Polymorphisme génétique de la dihydropteroate Synthétase de Plasmodium falciparum Pfdhps par la technique de séquençage des isolats collectés au cours de la chimio-prévention saisonnière du paludisme à Dioro, Missira et Sélingué
La présente étude s’inscrit dans le cadre du Projet PMI 2018-2019 intitulé : "Evaluation de l’approche CPS : caractérisation moléculaire des gènes dhfr et dhps de Plasmodium falciparum associés à la résistance à la sulfadoxine et à la pyriméthamine à Sélingué, Missira et Dioro (Mali)".
L’étude réalisée est de type transversal analytique et s’est déroulée d’octobre 2018 à juin 2019. Elle a porté sur des enfants de 3 à 59 mois se présentant au centre de santé et ayant reçus des doses de SP/AQ au cours de la campagne de CPS. L’étude consistait à prélever des échantillons de sang pour l’analyse des marqueurs moléculaires de la résistance à la SP. Objectif était de savoir si l’utilisation de la SP/AQ durant la campagne de protection contre le paludisme chez les enfants de 3 à 59 moins ne conduit pas aux parasites résistants a la sulfadoxine. C’est dans ce but que nous avons choisi 58 échantillons pour cette étude tous âgés de 0 3 à 59 mois et soumis au séquençage de leur gène Pfdhps. Un total de 13 séquences de bonne de qualité sur 58 a été obtenu et sélectionné pour la recherche de mutation sur le gène Pfdhps, soit un taux de réussite de 22,41%. Au cours de notre étude, nous avons détecté 13 souches portant les mutations sur les positions S436A, A437G, K540 E, A613S/P comme mutation connues à la résistance à la sulfadoxine mais aussi des mutations non connues à la position 613 aux acides aminés (F, G, Y et codon stop) de ce gène.
Après analyse des séquences du gène Pfdhps, nous avons obtenu des mutations de type « transversion » aux positions 436 ; 437 et 540 qui entraine des variations génétiques entrainant des changements d’acides aminés. Il s’agissait notamment des changements respectifs suivants : TCT2>GCT Serine (S) en Alanine (A), GCT ->GGT Alanine (A) -> Glycine (G) et AAA-> GAA Lysine (K) ->Acide Glutamique (E)
Au niveau de la position 613 nous avons révélé deux souches qui portent une variation génétique de type « transversion » entrainant un changement d’acide aminé GCC ->CCC Alanine (A) -> Proline (P) et GCC ->CCC Alanine (A) -> Serine (S) Cette position reste toute de même polymorphe avec la présence du changement d’acide aminé Alanine (A) en phénylamine (F), Tyrosine, Glycine (G) et codon stop
Après avoir examiné la diversité du gène Pfdhps dans ces 3 sites d’études (Missira, Sélingué et Dioro), à partir du logiciel Geneious Prime version 2019.2.1 par la méthode Neighbor-joining et Tamura-Nei model utilisé comme modèle de distance génétique, nous avons observé qu’après L’analyse phylogénétique que les 13 souches sont génétiquement différentes
Péritonite par perforations iléales d’origine typhique au service de chirurgie de l’Hôpital Sominé Dolo de Mopti
Bien que relativement rare dans les pays développés, les perforations iléales d’origine typhique constituent un problème de santé publique dans les pays en voie de développement notamment au Mali.
Notre étude avait pour but d’étudier les perforations iléales d’origine typhique dans le service de chirurgie de l’Hôpital Sominé Dolo de Mopti.
Il s’agissait d’une étude prospective qui s’est déroulée du 1er novembre 2017 au 31 Octobre novembre 2018 soit 12 mois.
Au cours de cette étude nous avons observé 58 cas de perforations iléales d’origine typhique soit 8,41% des consultations d’urgence.
Il s’agit d’une pathologie du sujet jeune avec un âge moyen de 19,7 ans.
Nous avons trouvé une prédominance masculine (63,8% d’hommes) avec un sexe ratio de 1,76.
L’urgence a été le mode de recrutement le plus fréquent (91,4%).
Le délai moyen de consultation était 13,1 jours après les symptômes.
