mind, psyche, and body. The long-term practice of Manonighraha helps an academically stressed individual to better cope with stress and has finer mental and physical health.

    Ayurvedic management of pseudophakic cystoid macular edema: A case report

    After cataract surgery, pseudophakic Cystoid macular edema is one of the main challenges to deal with for an ophthalmologist. It is a painless condition in which swelling or thickening occurs in the central retina (macula). Surgical trauma triggers the cascade of inflammatory reactions, leading to the breakdown of the blood-retinal barriers that lead to increased retinal capillary permeability which further causes edema. A 65-year-old female patient visited Shalakya tantra OPD with complaints of diminished vision in the right eye for two months. After a fundoscopic examination, she was diagnosed with right eye pseudophakic cystoid macular edema. The diagnosis was confirmed with OCT findings of the right eye. Intravitreal injections are costly and do not guarantee the reduction in pseudophakic CME. The patient was given Shamanaushadhis like punarnavasava vrihatavasakadi kwath, gokshuradi guggulu, chandraprabha vati and eranda taila as rechana dravya along with jalaukavacharan to reduce the edema caused by the fluid accumulation in the macular region. Significant improvement was seen in the patient’s visual acuity along with the reduction in macular edema.

    Phytochemical and Physicochemical analysis of Siddha polyherbal formulation KaranthaiChooranam

    Aim: Standardization of a Siddha Poly herbal compound formulation is essential for establishing its authentication quantity and efficacy. In this present study, an attempt has been made to standardize the KaranthaiChooranam (a polyherbal formulation that contains 14 plant materials). Hence standardization of traditional drugs becomes highly essential to explore their potency and efficacy in the global market. It has been increasing public interest and acceptance of traditional medicine. Methods: Physico-Chemical analysis, Phyto-Chemical Screening, Particle Size, Pesticide Residue, and Sterility test were performed as per PLIM (Pharmacopeia Laboratory of Indian Medicine) guidelines. Results: The loss of drying of the tested drug was 105°c .KC contains 0.59% acid-insoluble ash and solubility in the water was 9.63 %. Phytochemical screening KC has shown the presence of Alkaloids, Glycosides, Saponins, Phytosterols, Fixed Oil, Resins, Phenols, Flavonoids and Tannins. Microscopic observation of the particle size analysis reveals that the average particle size of the sample was found to be 115.5 ± 42.83 μm. Conculsion: From this set of parameters, it can be concluded that it is sufficient to evaluate the authenticity of KaranthaiChooranam can be used as a reference standard for the preparation of the standardized pharmaceutical product and further quality content research. This study suggests that quality specifications of KC can be developed using parameters described in Siddha along with analytical tools available today

    Aqueous extract of Daucus carota exerts a protective effect on the Renal, Hepatic and Duodenal mucosal histology in Diclofenac Induced Tissue Injury

    Objective: The use of ethnobiology in treatment of many diseases especially in rural residents with limited access to medical technology, treatment and equipment is beneficial and necessary. Daucus carota (DC) root extract was used as pre-treatment in adult Wistar rats exposed to diclofenac sodium (DF) to investigate a protective effect on the histology of liver, kidney and duodenum. Materials and Method: Twenty-five adult Wistar rats were used for this study and were randomly divided into 5 groups of 5 rats each, which included: Group A - Normal control; Group B – 50mg/kg DF control; Group C – 50mg/kg DC + 50mg/kg DF, Group D – 100mg/kg DC + 50mg/kg DF; and, Group E – 140mg/kg DC + 50mg/kg DC. Results and Discussion: The results showed a reduction in damage to the hepatocytes, maintenance of sinusoidal integrity and reduction in the number of inflammatory cells in the hepatic parenchyma, In the kidney tissue, the extract preserved the glomerular capillary tuft, renal tubular epithelial cells, conserved Bowman’s space and lining epithelium of the capsule in a dose dependent manner. The intestinal mucosa in groups treated with higher doses of the extract were completely preserved and intact with minimal erosion of  epithelial lining along with preservation of lamina propria and intestinal glands. Conclusion: Pre-administration of DC, preferably at concentrations of 100mg/kg and 140mg/kg reduced hepatotoxicity, renal tissue and mucosal layer damage in the duodenum following administration of DF. This preservation of tissues improved as concentration of the extract increased verifying that its efficiency was dose dependent.

