Evaluation de la pratique de la transfusion sanguine au Service d’Accueil des Urgences du CHU Gabriel Touré
La transfusion sanguine est un acte médical qui consiste à administrer le sang ou l’un de ses composants (GR, GB, plaquettes) provenant d’un ou de plusieurs sujets appelés « donneurs » à un ou plusieurs sujets malades appelés « receveurs ».
En dépit des moyens mis en œuvre pour améliorer la sécurité transfusionnelle, la transfusion sanguine reste une thérapie à risque, tant par le risque de transmission de maladies infectieuses, que par celui de survenue d’accidents immuno-allergiques. C’est la raison pour laquelle nous nous sommes proposés d’évaluer la pratique de la transfusion sanguine au service d’accueil des urgences du CHU Gabriel Touré.
Nous avons mené une étude prospective et descriptive qui s’est déroulé du 01 Septembre 2018 au 31 Aout 2019 soit une période de 12 mois au cours de laquelle nous avons enregistré 194 cas de transfusion sur un total de 19673 patients soit une prévalence faible de 1%.
La transfusion a été plus fréquente chez les sujets jeunes, avec une prédominance du sexe masculin soit 58%;le polytraumatisme était la pathologie la plus pourvoyeuse de transfusion sanguine soit 44,3 % ;le délai d’acquisition de la 1ere unité de PSL dans la plupart des cas est assez long dans notre contexte d’urgence soit dans 50,5% des cas ;le taux de satisfaction des demandes en PSL était de 64,07% ,ce qui est encore faible; la fièvre et le frisson étaient les incidents les plus rencontrés dans notre étude soit respectivement 4,12 et 2,06 %
Préférences des femmes âgées de 14 à 45 ans en matière de contraception dans la commune urbaine de Koulikoro
Notre étude prospective mixte c'est-à-dire avec un volet quantitatif et un volet qualitatif portant sur 214 femmes âgées de 14 à 45 ans, s’est déroulée dans la commune urbaine de Koulikoro du 25 Juillet 2018 au 31 Juillet 2018.
L’objectif visé était de découvrir les préférences des femmes en matière de contraception, déterminer la durée d’utilisation des méthodes de contraception préférées des femmes, identifier les raison du choix des femmes parmi les méthodes de contraception et identifier les caractéristiques sociodémographiques des participantes. Parmi les femmes enquêtées, 40,2% ont une préférence pour le Jadelle en raison de longue durée.
Parmi nos femmes enquêtées 44,4% de l’échantillon ont été orienté dans leur choix par un agent de santé, et le reste des femmes ont été orienté par : automédication, partenaires, parents, amie, voisine. La tranche d’âge la plus représentée était de 20-24 ans avec 29,4% de l’échantillon, ce qui montre qu’elles sont les plus concernées par la contraception, de même que les femmes mariées avec 83,2% de l’échantillon.
Les femmes scolarisées et non scolarisées pratiquent toutes la contraception, mais elle est plus pratiquée par les femmes scolarisées avec 71,1% de l’échantillon
Adherence to cholera interventions: associated factors, enablers and barriers in Chipata and George compounds of Lusaka, Zambia
ThesisCholera is an acute, diarrheal illness caused by the bacterium Vibrio cholerae. Cholera is
strongly linked to consumption of unsafe water, food, poor sanitation and overcrowded living
conditions.
Despite implementation of Cholera interventions in Lusaka, Cholera continues to affect the
lives of the people especially in the Peri urban areas of Lusaka. To date there is no empirical
evidence on factors associated with household adherence to Cholera interventions as well as
the enablers and barriers to adherence in these areas.
The purpose of this study was to establish enablers and barriers and factors associated with
adherence to Cholera interventions in Lusaka Zambia.
The data was collected using structured interview questionnaires from 400 participants.
Multistage sampling was adopted for selecting zones and Systematic random sampling for
selection of households. In-depth interviews were conducted for selected heads of household
and health committee chairpersons using maximum variation sampling.
The study used a Sequential Explanatory Design. Epidata management software was used for
database designing and questionnaire data entry and validation. The data was exported and
analysed using STATA software version 13. Categorical variables such as sex were stated as
proportions; Chi square test, Bivariate and multivariate logistics regression were used to test
for association between independent variables and household adherence to Cholera
interventions. A step wise multiple regression approach was used to select the best predictors.
Thematic analysis was used to identify enablers and barriers to Cholera interventions.
Only 5.8% of the households adhered to Cholera interventions. Factors associated with
adherence after controlling for other variables were age of household head (AOR=0.081;
95%CI=0.016-0.409) and mode of receipt of health messages (AOR=0.219; 95%CI=0.103-
0.466). Enablers were availability of water containers, water tanks, water kiosks, health
information and knowledge of hand washing behaviour. Barriers identified were
contamination and water leakages, shallow wells, queues at water points, distance from water
points, expensive charcoal, inadequate and expensive chlorine, fear of potential health effects
from use of chlorine, shared toilets and non-collection of garbage.
