    Occupational stress among probation officers in central province of Kenya

    RC 963.48.M8 2014Probation is a people business, oftentimes requiring intense, stressful confrontations with unmanageable offenders. Presently, there are more felons receiving probation than ever before. The impact of current probation supervision conditions on probation officers has not been fully documented. Since human capital is the greatest investment an organization can make, the ongoing assessment and management of employee stress effects becomes critical to maximizing the effectiveness of probation. Considering the fact that probation officers have contact with more offenders than most other justice practitioners, and probation caseloads have continued to increase at unprecedented levels, there is a need to understand more about the work of probation. The proposed study sought to investigate occupational stress among probation officers in Central Province of Kenya. It aimed at determining the prevalence, the causes and effects of occupational stress among probation officers, and the role of counseling in addressing occupational stress among the officers. The Rational Emotive Behavioral and Person-Environment Fit Theories formed the theoretical framework of understanding the concept of occupational stress. The study was conducted in Central province of Kenya. The population of the study comprised of 98 probation officers serving in Central Province at the time of the study. The study employed convenient sampling of the existing five counties in Central Province to arrive Nyandarua and Nyeri Counties. All the officers in these two counties totaling to 24 were the respondents of this study. Structured questionnaires were used for data collection. Descriptive and inferential statistics were used, and methods of data analysis included percentage score and mean. Data presentation was done by the use of tables. The findings revealed that probation officers experienced normal range of occupational stress with mild levels of stress on Role Ambiguity and Responsibility with combined T-scores of 57.92 and 51.04 respectively. Low remuneration, lack of sufficient resources to carry out job responsibilities and lack of opportunities for career development were found to be significant causes of occupational stress among probation officers. These causes of occupational stress were organizational in nature and not job related. The effects of occupational stress on the officers that were found to be statistically significant (p<0.05) were burnout, demotivation, anxiety/depression, conflict with colleagues, and low productivity. Counseling was cited by the respondents as being functional and pivotal in addressing incidences of occupational stress among them through referrals for counseling, psych education, supervision and debriefing. The study findings are useful to the probation department in Kenya in developing policies and effective Employee Assistance Programs (EAP) and other strategies for prevention and management of stress among probation officers. Furthermore, this study on occupational stress among probation officers in Kenya enlightens the public more on this crucial yet less recognized element of the criminal justice system

    Il s'agissait d'une étude prospective, randomisée, Cas versus Témoin réalisée à la maternité du centre de santé de référence de la commune V du District de BAMAKO du 01 Novembre 2017 au 31 Avril 2018 soit 6mois. L'objectif de notre étude était de déterminer la relation entre prééclampsie et le pronostic du nouveau-né. Deux groupes de 200 nouveau-nés chacun ont été constitués. Le groupe « Cas » était constitué de nouveau-nés issus de grossesses compliquées de prééclampsie, et le groupe « Témoin » ceux issus de grossesses normales. Les variables étaient les caractéristiques maternelles, foetales et les suites néonatales. L'analyse statistique a été faite avec SPSS 20.0 avec les tests de Khi-2, Student, Man Whitney et la régression logistique avec un risque alpha à 5 p.100 . Seuls 75 nouveau-nés du groupe « Cas » ont pu être mis immédiatement au sein maternel. Des différences significatives ont été observées entre les deux groupes sur les mesures anthropométriques et les données de morbidité foetale. En analyse multivariée il existait une association statistiquement significative entre prééclampsie et le risque de prématurité, d'hypotrophie foetale. La prééclampsie est génératrice de complications foetales comme le retard de croissance intra-utérin, la prématurité et l'hypotrophie foetale, mais aussi un recours plus important aux soins de réanimation et une mortalité plus élevée

