Role of Panchakarma in the Management of Spinal Canal Stenosis - A Case Study
Spinal canal stenosis is a painful, degenerative condition, and is localized to facets, joints, and ligamentum flavum of the vertebra which is often referred to for surgical correction. The main cause of stenosis is herniated vertebral disc. If there is no compression, the spinal canal can be referred to as narrow, but not stenotic. Certain studies have defined lumbar spinal stenosis as the narrowing of the osteo-ligamentous vertebral canal or the intervertebral foramina, which causes compression of the thecal sac or the caudal nerve roots. The narrowing may impact the entire canal or just a portion of it, but it occurs at a single vertebral level. In this condition, it may, drastically affect the quality of life due to severe pain in doing daily routine activities. In contemporary science, there is no treatment except surgical intervention which is having a high chance of complication and the chances of recurrence. Surgery is necessary when there are clinically significant motor deficiencies or symptoms of cauda equina syndrome. Based on the manifestation of clinical signs and symptoms it can be correlated with Gridhrasi, according to Ayurveda. Gridhrasi can be treated successfully by the intervention of Panchakarma procedures along with Shamana Chikitsa. Here is a case study of a patient suffering from low back pain radiating to bilateral lower limbs, stiffness, numbness, burning sensation in the bilateral sole region, and unable to walk without support, who was previously diagnosed as a case of Spinal Canal Stenosis. He was treated with a Panchakarma procedure, Shamana Chikitsa along with Physiotherapy Exercises. The patient showed marked improvement and could do daily routine activities properly
Standardisation of Kuvalayaghrta: A Potential Nootropic Supplement
Nootropics are groups of medicines which are known to act as memory enhancers. There is a growing need for such medicines from alternate systems of medicine to prevent or delay memory loss and related disorders. Classical textbooks in Ayurveda describe a group called as Medhya Rasayana, which act as neuro-nutrients. Chathushkuvalaya Rasayana is one such ghee preparation mentioned in Ashtangahrdaya. In it, Kuvalaya, botanically identified in Kerala as Monochoria vaginalis Presl. is the only herbal ingredient. Though Kuvalaya has been considered widely as a memory enhancer, there is scarce published literature regarding standardisation aspects of Kuvalaya ghrta. In this work, we have phytochemically standardized the ingredients of Kuvalayaghrta including its Atomic Absorption spectroscopy, Thin Layer Chromatography (TLC) and High-Performance Thin Layer Chromatography (HPTLC) profiling followed by the preparation of the ghrta(ghee) according to the standard procedure mentioned in Ayurvedic Formulary of India. Kuvalayaghrta was also standardised physicochemically and was found to be safe for internal administration. All the observations obtained in this study can be used for future reference
Comparative clinical study of Yashtimadhu ghrita tarpana and Sodium hyaluronate (0.1%) eye drop in the management of Shushkaakshipaka w.s.r to dry eyes
Shalakyatantra is one of the branch from the eight part which is concern with the management of disease affecting the regions of the body above the clavicles i.e. Ear, eyes the oral and the Nasal cavities etc. Shushkaakshipaka is vata pitta pradhan sarvagatavyadhi. It can be co-related with dry eye syndrome in modern aspect. Modern science has lacuna in complete curative management of disease. Ancient Ayurved texts e.g., Brihatrayees and Laghutrayees explained ample of therapies regarding management of Shushkaakshipaka. Dry eye syndrome is group of symptoms which includes ocular irritation, redness, tenderness, and dryness. yashtimadhu ghrita is vatapittaghna, chakshushya and having properties of snigdha guna so use of yashtimadhu ghrita tarpana will be beneficial in the management of shushkaakshipaak.
Aim: Comparative clinical study of Yashtimadhu Ghrita Tarpan and Sodium Hyaluronate (0.1%) Eye Drop in the management of Shushkakshipaka with special reference to Dry Eye Syndrome. Materials and Method: A total 70 patients of the age group 18-80 years presenting with signs and symptoms of Shushkaakshipaak w.s.r to Dry eye syndrome were selected randomly from OPD of the department of Shalakyatantra. The 35 patients of trial group were treated with Yashtimadhu Ghrita Tarpan and 35 patients of control group were subjected to Sodium Hyaluronate (0.1%) Eye Drop. Results: Yashtimadhu Ghrita Tarpan is more effective as compared to Sodium Hyaluronate (0.1%) Eye Drop. Trial drug provided more relief in most of the symptoms of shushkakshipaka. Conclusion: Yashtimadhu Ghrita Tarpan is an effective, safe and potent treatment of shushkakshipaka w.s.r. to Dry Eye Syndrome.

