    Etude comparative de la précarité et de l'accouchement non assisté à domicile de deux quartiers du district sanitaire de la commune v de Bamako/Mali

    Introduction : Nombreuses sont les mères qui n'ont pas accès à des services de soins de santé modernes. On estime que 60 à 80 p.100 des femmes des pays en voie de développement continuent d'accoucher à domicile sans aucune assistance ou avec l'aide de personnes non qualifiées et d'ordinaire dans un environnement peu salubre. Cette étude avait pour objectif d'étudier l'accouchement non assisté à domicile dans deux quartiers du District Sanitaire de la Commune V de Bamako Mali. Méthode : Il s'agissait d'une étude transversale analytique. Nous avons comparé deux groupes de femmes résidant dans deux quartiers différents (Sabalibougou et Badalabougou) et ayant accouché à domicile sur une période de 12 mois, du 1er mars 2015 au 28 Février 2016. Résultats : Au terme de cette étude, nous avons enregistré 2020 accouchements aux CSCom de Sabalibougou dont 263 accouchements à domicile soit 13 p.100 des accouchements et 1962 accouchements au CSRéf de la commune V venant de Badalabougou dont 62 accouchements à domicile soit 3 p.100 des accouchements. La proportion des femmes non scolarisées était de 74,64 p.100 à Sabalibougou contre 25,36 p.100 à Badalabougou avec une différence statistiquement significativement p égal 0,01. A Sabalibougou 89,40 p.100 des femmes avaient accouché à domicile par manque des moyens financiers contre 10,60 p.100 des femmes vivant à Badalabougou avec une différence statistiquement significative p égal 10 -5 . L'hémorragie par atonie utérine était la complication la plus représentée dans les deux quartiers soit 72,92 p.100 à Sabalibougou et 27,08 p.100 à Badalabougou sans significativité (p égal 0,44). Conclusion : Cette étude a montré que l'accouchement non assisté demeure une réalité en commune V du district de Bamako. Des actions sont nécessaires pour minimiser ce phénomène à travers la réduction du coûts des CPN et de l'accouchement dans les structures sanitaires

    Résume L’objectif de ce travail était d’étudier les aspects épidémiologiques et cliniques des intoxications aigues chez les enfants. Il s’agit d’une étude rétrospective et descriptive. Les données ont été recensées de janvier 201 6 à décembre 2018 soit une période de 3 ans. Nous avons pu colliger 80 cas d’intoxication aigue représentant de 1 ,47% de l’ensemble des hospitalisations aux urgences pédiatriques pendant la même période. Le sexe ratio était de 1, 28. La tranche d’âge 13-48 mois était la plus touchée avec 65% des cas. La garde des patients était assurée dans la majorité des cas par leurs mères qui sont essentiellement des femmes au foyer (76,3%). Les produits mis en cause sont majoritairement les médicaments suivis des produits industriels. L’administration de lait a été une attitude fréquente. Les signes cliniques les plus fréquents étaient les troubles et les troubles respiratoires. La prise en charge était essentiellement symptomatique. Cependant la létalité reste élevée (17,5%

    La maladie thromboembolique constitue une thématique majeure de santé publique par sa fréquence et sa gravité croissante. L'incidence pourrait être diminuée par une prévention en milieu médical. Objectif : Déterminer la fréquence de survenue des maladies thromboemboliques dans le service de Médecine Interne, évaluer les facteurs de risque, décrire les étiologies de la MTEV, décrire les traitements utilisés enfin d'évaluer le pronostic. Méthodologie : Il s'agissait d'une étude rétrospective et prospective et descriptive de janvier 2016 à décembre 2016 et de juillet 2018 à septembre 2018 et a concerné tous les malades hospitalisés durant les périodes d'étude. Résultats : La fréquence de risque de MTVE était de 86 p.100 patients hospitalisés dont 56 p.100 de sexe féminin. La tranche d'âge entre 61-74 ans représentait 24,9 p.100 et la profession ménagère était représentée avec 36,8 p.100 . 70 p.100 de nos patients vivaient en milieu urbain et 30 p.100 en milieu rural. Pour les patients hospitalisés 86,2 p.100 avaient un facteur de risque dont 56,4 p.100 de femme. 52,9 p.100 de nos patients ont une infection aigue, l'alitement prolongé a été retrouvé chez 48,8 p.100, le diabète était incriminé dans 28,4 p.100 et l'AVC 13,4 p.100 . Le niveau de risque modéré représentait 38,7 p.100, élevé 14,9 p.100 et très élevé 13,8 p.100 . La durée moyenne d'hospitalisation est de 19 jours avec un minimum de 4 jours et un maximum de 98 jours. Parmi les patients ayant un facteur de risque 53,8 p.100 ont reçu un traitement antithromboembolique prophylactique ; nous avons enregistré 3,1 p.100 de décès était probablement due au MTEV

    Evaluation of the relationship between dietary patterns and colorectal cancer among Meru County residents