    To explore the prevalence and associated socio-demographic factors of hepatitis B in HIV positive and HIV negative pregnant women attending antenatal care at the University Teaching Hospital, Lusaka, Zambia.To explore the prevalence and associated socio-demographic factors of hepatitis B in HIV positive and HIV negative pregnant women attending antenatal care at the University Teaching Hospital, Lusaka, Zambia. This was a comparative cross-sectional study with a total of 316 (158 HIV negative and 158 HIV positive) pregnant women, aged 16-46years. Participants were recruited from the antenatal ward using convenient sampling method from women with a known and documented HIV status. A structured questionnaire was administered for sociodemographic data and bloods for HBsAg screening werecollected. Data collection was done between 15th Dec 2016 and 30th May, 2017. The relationship between study variables and presence of HBV and HBV/HIV coinfection was examined using logistic regression. The selection for entry into the logistic regression model was considered at level p<0.05. Results: Of the 316 study participants 11(3.5%) tested positive for HBsAg. There was no statistical difference in the prevalence of HBV in the HIV negative and HIV positive pregnant women (3.8% and 3.2% respectively, P=0.76). Similarly, there was no association between the age, marital status, parity, residence, religion, education level or form of employment with HBV infection. Being on combined anti-retroviral therapy (cART) had a 91% reduced odds for HBV co-infection[OR = 0.09, CI = 0.01 – 0.63, P = 0.02] Conclusion: There was no significant difference in the prevalence of HBV between the HIV positive and HIV negative pregnant women. However, HIV antiretroviral treatment seems to have a protective effect on acquisition of HBV infection. Therefore, regardless of their HIV status or socio-demographic characteristics, all pregnant women should routinely be screened for HBV so that babies born to high risk mothers can receive the birth doses of HBV vaccine and immunoglobulins to prevent transmission to newborns.Office of Global AIDS/Department of State

    Calcium is the molecular switch shifting the phytosulfokine receptor 1 (PSKR1) from kinase to guanylate cyclase activity

    Conference presentationMany plant responses are mediated by interactions between intracellular calcium and the second messenger cGMP formed by guanylate cyclases (GCs). Previously we identified a novel class of receptor-GCs containing the GC catalytic center embedded within the kinase domain and showed that the recombinant cytoplasmic domain of phytosulfokine receptor AtPSKR1 has both guanylate cyclase and kinase activity in vitro (Kwezi et al. 2011 J Biol Chem 286: 22580-8). We now show that physiological increases in calcium levels enhance GC activity of AtPSKR1 whereas these calcium levels reversibly inhibit kinase activity. In addition PSKR1 kinase activity is reduced in the presence of the GC product cGMP. Recombinant AtPSKR1 can undergo in vitro autophosphorylation and we have confirmed it has 14 phosphorylation sites in its cytoplasmic domain including 8 serine, 3 threonine and 3 tyrosine residues. Three phospho-serine residues at the juxta-membrane position were mutated to either mimic phosphorylation on or off states. Kinase activity was enhanced in the on mutant and suppressed in the off mutant while GC activity was unaffected suggesting calcium acts as a molecular switch of PSKR1- mediated signalling that can be modulated by the phosphorylation state.     Le cancer de l'estomac, bien qu'en régression dans la plupart des pays industrialisés, reste un problème majeur de santé publique au Mali. Nous avons mené cette étude, dans le but d'étudier les aspects épidémiologiques, cliniques et anatomopathologiques de cette affection dans notre pays, Il s'agit d'une étude rétrospective ayant porté sur 55 cas des cancers de l'estomac diagnostiqués dans le service de Chirurgie « A » ; dans le service d'Anatomie et Cytologie Pathologiques du CHU du point "G" de 2013 à 2017. La connaissance du profil épidémiologique nous apparaît comme une nécessité car, elle permettra d'identifier les populations cibles. La taille de notre échantillon était de 55 cas. Il est plus fréquent chez l'homme que chez la femme avec un effectif de 72,7 p.100 . L'âge moyen des patients était de 57, 89 ans. Les classes sociales les plus démunies (Les Cultivateurs/Eleveurs) étaient les plus représentées. L'épi gastralgie était le principal motif de Consultation (94,5 p.100 ), les vomissements (83,6 p.100 ) et Amaigrissement (72,7 p.100 ). Les principaux antécédents ont été : l'ulcère gastrique (58,2 p.100 ) ; moignon de gastrectomie (5,5 p.100 ) et cancer de l'estomac (1,8 p.100 ). La moitié de nos patients avait un index de karnofski compris entre 80 et 100. L'antre était le siège de prédilection des cancers avec 47,3 p.100 des cas et le type histologique le plus fréquent était l'adénocarcinome (87,3 p.100 ). L'amélioration du pronostic et la baisse de l'incidence passent par la prise en Charge des lésions précancéreuses, l'éradication de le H. pylori, le changement des modes de conservations des aliments, le diagnostic et la prise en charge précoce des cas de cancer

    Status of Health workers Knowledge on Maternal and Neonatal Health related to Service Delivery in Makueni County