    A case report study on long term follow up treatment of T-AYU-HM Premium in Sickle cell anaemia Patient

    Sickle cell anaemia is a haemoglobin condition for which the Indian government has set up particular support provisions within the ministry of tribal affairs. Despite epidemiological and recent advances in the treatment of sickle cell anaemia over the past few years, it remains a global concern for everyone. To demonstrate the impact of T-AYU-HM Premium, the long-term treatment outcomes of a patient with sickle cell trait are detailed in this case study. A 35-year-old female patient with a family history of sickle cell anaemia was unable to finish her studies because she had blood transfusions every other month and experienced excruciating pain. Her reported painful crises, treatment compliance, the number of blood transfusions she received, and the number of hospitalizations she needed were all reviewed in this case study. Her past medication and medical history were assessed in 2008 following which the T-AYU-HM Premium treatment was started once she approached the clinic with these complaints. It was noted that the patient followed the prescribed line of treatment, had symptomatic relief from episodic pain, and was able to perform day-to-day tasks. It was noted that the patient only required one blood transfusion and one hospital stay throughout the course of the 15 years of follow-up. This case study would further the understanding and practical application of complementary medicine in the treatment of sickle cell anaemia. The potential for long-term treatment using alternative medical systems may well be established.

    The Incidence of Stroke with Different BMI Ranges in Diabetic Patients

    Introduction: Identifying the risk factors of stroke as an initial prevention is the best step to prevent it from happening. One of the risk factors for stroke in diabetic patients seems to be the presence of high BMIs in these patients, so given the importance of this issue and that little studies have been done so far, this 7 -year study on files has been done. Diabetic patients in Behbahan were performed with the aim of determining the incidence of stroke with different BMI areas in diabetic patients. Materials and Methods: In this study, which examines 1714 patients with diabetes and a detailed cross -sectional descriptive study. The case of these patients, which was diagnosed with diabetes and history of the disease and referred to Shahidzadeh Hospital in Behbahan from 2014 to 2020, was extracted in the medical evidence of the hospital. In this study, the demographic and clinical information of patients. The data then entered the SPSS 17 software and used the descriptive statistics tests of the metaphor, mean, standard deviation and variance and analytical tests and analytical tests, Anova, Pearson and a significant level p <0/ 05 were analyzed. Results: This study consisted of 1714 people with diabetes with an average age of 18.62 ± 58.56 years. Of these, 984 (57.4 %) were male and the rest were females. In the present study, 314 people (18.3 %) had a history of stroke. Of which 26 (8.2 %) have BMI between 25.5, 51 (16.2 %) with BMI more than 25 and less than 30, 71 (22.6 %) with BMI in 30-35, 77 (24.5 %) It had BMI between 35 and 40, and 89 (28.3 %) had a BMI of more than 40. In this study, a statistical relationship was found between BMI levels and a history of stroke (P = 0.008), with patients with higher BMIs more likely to have a stroke. Conclusion: In this study, a statistical relationship was found between BMI levels and a history of stroke (P = 0.008), as patients with higher BMI had a higher incidence of stroke. Therefore, given the relationship between BMI and stroke, it is necessary to give diabetic and high BMI patients at risk of stroke, training necessary weight loss and healthy diet with proper diet.     Herbs used in cardiovascular diseases

In this review, we sum up the ongoing information on homegrown medicines, essentially utilized in cardiovascular treatment. We show how there is frequently no scientific evidence to support the use of these medications to treat cardiovascular disorders. Despite the fact that they seem to affect organic cycles connected with the cardiovascular framework, there is little data on the restorative impacts of the majority of these herbs. Concerns are raised by the potential for medication contamination or drug substitution with other prescriptions, as well as by related negative effects like a higher risk of drug interactions. Doctors should review any potential benefits and drawbacks of their patients\u27 use of herbal remedies in every situation.