Parmi les signes évocateurs de la perforation intestinale, la douleur abdominale (100%), la défense ou contracture abdominale (100%), la douleur au toucher rectal (94,9%) et l’abolition de la respiration abdominale (74,1%) ont été les plus fréquents. L’ASP a posé le diagnostic de perforation intestinale à travers le croissant gazeux dans 53,4% des cas.
Le sérodiagnostic de Widal était positif dans 74,1% des cas.
L’excision-suture a été la technique opératoire la plus utilisée dans notre série (58,6%).
La morbidité précoce a été de 51,5% et la mortalité globale a été de 29,3% soit 17 décès sur 58 cas de perforations iléales d’origine typhique
Assessment of nutrition counseling services offered to diabetic patients and its effect on adherence to therapies and treatment at the Nakuru Provincial General Hospital, Kenya
RC 628 .K3 2014Poor health has been a global issue which has been associated with the impediment of productivity, economic growth and poverty eradication. It has been estimated that 32 percent of the global burdens of disease are caused by dietary complications. All millennium development goals are in one way or the other related to good nutrition intake. Nutritional Counseling has been credited for ensuring that people are aware of their dietary choices hence avoiding nutritional complications. This is more essential to diabetic patients. The purpose of this study was to assess the effectiveness of Counseling services offered to diabetic patients in Nakuru Provincial General Hospital, Kenya. In order to have the study carried out successfully, a descriptive survey was used to assess the effectiveness of nutritional Counseling services. The sample consisted of 283 respondents that included 8 dieticians and 275 diabetic patients. Stratified random sampling where the total population was split into two distinct samples (diabetic patients and dieticians) based on their demographics. After obtaining permission from all relevant authorities, the researcher collected the data in form of responses through structured questionnaires. Expert judgment was used to improve on content and construct validity of the items. The Statistical Package for Social Sciences (SPSS) was used to analyze the data. The study established that there was a significant relationship between the patient’s nutritional knowledge and recovery rate. Majority of the patients were not adhering to the recommended therapy. There was a statistically significant influence of nutritional counselors’ knowledge of diabetic management and diabetics’ recovery rate. There was also a significant relationship between the economic status of patients and adherence to therapy. This study recommends that the government must take an active role in promoting the nutritional counseling services offered in hospitals by employing more dieticians, intensifying awareness campaigns on the recommended diet for the diabetics. Civil society and religious groups must also take an active role by holding workshop and seminars to sensitize the community about the diabetic condition, psychological support and fighting against stigma to the patients. Additionally, dieticians need to be aware of the current recommendations for diabetic patients so as to pass it on to their clients. Male nutritionists/dieticians should also be trained to encourage more male patients to attend counseling sessions. The researcher suggests a further research on the level of stakeholders input in promoting nutritional counseling in hospitals and an assessment on whether patients actually adhere to the diet by doing a 24 hour recall
Prévalence des échecs virologiques des adolescents infectés par le VIH-1 et sous traitement ARV
Le Mali est le 3ème pays africain à avoir rendu gratuit l’accès aux traitements ARV et le suivi biologique de l’infection à VIH en Juillet 2004.
Nous avons mené une étude sur la prévalence de l’échec virologique chez les adolescents infectés par le VIH-1 et sous traitement ARV au CHU Gabriel Touré entre novembre 2016 et décembre 2017. Nous avons mené une étude transversale qui portait sur 41 adolescents. Les données ont été recueillies à partir des dossiers des patients. La tranche d’âge 14-15 ans était la plus représentée (46,34%), les garçons étaient les plus nombreux (63,41%). Le schéma thérapeutique EFV/ABC/3TC était le plus prescrit à nos patients (43,90%). Nous avons évalué la prévalence de l’échec virologique chez 41 patients sous traitement ARV infectés par le VIH-1 soit un taux d’échec de 39,02%.
La prévalence de l’échec virologique était 31,71% à 6 mois et 39,02% à 12 mois de ARV.
Dans l’ensemble, nos résultats ont montré que l’observance était un facteur déterminant pour obtenir une augmentation significative du taux de lymphocyte T CD4 et une diminution considérable de la charge virale plasmatique.