The households of Chipata and George Compounds still experience several barriers to
Cholera interventions. This has negatively affected the households’ with the majority of the
people not adhering to Cholera interventions.
The study recommends increasing the number of water points for households’ easy access to
water. Construction of flushable toilets due to lack of space for erection of pit latrines.
Regular collection of garbage to avoid disease transmission and prevention of households
from drawing water from shallow wells by burying them.

Microalbuminuria and serum creatinine in type 2 diabetes mellitus patients attending renal clinic at university teaching hospital in Lusaka,Zambia
ThesisThe study sought to investigate the presence of Micro-albuminuria (MA) in urine. MA urinary albumin excretion is in the range of 30 mg/24 hrs -300 mg/24 hrs. Consequently, urine albumin excretion has become a key therapeutic target in the management of patients with diabetes mellitus (DM). We aimed at describing the association between Micro-albuminuria, Serum Creatinine and fasting blood glucose in type 2 diabetes mellitus in patients attending Renal Clinic at University Teaching Hospital (UTH).This was an unmatched case control study involving 90 participants from UTH Renal Clinic. The cases included 45 type 2 diabetic patients who met the selection criteria and 45 non-diabetic controls. Blood and Spot urine samples were collected and analyzed for serum creatinine and micro-albuminuria respectively. We found a significant difference in the mean serum creatinine levels between the diabetics (95% CI= 4.231 - 4.438) and controls (95% CI= 3.943-4.163), p= 0.0003. The mean Micro-albuminuria levels between diabetics (95% CI= 3.405 – 3.737) and controls (95% CI = 2.967-3.266), also showed a significant difference, p=0.0001. Serum creatinine showed a weak positive correlation with fasting blood glucose, (r =0.2995 p =0.004) while Micro-albuminuria showed a strong positive correlation with fasting blood glucose, (r =0.3092, p =0.0030). In Zambian setting, both serum creatinine and Micro-albuminuria levels are significantly higher in type 2 diabetes patients compared to controls. Fasting blood glucose levels positively correlated with serum creatinine levels and Micro-albuminuria.

Factors associated with Elevated Plasma levels of Liprotein (a) in indegenous black Zambians with Diabetes Mellitus type 2 in the outpatient medical clinic at the University Teaching Hospital,Lusaka,Zambia
Introduction: Diabetes mellitus (DM) is an established risk factor for cardiovascular disease (CVD) and is considered to be a CVD equivalent. Lipoprotein(a) [Lp(a)] is an independent risk factor for cardiovascular diseases. Since Diabetes Mellitus Type 2 (T2DM) patient numbers continue to rise, and since patients continue to present with cardiovascular disease-related complications, it is possible that some of these patients have high plasma levels of Lp(a). The aim of the study was to investigate the plasma levels of Lp(a) in T2DM patients and also to assess the factors that may be associated with the plasma levels of Lp(a) among indigenous black Zambians with T2DM.
Materials and methods: We conducted a cross sectional study that enrolled 155 participants, 79 T2DM patients attending the outpatient medical clinic of the University Teaching Hospital and 76 community-based healthy individuals. A short questionnaire was used to record the social demographic characteristics and anthropometric measurements. 4ml of venous blood was collected from which all the analytes were measured. The factors that were assessed for association with Lp(a) included; social demographic characteristics, social economic status, duration of illness, dietary composition, physical fitness, BMI, fasting blood sugar (FBS), renal function, hepatic function, acute phase response, lipid profile, and glycaemic control. Therefore the variables that were measured were as follows; Age, sex, marital status, occupation, residence, dietary fat content, dietary carbohydrate source, dietary protein source, frequency of exercise, BMI, FBS, ALT, urea, creatinine, C-reactive protein, Triglycerides, total cholesterol, low density lipoprotein, high density lipoprotein, HbA1c and lipoprotein(a) respectively. The data were expressed as median (interquartile range). The Mann-Whitney U test was used to compare the median values between the two groups of the study participants (Diabetics and healthy individuals) for continuous variables, the Wilcoxon rank-sum test or Kruskal-Wallis test for the ordinal data whereas the Chi-squared test was used to compare the proportions for the nominal data. SPSS version 21(IBM) was used to perform a multiple linear regression analysis to identify the *9/et of variables that would best predict the plasma levels of Lp(a)
Results: The median plasma levels of Lp(a) in the diabetics (20.0 (11.8-37.4)mg/dl) was significantly higher (p<0.001,) than the healthy individuals (13.6 (9.4-21.5)mg/dl). 17 % of the diabetics had plasma levels of Lp(a) higher than 30 mg/dl. Of all the independent variables assessed, the results showed that glycemic control (HbA1c), FBS (glucose) triglycerides and residence were significant (p<0.001, p=0.030, p=0.040, p= 0.004 respectively) predictors of plasma levels of Lp(a). The linear relationships showed that the plasma levels of Lp(a) had a positive relationship with HbA1c (r=5.220) and FBS (r=0.660) whereas the relationship with triglycerides (r= -4.794) and residence (r= -7.165) were inverse Conclusion: . The plasma levels of Lp(a) in the T2DM patients were significantly higher than the non-diabetic healthy individuals. Glycemic control (HbA1c) , triglycerides (TG), fasting blood glucose (FBS) and social economic status (medium density residence) were predictors of serum levels of Lp(a)
Etude des échecs thérapeutiques de deuxième ligne chez les patients sous thérapie anti rétrovirale suivis au centre d'écoute de soins et d'animation et de conseils de Bamako.
