Get PDFData of 130 patients with thunderclap headache are presented. The data include clinical manifestation analysis and neuroimaging results (magnetic resonance angiography). Magnetic resonance angiography was performed 15 days after acute clinical manifestation and permitted to verify cerebral vasoconstriction, that led to treatment modification with clinical and neuroimaging signs of vasoconstriction reverse.Представлены результаты обследования 130 пациентов с громоподобной головной болью, анализ клинических данных, результатов метода нейровизуализации — магнитно-резонансной ангиографии (МРАГ). Анализ результатов МРАГ спустя 15 дней после клинического проявления позволяет верифицировать наличие церебральной вазоконстрикции, что обеспечивает выбор терапии, положительные клинические и нейровизуализационные признаки регресса вазоспазма

No full textThis article provides information about modern representation of non-alcoholic fatty liver disease, as well as the structure, role and function of vascular endothelium in the pathogenesis of steatosis. The article presents the results of remaxol in clinical practice in patients with nonalcoholic fatty liver disease. Investigated hepatic and endothelial protective effects of remaxol 5–10 day treatment and 30 days after of therapy. Early clinical effect was expressed in the normalization of endothelial function, which is the first marker of positive action endothelial protective effects of remaxol, compared with laboratory and sonographic evidence. В статье приведены сведения о современном представлении о неалкогольной жировой болезни печени, а также о структуре, роли и функции сосудистого эндотелия в патогенезе гепатостеатоза. В статье представлены результаты применения ремаксола в клинической практике у пациентов с неалкогольной жировой болезнью печени. Исследовано гепатопротективное и эндотелиопротективное действие ремаксола на 5–10-й день терапии и 30-й день после окончания терапии. Ранний клинический эффект выразился в нормализации функции эндотелия, который является первым маркером положительного эндотелиопротективного действия ремаксола, по сравнению лабораторными и сонографическими данными.

No full textАbout 80% of all ostearthritis localised in knee joints affecting non-bone structures. Ultrasound examination visualisating such abnormalities. There are no neither unigorm standart nor protocol for ultrasound examination on joints. The aim was analysung difference between interpritation of pathological chances in X-ray 0 stage of gonarthritis and revealing the most robust ultrasound criteria. Material and methods. Ultrasound examination was performed for 19 patients with X-ray 0 stage of gonarthritis by three doctors independtly with interval between examinations three days. The results. The most obgective criteria were changes in soft tissues (84,2%), cartilage echoic changes of patellar block 57,9%), patella ligament (84,2%), synovitis (89,5%), fluid os syprapatellar bursa (89,5%), changes of medial meniscus’ configuration (84,2%), and hyperehoic foci of lateral meniscus. Заключение: ultrasound examination the most informative for synovitis, changes of medial meniscus’ configuration, cartilage echoic changes of patellar block. До 80% всех случаев остеоартрита приходится на коленные суставы, при этом сначала поражаются некостные структуры, ультразвуковое исследование (УЗИ) позволяет визуализировать изменения. В России нет единого стандарта и протокола описания УЗИ сустава. Цель: анализ различий трактовки изменений при рентгенологической 0 стадии гонартрита и выявление наиболее надежных УЗ-признаков. Ультразвуковое исследование проводили 19 пациентам с рентгенологически нулевой стадией независимо друг от друга три врача УЗИ, интервал между исследованиями до трех дней. Наиболее объективными критериями были изменения мягких тканей (84,2%), изменение эхогенности хряща пателлярного блока (57,9%), собственной связки надколенника (84,2%), синовит тибиофеморального сочленения (89,5%), скопление жидкости супрапателлярной сумки (89,5%), изменения конфигурации менисков (84,2%) и гиперэхогенные включения латерального мениска (89,5%). Заключение: на 0 стадии гонартрита УЗИ информативно при синовите, изменениях конфигурации медиального мениска, эхогенности хряща пателлярного блока.

Get PDFExternal beam radiation therapy (EBRT) is currently an important component of the treatment of gastric cancer (GC). In accordance with international and domestic recommendations, it can be used in the postoperative period, before the operation and as a sole option, with a curative and palliative purpose, usually in combination with chemotherapy. With the transition of RT to a new technological level, which involves the use of conformal, intensively modulated regimens, and image guidance the revision of indications for and doses of EBRT is required. The article discusses modern approaches to the determination of irradiated volumes in various clinical scenarios of current radiation therapy for GC and strategies aimed at reducing the dose in critical organs at risk and increasing the overall effectiveness of treatment.Дистанционная лучевая терапия (ЛТ) в настоящее время является важным компонентом лечения рака желудка (РЖ). В соответствии с международными и отечественными рекомендациями она может быть использована в послеоперационном периоде, перед операцией и самостоятельно, с лечебной и паллиативной целью, как правило, в сочетании с химиотерапией. В связи с переходом ЛТ на новый технологический уровень, предусматривающий использование конформных, интенсивно-модулированных режимов, требуется пересмотр показаний, доз и режимов дистанционного облучения. В статье рассмотрены современные подходы к определению облучаемых объемов при различных клинических сценариях современной ЛТ по поводу РЖ и обсуждены стратегии, направленные на снижение дозы в критических органах и повышение эффективности лечения

No full textThe results of autopsy material of preterm newborns very low birth weight (VLBW) and extremely low birth weight (ELBW) brains are presented. In life the brains lesions were detected in the examined newborns with cranial ultrasound and MRI (intraventricular herminal hemorrhages/IVH and hypoxic — ischemic encephalopathy /HIE). In this researching were been using method of microscopy. Postmortem examination confirmed brain cell migration continues after birth in all examined newborns. This study have detailed the directions, positions, thickness cells migration columns. There were found that cells migration thickness and length depend on gestation age of preterm newborn with IVH and HIE. Besides we found the age morphologic differences in cell structures cerebral cortex in preterm newborns.