Ces résultats suggèrent la nécessité de renforcer les capacités des laboratoires (Bamako et capitales régionales) pour le suivi biologique des PV VIH. Ceci permettra la prise en charge précoce des situations d’échec virologique et de ses causes (inobservance, résistance aux ARV).
Experiences of health research stakeholders with international health research collaborations in Zambia
ThesisThe promotion of collaborative partnerships involving researchers and institutions from Low-and Middle-Income Countries with those from High-Income Countries has been a major development in health research. While these initiatives have shown great promise and attracted growing interest, research examining the processes that improve or hinder joint working and the inner workings of international health research collaborations has been limited. The study aimed to explore the experiences of health research stakeholders with international health research collaborations.
A qualitative phenomenological inquiry was employed using In-depth interviews with 20 key informants involved in North-South health research partnerships conducting health research activities in different parts of Zambia. Thematic analysis was used to analyse the data with the aid of Nvivo 12 data management software.
The findings of this study revealed that partnerships produce benefits for Southern partners which include generating of evidence to influence policy, improved service delivery, infrastructure development and designing of interventions to improve the healthcare of populations in most need. Most importantly through partnerships, there is the availability of financial resources to accomplish partnership goals. To enhance international health research partnerships, effective communication, local leadership, values, and accountability were identified as important in the process of partnership functioning. Trust interacts with different elements that create partnerships where there is co-ownership of study rewards. When this trust has been created, more opportunities for partnerships are formed. Negative loops of interaction in health research collaborations are largely due to funding mechanisms where ninety percent of the funding for health research is from Northern partners. This funding mechanism results in power imbalances that lead to publication challenges, dictation of research agenda and ownership of samples and data leading to misunderstandings between partners and a general lack of motivation to collaborate.
The study highlights the importance of international health research collaborations and its many benefits which include strategies to address community health challenges through the supply of ideas, translating research into interventions and designing health strategies. Challenges experienced result from unequal power relations where most of the funding comes from Northern partners. Acknowledging and reporting both positive and negative processes maximises learning in health research collaborations and highlight areas that partnerships need to focus on to make the most of joint workings.

Determinants of availability of tracer essential medicines in public health facilities in Nyeri County, Kenya
The goal of the health system is to improve health and health equity in ways that are responsive, financially fair and make the most efficient use of the available resources. This is supported by six Health Systems Strengthening (HSS) pillars of medicines, vaccines and technologies, human resource, financing, leadership and governance, service delivery and health information systems. A well-functioning health care system ensures equitable access to medicines, vaccines and technologies which results in overall HSS. Tracer essential medicines are supposed to be 100% available at all times to treat the common ailments in a region. Despite the large number of patients that visit public health facilities, the inadequacy of trained pharmaceutical personnel, inefficiencies of the supply chain system and inadequate funding, no study has been carried out to ascertain the availability of tracer essential medicines in these facilities. The objectives of the study were to determine the stocking level of tracer essential medicines, to establish human resource factors affecting availability of tracer essential medicines, to find out the supply chain factors affecting availability of tracer essential medicines and to assess the organizational factors affecting availability of tracer essential medicines. A cross sectional study design with a mixed method approach was used. A questionnaire was used to collect data on the human resource factors and supply chain factors affecting availability of tracer essential medicines. A Focus Group Discussion (FGD) guide was also conducted with seven respondents comprising of six sub County public health nurses and the County pharmacist. The sample population comprised of 17 public health facilities and 30 pharmacy personnel. The response rate was 25 (83.3%). The average stocking level in tier three facilities was 94% while that in tier two facilities was 63.75%. The overall mean percentage stocking level of the 20 tracer essential medicines in all the facilities was 72.65%. Human resource factors that affected availability were