    A thesis submitted in partial fulfillment for the conferment of Master of Science degree in Human Nutrition and Dietetics of Kenya Methodist UniversityColorectal cancer (CRC) is the fourth leading cause of death and third most commonly diagnosed cancer around the globe. Although diet plays an essential role in the development of colorectal cancer, there is little information on the role of diet in the rising CRC cases observed in Meru. The study sought to determine whether there is a connection between dietary patterns and colorectal cancer etiology in Meru County. The research was a retrospective case control study design involving 25 CRC patients and 50 CRC undiagnosed participants as controls. All stable CRC patients receiving treatment in the three cancer treatment and palliative care centers in Meru County between April and June 2017 were included in the study. Each CRC patient was identified and matched with two persons from the general population according to similarity by sex, date of birth, and place of residence. A food frequency questionnaire was used to collect data which was then averaged to daily consumptions. The data was analyzed using t-test, Chi-square and Odds Ratio. The study revealed that dietary patterns of Meru County residents include moderate intake of fruits (2.5 servings), vegetables (4.0 servings) , and starch staples (9.2 servings), low intake of pulses (1.5) , white meat (1.2), milk and its products(1.3) , and nuts and seeds (0.3) ,and high intake of red meat (5.1 ) servings for daily pattern intakes with no significant statistical difference between dietary patterns of CRC and non CRC persons (p- values > 0.05). No statistical association was established between dietary patterns of red meat and risk of developing CRC (p- values > 0.05) on chi-square test and Odds ratio (within the 95% CI). Our study therefore indicates that, there is no conclusive evidence associating dietary patterns and colorectal cancer etiology and incidences in residents of Meru County. This research recommends that other causes of cancer be explored so as to establish evidence based conclusions on the main causes of colorectal cancer in Meru Count

    L’objectif était d’évaluer l’indice CAO et la FGC en fonction du sexe, de l’âge, du niveau d’étude, de l’ethnie, de l’occupation des parents. Il s’agit d’une étude épidémiologique descriptive transversale a visée évaluative sur l’état buccodentaire chez les élèves du lycée Alfred Garçon dans la commune II du district de Bamako. L’étude s’est déroulée du 12 juillet au 13 novembre 2019. L’étude a porté sur un échantillon de 184 élèves de tout âge dont 115 Garçons et 69 Filles avec un sex-ratio de 1,67. L’indice CAO a été de 2,53 avec une fréquence globale de 83,15%%. Il n’y avait pas de liaison statistiquement significative entre le sexe et la carie dentaire (Khi² = 3,712 ; ddl = 1 ; p = 0,077

    Most referral health facilities are faced with challenges ranging from congestion of patients at these health facilities, limited resources both human and material to deal with the voluminous clients, slow rate of service delivery due to high numbers as a result of the by-passing tendencies. Self-referrals normally results in compromised quality of medical care provided to the patients. The problems mentioned above affects most of the high level health facilities which are traditionally meant to be referral health facilities. The study conducted in the month of August 2017 therefore aimed to find out the determinants of a functional referral system in two level 5 health facilities; Jaramogi Oginga Odinga Referral Hospital (JOOTRH) and Kisumu County Referral Hospital (KCH). The objectives of the study were to; examine the extent to which the primary health care centres characteristics, patients’ characteristics, receiving health facility characteristics and proximity to a health facility influence a functional referral system. A cross sectional study was conducted using a qualitative approach to data collection. Three hundred and thirty eight patients were systemically selected to participate in the study. Data was collected using an individual questionnaire through one on one interview between the dates 14th- 22nd August 2017. The collected data were coded and analyzed using the SPSS 25.0 Computer program. The qualitative data collected were analyzed quantitatively using the descriptive statistics and linear regression models. The results indicates that there was a positive and significant influence of the primary health facility characteristics (r=.474**, p<0.001) and receiving facility characteristics (r=.475**, p<0.001) on a functional referral system at the two high level facilities. There was also a negative but significant influence of proximity to health facility (r=- 0.137**, p=0.017) on a functional referral system at the two level 5 facilities. On patients characteristics, a significant interaction was found on gender χ2 (1) = 4.64, p= 0.031, education level χ2(2) = 20.37, p<0.001 and Marital status χ2(2) = 8.78, p= 0.012 with a functional referral system. Conclusion – Factors influencing functional referral system are primary health centre characteristics, patients’ characteristics, proximity to a health facility and referral health facility characteristics. It is therefore recommended that the County government of Kisumu should improve the supply of drugs and quality of service delivery at the primary health facilities to attract more patients seeking primary care services, specialists movement especially to the lower level health facilities need to be reinforced by the Ministry of Health to boost the citizen’s confidence in quality of care at these facilities, more sensitization strategies need to be put in place to educate both the patients and health care providers on the recommended referral practices that should be upheld to improve efficiency and effectiveness of service delivery

    Aspects épidémiologiques, cliniques et thérapeutiques des traumatismes rénaux au service d’urologie du CHU Gabriel TOURE

