A Retrospective Observational Study on the Use of Point-of-Care Ultrasound (POCUS) in a Gastroenterology and Nephrology Ward
Point-of-care ultrasound (POCUS) is gaining attention for its utility in various clinical settings. This retrospective observational study examined the use of a handheld ultrasound device in a university hospital ward and considered factors influencing its adoption among healthcare staff. The device, Pocket Echo miruco (Nippon Sigmax Co., Ltd.; 3.5 MHz convex probe), was installed in the gastroenterology and nephrology ward at Tottori University Hospital. Usage data were collected from a self-recorded log between May 2021 and May 2022. A total of 132 uses were recorded, all by physicians (98% gastroenterologists, 2% nephrologists). The most frequent use was for paracentesis guidance (60%), followed by post-procedural bleeding checks, ascites evaluation, and inferior vena cava assessment. The median usage time was 30 minutes (range: 5–70). Usage increased following an internal awareness initiative, suggesting that simple promotional activities can encourage behavioral change and facilitate the spread of handheld POCUS. No usage by nurses was observed, indicating the need for improved training opportunities and clarification of roles. These findings highlight the importance of both equipment availability and effective communication strategies to support broader, multidisciplinary use of handheld POCUS in inpatient care
Effective Facilitation to Support Reflection for Novice Nurses
This study aims to identify the characteristics of effective facilitation that foster new self-awareness through reflection support. Semi-structured interviews were conducted with four nurses in their second or third year of service who participated in a reflection training program, and five facilitators at Tottori University Hospital. The young nurses often began reflecting when they faced challenges in their daily practice. They highlighted the importance of a supportive environment and interpersonal relationships. Reflection enabled them to calmly review events and approach future practice more positively. Facilitators reflected not only when approached by young nurses but also during role-specific moments, such as monitoring novice nurses’ well-being. They encouraged insight through intentional questioning to deepen reflection. Furthermore, when young nurses gained insights during or after the reflection process, the facilitators themselves also experienced a sense of accomplishment. Effective facilitation thus supports both personal and professional development in new nurses. Moreover, it is believed that such reflection practices extend beyond individual growth, positively influencing the entire staff and enhancing the overall quality of nursing within the organization