unavailability of Standard Operating Procedures (SOPs), inadequate training of pharmacy personnel and pharmacy personnel not aware of tracer medicines. There was a significant relationship between availability of tracer essential medicines and pharmacy personnel awareness of tracer list (r=0.850), pharmacy personnel receiving training in commodity management (r=0.834), availability of SOPs (r=0.817) and designation of pharmacy personnel (r=0.746). The supply chain factors that affected availability of tracer essential medicines were long lead times, unavailability at Kenya Medical Supplies Agency (KEMSA), long ordering and delivery schedules and incorrect quantification. The organizational factors that affected availability were inadequate supportive supervision, inadequate financing and absence of Revolving Drug Funds (RDFs) within the facilities. The study concluded that the stocking level of tracer essential medicines in Nyeri County was below the World Health Organization recommendation of 100%. In addition, human resource factors, supply chain factors and organizational factors affected availability. The researcher recommends that a community pharmacy with subsidized medicines prices be introduced in the facilities; the County should facilitate training of all pharmacy personnel on commodity management, all facilities should have buffer stocks to cover at least two months as they await new stocks and the County should provide supportive supervision to the facilities
Prevalence of CCR2 gene polymorphisms among HIV infected individuals from different regions in Kenya
P 270-274Chemokine receptor-2 (CCR2) is a co-receptor for the entry of human immunodeficiency virus- I (HIV-I) into the target cells. Patients with the CCR2 mutations may progress to AIDS at least 2-4 years later than individuals carrying the normal gene. A G-to-A transition at position 190 characterizes the CCR2-64I mutation. This mutation has been identified as an important factor for delaying progression to AIDS. The effect of CCR2 polymorphism on HIV-1 disease progression has not been explored in depth within Africa. The status of CCR2 gene polymorphisms among HIV infected individuals in the Kenyan population is unknown. The objective of this study was to determine the existence and distribution of CCR2 gene mutations and to identify the different polymorphic groups of the co-receptor gene among HIV infected individuals in the Kenyan population

Aspects épidemio-cliniques, diagnostiques, thérapeutiques et évolutifs de la méningo-encéphalite herpétique dans le service des maladies infectieuses du CHU du Point G de Bamako
L’objectif général de notre étude était d’étudier les aspects épidémiocliniques, diagnostiques, thérapeutiques et évolutifs de la méngo-encéphalite herpétique. Il s’est agi d’une étude transversale avec recueil rétrospectif les données allant du 1er Janvier 2018 au 31 janvier 2019.
Résultat : Quatre cas de méningo-encéphalite herpétique ont été recensés dans le Service des Maladies Infectieuses soit 0,008% des admissions avec un taux de positivité de 26,67%. Tous ces patients résidaient à Bamako et le sexe masculin représentait un cas sur quatre. Les patients âgés de plus de 30 ans représentaient 75%. Les ménagères représentaient 50%. 100% de nos patients ont présenté trois critères majeurs associés à au moins deux critères mineurs d’une méningo-encéphalite herpétique. Le délai de consultation dans un centre de santé après apparition des premiers symptômes était d’au moins 14 jours. 25% des patients avaient une TDM cérébrale en faveur d’une encéphalite et HVS 1 a été isolé dans le LCR de 50% des patients. Un traitement par Aciclovir a été initié chez tous les patients avec un délai d’au moins 20 jours après les premiers symptômes et le taux de mortalité était de 75%
Qualité de l’eau alimentant le circuit de dialyse au centre hospitalier universitaire de point G.
Le but de notre étude était de suivre le traitement de l’eau utilisée en dialyse, ainsi que de faire l’état des lieux de la qualité de cette eau produite.
L’étude a été faite pendant une période de 6 mois dans la salle de traitement d’eau pour le suivie de l’installation, dans le laboratoire national de la santé pour les analyses physico-chimiques et les analyses bactériologiques.
Le suivi a été fait tous les jours et aucun dysfonctionnement n’a été constaté durant la période d’étude.
Les résultats des analyses bactériologiques et physico-chimiques étaient conformes aux seuils fixés par la pharmacopée française.
La qualité de l’eau utilisée dans le traitement par hémodialyse de l’insuffisance rénale chronique constitue un élément essentiel de l’efficacité et de la sécurité de cette thérapeutique ; différentes précautions doivent donc être prises pour réduire ou éviter les risques de mortalité, de morbidité ou d’inconfort liés à des défauts de cette qualité. Pour satisfaire aux dispositifs proposés par la pharmacopée, l’eau doit subir un traitement spécifique, même si elle provient d’un réseau de distribution publique.
Le mode de production doit donc être extrêmement efficace et faible. Pour cela il est indispensable que diverses règles techniques et méthodes soient respectées pour la conception, l’exploitation, l’entretien, la surveillance et le contrôle des installations