    Il s’agit d’une étude prospective portant sur 13 cas de traumatisme du rein, colligés au service d’urologie du CHU Gabriel TOURE de Bamako sur une période d’une année. Les patients se répartissaient en 13 hommes et pas de femmes. Nous avons enregistré 10 cas de traumatisme fermé et 3 cas de plaie pénétrante du rein. L’âge moyen était de 26 ans. Les étiologies étaient dominées par les accidents de la circulation. Les principaux signes cliniques étaient l’hématurie et les lombalgies. L’état hémodynamique était stable à l’entrée chez 77% de nos patients. L’échographie puis la tomodensitométrie rénale ont été réalisées chez tous les patients permettant le diagnostic de traumatisme rénal. Le bilan radiologique permettait de classer les traumatismes selon l’AAST (American Association for the Surgery of Trauma) en : 9 cas de grade I (69,2%), 3 cas de grade III (23,1%) et 1 cas de grade IV (7,7%). Notre prise en charge, a consisté en un traitement conservateur chez 12 patients, et chirurgical chez 1 patient dont une néphrectomie totale. Pas de décès enregistré, les 13 patients, ont évolué favorablement, aucune complication n’est observée avec un recul de 2 mois en moyenn

    Factors Influencing Rational Drug Use in Public Hospitals among Doctors and Pharmacists in Meru County

Irrational drug use is influenced by lack of essential drugs, lack of training and weak implementation of policies. The purpose of this study was to assess the factors that influence rational drug use among doctors and pharmacists in public hospitals in Meru County. Specific objectives were to examine how staff awareness on policy guidelines, management practices, product selection and inventory management influence rational drug use. A cross-sectional research design was adopted for this study. The sample size was 102 doctors and pharmacists in the public hospitals in Meru County. A Likert based questionnaire was used to collect data. There was a significant relationship between staff awareness (r=.232, p<0.05) and inventory management (r=.324, p<0.05) on rational drug use. Coefficient of determination (R) of 0.402 was obtained compared to overall R2 of 0.162 and this explains 40.2% of total variations that explained factors that influence rational drug use among the healthcare workers in Meru County. The ANOVA findings (p<0.05) shows that there was correlation between the predictors variables and the dependent variable RDU. The study recommends the following to the Hospital Management Team i) to offer training on rational drug use, ii) offer support supervision and appraisals iii) Constitute active Drugs and Therapeutics Committees iv) to form procurement committee with all stakeholders to strengthen inventory management

    Factors associated with the participation of safe motherhood action groups in accelerating institutional deliveries among maternal women in ShibuyunjiI rural district of Lusaka province

    ThesisThe low utilization of health facilities for delivery in Sub-Saharan Africa is a major concern despite the availability of global Motherhood initiatives to increase health facility usage. The concept of using Safe Motherhood Action Groups (SMAGs) is emerging to be a useful tool for referral of mothers to health facilities either for care without delay. Despite the availability of SMAGs in Shibuyunji District, statistics have shown that institutional deliveries were below (48%) the MoH‟s target of 60% in 2016, with increased community-based maternal deaths. We therefore examined the factors associated with the participation of SMAGs in acceleration of institutional deliveries among maternal women in Shibuyunji rural district. An analytical cross sectional study was conducted involving 239 maternal women. A check list was used to collect data of respondents from the health facility delivery register and postnatal care register. A semi-structured questionnaire was administered. Step-wise, backward regression analysis using Stata Version 14 was used to estimate adjusted odds ratios (AOR) and 95% confidence intervals (CIs) of associated factors with SMAGs accelerating institutional deliveries among maternal women. All independent variables that were significant at univariate level were included in the Multivariable regression analysis until the final model was obtained. The study revealed that 93% of the women were assisted by SMAG to access the health facility for delivery. The study showed the key predictors for as shown; Women who lived 10 km away from the health facility had a reduced odds of delivering at the health facility by 90% [AOR 0.1; 95% CI 0.03-0.39; P=0.001] [AOR 0.1; 95% CI 0.03-0.39; P=0.001], The pregnant women who did not know the benefits of delivering from the health facility had a reduced odds of delivering from the facility by 93% [AOR 0.07; 95% CI 0.014-0.37; P= 0.002]. Pregnant women who reported not being visited at their home by SMAGs had a reduced odds of delivering from the health facility by 70% [AOR 0.3; 95% CI 0.09-0.86; P= 0.027]. Evidence in this study has demonstrated that district institutional deliveries were low against the MoH target of 60% health facility deliveries. Nonetheless, participation of SMAGs in accelerating institutional deliveries among women in Shibuyunji district had a relative influence in the community, given evidence that women who were home visited by SMAGs, pregnant women who knew the benefits of delivering from the health facility and women who lived within 0 – 5km to the HF were more likely to deliver at the HF, providing a differential imbalance among maternal women who lived in further places of Nakaiba, Chintanga and Masiteki. These findings may be a reflection of inequalities associated with access and limitations in awareness efforts of the SMAG program in Shibuyunji. Thus, a need for effective and sustainable means of transport to improve access by linking communities to the health facility care through SMAGs who should be placed in strategic areas of the district.