Nous avons réalisé une étude retro prospective transversale au Centre d'écoute de soins, d'animation et de conseils de Bamako (CESAC). Aucours de notre étude, nous avons enregistré 20 patients sous traitement de troisième ligne. La prédominance féminine a été nette, c'est à dire 55 p.100 de la population d'étude étaient des femmes. La tranche d'âge de 30 à 40 ans était la plus représentée avec 11 patients soit 40 p.100 . L'inobservance était principalement due au refus de prendre et à l'intolérance aux ARV dans 30 p.100 des cas. Les patients VIH1 étaient majoritaires avec 85 p.100 . La proportion des patients ayant un taux de CD4 inférieur à 200 était passée de 45 p.100, à M6, à 55 p.100 à M12 ; nous avons constaté une augmentation du nombre des patients ayant une charge virale supérieure à 3000 copies/ml, passant ainsi de 73,3 p.100 à M6, à 88,2 p.100 à M12. Le schéma de première ligne 2 INRT+1 INNRT était le plus prescrit chez 85 p.100 de nos patients. Le schéma de deuxième ligne 2 INNRT+1IP BOOSTE était le plus prescrit après échec de première ligne avec un taux de 80 p.100 . TDF+3TC+Darunavir+Novir+Raltegravir était le schéma de troisième ligne le plus prescrit avec 65 p.100 des cas. L'échec thérapeutique est une réalité préoccupante dans notre contexte. Durant notre étude nous n'avons pas eu accès au test de genotypage de nos patients
Etude des traumatismes crâniens de l'enfant dans le service de Neurochirurgie du CHU Gabriel Touré de Bamako, de Mars 2017 au Mars 2018
Il s'agit d'une étude descriptive et prospective portant sur 202 patients, âgés de 0 à 15 ans victimes de traumatisme crânien, étalée sur 12 mois (de Mars 2017 au Mars 2018). Notre étude a montré une forte prédominance masculine avec 65.35 p.100 et un sex- ratio de 1.88. Les victimes avaient un âge compris entre 2 mois et 15 ans. Les chutes et les AVP ont été incriminés pour la plupart dans l'étiologie des traumatismes crâniens touchant en général les enfants en age prescolaire. 0.99 p.100 de nos patients avaient un GCS inférieur à 8 à l'admission ; Comme signes fonctionnels, nous avons noté des céphalées,des crises convulsives, la perte deconnaissance,lasomnolence voire le coma. Les signes physiques étaient la déformation du crâne, une rhinorrhée du LCR, des plaies crânio-faciales et des oedèmes autours de l'embarrure. la totalité des victimes de traumatisme crânien ont pu réaliser une TDM cérébrale soit 100 p.100 des cas. Comme traitement médical, les antalgiques et les antibiotiques ont été utilisésch ez tous nos patients opérésles techniques chirurgicales les plus utilisées étaient les lévés d'embarrure et les évacuations d'hematome à travers un volet. En terme d'évolution,12.48 p.100 de nos patients avaient présenté descomplications, c ontre 87.52 p.100 qui n'ont pas manifesté.10.89 p.100 des patients ont présenté des séquelles. Le taux de mortalité dans notre série a été de 1.49 p.100 soit 3 décès
Variations climatiques et risque d’accident vasculaire cérébral
L’objectif était d’étudier le risque et la prévisibilité de la survenue des accidents vasculaires cérébraux en fonction des variations climatiques. Il s’agissait d’une étude rétrospective descriptive et analytique portant sur des dossiers des patients hospitalisés au service de Neurologie du CHU Point-G résident à Bamako sur une période de 10ans (Janvier 2008 à Décembre 2017).