Training and detraining management of performance measures by Yoga among middle-aged blue-collar men
To observe the effect of yogasanas and yoga breathing maneuvers (YBM) on body flexibility, balance, muscular strength and reaction time on middle aged blue-collar individuals, 8 healthy blue collar, males (age 37±6.2 years) were trained in yoga for 12 weeks. First 6 weeks, they practiced mostly yogasanas. From 7th week, progressively the YBM practice increased up to 12th week. Their body flexibility, balance, maximal hand grip strength (HGS) and HGS endurance at 50 % maximal strength for both hands, back -leg strength (BLS), reaction times for both visual (VRT) and auditory (ART) mode, were measured before and at the end of 6th and 12th weeks yoga training. Data was analysed by ANOVA. With more yogasanas practice, their body flexibility improved from base line 14.1±4.5 cm to 18.3±4.3 cm (P<0.001, magnitude 29.8%) after 6 weeks of yoga training. With proportionately greater YBM practice, still it increased in lower magnitude (9.3%) to 20±4.6 cm (p<0.001) after 12 weeks of training. Balance showed similar pattern of changes like flexibility. In VRT, baseline value of 281.4±16.4ms reduced to 271.14±16.1 ms and 251.7±15.4 ms after 6 weeks and 12 weeks respectively, indicating greater reaction time improvement by YBM. ART also showed similar pattern of changes. Left and Right HGS endurance have improved by 19% and 23.1% respectively, after 6 weeks. BLS and HGS improved in small magnitude. Detraining and age related deterioration of body flexibility, balance, reaction time and strength endurance could be controlled by yogasanas. YBM have greater positive role in reaction time
Efficacy of Sarjadi Lepa Gutika and Terbinafine Ointment in Dadru (Tinea Corporis)
The term \u27Kushtha\u27 can be referred to various skin disorders. \u27Dadru\u27 is a type of Kushtha. In Dadru there is pradhanata of Kaphaa nd Pitta Dosha. It exhibits clinical features of Kandu, Raga, Pidika ,Utsanna Mandala. On basis of clinical features Dadru is compared with Tinea by many scholars. Tinea is superficial fungal infection in which the fungi colonises dead keratinized epidermal tissues of skin, hair and nails and produces annular lesions over skin surface. Aim- Efficacy of Sarjadi Lepa Gutika and Terbinafine Ointment along with Tiladi Churna internally in Dadru (Tinea Corporis). Material and Methods – Study contains 60 patients of Dadru which were divided into two groups (3o in each group). In Group A (Intervention) Sarjadi Lepa Gutika for local application twice daily and Tiladi Churna 6gm at morning after meal for 30 days was given. In Group B (Experimental group) Terbinafine Ointment for local application twice daily and Tiladi Churna 6gm at morning after meal 30 days was given. Assessment was recorded on every 15th day (15th day, 30th day and 45th day). Result – Both the groups were equally effective in reducing Kandu, Raaga and Manadala utpatti. Conclusion – : Sarjadi Lepa Gutika is as effective as Terbinafine ointment in the management of Dadru (Tinea corporis) and it may prevent recurrence if combined with Tiladi Churna as Abhyantar Chikitsa
The Study of add on effect of Madhusudan Vati and Arogyavardhini Vati in Prameha with special reference to Type 2 Diabetes Mellitus
Prameha (Diabetes mellitus) is an age long disease known from vedic period and now it is a leading lifestyle disorder. Diabetes mellitus is a group of metabolic syndromes of fat, protein and carbohydrate which is due to absolute or relative deficiency of insulin, characterized by hyperglycemia over a prolonged period with polyuria, polydipsia, and polyphagia like symptoms. The estimates in 2019 showed that 77 million individuals had diabetes in India, which is expected to rise to over 134 million by 2045. It has turned out to be the biggest “silent killer” in today’s world. This is an Open label Comparative clinical study with pre-test and post-test design where in 60 patients suffering from Prameha (type 2 DM) of either gender between the age group of 25 to 75 years were randomly selected and grouped into Group A and Group B. The 30 patients in Group A were treated with Arogyavardhini vati & Madhusudhan vati with Metformin (500 mg) and Glimepiride (1mg) BD. Group B of 30 patients were given Metformin (500mg) and Glimepiride (1 mg) for 120 days. Results obtained in subjective and objective parameters were analyzed for the statistical significance by adapting paired T test, unpaired T test, Mann-Whitney Z and Wilcoxon Signed Ranks Test Z. The study revealed that Group A was found to be more effective in bringing symptomatic relief and improving biochemical markers in the patients of Prameha.