    Globally, new born mortality remains unacceptably high. Neonatal mortality accounts for 60% of infant mortality rate in Kenya. In Makueni County, 1 in 22 children does not live to his or her first birth day. This unacceptable high mortality is despite existence of cheap and tested interventions which could reduce up to 70% of current deaths. Evidence of mismanagement of clients in the County health facilities has raised concerns whether health workers attending to mothers and neonates are knowledgeable. The purpose of this study was to determine health worker knowledge on maternal and neonatal health related to service delivery in Makueni County. Specific objectives were to determine prenatal care, preeclampsia and postnatal care knowledge among primary care givers and how it relates to service delivery. Methods: we used a cross sectional research design. A total of 163 health workers selected through simple random sampling were involved. Data was collected using a structured questionnaire and observational checklist. Results: On prenatal care, majority of health workers 89(54.6%) had good knowledge (X2 =45.1 p<0.05). Practice didn’t conform to guidelines since 30.3% and 69.7% of antenatal mothers didn’t have full history taken and general physical examination done respectively. On pre-eclampsia, majority of caregivers (78, 49.4%) had poor knowledge while on postnatal care 53(32.5%), 58(37.4%) and 132 (80%) had good knowledge in new-born care, management of neonatal infections and young infant feeding respectively. Conclusions: The study concluded that health workers were not following guidelines and lacked adequate knowledge in management of pre-eclampsia, newborn care and neonatal infections. Recommendations: The study recommends the following to Makueni County health managers (i) to have refresher course, targeted continuous medical education and standard operating procedure on management of pre-eclampsia (ii) to strengthen supportive supervision and ensure guidelines are adhered to (iii) to retrain workers on new-born care and management of neonatal infection

    Trends and factors associated with acute respiratory infection among under-five children in Zambia

    ThesisAcute Respiratory Infection (ARI) is one of the leading causes of morbidity and mortality among children under the age of five years globally accounting for 16% of deaths. In Zambia, ARI accounts for 30-40% of children’s outpatient attendance and 20-30% of hospital admissions. The study therefore sought to assess trends and factors associated with ARI using demographic health survey data from 1996 to 2014. We conducted a cross-sectional secondary analysis of Zambia Demographic and health survey data from 1996, 2002, 2007 and 2014 involving under five children and their mothers. We extracted data using a data extraction tool from the women’s file and prepared for analysis. We assessed trends using chi-square for trends. We conducted a complex survey logistic regression analysis using STATA version 14.0 and reported adjusted odds ratios (AOR) 95% confidence intervals (CI) and p-values. The highest prevalence of ARI was in 2002 and a drop in 2007 and 2014 (Non-Parametric-trend p-0.001). Occurrence of ARI was associated with nutritional status, underweight children were more likely to develop ARI (Adjusted odds ratio [AOR] 1.50 95% [confidence interval] CI 1.25 – 1.68) compared with children who were not. Use of cooking fuels such as charcoal and firewood were associated with high odds for ARI compared to electricity use (AOR 2.67 95%CI 2.09 – 3.42 p-<0.001 and 2.79 95%CI 2.45 -3.19 p<0.001) respectively. Children with co-morbidities such as malaria and diarrhea (AOR 5.85 95%CI 5.01 – 6.64 p-<0.001 and AOR 1.45 95%CI 1.25 – 1.68 p-<0.001 respectively) compared to those who did not have. ARI was less likely among children with mothers who had secondary or higher education (AOR 0.30 95% 0.15-0.58 p-<0.001). Receiving micronutrients such as vitamin A was protective from ARI (AOR 0.46 95%CI 0.40 – 0.52). Interventions to reduce the burden of ARI should be targeted at health promotion and sensitization of mothers to avoid crowded places, use of cooking fuels such as firewood and charcoal as well as seeking healthcare early should signs of ARI occur. Furthermore, vaccination monitoring and ARI surveillance should continue in the community and health facilities in order to identify high burden areas.     Utilisation des services de Planification Familiale dans l’aire de santé de la commune rurale de Farako du district sanitaire de Ségou du 1er janvier au 31 décembre 2018

