A CASE OF LAPAROSCOPIC HEPATECTOMY FOR PRIMARY HEPATIC NEUROENDOCRINE TUMOR DIAGNOSED PREOPERATIVELY BY SOMATOSTATIN RECEPTOR SCINTIGRAPHY
Get PDF症例は73歳男性で，背部痛を主訴に来院し，精査目的に施行した腹部造影CTで偶発的にリング状濃染を伴う肝腫瘤を認めた．腹部超音波検査では肝S5に境界明瞭辺縁整，内部不均一の27㎜の腫瘤を認め，生検を行ったところNET G1と診断した．その他の検査では他臓器に原発腫瘍を指摘し得なかった．ソマトスタチン受容体シンチグラフィで肝S5に異常集積を認めたが，その他の肝および他臓器に集積は認めなかった．以上より，肝原発NETと診断し根治切除を行った．手術は腹腔鏡下肝 S5部分切除術を施行し，術後 5 日目に退院となった．病理診断では腫瘤径は20㎜で，synaptophysin，D56がそれぞれ陽性であった．核分裂像は 2 個/10 HPF，Ki-67 indexは1.3%であり，術前と同様にNET G1の診断であった．肝原発NETはNET全体のうち0.3%と稀な疾患である．今回我々はソマトスタチン受容体シンチグラフィが有用であった肝原発NETの 1 例を経験したので，文献的考察加えて報告する．departmental bulletin pape