Management of Amlapitta (Hyperacidity) with Patoladi kwath: A Case Study
Amlapitta (Hyperacidity) is one of the gastrointestinal disorders. It is closely related with food habits, behavioral pattern & life style. It is not described in basic compendium of Ayurveda like Charak samhita, Sushruta samhita & Ashtang hridaya. Its detail description is available in Madhavnidan, Kashyap samhita, Yogratnakar etc. Consumption of etiological factors leads to provocation of tridoshas & causes Agnimandya. Vitiated Agni unable to digest the food properly & leads to fermentation of food. In Amlapitta (Hyperacidity) pachak pitta is increases in its quantity & its normal bitter taste (katu) gets converted to sour (amla) taste due to fermentation of food. Tikta-Katu rasatmak drugs are mainly used for treatment as they have Aampachan & Agnideepan properties. Patoladi Kwath was used for shaman chikitsa in present case. A 42 years male patient having chief complaints of Udar-uro daha (burning sensation in abdomen & chest region), Amlodgar (Sour belching), Aruchi (Anorexia), Hrillas (feeling of Nausea), Chardi (episodes of vomiting occasionally), Udarshula (Abdonimal pain), Klama (Fatigue) & Bhrama (Vertigo) was treated with Patoladi Kwath for 15 days. Follow-up of patient was taken after each 7 days. This Kwath (Decoction) is an excellent combination of pittashamak dravyas, performs functions like Agnideepana, pachana & helps to improve digestion. After 15 days treatment, symptoms like Udar-uro daha, Amlodgar, Hrillas, Chardi, Udarshula & Bhrama got complete relief while in symptoms like Aruchi & klama relief was 80% & 75% respectively. Overall percentage of relief was found to be 90%.
Association between early menopause and incidence of Osteoarthritis in patients of Wardha district - An observational study
Background: Osteoarthritis (OA) is a progressive musculoskeletal disease involving the degeneration of cartilage and bone, and is one of the leading causes of pain and disability worldwide. It is a multi-factorial, slowly progressing, and primarily non-inflammatory degenerative disorder of the synovial joints that is often age related and/or trauma induced. Women are more prone than men. It rises in prevalence, incidence, and severity with polyarticular involvement following menopause. Aim and Objectives: To evaluate the association between early menopause and incidence of Osteoarthritis in patients of Wardha District. Methodology: A cross sectional study was conducted among 60 post-menopausal female subjects having normal menopause and early menopause between the age group of 50 to 75 years of Wardha District. Group A comprised normal menopause subjects and Group B comprised early menopausal subjects. Then incidence of Osteoarthritis in both groups was assessed on the basis of diagnostic criteria by filling predesigned proforma and comparison was done by statistical analysis. Observation and Discussion: Patients of group B showed diagnostic features of Osteoarthritis like crepitus, restricted range of movement , bony enlargement and morning stiffness <30 minutes in more number of patient as compared to group A. This clearly indicates that incidence of Osteoarthritis was more in patients having early menopause as compared to normal menopause. Conclusion: This study concluded that there is strong association between early menopause and occurrence of Osteoarthritis.
A Physico-chemical assay of a herbal formulation - Smrutisudha granules
Ayurveda is an ancient medical practise that focuses on the prevention of illness and the treatment of conditions that are related to the human body. Ayurveda provides information on a wide variety of herbal remedies that can be used to cure a variety of disorders. In such a scenario, standardisation is necessary for ensuring uniformity from one batch of drug production to the next and for carrying out routine drug manufacturing on a large scale. In this age of shifting lifestyles and increased public interest, the process of modifying older dosage forms and the development of new dosage forms is an ever-evolving process that makes a significant contribution to the expansion and development of scientific knowledge. Granules are produced from a refined form of Ghana, which is a solid preparation of herbal extract, and Khanda Kalpana, which are solid preparations that are comparable to granules. Because it contains medications that are medhya, Smrutisudha granules are developed for their beneficial benefits, such as boosting children\u27s memories. In the current work, standardisation, physio-chemical properties, qualitative analysis, and chromatography (HPTLC) of Smrutisudha granules have been produced. Prior to its application in therapeutic settings, the standardisation of Smrutisudha granules will be aided by this research.
Analytical study and preparation of praval garbha pottali
The Pottali rasayana Kalpana is one among the four types of rejuvenation formulations (Chaturvidh rasayana Kalpana) of mercury (Parad). They are known for its specific method of preparation, unique end product, optimum potency, smaller dosage and larger therapeutic applicability. Praval garbha pottali is a herbo-mineral complex formulation. Gandhak paka method was selected for preparation as it is the most commonly used procedure. It also is a best method to enhance the efficiency of the drugs and keep them in a concise form. The description of Pottali rasayana Kalpana is found in numerous Rasagranthas (Classical books of Rasa shastra); however, very less research work is carried out on them, and minimal research is carried out on Praval garbha pottali. Pottali rasayana Kalpana are not well-versed in current ayurvedic practice due to the lack of research study. XRD, FTIR, and EDS mapping is carried out on the prepared pravalgarbha pottali