    Méthode : Nous avons effectué une étude descriptive, transversale, rétrospective visant à étudier l’utilisation des services de Planification Familiale dans la commune rurale de Farako du district sanitaire de Ségou. Notre étude s’est déroulée sur 08 mois allant du 1er janvier 31 août 2019 et l’exploitation des données de la PF du 1er janvier 31 décembre 2018. Ont été concerné par notre étude deux cent neuf (209) femmes âgées de 14 – 45 ans et deux (02) agents de santé impliqués dans la prestation des services de PF. Résultats : Les femmes âgées de moins de 30 ans représentaient (62,7%) avec une moyenne d’âge de 24,5 ans ; 84% des femmes étaient mariées ; (83,2%) étaient ménagères et (67,2%) étaient non instruites. La principale source d’information des femmes était le personnel médical (information donnée juste après l’accouchement) soit 47,9%. Nous avons trouvé que 53,6% des clientes avaient utilisé une contraception antérieure. Parmi les femmes ayant utilisé une méthode contraceptive 50,7% avait abandonné leur méthode pour désir de grossesse comme principale raison d’abandon (41,2%). Le contraceptif injectable (CIP) était la méthode contraceptive la plus utilisée (67,9%) suivi de l’implant (28,7%), le DIU (2,4%) et la pilule (1%) avec une prévalence de contraception à 5,6%. Le CSRef de Ségou et l’ONG Marie Stopes International Mali étaient les sources d’approvisionnement du dépôt de vente du centre en produits contraceptifs. Parmi les deux agents responsables de l’unité PF, seulement une avait reçu la formation et le recyclage sur les méthodes contraceptives durant l’année 2018. Seulement six méthodes contraceptives étaient disponibles au centre (CIP, implant, pilule, DIU, MAMA, préservatif masculin) et le centre ne disposait pas de matériels d’insertion de DIU. Conclusion : En définitive, nous pourrons dire que le taux de prévalence de la contraception n’atteint pas le taux souhaité au niveau national (16%) ; cependant les efforts restent à faire notamment en milieu communautaire par le renforcement des IEC/CCC en matière de PF par les acteurs communautaires (relais, ASC)

    Complications post opératoires en milieu de réanimation

    Notre étude sur les complications post opératoires en milieu de réanimation du CHU Gabriel Toure était une étude prospective, descriptive et analytique qui s’était déroulée sur une période de 09 mois allant du 1er octobre 2018 au 1er juin 2019. Ont été inclus dans l’étude les patients admis en réanimation et ayant présente une complication post opératoire. Les données ont été recueillies sur les fiches d’enquêtes. La collecte et l’analyse ont été réalisées sur le logiciel SPSS 19.0. La mise en tableau a été faite à l’aide de Microsoft Office Excel 2016 .Le traitement de texte a été réalisé sur Microsoft Office Word La fréquence des complications post opératoires était de 55,12%. Le sexe féminin était majoritairement représenté avec 63,7%. La tranche d’âge la plus présenté était celle comprise entre [21-40]. Les complications hémorragiques et cardiovasculaire étaient les plus représenté avec 54,86%.Les facteurs de risque identifies était la chirurgie en urgence et les classes Altemeier III et IV. L’évolution était favorable chez la majorité des patients mais le taux de décès de 29,2% reste encore élevé. La prédominance des complications médicales comme facteurs létaux prédictifs montre bien qu’une prise en charge médicalisée efficace peut améliorer le pronostic

    Une diminution de l'incidence du paludisme consécutive à la mise en oeuvre de stratégies de contrôle telles que l'utilisation de ACT, de MILD, du traitement préventif intermittent pendant la grossesse et du traitement préventif intermittent chez les nourrissons et de la chimioprévention du paludisme saisonnier a été observée dans certaines régions d'Afrique. En outre, une étude récente a montré que les enfants plus âgés étaient plus susceptibles au paludisme après le renforcement des stratégies de contrôle. Ces changements pourraient également être accompagnés par une modification des caractéristiques cliniques du paludisme grave (SM). Pour tester notre hypothèse, nous avons comparé la distribution des phénotypes cliniques du paludisme grave dans le contexte de mise à échelle des stratégies de lutte. Nous avons mis à profit une étude sur le paludisme grave menées de 2014 à 2018 à Bandiagara, dans le service de pédiatrie de l'hôpital du Mali de Bamako et à Sikasso pour déterminer la distribution des phénotypes cliniques du paludisme grave. L'analyse préliminaire des données recueillies après la mise à l'échelle des stratégies de contrôle a montré que parmi 79 cas de paludisme grave, le phénotype clinique de l'anémie paludéenne prédominante (48,1%) prédominait, tandis que la fréquence de neuropaludisme était de 35,4% et 16,5% des cas avaient un phénotype clinique mixte. Avant la mise à échelle des stratégies de contrôle, la part des formes neurologiques dans le paludisme grave était supérieure à 50%. La mise à échelle des stratégies de lutte aurait aussi un impact sur l’expression clinique du paludisme grave