Sur 892 patients colligés, l’étude a abouti aux résultats suivants :
Une prédominance masculine de 54,7% avec un sexe ratio de 1,45
Un âge moyen de 59,1ans avec des extrêmes de 5 et 95 ans (Sujets âgés les plus atteints)
L’ischémie à la TDM a été prédominante dans 72,6%
L’HTA et l’âge étaient les Facteurs de risque les plus retrouvés
Les patients provenaient des urgences à 51,35%
Le risque était élevé en saison des pluies avec une pluviométrie de 0 à 150 mm (65,6%), une température moyenne de 24 à 28°C (46,46%), une humidité relative moyenne de 50 à 75% (41,96%). La survenue était en journée le matin (85,2%) et au mois d’Août (12%)
Sur 3 périodes après l’étude c’est-à-dire en 2020, d’après la prévision linéaire, le risque d’AVC peut probablement croit dans l’ensemble de l’année avec des taux de 1 à 7% et une diminution de la pluviométrie et l’humidité relative, une légère augmentation de la température moyenne de 0,5°C et de la vitesse de vent 0,3m/
Haematological and serological findings in Cercopithecus aethiops(African green monkeys) with Cyclospora infections in Kenya
P 122-131Cyclospora,is a common cause of gastroenteritis in humans resulting in protracted life threat¬ening diarrhoea in immunocompromised patients worldwide.Cyclospora infections have been reported in African green monkeys. The objective of the study was to determine the haema¬tological and serological parameters in African green monkeys with Cyclospora infections.A cross-sectional laboratory based study was done at Institute of Primate Research, Nairobi, from March 2008 to June 2009. Thirty three African green monkeys comprising of 10 male adults, 7 female adults, 3 male juveniles and 13 female juveniles were analysed for blood cell counts using a haematological analyser and screened for Cyclospora by ELISA. The red blood cell counts decreased in all study animals with mean decline of 1.2-1.5 (x 106/µl). The white blood cell counts varied among the study animals with highest increase among female adults from4.3-7.1 to 5.4-8.8 (x 103 /µl) and a decrease among the male adults from 3.9-8.7 to 1.5-5.3 (x 103/µl). Positive antibody responses to Cyclospora were observed in 20 of the study animals with highest mean optical density (OD) values of 0.816 ± 0.100 observed among the female juveniles and lowest among the male juveniles at mean OD value of 0.646 ± 0.055.
In conclusion, Cyclospora infections triggered both cellular and humoral responses in the African green monkeys
A case study of mama Lucy Kibaki hospital.
Health service delivery is a key pillar of the health system management .The World Health Organization recently emphasized the need to develop adolescent -friendly health services to improve the care provided to young people throughout the world. However, there is limited peer reviewed literature on this subject therefore necessitating assessment of whether the existing health facilities are prepared to implement the adolescent friendly health services. Adolescent friendly health services remains a relatively new and sensitive area mainly due to restrictive norms and policies guiding the services. After International Conference on Population and Development in 1994, countries started implementing adolescent friendly health services. The Government of Kenya together with partners in an attempt to address the health challenges came up with the Adolescent package of care (APOC) in 2013 whose guidelines were finalized in November 2014 and released for use by service providers. Despite this package of care, there is still ineffective staff capacity in relation to skills and knowledge gap of health professionals, training needs, health resources as well as health system factors that can affect implementation of AFHS. The study explored ways of mitigating or addressing the barriers to implementation of these services. The study used both quantitative and qualitative approaches to collect data. The study utilized survey research adapting descriptive cross sectional design and semi-structured questionnaire to interview 348 health care providers and 472 adolescents in Mam Lucy Kibaki Hospital from 3rd May 2014 to 16 June 2014 .The respondents were mainly nurses, clinical officers and Medical doctors who were working at the health service delivery area at the time of study and were interviewed using an interview guide. The managers at the hospital were interviewed using an in-depth interview guide while the adolescents were interviewed through interview schedule. A total of four (4) focus group discussion was held with adolescents. Quantitative data was analyzed using SPSS Version 18.0. Descriptive statistics and chi-square tests were performed to determine significant associations. The study established that sex, age, level of education and adolescent awareness about existence of friendly health services offered were significantly associated with utilization at p<0.05. Long queues, unfavorable working hours and lack of money negatively affected consumption of AFHS. The study concluded that the utilization of health services among the adolescents was low largely due to unfriendliness of the health care providers at health facilities and lack of awareness of AFHS services. In view of the findings, this study recommends need for the Government through the Ministry of Health and partners in health service provision to increase the number of AFHS and ensure that the recommendations of adolescent package of care is implemented fully with good evaluation strategies in place. Laborious awareness drives to sensitize the adolescents about availability of adolescent friendly services through rigorous health education and increased involvement of both parents/guardians and teachers to scale up implementation are also recommended.
